The Effect of Recombinant Bovine Somatotropin on Patterns of Milk Production, Lactational Milk Estimates and Net Farm Income
Bovine somatoropin (bST) alters total milk production and production patterns in dairy cows and understanding the economic benefits of bST for the dairy producer are critical. Holstein cows (n = 555) from four Michigan dairy farms were randomly assigned as untreated controls or to receive 500 mg of bovine somatotropin (PosilacR) administered every 14 days beginning at 63 to 69 days of lactation and continuing until approximately 21 days prior to the end of lactation or until the animal was removed from the herd. Average peak milk production was 50.8 kg / day and occurred at an average of 113 9 days of lactation for bST-treated cows while average peak production was 48.9 kg / day occurring at an average of 86.4 days of lactation for control cows; both parameters were significantly greater for bST-treated cows compared to controls. Study cows treated with bST were significantly more persistent in lactation (7% greater lactational persistency) compared to control cows. All DHIA estimates and actual milk produced were not significantly different between the study treatment groups for any of the four comparisons made (first, second, third monthly tests after bST treatment initiation and final (305-day) DHIA production estimates); however, the accuracy of DHIA production estimates was significantly affect by the amount of time elapsed since bST but became non-significant by the third DHIA test date. The use of bST changed NFI for each of the four study farms by 3.57, 7.15) per bST-treated cow, respectively during the trial period (from 63 to 305 days of lactation). The overall average change in NFI attributable to bST was $43.01 per bST-treated cow. 2 Profitability of bST use was observed to be quite variable between farms studied because many factors were found to affect the change in NFI per cow resulting from bST use; the level of production response and the price received for milk had the largest effects on the change in NFI associated with bST use; by contrast, price paid for bST itself and feed had only minimal effects on bST-associated profitability. The development of the passé composé in lower-intermediate learners of French as a second language
In this study we tracked the development of the passe compose in second-language learners of French whose first language is English. Although the passe compose is a highly used tense among native speakers of French and it appears to present particular difficulty for first-language English speakers, its second-language development has been surprisingly under-researched. In order to trace developmental patterns of the passe compose we obtained a corpus of obligatory context use of this tense by 30 Year-12 (lower-intermediate) students at two time points six months apart and analysed the data both quantitatively and qualitatively. Our findings suggest that these students used remarkably few memorized formulas, that they passed through five distinct stages in their acquisition of the passe compose, that those early stages were characterized by transfer errors, and that the presence of the auxiliary, whether correct or incorrect, formed a crucial stage in the development of the tense. Theoretical explanations for the findings are presented together with some tentative pedagogical implications
Who should we hire?: Examining coaching succession in NCAA Division I women’s basketball
The purpose of this study was to evaluate the performance of newly hired coaches in relation to their predecessors, and utilize the analysis to provide guidance to decision makers in college athletic departments. This study examined 185 coaching changes in Division I women’s basketball in 16 conferences between 2000 and 2009. Data were collected from online sources including institutional websites, media guides, and media articles. Latent class analysis was employed to reduce the data to one item per factor. Factors included demographics, coaching ability, coaching experience, past team performance, hiring factors (coaching level change, inside/outside hire, interim, conference affiliation), and institutional factors (public/private, demographic market area, enrollment, budget, and National Association of Collegiate Directors of Athletics standings). Mixed models analysis was performed to identify which categories have a relationship with changes in the number of wins following a coaching change. Results suggest that past team performance was the strongest indicator of future performance after a coaching change
A Post-Succession Analysis of Factors Influencing Coaching Success in NCAA Division I Men’s Basketball – Journal of Issues in Intercollegiate Athletics
Based on the reciprocal determinism component of social learning theory, a total of 736 men’s NCAA Division I basketball coaching changes between 1999 and 2014 were examined to establish which factors were related to conference success following a coaching change. Results from an exploratory latent class analysis indicated that many demographic, environmental, and experiential variables assumed to be important in hiring a new coach are insignificant. However, a program’s previous success, individual coaching ability, and previous coach vacancy circumstance are all significantly related to conference winning differential after a coaching change. Results also indicated a regression to the mean occurs after most coaching changes except for the most elite programs. Pragmatically, however, findings show relatively small increments in winning or losing following a coaching change, suggesting that the impact of a coach is often overstated. Stakeholders can use this information to evaluate coaches, programs, and hiring practices in men’s Division I basketball
Pedometers and Aerobic Capacity: Evaluating an Elementary After-School Running Program
Childhood obesity affects 1 of every 6 youth in the United States. One contributing factor to this statistic is a lack of physical activity (PA). Demands related to accountability which are placed on educators to demonstrate academic achievement often result in resistance to allocating time during the school day for PA. One possible solution is to consider utilizing time after school to integrate PA programs. The purpose of this study was to assess the impact of a 12-week after-school pedometer-focused PA program on aerobic capacity and to examine the relationship between step count and aerobic capacity in elementary school aged children. A group of elementary students (n=24; 9.5±0.9 years) participated in a 12-week pedometer-focused PA program that included pretraining and posttraining fitness testing via the 20-meter version of the PACER test. Paired sample t-tests revealed significant differences between the pretest (M=21.0 laps, SD=9.9) and posttest (M=25.2 laps, SD=12.2) scores (t=4.04, P≤0.001). A Pearson correlation revealed no significant relationship between individual step count and the difference between PACER pre- and posttest (r=0.318, P=0.130). The program improved aerobic capacity, but an increase in pedometer-calculated step count was not a predictor
Cementless unicompartmental knee replacement achieves better ten year clinical outcomes than Cemented:A systematic review
Purpose
The aim of this study was to report and compare the long-term revision rate, revision indications and patient reported outcome measures of cemented and cementless unicompartmental knee replacements (UKR).
Methods
Databases Medline, Embase and Cochrane Central of Controlled Trials were searched to identify all UKR studies reporting the ≥ 10 year clinical outcomes. Revision rates per 100 component years [% per annum (% pa)] were calculated by fixation type and then, subgroup analyses for fixed and mobile bearing UKRs were performed. Mechanisms of failure and patient reported outcome measures are reported.
Results
25 studies were eligible for inclusion with a total of 10,736 UKRs, in which there were 8790 cemented and 1946 cementless knee replacements. Cementless fixation had reduced long-term revision rates compared to cemented for the Oxford UKR. For the non-Oxford UKRs, the revision rates of cementless and cemented fixation types were equivalent. Therefore, cementless UKRs offer at least equivalent if not lower revision rates compared to cemented UKRs.

The Role of Palliative Surgery for Malignant Bowel Obstruction and Perforation in Advanced Microsatellite Instability-High Colorectal Carcinoma in the Era of Immunotherapy: Case Report.
The role of palliative surgery in the management of acute complications in patients with disseminated malignancy remains controversial given the complexity of assessing acute surgical risk and long-term oncologic outcome. With the emergence of checkpoint blockade immunotherapy, there appears to be an increasing role for historically palliative procedures as a bridge to systemic immunotherapy. This is especially evident in advanced microsatellite instability-high (MSI-H) colorectal cancer where malignant obstruction and fistula formation are more common and where immunotherapy with checkpoint blockade (anti-PD-1/PD-L1, anti-CTLA-4) has a high response rate with potential for favorable oncologic outcomes. We present a series of three patients with MSI-H metastatic colorectal cancer complicated by malignant bowel obstruction and fistula formation, who having progressed on standard chemotherapy, underwent palliative intervention as a bridge to immune checkpoint blockade with durable and clinically meaningful anti-cancer responses. These cases highlight the need to re-evaluate the role of historically palliative operations in the setting of disease progression for immunotherapy-responsive tumors
Physical Activity and Public Health in Older Adults: Recommendation From the American College of Sports Medicine and the American Heart Association
Objective - To issue a recommendation on the types and amounts of physical activity needed to improve and maintain health in older adults.
Participants - A panel of scientists with expertise in public health, behavioral science, epidemiology, exercise science, medicine, and gerontology.
Evidence - The expert panel reviewed existing consensus statements and relevant evidence from primary research articles and reviews of the literature. Process: After drafting a recommendation for the older adult population and reviewing drafts of the Updated Recommendation from the American College of Sports Medicine (ACSM) and the American Heart Association (AHA) for Adults, the panel issued a final recommendation on physical activity for older adults.
Summary - The recommendation for older adults is similar to the updated ACSM/AHA recommendation for adults, but has several differences including: the recommended intensity of aerobic activity takes into account the older adult\u27s aerobic fitness; activities that maintain or increase flexibility are recommended; and balance exercises are recommended for older adults at risk of falls. In addition, older adults should have an activity plan for achieving recommended physical activity that integrates preventive and therapeutic recommendations. The promotion of physical activity in older adults should emphasize moderate-intensity aerobic activity, muscle-strengthening activity, reducing sedentary behavior, and risk management
Long term in vivo wear of different bearing types used for the Oxford Unicompartmental Knee Replacement
Objectives: The aim of this study was to determine the polyethylene wear rate of Phase 3 Oxford Unicompartmental Knee Replacement bearings and to investigate the effects of resin type and manufacturing process.Methods: A total of 63 patients with at least ten years’ follow-up with three bearing types (1900 resin machined, 1050 resin machined, and 1050 resin moulded) were recruited. Patients underwent full weight-bearing model-based radiostereometric analysis to determine the bearing thickness. The linear wear rate was estimated from the change in thickness divided by the duration of implantation.Results: The wear rate for 1900 resin machined (n = 19), 1050 machined (n = 21), and 1050 moulded bearings (n = 23) were 60 µm/year (sd 42), 76 µm/year (sd 32), and 57 µm/year (sd 30), respectively. There was no significant difference between 1900 machined and 1050 machined (p = 0.20), but 1050 moulded had significantly less wear than the 1050 machined (p = 0.05). Increasing femoral (p < 0.001) and tibial (p < 0.001) component size were associated with increasing wear.Conclusion: Wear rate is similar with 1050 and 1900 resin, but lower with moulded bearings than machined bearings. The currently used Phase 3 bearings wear rate is low (1050 moulded, 57 µm/year), but higher than the previously reported Phase 2 bearings (1900 moulded, 20 µm/year). This is unlikely to be due to the change in polyethylene but may relate to the minimally invasive approach used with the Phase 3. This approach, as well as improving function and thus increasing activity levels, may increase the risk of surgical errors, such as impingement or bearing overhang, which can increase wear. Surgeons should aim to use 4 mm thick bearings rather than 3 mm thick bearings in young patients, unless they are small and need conservative bone resections
Post-operative determinants of chronic pain after primary knee replacement surgery:Analysis of data on 258,386 patients from the National Joint Registry for England, Wales, Northern Ireland and the Isle of Man (NJR)
Objective: To identify post-operative risk factors for the development of chronic pain after knee replacement. Design: Primary knee replacements in persons aged ≥18 years between April 2008 and December 2016 from the National Joint Registry, linked with English Hospital Episode Statistics data, and Patient Reported Outcome Measures. The outcome was chronic pain 6-months after surgery (Oxford Knee pain score). Logistic regression modelling identified risk factors for chronic pain outcome. Results: 258,386 patients; 56.7% women; average age 70.1 years (SD ​± ​8.8 years). 43,702 (16.9%) were identified as having chronic pain 6-months post-surgery. Within 3 months of surgery complications were uncommon: intra-operative complications 1224 (0.5%); ≥1 medical complication 6073 (2.4%)); 32,930 (12.7%) hospital readmissions; 3848 (1.5%) re-operation; 835 (0.3%) revision. Post-surgical risk factors of chronic pain were: mechanical complication of prosthesis odds ratio (OR) 1.56 (95% Confidence Interval 1.35, 1.80); surgical site infection OR 1.13 (0.99, 1.29); readmission OR 1.47 (1.42, 1.52); re-operation OR 1.39 (1.27, 1.51); revision OR 1.92 (1.64, 2.25); length of stay e.g. 6+ vs. <2 days OR 1.48 (1.35, 1.63), blood transfusion OR 0.47 (0.26, 0.86) and myocardial infarction OR 0.69 (0.49, 0.97). Discriminatory ability of the model was only fair (c-statistic 0.71) indicating that post-surgical predictors explain a limited amount of variability in chronic pain. Conclusions: We identified a number of post-operative factors relating to the operation and early recovery that are associated with chronic pain following primary knee replacement. The model had weak discriminatory ability indicating that there remains considerable unexplained variability in chronic pain outcome
