The Impact of Script Concordance Testing on Clinical Decision-Making in Paramedic Education
This study investigates the effectiveness of the Script Concordance Test (SCT) in enhancing clinical reasoning skills within paramedic education. Focusing on the Medical University of Lublin, we evaluated the SCT’s application across two cohorts of paramedic students, aiming to understand its potential to improve decision-making skills in emergency scenarios. Our approach, informed by Van der Vleuten’s assessment framework, revealed that while the SCT’s correlation with traditional methods like multiple-choice questions (MCQs) was limited, its formative nature significantly contributed to improved performance in summative assessments. These findings suggest that the SCT can be an effective tool in paramedic training, particularly in strengthening cognitive abilities critical for emergency responses. The study underscores the importance of incorporating innovative assessment tools like SCTs in paramedic curricula, not only to enhance clinical reasoning but also to prepare students for effective emergency responses. Our research contributes to the ongoing efforts in refining paramedic education and highlights the need for versatile assessment strategies in preparing future healthcare professionals for diverse clinical challenges
Differentiation of Bacillus anthracis and other Bacillus cereus group bacterial strains using multilocus sequence typing method
The study describes the preparation of the phylogenetic differentiation of Bacillus cereus strains. The Bacillus cereus group of bacteria is very important for human and animal health. The multilocus sequence typing scheme has been used to present this group of bacteria’s phylogenetic relationship and structure. The MLST system was established using 60 isolates of B. anthracis, B. cereus sensu stricto, B. thuringiensis, and transitional environment strains of Bacillus spp. As a negative control, five strains of B. subtilis and B. megaterium were used. Primers for amplification and sequencing were designed to target highly conserved internal fragment of seven housekeeping genes: glpF, gmk, ilvD, pta, pur, pycA, and tpi. A total of 22 different sequence types (STs) were distinguished. Analysis of the sequence data showed that all of the Bacillus cereus strains are very closely related. The MLST scheme exhibited a high level of resolution that can be used as an excellent tool for studying the phylogenetic relationship, epidemiology, and population structure of the Bacillus cereus group strains. The MLST method additionally allows us to define the phylogenetic relationship between very closely related strains based on a combination of the sequences of all seven alleles fragments and each of them separately. Thus, this genetic investigation tool is very useful in epidemiological investigation of potential military/ bioterrorist use of B. anthracis
The regression analysis of predictors for selected health behaviors among elderly in Poland
The meaning of the word health takes on a different connotation in relation to certain individuals or groups. The trend is visible when taking under consideration the health of elderly individuals or the entire population in this age category. Even more, there has been noticed a great spread in individuals belonging to this age group, given that the elderly person is considered to be one that completed 65 years of life untill the diversified its end. Therefore, such a wide age range covering approximately 20 - 30 years demands diverse criteria for persons aged 70 years and the ones aged 90 years. The purpose of the study was to identify predictors influencing the health behaviors of people over 65 years of age in Poland. On the one hand, older age allows a sense of satisfaction, fulfillment and understanding for the meaning of the whole past life. On the other hand, it can be a source of physical and mental pain, the cause of dissatisfaction and frustration. All in all, the manner in which this period is experienced depends on numerous factors. These include both genetic and individual factors as well as social aspects. However, the most important ones are considered to be health behaviors of seniors. For that reason, taking into account the perspective of aging Poland, joint preparation for old age should become a priority undertaking
The severity of traumatic stress associated with COVID-19 pandemic, perception of support, sense of security, and sense of meaning in life among nurses : research protocol and preliminary results from Poland
The COVID-19 pandemic can not only affect physical health, but also mental health, resulting in sleep problems, depression, and traumatic stress. Our research investigates the level of posttraumatic stress, perceived social support, opinions on positive and negative consequences of the pandemic, sense of security and sense of meaning among nurses in the face of this new and not fully understood global epidemiological phenomenon. For this purpose, computer-assisted web interviews were conducted between May 1 and May 15, 2020. Participating nurses completed the following research tools: The Impact Event Scale-Revised (IES-R), The Multidimensional Scale of Perceived Social Support (MSPSS), The Changes in Outlook Questionnaire (CIOQ), The Safety Experience Questionnaire (SEQ) and The Meaning in Life Questionnaire (MLQ). Three hundred and twenty-five nurses of an average age of 39.18 ± 11.16 years and working throughout Poland joined the study. The average overall IES-R score in the study group was 1.78 ± 0.65. Among the dimensions of traumatic stress, the highest score was obtained in the “avoidance” dimension was 1.86 ± 0.73. Amongst participating nurses, the highest support rates were provided by significant others (22.58 ± 5.22). Higher average scores were noted among participants in the subscale measuring positive psychological changes (18.56 ± 4.04). The mean MLQ score was 5.33 ± 0.87. A slightly higher result was observed in the subscale “presence” (5.35 ± 1.14). The results of the research implemented during the period of severe psychological pressure associated with the COVID-19 pandemic provided information on symptoms of traumatic stress in the examined group of nurses. Their sense of security has been lowered and accompanied by an intensified reflection on issues concerning security. However, their current sense of meaning in life remains higher than the tendency to searching for it. The surveyed nurses received individual support mostly from significant others (i.e., other than family and friends). They see positive changes resulting from painful experiences related to the COVID-19 pandemic, which can be characterized by adaptation in the form of post-traumatic growth
In situ simulation training in First Aid. Pilot study. First aid in a dangerous workplace
Abstract Purpose The aim of the study was to evaluate the newly develop course prepared for the employees working in the forest and mountain environmentMethodology 31 people participated in the course. They were employees of Roztocze National Park and the Forestry Commission Lutowiska. A diagnostic survey was implemented in a form of a questionnaire. The respondents were provided with two original questionnaires and a telephone survey. Surveys were anonymous and voluntary.Findings The average assessment of First Aid knowledge before the workshop was 2.48 and majority of participants assesed First Aid as difficult. After the workshop, the respondents assessed the knowledge on average as 3.87 and as much as 58% declared that definitely would provide First Aid to a stranger; 81% to a close person. Over 80% of respondents noticed the need of regular training in First Aid.Research implication The analysis showed that regular improvement of First Aid skills is required by the participants. The training should be adjusted to the group’s needs in terms of the program, teaching techniques and the place of training. There is a need to implement such training on a wider scale among forestry and mountain workers.OriginalityUncovering the gaps in First Aid training in mountain and forest workers in their professional training
Current Response and Management Decisions of the European Union to the COVID-19 Outbreak: A Review
COVID-19 has proven to be a formidable challenge for many countries in the European Union to manage effectively. The European Union has implemented numerous strategies to face emerging issues. Member States have adopted measures such as the closure of borders and significant limitations on the mobility of people to mitigate the spread of the virus. An unprecedented crisis coordination effort between Member States has facilitated the ability to purchase equipment, personal protective equipment, and other medical supplies. Attention has also been focused on providing substantive money for research to find a vaccine and promote effective treatment therapies. Financial support has been made available to protect worker salaries and businesses to help facilitate a return to a functional economy. Lessons learned to date from COVID-19 in the European Union are many; the current crisis highlights the need to think about future pandemics from a population-based management approach and apply outside the box critical thinking. Due to the complexity, intensity, and frequency of complex disasters, global leaders in healthcare, government, and business will need to pivot from siloed approaches to decision-making to embrace multidisciplinary and transdisciplinary levels of cooperation. This cooperation requires courage and leadership to recognize that changes are necessary to avoid making the same mistakes we have planned countless times on avoiding. This study focuses on the European Union’s initial response to the COVID-19 pandemic, starting with how the European Union first learned and processed the global information arising out of China, followed by the incremental population-based medicine/management decisions made that currently are defining the European Union’s capacity and capability. The capacity to organize, deliver, and monitor care to a specific clinical population under a population-based management target includes strict social distancing strategies, contact testing and tracing, testing for the virus antigen and its antibodies, isolation, and treatment modalities such as new mitigating medications, and finally, a vaccine
Brucella – Virulence Factors, Pathogenesis and Treatment
Brucellae are Gram-negative, small rods infecting mammals and capable of causing disease called brucellosis. The infection results in abortion and sterility in domestic animals (sheeps, pigs, rams etc). Especially dangerous for humans are: Brucella melitensis, Brucella suis, Brucella abortus, and Brucella canis that trigger unspecific symptoms (flu-like manifestation). Brucella rods are introduced via host cells, by inhalation, skin abrasions, ingestion or mucosal membranes. The most important feature of Brucella is the ability to survive and multiply within both phagocytic and non-phagocytic cells. Brucella does not produce classical virulence factors: exotoxin, cytolisins, exoenzymes, plasmids, fimbria, and drug resistant forms. Major virulence factors are: lipopolysaccharide (LPS), T4SS secretion system and BvrR/BvrS system, which allow interaction with host cell surface, formation of an early, late BCV (Brucella Containing Vacuole) and interaction with endoplasmic reticulum (ER) when the bacteria multiply. The treatment of brucellosis is based on two-drug therapy, the most common combinations of antibiotics are: doxycycline with rifampicin or fluoroquinolones with rifampicin. Reduction of Neuropathic and Inflammatory Pain through Inhibition of the Tetrahydrobiopterin Pathway
Human genetic studies have revealed an association between GTP cyclohydrolase 1 polymorphisms, which decrease tetrahydrobiopterin (BH4) levels, and reduced pain in patients. We now show that excessive BH4 is produced in mice by both axotom- ized sensory neurons and macrophages infiltrating damaged nerves and inflamed tissue. Constitutive BH4 overproduction in sensory neurons increases pain sensitivity, whereas blocking BH4 production only in these cells reduces nerve injury-induced hy- persensitivity without affecting nociceptive pain. To minimize risk of side effects, we targeted sepiapterin reductase (SPR), whose blockade allows minimal BH4 production through the BH4 salvage pathways. Using a structure-based design, we developed a potent SPR inhibitor and show that it reduces pain hypersensitivity effectively with a concomitant decrease in BH4 levels in target tissues, acting both on sensory neurons and macrophages, with no development of tolerance or adverse effects. Finally, we demonstrate that sepiapterin accumulation is a sensitive biomarker for SPR inhibition in vivo
