Surgical management of a diabetic calcaneal ulceration and osteomyelitis with a partial calcanectomy and a sural neurofasciocutaneous flap
The treatment of calcaneal osteomyelitis in diabetic patients poses a great challenge to the treating physician and surgeon. The use of a distally based sural neurofasciocutaneous flap after an aggressive debridement of non-viable and poorly vascularized tissue and bone that is combined with a thorough antibiotic regimen provides a great technique for adequate soft tissue coverage of the heel. In this case report, the authors describe the aforementioned flap as a versatile alternative to the use of local or distant muscle flaps for diabetic patients with calcaneal osteomyelitis and concomitant large wounds
Evaluation of a text supported weight maintenance programme βLighten Up Plusβ following a weight reduction programme: randomised controlled trial
Background Many overweight people find it difficult to maintain weight loss after attending a weight reduction programme. Self-weighing and telephone support are known to be useful methods for self-monitoring for weight loss. We examined the effectiveness of an SMS-text messaging based weight maintenance programme to encourage regular self-weighing in adults who had completed a 12 week commercial weight loss programme. Methods Randomised controlled trial of 380 obese or overweight men and women. The intervention group (n=190) received a single maintenance support phone call and SMS-text based weight maintenance messages over 12 weeks to encourage regular self-weighing after completing their weight loss programme. The primary outcome was change in weight at nine months follow up. Results Our sample (N=380) had a mean age of 47.4 years (SD 13.4), mean baseline weight and BMI of 93.1kg (16.1) and 34.4 kg/m2 (5.0) respectively, as well as majority female (87.3%) and White British (80.0%). Using intention to treat analysis both groups regained weight at nine months follow up; the intervention group regained an average of 1.36 kg while the control group regained 1.81 kg. Adjusting for covariates resulted in a mean difference of 0.45 kg (95% CI -0.78, 1.67) favouring the intervention group at nine month follow up. Conclusions We found no evidence that an SMS based weight maintenance intervention encouraging adults to weigh themselves weekly prevented weight regain at three or nine months after completing a commercial weight loss programme. <br/
Energy-efficient casting processes
Metal casting is one of the most energy-intensive manufacturing processes that have developed along the evolution of mankind. Although nowadays its scientific and technological aspects are well established, in the context of future resource scarcity and environmental pollution pressures, new studies appear necessary to describe the βfoundry of the futureβ where energy and material efficiency are of great importance to guarantee competitiveness alongside environmental protection. In this chapter, both managerial and technical good practices aimed at implementing energy-efficient casting processes are presented alongside a few examples. The βSmall is Beautifulβ philosophy is presented as a systematic approach towards energy resilient manufacturing and, potentially, sustainability in the long term. Thus, this chapter aims at providing an overview of the different aspects comprising the state of the art in the industry and examples of research themes in academia about energy-efficient casting processes
Building social capital through breastfeeding peer support: Insights from an evaluation of a voluntary breastfeeding peer support service in North-West England
Background:
Peer support is reported to be a key method to help build social capital in communities. To date there are no studies that describe how this can be achieved through a breastfeeding peer support service. In this paper we present findings from an evaluation of a voluntary model of breastfeeding peer support in North-West England to describe how the service was operationalized and embedded into the community. This study was undertaken from May, 2012 to May, 2013.
Methods:
Interviews (group or individual) were held with 87 participants: 24 breastfeeding women, 13 peer supporters and 50 health and community professionals. The data contained within 23 monthly monitoring reports (January, 2011 to February 2013) compiled by the voluntary peer support service were also extracted and analysed.
Results:
Thematic analysis was undertaken using social capital concepts as a theoretical lens. Key findings were identified to resonate with βbondingβ, βbridgingβ and βlinkingβ forms of social capital. These insights illuminate how the peer support service facilitates βbondsβ with its members, and within and between women who access the service; how the service βbridgesβ with individuals from different interests and backgrounds, and how βlinksβ were forged with those in authority to gain access and reach to women and to promote a breastfeeding culture. Some of the tensions highlighted within the social capital literature were also identified.
Conclusions:
Horizontal and vertical relationships forged between the peer support service and community members enabled peer support to be embedded into care pathways, helped to promote positive attitudes to breastfeeding and to disseminate knowledge and maximise reach for breastfeeding support across the community. Further effort to engage with those of different ethnic backgrounds and to resolve tensions between peer supporters and health professionals is warranted
Football fans in training: the development and optimization of an intervention delivered through professional sports clubs to help men lose weight, become more active and adopt healthier eating habits
<p>Background: The prevalence of obesity in men is rising, but they are less likely than women to engage in existing weight management programmes. The potential of professional sports club settings to engage men in health promotion activities is being increasingly recognised. This paper describes the development and optimization of the Football Fans in Training (FFIT) programme, which aims to help overweight men (many of them football supporters) lose weight through becoming more active and adopting healthier eating habits.</p>
<p>Methods: The MRC Framework for the design and evaluation of complex interventions was used to guide programme development in two phases. In Phase 1, a multidisciplinary working group developed the pilot programme (p-FFIT) and used a scoping review to summarize previous research and identify the target population. Phase 2 involved a process evaluation of p-FFIT in 11 Scottish Premier League (SPL) clubs. Participant and coach feedback, focus group discussions and interviews explored the utility/acceptability of programme components and suggestions for changes. Programme session observations identified examples of good practice and problems/issues with delivery. Together, these findings informed redevelopment of the optimized programme (FFIT), whose components were mapped onto specific behaviour change techniques using an evidence-based taxonomy.</p>
<p>Results: p-FFIT comprised 12, weekly, gender-sensitised, group-based weight management classroom and βpitch-sideβ physical activity sessions. These in-stadia sessions were complemented by an incremental, pedometer-based walking programme. p-FFIT was targeted at men aged 35-65 years with body mass indexββ₯β27 kg/m2. Phase 2 demonstrated that participants in p-FFIT were enthusiastic about both the classroom and physical activity components, and valued the camaraderie and peer-support offered by the programme. Coaches appreciated the simplicity of the key healthy eating and physical activity messages. Suggestions for improvements that were incorporated into the optimized FFIT programme included: more varied in-stadia physical activity with football-related components; post-programme weight management support (emails and a reunion session); and additional training for coaches in SMART goal setting and the pedometer-based walking programme.</p>
<p>Conclusions: The Football Fans in Training programme is highly acceptable to participants and SPL coaches, and is appropriate for evaluation in a randomised controlled trial.</p>
Analysis of Mice Lacking DNaseI Hypersensitive Sites at the 5β² End of the IgH Locus
The 5β² end of the IgH locus contains a cluster of DNaseI hypersensitive sites, one of which (HS1) was shown to be pro-B cell specific and to contain binding sites for the transcription factors PU.1, E2A, and Pax5. These data as well as the location of the hypersensitive sites at the 5β² border of the IgH locus suggested a possible regulatory function for these elements with respect to the IgH locus. To test this notion, we generated mice carrying targeted deletions of either the pro-B cell specific site HS1 or the whole cluster of DNaseI hypersensitive sites. Lymphocytes carrying these deletions appear to undergo normal development, and mutant B cells do not exhibit any obvious defects in V(D)J recombination, allelic exclusion, or class switch recombination. We conclude that deletion of these DNaseI hypersensitive sites does not have an obvious impact on the IgH locus or B cell development
Efficiency of Organelle Capture by Microtubules as a Function of Centrosome Nucleation Capacity: General Theory and the Special Case of Polyspermia
Transport of organelles along microtubules is essential for the cell metabolism and morphogenesis. The presented analysis derives the probability that an organelle of a given size comes in contact with the microtubule aster. The question is asked how this measure of functionality of the microtubule aster is controlled by the centrosome. A quantitative model is developed to address this question. It is shown that for the given set of cellular parameters, such as size and total tubulin content, a centrosome nucleation capacity exists that maximizes the probability of the organelle capture. The developed general model is then applied to the capture of the female pronucleus by microtubules assembled on the sperm centrosome, following physiologically polyspermic fertilization. This application highlights an unintuitive reflection of nonlinearity of the nucleated polymerization of the cellular pool of tubulin. The prediction that the sperm centrosome should lower its nucleation capacity in the face of the competition from the other sperm is a stark illustration of the new optimality principle. Overall, the model calls attention to the capabilities of the centrosomal pathway of regulation of the transport-related functionality of the microtubule cytoskeleton. It establishes a quantitative and conceptual framework that can guide experiment design and interpretation
Measurement of the branching fraction and CP content for the decay B(0) -> D(*+)D(*-)

Copyright @ 2002 APS.We report a measurement of the branching fraction of the decay B0βD*+D*- and of the CP-odd component of its final state using the BABAR detector. With data corresponding to an integrated luminosity of 20.4ββfb-1 collected at the Ξ₯(4S) resonance during 1999β2000, we have reconstructed 38 candidate signal events in the mode B0βD*+D*- with an estimated background of 6.2Β±0.5 events. From these events, we determine the branching fraction to be B(B0βD*+D*-)=[8.3Β±1.6(stat)Β±1.2(syst)]Γ10-4. Measurement of D-s(+) and D-s(*+) production in B meson decays and from continuum e(+)e(-) annihilation at βs=10.6 GeV

continuum events near the Ξ₯(4S) resonance are presented. Using 20.8 fb-1 of data on the Ξ₯(4S) resonance and 2.6 fb-1 off-resonance, we find the inclusive branching fractions B(BβDs+X)=(10.93Β±0.19Β±0.58Β±2.73)% and B(BβDs*+X)=(7.9Β±0.8Β±0.7Β±2.0)%, where the first error is statistical, the second is systematic, and the third is due to the Ds+βΟΟ+ branching fraction uncertainty. The production cross sections Ο(e+e-βDs+X)ΓB(Ds+βΟΟ+)=7.55Β±0.20Β±0.34pb and Ο(e+e-βDs*Β±X)ΓB(Ds+βΟΟ+)=5.8Β±0.7Β±0.5pb are measured at center-of-mass energies about 40 MeV below the Ξ₯(4S) mass. The branching fractions Ξ£B(BβDs(*)+D(*))=(5.07Β±0.14Β±0.30Β±1.27)% and Ξ£B(BβDs*+D(*))=(4.1Β±0.2Β±0.4Β±1.0)% are determined from the Ds(*)+ momentum spectra.
