Surveillance as an Option for the Treatment of Small Renal Masses
OBJECTIVES. To review the natural history and biological potential of small renal masses in order to evaluate surveillance as a treatment option. METHODS. Literature search of Medline and additional references from non-Medline-indexed publications concerning surveillance of small renal masses. RESULTS. The natural history and biological potential of small renal masses can still not be unambiguously predicted at present. There seems to be no clear correlation between tumour size and presence of benign histology. The majority of small renal masses grow and the majority are cancer, but one cannot safely assume that a lack of growth on serial CT scans is the confirmation of absence of malignancy. Needle core biopsies could be used to help in decision making. They show a high accuracy for histopathological tumour type but are less accurate in evaluating Fuhrman grade. CONCLUSIONS. At present, surveillance of small renal masses should only be considered in elderly and/or infirm patients with competing health risks, in those with a limited life expectancy, and in those for whom minimal invasive treatment or surgery is not an option. In all other patients, active surveillance should only be considered in the context of a study protocol. Long-term, prospective studies are needed to provide a more accurate assessment of the natural history and metastastic potential of small renal masses
A Giant Mucinous Adenocarcinoma Arising within a Villous Adenoma of the Urachus: Case Report and Review of the Literature
We present an exceptional case of a giant urachal tumor, consisting of both villous adenoma and mucinous adenocarcinoma of the urachus. The tumor was incidentally discovered during investigations for renal failure. Initial transurethral biopsies showed only a villous adenoma of the urachus. Although the biopsies showed no malignancy, a radical cystoprostatectomy and broad excision of the urachus and umbilicus were performed. At the same time, a bilateral nephroureterectomy was performed because of reflux-nephropathy and renal failure. The indication for surgery was based on the typical imaging aspects, raising the suspicion of an underlying urachal adenocarcinoma (size and location). Indeed, at final histopathology a concomitant well-differentiated adenocarcinoma of the urachus confined to the urachal mucosa was found. Preclinical Evidence for the Benefits of Penile Rehabilitation Therapy following Nerve-Sparing Radical Prostatectomy
Erectile dysfunction following radical prostatectomy remains a frequent problem despite the development of nerve-sparing techniques. This erectile dysfunction is believed to be neurogenic, enhanced by hypoxia-induced structural changes which result in additional veno-occlusive dysfunction. Recently, daily use of intracavernous vasoactive substances and oral use of PDE5-inhibitors have been clinically studied for treatment of postprostatectomy erectile dysfunction. Since these studies showed benefits of “penile rehabilitation therapy,” these effects have been studied in a preclinical setting. We reviewed experimental literature on erectile tissue preserving and neuroregenerative treatment strategies, and found that preservation of the erectile tissue by the use of intracavernous nitric oxide donors or vasoactive substances, oral PDE5-inhibitors, and hyperbaric oxygen therapy improved erectile function by antifibrotic effects and preservation of smooth muscle. Furthermore, neuroregenerative strategies using neuroimmunophilin ligands, neurotrophins, growth factors, and stem cell therapy show improved erectile function by preservation of NOS-containing nerve fibers
Metabolic Changes after Urinary Diversion
Urinary diversion is performed on a regular basis in urological practice. Surgeons tend to
underestimate the metabolic effects of any type of diversion. From the patient's perspective, diarrhea
is the most bothersome complaint after urinary diversion. This might be accompanied by
malabsorption syndromes, such as vitamin B12 deficiency. Electrolyte abnormalities can occur
frequently such as hyperchloremic metabolic acidosis, or less frequently such as hypokalemia,
hypocalcaemia, and hypomagnesaemia. Bone health is at risk in patients with urinary diversion. Some
patients might benefit from vitamin D and calcium supplementation. Many patients are also subject
to urinary calculus formation, both at the level of the upper urinary tract as in intestinal reservoirs.
Urinary diversion can affect hepatic metabolism, certainly in the presence of urea-splitting bacteria.
The kidney function has to be monitored prior to and lifelong after urinary diversion. Screening for
reversible causes of renal deterioration is an integral part of the followup
Sense and Nonsense of an Extended Pelvic Lymph Node Dissection in Prostate Cancer
Lymph node metastases associated with prostate cancer (PCa) has been shown to be a poor prognostic factor. The role of pelvic lymph node dissection (PLND) itself in relation to survival remains unclear, however. A Medline search was conducted to address this issue. The following conclusions were drawn. Only recently, improved survival due to completion of radical prostatectomy (RP) (compared to abandoning RP) in known or presumed lymph-node-positive patients has been shown. Lymph node sampling can only be considered representative if an adequate number of nodes is removed. While several authors have suggested that a therapeutic benefit in patients undergoing RP is not provided by PLND, the reliability of these studies is uncertain. Contrary to this, several studies have indicated the possibility of long-term survival even in the presence of limited lymph node metastases. The role and timing of initiation of adjuvant androgen deprivation therapy (ADT) in patients who have node-positive disease after RP is controversial. Recent studies suggest that delaying ADT may not adversely impact survival
EAU guidelines on prostate cancer
Objetivos: Presentar un resumen de la versión del 2007 de la guía de la Asociación Europea de Urología (EAU) para el cáncer de próstata (CaP). Métodos: Se realizó por un grupo de trabajo una revisión de los nuevos datos presentes desde 2004 hasta 2007 en la literatura. Las guías han sido actualizadas y el nivel de evidencia/grado de recomendación ha sido añadido al texto basándose en una revisión sistemática de la literatura, que incluía una búsqueda de las bases de datos online y revisiones bibliográficas. Resultados: Una versión completa está disponible en la EAU Office o en www.uroweb.org. El método diagnóstico de elección es la biopsia sistematizada de la próstata bajo control ecográfico. El tratamiento activo es el recomendado en la mayoría de los pacientes con enfermedad localizada y una larga esperanza de vida, siendo la prostatectomía radical superior a la vigilancia ("watchful waiting") en un ensayo randomizado prospectivo. La prostatectomía radical con preservación de bandeletas es la técnica de elección en la enfermedad órgano-confinada; el bloqueo androgénico neoadyuvante no ha demostrado una mejoría en las variables de resultados. La radioterapia debe realizarse con al menos 72 Gy en el CaP de bajo riesgo y con 78 Gy en el de intermedio - alto riesgo. El bloqueo androgénico en monoterapia es el estándar del tratamiento en el CaP metastásico; el bloqueo androgénico intermitente podría ser un tratamiento alternativo en pacientes seleccionados. El seguimiento se basa principalmente en los niveles de PSA y en la anamnesis específica de la enfermedad, estando las pruebas de imagen sólo indicadas cuando aparecen los síntomas. La quimioterapia con docetaxel ha surgido como el tratamiento de referencia para el CaP metastásico hormonorefractario. Conclusiones: El conocimiento en el campo del CaP está rápidamente cambiando. Complications and Functional Results of Surgery for Locally Advanced Prostate Cancer
The role of surgery in clinical stage T3 prostate cancer (cT3 PCa) is still subject to debate. We reviewed the records of 139 consecutive patients who underwent a radical prostatectomy (RP) for cT3 PCa with a mean follow-up of 8 years. All data related to surgical and perioperative complications were collected. Continence and erectile function were assessed at 12 months postoperatively and long-term oncologic outcomes were analyzed. Rectal injury and injury of the obturator nerve occurred both in 0.7% of cases. No serious in-hospital complications were noted and no reintervention was needed. Lymphatic leakage was noted in 2.2% of patients and 1.4% experienced prolonged drainage of urine. In 7.2%, wound-related problems occurred. Anastomotic stricture occurred in 2.9%. These complication rates were not different compared to surgical series of RP in localized PCa. At 12 months, complete continence was 87.8% and erectile function had fully recovered in 6% and 10% of patients who underwent a non-nerve sparing or unilateral nerve-sparing procedure, respectively. 10-year estimated biochemical PFS, clinical PFS, CSS and OS were 51.8%, 85.6%, 94.6% and 85.9%, respectively.
