    Mathematical modelling for solar cell, panel and array for photovoltaic system

    Renewable energy is considered as next alternative to fossil fuels and nowadays, it attracts much attention in agriculture and environmental protection. Application of solar photovoltaic system is drying and dehydration of products, heating, irrigation, greenhouse and power generation etc. Temperature and sun radiation varies nonlinearly. Power generation varies with reference to radiation and temperature in photo-voltaic (PV) system. PV characteristic is nonlinear and  PV cell is the basic unit for electricity generation. To get the characteristic response of PV, it aimed to develop a solar cell/panel model and array on a platform like MATLAB. In this research paper, step by step procedure has been defined for modelling solar cell, panel, and array models of the photovoltaic system. Kyocera solar KC-200GT 200W solar panel is used as a reference model for further modelling. The PV array characteristic are simulated for different irradiance(200W/m2,400 W/m2 ,600 W/m2 ,800W/m2 ,1000W/m2)and temperature variation(25°C, 35°C, 45°C, 55°C, 75°C). The output characteristic of the reference model matches with simulated results. The output reduced when the solar irradiation reduced from 1000 to 200 W/m2. As the temperature increased, the output voltage decreases, whereas the output current increases slightly. This model would be useful for investigating the effect of different parameters like series resistance, shunt resistance, thermal voltage, solar cell temperature coefficient of short circuit current etc. It would also be useful for investigating the working parameters like temperature & radiation condition and different series and parallel combinations of panels. This modelling is useful in investigating the performance of solar arrays in different applications of solar power generation, as well as modelling provides a major role in the mounting of PV panels.

    Providers’ Perception of Alert Fatigue After Implementation of User-Filtered Warnings

    Alert fatigue is a complex problem that many health institutions face when using an electronic health record (EHR). The addition of user-filtered warnings (UFW) is a physicians’ proposed intervention at Inova Health System (IHS), a large 5-hospital health system in Northern Virginia, that allows prescribers to filter out specific drug-drug interactions and pregnancy and lactation medication alerts for a 30-day period. This study aims to determine the impact of UFW on physicians’ perception of alert fatigue and to calculate the reduction of medication alerts. It was hypothesized that the reduction in alerts will significantly impact physicians’ perception of alert fatigue in a positive manner. Physician perception of alert fatigue was assessed using online surveys before and after the implementation of UFW. Data from Medications Warnings Statistics reports were used to assess the reduction of alerts fired post-implementation of UFW. For the primary outcome, there was no significant difference in the overall perception of alert fatigue before and after the implementation of UFW. For the secondary outcome, the number of medication alerts was decreased by 16.7% post UFW implementation. Overall, the data does not support UFW to reduce alert fatigue

    Feto-maternal outcome in pregnancy beyond 40 weeks

    Background: Fetal, Neonatal and Maternal complications associated with pregnancy beyond 40 weeks have always been underestimated. However emerging evidence demonstrates that the incidence of complications increases after 40 weeks of gestation. The present study conducted to find out the fetomaternal outcome of such prolonged pregnancy.Methods: A prospective cross sectional study of 84 patients with uncomplicated prolonged pregnancy fulfilling the inclusion and exclusion criteria and admitted in department of obstetrics and gynecology at a tertiary care hospital. The aim of the study was to know fetomaternal outcome in pregnancy beyond 40 weeks in consideration of spontaneous and induced labour.Results: Out of 84 patients, 58 (69.05%) were in the age group of 20-30 years, 44 (52.38%) were between 41-42 weeks of gestation according to their LMP and 38(45.24%) were between 40-41 weeks of gestation. In 27 (32.14%) patients mode of delivery was caesarean section, in whom most common indication being fetal distress in 48.15% followed by failure to progress in 22.22%. In present study perinatal morbidity like IUFD, neonatal asphyxia, MAS, RDS were 4.76%, 9.52%, 7.14% and 3.57% respectively. Maternal morbidity like prolonged labor, PPH, fever, wound infection were 10.71%, 5.95%, 3.57% and 3.57% respectively.Conclusions: With Regular antenatal check-up, incidence of postdate pregnancy can be decreased and it is important because of definite risk to fetus as pregnancy continuing beyond 40 weeks of gestation is associated with increased perinatal morbidity and mortality especially those who do not come for regular antenatal check-up. Confirmation of diagnosis of exact term of pregnancy is very important as many patients don’t have regular menstrual history and LMP. Diagnosis can be confirmed by first trimester ultrasound which is most important non-invasive method and readily available

    Amiodarone Toxicity: A Case Report

    Amiodarone is one of the commonly used drug in arrhythmias. It is first line of treatment for ventricular tachycardia particularly in treatment of supraventicular tachycardia like atrial fibrillation. Amiodarone has a wide range of adverse effects ranging from endocrine to cardiac system. A case of raised thyroid stimulating Hormone (TSH)  with amiodarone was reported in our Adverse drug  monitoring centre (AMC), in which several other adverse events such as raised raised billirubin and hyponatremia were present. Patient was treated with thyroxine while amiodarone along with atorvastatin and metoprolol were withdrawn. We present a case who developed subclinical hypothyroidism, electrolyte imbalance and liver dysfunction with amiodarone therapy

    Venoarterial PCO2 difference: a marker of postoperative cardiac output in children with congenital heart disease

    OBJECTIVE: To determine the relationship between venoarterial carbon dioxide gradient (DeltapCO2) and central venous oxygen saturation (ScvO2) in children after cardiac surgery. STUDY DESIGN: A cohort study. PLACE AND DURATION OF STUDY: The Paediatric cardiac intensive care unit of the Aga Khan University Hospital, Karachi, from June 2006 to May 2007. METHODOLOGY: All children admitted in the paediatric cardiac intensive care after complete repair of congenital heart defect using cardiopulmonary bypass were included in the study. Simultaneous arterial and central venous blood gas samples were obtained from a catheter placed in the artery (either radial or femoral) and superior vena cava respectively. Linear regression analysis was performed between ScvO2 and DeltapCO2. RESULTS: Fifty seven children aged from 5 days to 14 years were included and 272-paired simultaneous arterial and central venous samples were analyzed. Mean venous pCO2 was 47.82+/-9.03 mmHg and mean arterial pCO2 was 40.50+/-9.06 mmHg. One hundred seventy four samples had ScvO2 \u3e 70% with mean DeltapCO2 of 5.44+/-2.55 mmHg and 98 samples had ScvO2 \u3c 70% with mean DeltapCO2 of 9.07+/-3.90 mmHg. With ScvO2 \u3c 70%, 77 samples had DeltapCO2 of \u3e 6 mmHg while only 21 samples had DeltapCO2 of \u3c 6 mmHg (p \u3c 0.001). On the contrary with ScvO2 \u3e 70%, 71 samples had DeltapCO2 of \u3e 6 mmHg and 103 samples had DeltapCO2 of \u3c 6 mmHg. Coefficient of correlation (R2) between 0.340 was ScvO2 and DeltapCO2. CONCLUSION: Elevated DeltapCO2 is practical and can be utilized as a useful adjunct to low ScvO2 in the assessment of low cardiac output syndrome in children after cardiac surgery

    MAPS-SNMA ShaD.O.w D.O. Day Rowan SOM Pipeline Program: A Pilot Study

    Prior to being a medical student, a majority of pre-medical students have not encountered the course load intensity, academic rigor, and time management required by a medical school curriculum. Diversifying the workforce has been shown to have benefits, especially at the micro or patient level, since having racial or ethnic concordance between providers and patients has been associated with increased levels of satisfaction and improvements in certain clinical outcomes. The number of MDs increased to 841,321 or 91.8% versus 72,961 DOs or 8.0% creating the need for minorities to be knowledgeable of both MD and DO paths to becoming a physician. To provide more knowledge of osteopathic medicine and osteopathic physicians, the Minority Association of Premedical Students (M.A.P.S.) and the the Student National Medical Association (S.N.M.A.) of Rowan University School of Osteopathic Medicine have implemented a ShaDOw DO Day program for the graduate students of Rowan University Graduate School of Biomedical Sciences. The ShaDOw DO Day program will give MAPS students the opportunity to witness the resilience, discipline, and motivation that successful medical students possess.This program will also provide MAPS students with the opportunity to explore a potential career in medicine. Students will gain a perspective on osteopathic principles and osteopathic manipulative medicine. Participants of the program will also be introduced to the quality education an osteopathic medical school such as RowanSOM instills in its students to produce competent and compassionate physicians. Therefore, it is imperative that graduate students learn from medical students to increase their knowledge of osteopathic medicine. To analyze the effectiveness of this program, pre-assessment and post-assessment surveys were given to the participants. Overall, an increase in the understanding of osteopathic medicine and consideration of a career in a health profession and/or medicine was observed demonstrating both the need and success of our program initiative

    Beyond the horizon, backhaul connectivity for offshore IoT devices

    The prevalent use of the Internet of Things (IoT) devices over the Sea, such as, on oil and gas platforms, cargo, and cruise ships, requires high-speed connectivity of these devices. Although satellite based backhaul links provide vast coverage, but they are inherently constrained by low data rates and expensive bandwidth. If a signal propagated over the sea is trapped between the sea surface and the Evaporation Duct (ED) layer, it can propagate beyond the horizon, achieving long-range backhaul connectivity with minimal attenuation. This paper presents experimental measurements and simulations conducted in the Industrial, Scientific, and Medical (ISM) Band Wi-Fi frequencies, such as 5.8 GHz to provide hassle-free offshore wireless backhaul connectivity for IoT devices over the South China Sea in the Malaysian region. Real-time experimental measurements are recorded for 10 km to 80 km path lengths to determine average path loss values. The fade margin calculation for ED must accommodate additional slow fading on top of average path loss with respect to time and climate-induced ED height variations to ensure reliable communication links for IoT devices. Experimental results confirm that 99% link availability of is achievable with minimum 50 Mbps data rate and up to 60 km distance over the Sea to connect offshore IoT devices

    Bioactive Lipids and Circulating Progenitor Cells in Patients with Cardiovascular Disease

    Bone marrow-derived progenitor cells are mobilized into the peripheral blood after acute myocardial injury and in chronic ischemic heart disease. However, the mechanisms responsible for this mobilization are poorly understood. We examined the relationship between plasma levels of bioactive lipids and number of circulating progenitor cells (CPCs) in patients (N = 437) undergoing elective or emergent cardiac catheterization. Plasma levels of sphingosine-1 phosphate (S1P) and ceramide-1 phosphate (C1P) were quantified using mass spectrometry. CPCs were assessed using flow cytometry. S1P levels correlated with the numbers of CD34+, CD34+/CD133+, and CD34+/CXCR4+ CPCs even after adjustment for potential confounding factors. However, no significant correlation was observed between C1P levels and CPC count. Plasma levels of S1P correlated with the number of CPCs in patients with coronary artery disease, suggesting an important mechanistic role for S1P in stem cell mobilization. The therapeutic effects of adjunctive S1P therapy to mobilize endogenous stem cells need to be investigated

    Workplace mistreatment and mental health in female surgeons in Pakistan

    Background: Despite workplace mistreatment, which includes harassment, bullying and gender discrimination(GD)/bias, being serious problems for female surgeons, there are limited data from lower-middle-income countries like Pakistan. This study explored harassment and GD/bias experienced by female surgeons in Pakistan, and the effects of these experiences on mental health and well-being.Methods: A nationwide survey was conducted between July and September 2019 in collaboration with the Association of Women Surgeons of Pakistan, an organization consisting of female surgeons and trainees in Pakistan. An anonymous online survey was emailed directly, disseminated via social media platforms (such as Facebook, Twitter and Instagram), and sent to surgical programmes in Pakistan.Results: A total of 146 women surgeons responded to the survey; 67.1 per cent were trainees and the rest attending surgeons. Overall, 57.5 per cent of surgeons reported experiencing harassment, most common being verbal (64.0 per cent) and mental (45.9 per cent), but this mostly went unreported (91.5 per cent). On multivariable analysis adjusted for age and specialty, workplace harassment (odds ratio 2.02 (95 per cent c.i. 1.09 to 4.45)) and bullying (odds ratio 5.14 (95 per cent c.i. 2.00-13.17)) were significantly associated with severe self-perceived burnout, while having a support system was protective against feelings of depression (odds ratio 0.35 (95 per cent c.i. 0.16 to 0.74)). The overwhelming majority (91.3 per cent) believed that more institutional support groups were needed to help surgeons with stress reduction (78.8 per cent), receiving mentorship (74.7 per cent) and work-life balance (67.8 per cent).Conclusion: Workplace mistreatment, in particular harassment and bullying, has a damaging impact on the mental well-being of female surgeons, particularly trainees. The absence of support groups in Pakistan should be urgently addressed so that surgeons, especially trainees, may cope better with potentially harmful workplace stressors
