    Development of CNS-type Primitive Neuroectodermal Tumor in Metastatic Testicular Mixed Germ Cell Tumor.

    A 29-year-old man presenting with a retroperitoneal mass was found to have a testis lesion consistent with mixed germ cell tumor and the RPLND specimen showed teratoma with an area of central nervous system-type primitive neuroectodermal tumor (PNET) not present in the testis. Whether such primitive tumor components represent a de novo tumor component or represent progression from existing neuroepithelial teratomatous elements is unclear. Given the high likelihood of residual tumor and possibility of malignant transformation, post-chemotherapy RPLND remains vital in treating patients with testis cancer. PNET is chemo-resistant and lesions should be resected, without clear evidence for adjuvant chemotherapy

Part of the difficulty is due to an absence of any detailed theory regarding intelligence and particularly a theory that would demonstrate the relationship of intelligence to the overall adjustments of people perceptually, conceptually, and emotionally. Werner has attempted to supply some hypotheses to guide a more systematic kind of observation and research on feeblemindedness. Certain of these hypotheses supply the background for the study reported here, i.e., those hypotheses which center on the relationship between intelligence and perceptual adjustment and functioning and deal specifically with the genetic changes in perceptual activity is these are accelerated or retarded by the intelligence variable. The hypotheses advanced by this study ares that at a given mental age feebleminded individuals would show a level of functioning generally below that of normal individuals; further, that the adult feebleminded would indicate a level of functioning generally below that of the adolescent feebleminded individuals. It is also proposed that their functioning should be understandable in terms of the various developmental stages proposed by Werner. The Rorschach inkblot test was employed since it appeared well suited to collect perceptual data and it allows the testing of a wide range of age groups with the same material. To test the hypotheses, the Rorschach protocols of thirty normal adults, thirty adult defectives, thirty adelescent defectives, and one hundred and thirty seven children divided into six year levels frem three to ten were obtained. The median mental age of the eight year olds is closest to that of both defective groups, and thus they represent the normal group against whom the hypotheses are tested. The material was analyzed by use of a scoring schema developed by Friedman, operationally designed to reflect the structural aspects of perceptual activity in their genetically law and genetically high qualities.     The quality of the early hominin fossil record : implications for evolutionary analyses

    Hominins (the clade including modern humans and their fossil ancestors) were a taxonomically and morphologically diverse group during the Plio-Pleistocene, and their evolution documents the only known transition to obligate bipedalism in primates. However,many aspects of their shared evolutionary history remain frustratingly unclear due to uncertainty about whether change in the fossil record reflects genuine evolutionary change or variation in our sampling of the rock record. Here, a comprehensive assessment of the quality of the early African hominin fossil record is presented. A specimen database of all early African hominin fossils (>5000) has been compiled including taxonomic, geological, anatomical, and bibliographic information. Using a range of sampling metrics (fossil-bearing formations, collection effort, sampled area, and ghost lineage diversity), it is shown that the pulsedlike pattern of uncorrected (taxic) hominin diversity is almost entirely controlled by rock availability. By contrasting taxic with phylogenetically corrected diversity, hominin diversification appears unconstrained through the lateMiocene and Pliocene, with diversity constantly increasing until a single peak is reached in the early Pleistocene. Phylogenetically corrected diversity shows no discernible link with sampling metrics and there is no direct evidence that shifts in climatic conditions drove diversification. A study of specimen completeness through geological time shows that while sampling metrics (specifically sustained collection effort at rich deposits) have a major influence on patterns of specimen completeness, specimen completeness has only a moderate influence on diversity patterns. It also shows that specimen completeness is poorest during the period most pertinent to human origins, the estimated Pan-Homo divergence date, in large part due to under-sampling (<4% of Africa by sampled area). In combination, this work illustrates that the hominin fossil record is by no means an unbiased depiction of evolutionary events, and therefore its quality and incompleteness should be fully understood before any interpretation of macroevolutionary patterns

    Patient perceptions of physician empathy, satisfaction with physician, interpersonal trust, and compliance

    Objectives: This study was designed to investigate psychometric properties of the Jefferson Scale of Patient Perceptions of Physician Empathy (JSPPPE), and to examine correlations between its scores and measures of overall satisfaction with physicians, personal trust, and indicators of patient compliance. Methods: Research participants included 535 out-patients (between 18-75 years old, 66% female). A survey was mailed to participants which included the JSPPPE (5-item), a scale for measuring overall satisfaction with the primary care physician (10-item), and demographic questions. Patients were also asked about compliance with their physician\u27s recommendation for preventive tests (colonoscopy, mammogram, and PSA for age and gender appropriate patients). Results: Factor analysis of the JSPPPE resulted in one prominent component. Corrected item-total score correlations ranged from .88 to .94. Correlation between scores of the JSPPPE and scores on the patient satisfaction scale was 0.93. Scores of the JSPPPE were highly correlated with measures of physician-patient trust (r \u3e.73). Higher scores of the JSPPPE were significantly associated with physicians\u27 recommendations for preventive tests (colonoscopy, mammogram, and PSA) and with compliance rates which were \u3e .80). Cronbach\u27s coefficient alpha for the JSPPPE ranged from .97 to .99 for the total sample and for patients in different gender and age groups. Conclusions: Empirical evidence supported the psychometrics of the JSPPPE, and confirmed significant links with patients\u27 satisfaction with their physicians, interpersonal trust, and compliance with physicians\u27 recommendations. Availability of this psychometrically sound instrument will facilitate empirical research on empathy in patient care in different countries

All treatments resulted in weight gains and limited body condition score changes. The hay and high CGF pellet diet resulted in statistically different weight gains compared with the other supplementation programs
