    Synthesis of research issues and capacity building in water and land resources management in Ethiopia

    This paper presents an overview and synthesis of the key research and capacity building issues arising from the workshop presentations and the papers. Three days of intensive deliberations by professionals from various research, development and governmental organizations, and of diverse disciplines, backgrounds and nationalities have clearly acknowledged that water management issues remain very crucial for poverty alleviation and rural development in Ethiopia – the overwhelming proportion of the population depends on smallholder agricultural production, which is highly constrained by water availability (absence of perennial rivers, high spatial and temporal availability of rainfall, etc.). This situation, over the years, has generated a critical need for efficient water and land management to reduce production risks and hazards, and enhance stable agricultural and livestock production. Recent decades have witnessed various efforts in the area of irrigation and supplementary irrigation (and other development initiatives), employing various water harvesting technologies, construction of micro dams, diversions structures, etc., which were largely combined with traditional yield-enhancing methods to facilitate sustainable smallholder agricultural production.Most of these efforts did not only fall short of their desired objectives of improving smallholder production conditions but also generated a host of other problems which aggravated smallholder production conditions: the large scale irrigation schemes (e.g. Awash Basin and elsewhere) resulted in secondary soil salinization where large tracts of land have gone out of production; the spontaneous construction of micro-dams across the country (especially in Tigray) is associated with human and livestock health hazards that in some cases has resulted into abandonment of the dams; the production potential of extensive watersheds remain largely unexploited or inappropriately utilized, resulting in extensive degradation of fragile lands, and so on. The potential for effectively integrating crops and livestock management in the context of growing water in complementary crop-livestock systems remains largely unexploited, especially from the perspective of efficient water and land utilization.     Foreign Body Ingestion: Three Case Reports

    Foreign body ingestion in adults is rarely reported in our environment. While the incident is commonly deliberate in adults, it is often accidental in children. As illustrated in these case reports, diagnosis is often challenging if the act is not witnessed or reported by someone, even at the onset of complications. A seven-week-old boy ingested a used razor blade given to him by his three-year-old sister. The incident was reported and the object removed at laparotomy.A 38year old woman deliberately swallowed a padlock-and-key apparently to attract her husband’s attention. Plain X-rays demonstrated a radio-opaque object first in the chest and then in the stomach. Both padlock-and-key were removed at operation. A 24-year old male with a psychiatric disorder was brought for treatment of an abdominal surgical wound that had failed to heal four months after appendicectomy. Wound treatment failed and exploration of the wound and laparotomy extracted metals and plastic objects.Diagnosis of foreign body ingestion in our environment is achieved by documenting a proper history, physical evaluation and often, plain X-rays.     Influence of Cereal-Legume Rotation on Striga Control and Maize Grain Yield in Farmers’ Fields in the Northern Guinea Savanna of Nigeria

    On-farm trials were conducted in 2001, 2002, and 2003 in the northern Guinea savanna of Nigeria to evaluate integrated Striga hermonthica control measures under farmermanaged conditions. These included intercropping a Striga-resistant maize variety with cowpea for 3 years and also cropping this maize in rotation with legume trap crops - soybean and cowpea for 1-2 two years. Intercropping Striga-tolerant maize variety, Acr. 97TZL Comp. 1-W, with cowpea (Vigna unguiculata L.) consistently reduced Striga infestation in maize relative to continuously cropped sole maize over the threeyear period. Maize grain yield was lower in the intercrop than in the sole maize plot probably due to competition from cowpea. However, because of the high value of cowpea in the intercrop, crop value for this system was higher than sole cropped maize. Legume-maize rotation reduced Striga infestation by 35% after one year of legumes in the rotation and by 76% after two years of legumes in the rotation. Soybean was more effective in reducing Striga infestation and also gave higher maize grain yield than cowpea. The rotation of these two legumes with maize had clear advantage over continuously cropped maize. Farmers should therefore be encouraged to adopt the introduction of grain legumes into the cereal cropping systems of the Nigerian savanna

    Fighting COVID-19 in Freetown, Sierra Leone: the critical role of community organisations in a growing pandemic

    As the COVID-19 pandemic spreads in Africa, attention is increasingly shifting to the potential and ongoing impact on informal settlements, which face considerable challenges around the implementation of conventional control measures of social distancing, hand washing and self-isolation. In Freetown, Sierra Leone, residents of informal settlements have relied on local community organisations and groups, and their resourcefulness to provide essential preparedness, response and on-going support to alleviate the public health and economic risks associated with the effects of the COVID-19 outbreak. This is also premised on lessons drawn from dealing with previous epidemics, notably the Ebola virus disease in 2014–2015. This paper will explain the nature and form of community organisation that can be galvanised and leveraged for COVID-19 preparedness and responses that are suited for informal settlements. Secondly, it highlights the critical contribution of community organisations in social protection measures that tackle deeply entrenched inequalities in rapidly urbanising contexts. Finally, the cases examined seek to provide evidence of the value of processes of continuous learning within community organisation that are essential for both humanitarian assistance and emergency management.     Glycosylation of human cyclooxygenase-2 (COX-2) decreases the efficacy of certain COX-2 inhibitors.

    Prostanoids play an important role in a variety of physiological and pathophysiological processes including inflammation and cancer. The rate-limiting step in the prostanoid biosynthesis pathway is catalyzed by cyclooxygenase-2 (COX-2). COX-2 exists as two glycoforms, 72 and 74 kDa, the latter resulting from an additional glycosylation at Asn(580). In this study, Asn(580) was mutated, and the mutant and wild-type COX-2 genes were expressed in COS-1 cells to determine how glycosylation affects the inhibition of COX-2 activity by aspirin, flurbiprofen, ibuprofen, celecoxib, and etoricoxib. Results indicate that certain inhibitors were 2-5 times more effective at inhibiting COX-2 activity when the glycosylation site was eliminated, indicating that glycosylation of COX-2 at Asn(580) decreases the efficacy of some inhibitors

    Bringing the social into vaccination research: Community-led ethnography and trust-building in immunization programs in Sierra Leone

    Background Vaccine hesitancy is a complex, contested social phenomenon and existing research highlights the multifaceted role of trust in strengthening vaccine confidence. However, understanding public engagement with vaccination through the lens of (mis)trust requires more contextual evidence on trust's qualitative determinants. This includes expanding the geographic focus beyond current studies' focus on High Income Countries. Furthermore, obstacles remain in effectively integrating social science findings in the design of vaccine deployment strategies, and in ensuring that those who implement interventions and are affected by them are directly involved in producing knowledge about vaccination challenges. Methods We piloted a community-led ethnographic approach, training Community Health Workers (CHWs) in Kambia District, Sierra Leone, in qualitative social science methods. Methods included participant observation, participatory power mapping and rumour tracking, focus group discussions and key stakeholder interviews. CHWs, with the support of public health officials and professional social scientists, conducted research on vaccination challenges, analysed data, tested new community engagement strategies based on their findings and elicited local perspectives on these approaches. Results Our findings on vaccine confidence in five border communities highlighted three key themes: the impact of prior experiences with the health system on (mis)trust; relevance of livelihood strategies and power dynamics for vaccine uptake and access; and the contextual nature of knowledge around vaccines. Across these themes, we show how expressions of trust centered on social proximity, reliability and respect and the role of structural issues affecting both vaccine access and confidence. The pilot also highlighted the value and practical challenges to meaningfully co-designed research. Conclusion There is scope for broader application of a community-led ethnographic approach will help redesign programming that is responsive to local knowledge and experience. Involving communities and low-cadre service providers in generating knowledge and solutions can strengthen relationships and sustain dialogue to bolster vaccine confidence

    An intra-COVID-19 assessment of hand hygiene facility, policy and staff compliance in two hospitals in Sierra Leone: is there a difference between regional and capital city hospitals?

    Although hand hygiene (HH) is the most effective intervention to reduce the spread of infections, there are limited data on HH facilities, policy, and compliance in sub-Saharan Africa. This cross-sectional study is aimed at assessing HH using the WHO HH self-assessment framework, HH technical reference manual, and a modified infection control self-assessment tool in two hospitals in Sierra Leone. Only 10% and 9% of regional and capital city hospitals had running tap water, respectively. Veronica buckets were the resources for HH in 89% of units in the regional hospital and 92% of units in capital city hospital. Constant supply of soap and alcohol-based hand rub was available in 82% and 68%; and 74% and 79% of units in the capital city and regional hospitals, respectively. Only 10% of the units in both hospitals had hand-drying facilities and functional sinks. Overall HH compliance for the two hospitals was 18.6% and was higher in the regional (20.8%) than the capital city (17.0%) hospitals. The HH levels for the capital city and regional hospitals were 277.5 and 262.5 respectively. Despite the COVID-19 pandemic, there are still challenges with HH compliance in Sierra Leone. It is, therefore, necessary to strengthen the HH multi-modal strategy
