    Modeling of the aorta artery aneurysms and renal artery stenosis using cardiovascular electronic system

    <p>Abstract</p> <p>Background</p> <p>The aortic aneurysm is a dilatation of the aortic wall which occurs in the saccular and fusiform types. The aortic aneurysms can rupture, if left untreated. The renal stenosis occurs when the flow of blood from the arteries leading to the kidneys is constricted by atherosclerotic plaque. This narrowing may lead to the renal failure. Previous works have shown that, modelling is a useful tool for understanding of cardiovascular system functioning and pathophysiology of the system. The present study is concerned with the modelling of aortic aneurysms and renal artery stenosis using the cardiovascular electronic system.</p> <p>Methods</p> <p>The geometrical models of the aortic aneurysms and renal artery stenosis, with different rates, were constructed based on the original anatomical data. The pressure drop of each section due to the aneurysms or stenosis was computed by means of computational fluid dynamics method. The compliance of each section with the aneurysms or stenosis is also calculated using the mathematical method. An electrical system representing the cardiovascular circulation was used to study the effects of these pressure drops and the compliance variations on this system.</p> <p>Results</p> <p>The results showed the decreasing of pressure along the aorta and renal arteries lengths, due to the aneurysms and stenosis, at the peak systole. The mathematical method demonstrated that compliances of the aorta sections and renal increased with the expansion rate of the aneurysms and stenosis. The results of the modelling, such as electrical pressure graphs, exhibited the features of the pathologies such as hypertension and were compared with the relevant experimental data.</p> <p>Conclusion</p> <p>We conclude from the study that the aortic aneurysms as well as renal artery stenosis may be the most important determinant of the arteries rupture and failure. Furthermore, these pathologies play important rules in increase of the cardiovascular pulse pressure which leads to the hypertension.</p

    Evalution of Programmed Cell Death 4 (PDCD4) Expression in Patients with Acute Myeloid Leukemia

    Background: Acute myeloid leukemia (AML) is clinically, cytogenetically, and molecularly heterogeneous. Advances in molecular research have greatly improved our understanding of the leukemogenesis in AML. Programmed cell death-4 (PDCD4) is a novel tumor suppressor that inhibits neoplastic transformation and tumor progression and invasion by suppressing activator protein (AP)-1 activation and protein translation. In the present study, we examined the PDCD4 expression levels in de novo AML and investigated correlation between altered expression of PDCD4 with patients’ clinical characteristics. Methods: Peripheral blood samples were collected from 56 patients with AML at the time of diagnosis and also, 10 healthy individuals. Quantification of PDCD4 mRNA expression by Real Time Quantitative PCR was performed. Mann-Whitney U test was used to compare PDCD4 expression differences among healthy and AML samples. Findings: PDCD4 mRNA expression was significantly diminished in patients with AML at diagnosis in comparison to the PB as normal samples (P < 0.001). We observed a statistically lower expression in AML-M5 subtype in comparison to other AML subtypes (P = 0.028) . We have not found any significant correlation between PDCD4 expression level and clinical parameters of patients. Also, no significant correlation between the expression levels of PDCD4 and complete remission after induction therapy was observed. Conclusion: Our results shown that, like many epithelial cancers, the down-regulation of PDCD4 expression also occurs in AML

    Investigation of metabonomics technique by analyze of NMR data, which method is better? Mean center or auto scale?

    The factors such as disease can disrupt homeostasis, resulting in perturbations of endogenous biochemicals that are involved in key metabolic profiles. Metabonomics is useful technique to quantitative description of endogenous metabolites present in a biological sample such as urine, plasma and tissue. High resolution 1H nuclear magnetic resonance (NMR)-based metabonomics is a technique used to analyze and interpret multivariate metabolic data that correlate with changes of physiological conditions. Before any explanation for metabolite data, preprocessing the spectroscopic data is essential. In this paper, we show scaling effects in metabonomics investigation of patients diagnosed with Crohn's and Celiac disease. two techniques of scaling were applied as follows: mean centering and auto scaling. Results reveal that the mean centering is more useful to segregate patients from healthy subjects in the data set of Crohn's and Celiac disease

    Magnetized inulin by Fe3O4 as a bio-nano adsorbent for treating water contaminated with methyl orange and crystal violet dyes

    Current work focuses on fabricating a new bio-nano adsorbent of Fe3O4@inulin nanocomposite via an in-situ co-precipitation procedure to adsorb methyl orange (MO) and crystal violet (CV) dyes from aqueous solutions. Different physical characterization analyses verified the successful fabrication of the magnetic nanocomposite. The adsorbent performance in dye removal was evaluated by varying initial dye concentration, adsorbent dosage, pH and temperature in 5110 mg/L, 0.10.8 g/L, 111 and 283 – 338 K, respectively. Due to the pH of zero point of charge and intrinsic properties of dyes, the optimum pHs were 5 and 7 for MO and CV adsorption, respectively. The correlation of coefficient (R2) and reduced chi-squared value were the criteria in order to select the best isotherm and kinetics models. The Langmuir model illustrated a better fit for the adsorption data for both dyes, demonstrating the maximum adsorption capacity of 276.26 and 223.57 mg/g at 338 K for MO and CV, respectively. As well, the pseudo-second-order model showed a better fitness for kinetics data compared to the pseudo-first-order and Elovich models. The thermodynamic parameters exhibited that the dye adsorption process is endothermic and spontaneous, which supported the enhanced adsorption rate by increasing temperature. Moreover, the nanocomposite presented outstanding capacity and stability after 6 successive cycles by retaining more than 87% of its initial dye removal efficiency. Overall, the magnetized inulin with Fe3O4 could be a competent adsorbent for eliminating anionic and cationic dyes from water

    Violence against Nurses: A Neglected and Health-threatening Epidemic in the University Affiliated Public Hospitals in Shiraz, Iran.

    BACKGROUND: Nurses are more likely to be exposed to violence at their workplace in comparison with other employees. OBJECTIVE: To determine various aspects of violence against nurses in Shiraz public hospitals. METHODS: This cross-sectional study was conducted from 2017 to 2018, using a multistage random sampling method. Violence including verbal threats, verbal abuse, physical and sexual abuse as well as ethnical types, violence from patients, patients' companions and coworkers, and causes of violence were investigated using a checklist. RESULTS: 405 nurses with a mean age of 30.2 (SD 7.1) years and female to male ratio of 4.2 were interviewed. 363 (89.6%) nurses had experienced at least one kind of violence; 68.4% suffered from more than one type of violence. Verbal abuse (83.9%), verbal threats (27.6%), physical violence (21.4%), sexual abuse (10.8%), and ethnical harassment (6.1%) were the most common types of violence experienced by the nurses. Patients' companions, patients, and physicians were reported as the sources of violence in 70.6%, 43.1%, and 4.1% of cases, respectively. Nurses with non-official employment status and non-Farsi ethnicity, having a disease, with non-evening shift work, and those with short or long employment period were more affected. Unrealistic expectations by patients' companions and long working hours were the most common attributing factors. CONCLUSION: Violence against nurses, as a strenuous and health-threatening crisis, has become epidemic in public hospitals in our region.     An algorithm for differentiating food antigen-related gastrointestinal symptoms

    Aim: The aim of this clinical audit was to assess patient-reported outcomes on the effect of dietary intervention, to enhance our understanding of possible treatment options in irritable bowel syndrome (IBS). Background: A large number of food-related gastro-intestinal disorders have been attributed to IBS for decades. Methods: Patient-reported outcomes from the records of 149 IBS patients treated at secondary and tertiary Gastroenterology outpatients in two UK hospitals between January 2014 and July 2016 were audited. Patients all presented with symptoms fulfilling Rome III-IV criteria for IBS had negative coeliac serology and did not have other gastrointestinal (GI) conditions. A modified version of a low FODMAP diet had been recommended (gluten and lactose free diet (G/LFD)) and was implemented for 6 weeks. Outcomes and dietary adherence were recorded during outpatient's consultations. Results: A total of 134 patients complied with the diet optimally. The majority had an improvement rate >70% and continued with the diet. Fifty-three percent became completely or almost asymptomatic, while 27.6% had a poor response to the diet (scoring < 30%) to G/LFD. The improvement was excellent in patients with normal BMI and good in overweight and obese and where BMI <18. Over 50% did not require any follow-up within 12 months. Conclusion: Although it is unclear whether symptoms are triggered by gluten, fructans or lactose, elimination of gluten and lactose proved to be an effective treatment in patients with IBS. Multidisciplinary team management and implementation of detailed nutrition therapy using the audit algorithm might prove to be both cost effective and efficacious a treatment option in IBS
