    EFFECT OF WEIGHT LOSS ON INFLAMMATORY MARKERS IN SEVERELY OBESE ADULTS

    Introduction: The role that inflammation plays in the atherosclerotic disease process is well established. Obesity is coupled with a state of chronic inflammation and is associated with increased circulating inflammatory markers including C-Reactive Protein. Purpose: The purpose of this study was to evaluate the additive effect of aerobic or resistance exercise training to caloric restriction for weight loss on high sensitivity C-Reactive Protein changes compared to dietary restriction alone in class II and class III obese individuals. Methods: 24 healthy, sedentary, obese women underwent a 12-week diet and exercise intervention: Caloric restriction weight loss program with no exercise (DIET); Caloric restriction with aerobic endurance training (DIET+AT); and Caloric restriction with resistance training (DIET+RT). Blood was drawn at baseline and 12-weeks and assayed for hs-CRP. Results: Weight was decreased significantly in all groups in the intervention. Hs-CRP was unchanged at 12-weeks.Conclusions: In conclusion, this investigation was successful in producing weight loss, BMI decreases, decreases in body fat percentage, and positive changes in fitness markers, though no changes in hs-CRP were associated with weight loss or weight loss with exercise. Further investigations into the influence of weight loss and exercise on CRP levels among Class II and Class III individuals should be completed to examine and expand upon the results observed in this study

    Stop, Question, and Frisk: A Tool of Racial Control in New York City

    Broken Windows policing through the utilization of Stop, Question, and Frisk has been widely used by the New York City Police Department (NYPD) since the 1990s, as guaranteed by landmark Supreme Court Case Terry v. Ohio (1968). As a result, hundreds of minority citizens have been the victim of routine stops for minor offenses through this aggressive police tactic. This study utilizes 2017 NYPD Stop, Question, and Frisk Data to determine whether broken windows policing, through stop, question, and frisk, operates as a mode of racial control for African Americans in New York City. Through the utilization of chi-square analyses, binary logistic regression, and multi nominal logistic regression, statistically significant associations were found for several variables based on race

    Rehabilitating antisocial personalities: treatment through self-governance strategies

    Offenders with antisocial personality disorder (ASPD) are widely assumed to reject psychotherapeutic intervention. Some commentators, therefore, argue that those with the disorder are better managed in the criminal justice system, where, following the introduction of indeterminate sentences, engagement with psychological treatment is coercively linked to the achievement of parole. By comparison, National Institute of Clinical Excellence guidelines on the management and treatment of ASPD recommend that those who are treatment seeking should be considered for admission to specialist psychiatric hospitals. The rationale is that prison-based interventions are underresourced, and the treatment of ASPD is underprioritised. The justification is that offenders with ASPD can be rehabilitated, if they are motivated. One problem, however, is that little is known about why offenders with ASPD seek treatment or what effect subsequent treatment has on their self-understanding. The aim of this paper is to address these unresolved issues. It draws on the findings of Economic and Social Research Council (ESRC) funded qualitative study examining the experiences of sentenced male offenders admitted to a specialist personality disorder ward within the medium secure estate and the medical practitioners who treat them. The data are analysed with reference to Michel Foucault’s work on governmentality and strategy in power relations. Two arguments are advanced: first, offenders with ASPD are motivated by legal coercive pressures to implement a variety of Foucauldian-type strategies to give the false impression of treatment progress. Second, and related, treatment does not result in changes in self-understanding in the resistive client with ASPD. This presupposes that, in respect of this group at least, Foucault was mistaken in his claim that resistive behaviours merely mask the effectiveness of treatment norms over time. Nevertheless, the paper concludes that specialist treatment in the hospital setting can effect changes in the resistive offender’s self-understanding, but not if the completion of treatment results, as is commonplace, in his prison readmission

We interrogate how professionals make sense and meaning of policy in the real world and how professional perspectives drive and shape their contributions to policy implementation nationally and locally. Contemporaneously, these analyses enable us to critically examine the caricatures, stereotypes, and assumptions that can (mis)inform common constructions, representations, and understandings of youth justice policy trajectories, including those relating to contextual stability, conceptual clarity, robust evidence bases, and purported foundations in stakeholder consensus

    Alternatives to custodial remand for women in the criminal justice system: a multi-sector approach

    Throughout the world, women involved in criminal justice systems often present with substantial needs and vulnerabilities. Diverting vulnerable people away from prison is government policy in England and Wales, but full psychiatric and social assessments are expensive and hard to access. A screening and quick response initiative ‐ alternatives to custodial remand for women (ACRW) ‐ was implemented across three areas of London (West, South and East) to supplement existing court liaison and diversion services, to assess the feasibility of a supplementary custodial remand service as part of a women's specialist service pathway in the criminal justice system in England. Three mental health trusts and two voluntary sector providers offered this service enhancement – a screening and service link provision in three London boroughs between 2012 and 2014. We conducted a service evaluation using routinely collected service use record data. The service made 809 contacts, of whom 104 had contact on multiple occasions. Many were identified as at risk of self‐harm (46%) or had histories of hospital admission for mental disorder (36%), but few were referred either to the liaison and diversion service or specialist mental health services. The largest group of referrals was to women's community services outside the health service (e.g. counselling, domestic violence or sexual abuse services). 180 women had dependent children and 22 were pregnant, increasing the urgency to find non‐custodial alternatives. As well as confirming high levels of need amongst women entering the criminal justice system, this evaluation confirms the feasibility of working across sectors in this field, providing an extra layer of service that can complement existing liaison and diversion service provision. The service was responsive and most women using it were kept out of custody. Research is now required to understand the appropriateness of the referrals, the extent to which women follow them through and the impact on their mental health and desistance from offending

    Darbepoetin alfa given every 1 or 2 weeks alleviates anaemia associated with cancer chemotherapy.

    In part A of this study, patients were randomised to cohorts receiving darbepoetin alfa at doses of 0.5 to 8.0 m.c.g x kg(-1) x wk(-1) or to a control group receiving epoetin alfa at an initial dose of 150 U x kg(-1) three times weekly. In part B, the cohorts were darbepoetin alfa 3.0 to 9.0 m.c.g x kg(-1) every 2 weeks or epoetin alfa, initial dose 40 000 U x wk(-1). Safety was assessed by adverse events, changes in blood pressure, and formation of antibodies to darbepoetin alfa. Efficacy was assessed by several haematologic endpoints, including change in haemoglobin from baseline. The adverse event profile of darbepoetin alfa was similar to that of epoetin alfa. No relationship between the rapidity of haemoglobin response and any adverse event was observed. No antibodies to darbepoetin alfa were detected. Higher doses of darbepoetin alfa increased the proportion of patients with a haemoglobin response and decreased the median time to response. The overall dose of darbepoetin alfa required to produce a mean increase in haemoglobin does not increase when the dosing interval is increased from 1 to 2 weeks. Therapy with darbepoetin alfa is safe and effective in producing a dose-related increase in haemoglobin levels in patients with cancer receiving chemotherapy

    Managing the Socially Marginalized: Attitudes Towards Welfare, Punishment and Race

    Welfare and incarceration policies have converged to form a system of governance over socially marginalized groups, particularly racial minorities. In both of these policy areas, rehabilitative and social support objectives have been replaced with a more punitive and restrictive system. The authors examine the convergence in individual-level attitudes concerning welfare and criminal punishment, using national survey data. The authors\u27 analysis indicates a statistically significant relationship between punitive attitudes toward welfare and punishment. Furthermore, accounting for the respondents\u27 racial attitudes explains the bivariate relationship between welfare and punishment. Thus, racial attitudes seemingly link support for punitive approaches to opposition to welfare expenditures. The authors discuss the implications of this study for welfare and crime control policies by way of the conclusion
