    Role of the community matron in advance care planning and ‘do not attempt CPR’ decision-making: a qualitative study

    The community matron (CM) is often the key worker caring for patients with chronic, life-limiting, long-term conditions, but these patients are not always recognised as palliative cases. This study explored the experiences of CMs with regard to advance care planning (ACP) and ‘do not attempt cardiopulmonary resuscitation’ (DNACPR) decision-making to understand whether or not they felt adequately prepared for this aspect of their role, and why. Qualitative data were generated from six CMs using a broad interpretive phenomenological approach. Face-to-face recorded interviews were analysed using template analysis. The study found that although participants faced complex ethical situations around ACP and DNACPR almost on a daily basis, none had received any formal training despite the emphasis on training in national and local guidelines. Participants often struggled to get their patients accepted on to the Gold Standards Framework. The research found variability and complexity of cases to be the main barriers to clear identification of the palliative phase

    Carbon isotopes in bulk carbonaceous chondrites

    The chemical and physical processes involved in the formation of the solar system are examined. Primitive matter has been found on a microscopic scale in a variety of meteorites: fragments of small solar system bodies that were never part of a large planet. This primitive matter has, in most cases, been identified by the presence of anomalous abundances of some isotopes of the chemical elements. Of particular interest for carbon isotope studies are the primitive meteorites known as carbonaceous chondrites. Using a selective oxidation technique to sort out the carbon contained in different chemical forms (graphite, carbonates, and organic matter), four carbonaceous chondrites are analyzed. The presence of the (13) C-rich component was confirmed and additional carbon components with different, but characteristic, isotopic signatures were resolved

    Oxygen isotopic compositions of the high-temperatue phases in carbonaceous chondrites define a mixing line with an O-16 rich component and show little superimposed chemical isotope fractionation. Within a single inclusion in Allende, variations of delta O-18 and delta O-17 of 39% are found. The ordinary chondrites are slightly displaced from the terrestrial fractionation trend, implying that at least 0.2% of the oxygen in terrestrial rocks was derived from the O-16 rich component

    The Flux Ratio Method for Determining the Dust Attenuation of Starburst Galaxies

    The presence of dust in starburst galaxies complicates the study of their stellar populations as the dust's effects are similar to those associated with changes in the galaxies' stellar age and metallicity. This degeneracy can be overcome for starburst galaxies if UV/optical/near-infrared observations are combined with far-infrared observations. We present the calibration of the flux ratio method for calculating the dust attenuation at a particular wavelength, Att(\lambda), based on the measurement of F(IR)/F(\lambda) flux ratio. Our calibration is based on spectral energy distributions (SEDs) from the PEGASE stellar evolutionary synthesis model and the effects of dust (absorption and scattering) as calculated from our Monte Carlo radiative transfer model. We tested the attenuations predicted from this method for the Balmer emission lines of a sample starburst galaxies against those calculated using radio observations and found good agreement. The UV attenuation curves for a handful of starburst galaxies were calculated using the flux ratio method, and they compare favorably with past work. The relationship between Att(\lambda) and F(IR)/F(\lambda) is almost completely independent of the assumed dust properties (grain type, distribution, and clumpiness). For the UV, the relationship is also independent of the assumed stellar properties (age, metallicity, etc) accept for the case of very old burst populations.     Electron Positron Annihilation Radiation from SgrA East at the Galactic Center

    Maps of the Galactic electron-positron annihilation radiation show evidence for three distinct and significant features: (1) a central bulge source, (2) emission in the Galactic plane, and (3) an enhancement of emission at positive latitudes above the Galactic Center. In this paper, we explore the possibility that Sgr A East, a very prominent radio structure surrounding the Galactic nucleus, may be a significant contributer to the central bulge feature. The motivation for doing so stems from a recently proposed link between this radio object and the EGRET gamma-ray source 2EG J1746-2852. If this association is correct, then Sgr A East is also expected to be a source of copious positron production. The results presented here show that indeed Sgr A East must have produced a numerically significant population of positrons, but also that most of them have not yet had sufficient time to thermalize and annihilate. As such, Sgr A East by itself does not appear to be the dominant current source of annihilation radiation, but it will be when the positrons have cooled sufficiently and they have become thermalized. This raises the interesting possibility that the bulge component may be due to the relics of earlier explosive events like the one that produced Sgr A East.Comment: This manuscript was prepared with the AAS Latex macros v4.0 It is 37 pages long and has 16 figure

    Classificational parameters for acapulcoites and lodranites: The cases of FRO 90011, EET 84302 and ALH A81187/84190

    Acapulcoites and lodranites probably sample a common parent body, which has experienced a range of partial melting. We present classificational parameters which allow acapulcoites-lodranites to be distinguished from other groups of meteorites, as well as from each other. Petrography can complement oxygen isotopic compositions in separating these meteorites from other groups of stony-irons and primitive achondrites, while petrographic properties alone distinguish acapulcoites from lodranites. Acapulcoites differ from lodranites in having smaller grain sizes, abundant Fe, Ni-FeS as micron-sized veins and plagioclase which escaped melting. We have applied these criteria to three new members of the group. FRO 90011 is a typical lodranite; EET 84302 is intermediate in many properties between acapulcoites and lodranites; and ALH A81187/84190 are paired meteorites and are first low-FeO acapulcoites. These meteorites provide a wider spectrum of samples from the acapulcoite-lodranite parent body and suggests that this body may have had a complex structure

    Molecular epidemiology of human rhinovirus infections in Kilifi, coastal Kenya

    This study reports pediatric surveillance over 3 years for human rhinovirus (HRV) at the District Hospital of Kilifi, coastal Kenya. Nasopharyngeal samples were collected from children presenting at outpatient clinic with no signs of acute respiratory infection, or with signs of upper respiratory tract infection, and from children admitted to the hospital with lower respiratory tract infection. Samples were screened by real-time reverse transcriptase polymerase chain reaction (real-time RT-PCR) and classified further to species by nucleotide sequencing of the VP4/VP2 junction. Of 441 HRV positives by real-time RT-PCR, 332 were classified to species, with 47% (155) being HRV-A, 5% (18) HRV-B, and 48% (159) HRV-C. There was no clear seasonal pattern of occurrence for any species. The species were present in similar proportions in the inpatient and outpatient sample sets, and no significant association between species distribution and the severity of lower respiratory tract infection in the inpatients could be determined. HRV sequence analysis revealed multiple but separate clusters in circulation particularly for HRV-A and HRV-C. Most HRV-C clusters were distinct from reference sequences downloaded from GenBank. In contrast, most HRV-A and HRV-B sequences clustered with either known serotypes or strains from elsewhere within Africa and other regions of the world. This first molecular epidemiological study of HRV in the region defines species distribution in accord with reports from elsewhere in the world, shows considerable strain diversity and does not identify an association between any species and disease severity
