    The sentinel surveillance of sexually transmitted disease (STD) has been started in 8 model prefectures in Japan since 1998. The response rate of the surveillance in Tokushima was 87.7% in 1999, and 86.6% in 2000. From the results of the sentinel surveillance, the incidence rate of total STD per 100,000 person・year in Tokushima was about 410 in 1999, and about 440 in 2000. Chlamydial infection was most frequent especially in younger women, and the incident rate per 100,000 person・year was about 2,500 in the age ranged from 20 to 24 years old.     Eocene volcanism during the incipient stage of Izu–Ogasawara Arc: Geology and petrology of the Mukojima Island Group, the Ogasawara Islands.

    The Ogasawara Islands mainly comprise Eocene volcanic strata formed when the Izu-Ogasawara-Mariana Arc began. We present the first detailed volcanic geology, petrography and geochemistry of the Mukojima Island Group, northernmost of the Ogasawara Islands, and show that the volcanic stratigraphy consists of arc tholeiitic rocks, ultra-depleted boninite-series rocks, and less-depleted boninitic andesites, which are correlatable to the Maruberiwan, Asahiyama and Mikazukiyama Formations on the Chichijima Island Group to the south. On Chichijima, a short hiatus is identified between the Maruberiwan (boninite, bronzite andesite, and dacite) and Asahiyama Formation (quartz dacite and rhyolite). In contrast, these lithologies are interbedded on Nakodojima of the Mukojima Island Group. The stratigraphically lower portion of Mukojima is mainly composed of pillow lava, which is overlain by reworked volcaniclastic rocks in the middle, whereas the upper portion is dominated by pyroclastic rocks. This suggests that volcanic activity now preserved in the Mukojima Island Group records growth of one or more volcanoes, beginning with quiet extrusion of lava under relatively deep water followed by volcaniclastic deposition. These then changed into moderately explosive eruptions that took place in shallow water or above sea level. This is consistent with the uplift of the entire Ogasawara Ridge during the Eocene. Boninites from the Mukojima Island Group are divided into three types on the basis of geochemistry. Type1 boninites have high SiO2 (>57.0wt.%) and Zr/Ti (>0.022) and are the most abundant type in both Mukojima and Chichijima Island Groups. Type2 boninites have low SiO2 (<57.1wt.%) and Zr/Ti (<0.014). Type3 boninites have 57.6-60.7wt.% SiO2 and are characterized by high CaO/Al2O3 (0.9-1.1).     Prevention of the initial testosterone surge induced by a luteinizing hormone-releasing hormone analogue in prostate cancer patients:the endocrinological effects of pretreatment with chlormadinone acetate

    We investigated the endocrinological effects of pretreatment with chlormadinone acetate (CMA) in preventing the initial testosterone surge induced by a luteinizing hormone-releasing hormone (LH-RH) analogue. A total of 25 patients with previously untreated prostate cancer were included in this study. The patients were randomly assigned to 2 treatment groups:Group1;CMA therapy was begun 4 weeks before the initial LH-RH analogue injection. Group 2;CMA therapy was begun 2 weeks before the initial LH-RH analogue injection. After the initial LH-RH analogue injection, CMA was administered during this study. After LH-RH analogue application, the mean values of serum luteinizing hormone (LH) and testosterone increased in both groups on day 3. However, LH and testosterone levels remained beneath pretreatment values in both groups. The mean relative PSA levels did not significantly increased on day 3 and day 7in both groups. Our results indicate that pretreatment with CMA for 2 weeks was sufficient to prevent the initial testosterone surge in the maximal androgen blockade which was associated with CMA

    The aim of this study is to clarify the usefulness of PSA screening to detect prostate cancer subclinically in Tokushima City. Mass screening of serum PSA measurement only was started from 2002 in Tokushima City. Three years’ results are reported and analyzed in this paper. In 2002 to 2004,(9099,9345,10680) men over 55 years old were measured serum PSA level with annual check of health condition at various outpatient clinics in Tokushima City. The men with high PSA levels were recommended to visit the urologist for further examinations. By the urologists the men with high PSA levels were diagnosed by careful urologic procedures including DRE, TRUS, accurate prostatic biopsy and more. In (801,606,727) men, PSA levels were over normal range, and (455,257,358) men visited urologic clinics for further examinations. Careful examinations were performed and prostatic biopsies were done in (233,121,141) men. Finally, the prostate cancers were found out in (121,60,59) men and these men were entered suitable medical treatments immediately. Accordingly, (1.34%,0.64%,0.55%) of (9099,9345,10680) men with PSA measurements were diagnosed as prostate cancers. In the group of prostate cancer, patient number of early cancer group (Stage B) were (80:66.1%,42:70.0%,50:84.7%),which means that prostate cancers found out by PSA screening might be early curable cancers in many cases. These 3 years’ results indicate that PSA screening is a very useful modality to find out early prostate cancers and contribute the decrease of prostate cancer death in Tokushima City in the future

    Induction of expressions of HLA class I and class II antigens by interferon on human renal cell carcinoma or bladder cancer cell lines

    Interferon-induced expressions of HLA class I and class II (DR, DP, DQ) antigens on human renal cell carcinoma (RCC) cell lines and bladder cancer cell lines were analyzed by flowcytometry. Class I antigen was present on all six different RCC cell lines and the expression was enhanced with IFN-γ treatment except one cell line, Caki-2. Although no RCC cell lines expressed class II antigens without IFN-γ treatment, the treatment induced the expression of DR antigen in all cell lines except Caki-2. DP antigen was induced on KPK-1 and KPK-13 with exposure to IFN-γ. On the other hand, bladder cancer cell lines studied showed surface expression of class I antigen and enhanced expression of class I antigen with IFN-γ treatment. However, IFN-γ didn't enhance the expression of class II antigens on any of those bladder cancer cell lines. The minimum concentrations of IFN-γ to increase the expression of class I and DR andtigens on both KPK-13 and ACHN were 4 IU/ml and 100 IU/ml, respectively. The incubation time with IFN-γ to get the maximal expression of class I and DR antigens on KPK-13 were 36 hr and 48 hr, respectively. Meanwhile, although the expression of class I antigen on RCC cell lines was enhanced by IFN-α, IFN-α could not give any effect in class II antigens. Our results demonstrate the obvious effect of IFN-γ to enhance the expression of not only HLA class I antigen but also class II antigens on RCC cell lines

    Thermal and chemical evolution of the subarc mantle revealed by spinel-hosted melt inclusions in boninite from the Ogasawara (bonin) Archipelago, Japan

    Primitive melt inclusions in chrome spinel from the Ogasawara Archipelago (Japan) compose two discrete groups of high-SiO2, high-MgO (high-Si) and low-SiO2, low-MgO (low-Si) boninitic suites, with ultra-depleted dish- and V-shaped, and less-depleted flat, rare earth element patterns. The most magnesian melt inclusions of each geochemical type were used to estimate the temperature-pressure conditions for primary boninites, which range from 1345 °C at 0.56 GPa to 1421 °C at 0.85 GPa for the 48-46 Ma low-Si and high-Si boninites, and 1381 °C at 0.85 GPa for the 45 Ma low-Si boninite. The onset of the Pacific slab subduction at 52 Ma forced upwelling of depleted mid-oceanic ridge basalt mantle (DMM) to yield proto-arc basalt (PAB). With the rise of DMM, refractory harzburgite ascended without melting. At 48-46 Ma, introduction of slab fluids induced melting of the PAB residue and high-temperature harzburgite, resulting in the low-Si and high-Si boninites, respectively. Meanwhile, convection within the mantle wedge brought the less-depleted residue of PAB and DMM into the region fluxed by slab fluids, which melted to yield the less-depleted low-Si boninite at 45 Ma, and fertile arc basalts, respectively

    Biologic markers in prostatic intraepithelial neoplasia: immunohistochemical and cytogenetic analyses

    Objective:We evaluated the biological properties of High-grade prostatic intraepithelial neoplasia (PIN) by immunohistochemistry and fluorescence in situ hybridization (FISH) analysis in relation to normal tissue and carcinoma lesions. Materials and Methods:Immunohistochemical staining and FISH were performed on23formalin-fixed radical prostatectomy specimens taken from patients with PIN. Assays were performed using MIB-1, chromogranin A (CGA) and an anti-androgen receptor antibody (AR). A centromere probe for chromosome8was used to test for aneuploidy. Results:The MIB-1index of cancerous specimens (16.2±10.5%) was significantly higher than that of benign (1.9±1.6%, p<0.0001) or PIN (4.0±4.5%, p<0.0001) specimens. The percentage of CGA positive cells was significantly lower in normal tissue (1.2±1.8%) than in PIN (3.5±2.9%, p=0.012) or carcinoma (5.4±4.9%, p=0.005) lesions. Positive staining for AR was consistently observed in the nuclei of both benign and malignant epithelial cells, but positive cytoplasmic staining was also seen in PIN epithelial cells. No significant difference in FISH detected anomalies were found between PIN and carcinoma specimens. Conclusions:Our studies concerning proliferative activity, NE differentiation and chromosomal anomalies of prostatic specimens support the hypothesis that PIN is a biologically intermediate stage in the pathogenesis of prostatic carcinoma. The cellular distribution of AR was altered in PIN cells, but the role of AR in PIN is not yet clear

    Unusual trivial trauma may end with extrusion of a well-functioning penile prosthesis: a case report

    <p>Abstract</p> <p>Background</p> <p>Diabetes mellitus (DM) is the most common indication for insertion of a penile prosthesis and is a risk factor for infection of such prostheses.</p> <p>Case presentation</p> <p>Two patients presented with infected prostheses following unusual trivial penile trauma. Both patients underwent exploration and removal of the prostheses with uneventful recovery.</p> <p>Conclusion</p> <p>Appropriate sizing of the prosthesis should be taken into account to ensure good concealment and avoid easy exposure of the penis to unexpected trauma. Use of the newly designed antibiotic-coated prostheses appears preferable. As soon as signs of prosthesis infection appeared, extrusion of the device should be expedited.</p
