    Response to intervention : establishing effective introductory training

    The purpose of this project was to develop an introductory Response to Intervention training module that teachers, administrators, and instructional aides can understand and that will help them prepare for possible Response to Intervention implementation. Intended training participants have or work with general education and special education students who are in need of classroom and school wide interventions. Subjects for the pilot study were California State University Monterey Bay, Level 2- Education Specialist credential candidates. Data was also gathered from experts in the education field such as; teachers familiar with RTI, university professors, researchers, and PhDs to determine if the training was complete in content and format. Pilot results indicated that the training was complete but overwhelming. Expert results showed that the responses were positive and there was no mention of excluding components of the training. Experts did not suggest that the training was overwhelming. Some additions were suggested that provide opportunity for future research

    2,3-butanediol in experimental myocardial ischaemia in pigs

    To investigate the role of 2,3-butanediol in myocardial ischaemia we analysed this compound in pig's myocardium and blood. Ischaemia was induced by ligation of a coronary artery. In the first study we found significantly higher levels of 2,3-butanediol in the homogenate of ischaemic myocardium than in non-ischaemic myocardium. The lactate concentration was also significantly elevated. In the second study, where ischaemia was similarly induced, and where reperfusion was achieved by re-opening the ligated coronary artery after 20 min, 2,3-butanediol in peripheral blood was found to increase significantly. In the pigs in which the coronary artery was not re-opened, the 2,3-butanediol level in peripheral blood was unchanged. We conclude that in pigs' anaerobic myocardia accumulation of 2,3-butanediol occurs; if the myocardium is reperfused this metabolite also appears in the bloo

    Use of telehealth and outcomes before a visit to the emergency department: a cross-sectional study on walk-in patients in Switzerland

    AIMS OF THE STUDY An increasing number of patients are using telehealth before contacting the healthcare system. If we are to optimise future telehealth strategies and adequately respond to patient needs, we need to know more about the frequency and characteristics of telehealth use. Our objectives were (i) to investigate whether patients use telehealth before consulting the emergency department (ED), (ii) to compare patients with and without use of telehealth, and (iii) to investigate adherence, confidence and satisfaction. MATERIALS AND METHODS A survey was conducted among ED walk-in patients at a tertiary university hospital in Switzerland. Eligible patients were questioned about their use of telehealth before current presentation, during 30 shifts from 23 October to 15 December 2019. RESULTS A total of 183 (43.9%) of 417 surveyed patients used telehealth, with the telephone being the most commonly used modality, especially among elderly patients. Patients using telehealth were more likely to be male (53.5%, p = 0.001) and were similar in age to non-users. Telehealth users tended to be better educated. Telehealth was predominantly used for semi-urgent non-traumatic diseases that did not lead to hospitalisation. All age groups expressed satisfaction with telehealth, trusted the recommendations and adhered to them. The main reason for "non-use" of telehealth was lack of knowledge. CONCLUSIONS Lack of knowledge about telehealth opportunities and barriers in specific patient groups should be addressed to include all patients, and thus to exploit telehealth’s many advantages

Most profile measurement techniques cannot correctly characterize fibers with small or irregular refractive-index variations over their cross sections. Microinterferometric optical phase tomography (MIOPT) is a technique that allows measurement of fiber refractive-index profiles exhibiting such variations. We present the first demonstration, to our knowledge, of MIOPT. The profile of a polarization-maintaining fiber is measured by MIOPT and shown to be in agreement with (destructive) fiber end-face measurements. MIOPT is also applied to the limiting case of a symmetric single-mode fiber

    Telehealth use by walk-in patients who subsequently visited an academic emergency department: a repeated cross-sectional study during the COVID-19 summer 2021.

    AIM OF THE STUDY During the COVID-19 pandemic, telehealth use increased worldwide in a variety of medical specialities and reached new population groups. A baseline survey of telehealth use prior to admission to the emergency department (ED) conducted before COVID-19 concluded that predominantly well-educated men used telehealth. It is unclear how COVID-19 changed the use of telehealth in Swiss emergency patients. We therefore aimed to investigate (i) the frequency of telehealth use during the pandemic, and (ii) how the pandemic has influenced telehealth use and users. MATERIALS AND METHODS A repeated cross-sectional study was conducted among ED walk-in patients at a tertiary university hospital in Switzerland. The study took place one and a half years after the first confirmed COVID-19 case, during 30 shifts from 8 to 29 July 2021 and compared with the baseline survey conducted in 2019. Eligible patients were questioned about their use of, and attitudes to telehealth. RESULTS A total of 1020 patients were screened for the COVID survey and 443 complete questionnaires were evaluated. A trend towards a general increase (+6.4%) in telehealth use was demonstrated (50.3%, n = 223 COVID survey vs 43.9%, n = 183 baseline survey; p = 0.058), with a shift to more female patients using telehealth in the COVID survey (female 54.9%, n = 124 vs 45.1%, n = 102; p = 0.052). During the pandemic, first use of telehealth was reported by 12.2% (n = 54) of patients, with a significant increase among patients with low educational status, and the latter patients often indicated that they did not plan to use telehealth after the pandemic. The perceived usefulness of telehealth and adherence to recommendations increased in the COVID survey compared with the baseline survey (adherence 90.3%, n = 149, vs 78.0%, n = 131; p = 0.002). CONCLUSION We found a trend towards increased use of telehealth among Swiss ED patients. First-time users of telehealth were predominantly less educated and inclusion of these user groups may not be sustainable, as was indicated by the patients. COVID-19 led to greater adherence to telehealth recommendations and higher perceived usefulness. This could be due to the limited access to healthcare providers due to pandemic precautions. When offering telehealth, the needs of all patient groups must be considered, in order to ensure that telehealth provides the greatest benefit with lower barriers to use

    Medical Treatment of Radiological Casualties: Current Concepts

    The threat of radiologic or nuclear terrorism is increasing, yet many physicians are unfamiliar with basic treatment principles for radiologic casualties. Patients may present for care after a covert radiation exposure, requiring an elevated level of suspicion by the physician. Traditional medical and surgical triage criteria should always take precedence over radiation exposure management or decontamination. External contamination from a radioactive cloud is easily evaluated using a simple Geiger-MĂŒller counter and decontamination accomplished by prompt removal of clothing and traditional showering. Management of surgical conditions in the presence of persistent radioactive contamination should be dealt with in a conventional manner with health physics guidance. To be most effective in the medical management of a terrorist event involving high-level radiation, physicians should understand basic manifestations of the acute radiation syndrome, the available medical countermeasures, and the psychosocial implications of radiation incidents. Health policy considerations include stockpiling strategies, effective use of risk communications, and decisionmaking for shelter-in-place versus evacuation after a radiologic incident

    The "Multimat" experiment at CERN HiRadMat facility: advanced testing of novel materials and instrumentation for HL-LHC collimators

    The increase of the stored beam energy in future particle accelerators, such as the HL-LHC and the FCC, calls for a radical upgrade in the design, materials and instrumentation of Beam Intercepting Devices (BID), such as collimators Following successful tests in 2015 that validated new composite materials and a novel jaw design conceived for the HL-LHC collimators, a new HiRadMat experiment, named “HRMT36-MultiMat”, is scheduled for autumn 2017. Its objective is to determine the behaviour under high intensity proton beams of a broad range of materials relevant for collimators and beam intercepting devices, thin-film coatings and advanced equipment. The test bench features 16 separate target stations, each hosting various specimens, allowing the exploration of complex phenomena such as dynamic strength, internal damping, nonlinearities due to anisotropic inelasticity and inhomogeneity, effects of energy deposition and radiation on coatings. This paper details the main technical solutions and engineering calculations for the design of the test bench and of the specimens, the candidate target materials and the instrumentation system

    The development of a business intelligence web application to support the decision-making process regarding absenteeism in the workplace

    Nowadays, one of the biggest concerns of industries all over the world is situations regarding absenteeism, since it has a great impact on the productivity and economy of companies, as well as on the health of their employees. The major causes of absenteeism appear to be work accidents and sickness leaves, which lead to the attempt by companies of understanding how the workload is related to the health of their collaborators and, consequently, to absenteeism. Thus, this paper proposes the design and development of a Web Application based on Business Intelligence indicators in order to help the health and human resources professionals of a Portuguese company analyse the relation between absenteeism and the health and lifestyle of employees, with the intention of concluding whether the work executed on the company is harming workers’ health. Furthermore, it is intended to discover the principal motives for the numerous and more frequent absences in this company, so that it is possible to decrease the absenteeism rate and, hence, improve the decision-making process.     The rate of X-ray-induced DNA double-strand break repair in the embryonic mouse brain is unaff ected by exposure to 50 Hz magnetic fi elds

    Following in utero exposure to low dose radiation (10 – 200 mGy), we recently observed a linear induction of DNA double-strand breaks (DSB) and activation of apoptosis in the embryonic neuronal stem/progenitor cell compartment. No signifi cant induction of DSB or apoptosis was observed following exposure to magnetic fi elds (MF). In the present study, we exploited this in vivo system to examine whether exposure to MF before and after exposure to 100 mGy X-rays impacts upon DSB repair rates. Materials and methods : 53BP1 foci were quantifi ed following combined exposure to radiation and MF in the embryonic neuronal stem/progenitor cell compartment. Embryos were exposed in utero to 50 Hz MF at 300 m T for 3 h before and up to 9 h after exposure to 100 mGy X-rays. Controls included embryos exposed to MF or X-rays alone plus sham exposures. Results : Exposure to MF before and after 100 mGy X-rays did not impact upon the rate of DSB repair in the embryonic neuronal stem cell compartment compared to repair rates following radiation exposure alone. Conclusions : We conclude that in this sensitive system MF do not exert any signifi cant level of DNA damage and do not impede the repair of X-ray induced damage
