    Patient allocations in general practice in case of patients' preferences for gender of doctor and their unavailability

    <p>Abstract</p> <p>Background</p> <p>In some countries every citizen has the right to obtain a designated general practitioner. However, each individual may have preferences that cannot be fulfilled due to shortages of some kind. The questions raised in this paper are: To what extent can we expect that preferences are fulfilled when the patients "compete" for entry on the lists of practitioners? What changes can we expect under changing conditions? A particular issue explored in the paper is when the majority of women prefer a female doctor and there is a shortage of female doctors.</p> <p>Findings</p> <p>The analysis is done on the macro level by the so called gravity model and on the micro level by recent theories of benefit efficient population behaviour, partly developed by two of the authors. A major finding is that the number of patients wanting a doctor of the underrepresented gender is less important than the strength of their preferences as determining factor for the benefit efficient allocation.</p> <p>Conclusions</p> <p>We were able to generate valuable insights to the questions asked and to the dynamics of benefit efficient allocations. The approach is quite general and can be applied in a variety of contexts.</p

    Female labour market outcomes and the impact of maternity leave policies

    ABSTRACT: This paper shows how family policies aimed at reconciling the pressures of family and work generate substantial variation in labour market outcomes across developed countries. We use a life-cycle model of female labour supply and savings behaviour, calibrated to the US economy, to assess the effect of introducing to the US a maternity leave policy similar to Scandinavian-type policies. We focus on the impact on gender differences in participation and in wages. We distinguish between the effect of the job protection offered by maternity leave and the effect of income replacement. Job protection leads to substantial increases in participation of mothers with children under 6, but with little long term effects. The effects on wages are minimal, with negative selection effects offsetting the reduced human capital depreciation.     What’s a (Childless) Man Without a Woman? The Differential Importance of Couple Dynamics for the Wellbeing of Childless Men and Women in the Netherlands

    Using rich couple data from the Netherlands Kinship Panel Study, we investigated to what extent there were gender differences in couple dynamics within childless couples (N = 163). Though the childless partners reported similar relationship satisfaction, we found gender differences in the link between relationship conflict and relationship satisfaction - the childless men were more strongly affected by the negative aspects of the partnership. This gender difference was not evident for the association between partner support and relationship satisfaction - the positive aspects of the partnership were equally important for the male and the female childless partners. Furthermore, the association between relationship satisfaction and health was stronger for the childless men than for the childless women and this difference was particularly evident when the levels of relationship satisfaction were low. These results indicate that when they are in unsatisfying romantic relationships, childless men are at a greater risk than childless women of physical and mental ill health

    Parental leave benefit and differential fertility responses: evidence from a German reform

    This paper examines the causal effects of a major change in the German parental leave benefits on fertility. I use the unanticipated reform of 2007 to assess how a move from a means-tested to an earnings-related benefit affects higher-order births. By using data from the Mikrozensus, I find that the reform significantly affected the timing of higher-order births. Overall, mothers “just” affected by the reform initially reduce subsequent childbearing and start to compensate by the end of the third year. The negative effects are largely driven by lowest-income mothers, who are now worse-off and do not display any catch-up effects
