Drug availability and health facility usage in a Bamako Initiative and a non-Bamako Initiative Local Government Areas of Akwa Ibom State, South -South Nigeria
Background: The availability of drugs on a continuous basis is paramount to the success of any health care system. The Bamako Initiative (BI) had provision of essential drugs as one of its key thrusts in order to improve the utilization of health facilities. This study compared the perceived availability of essential drugs and patronage of health facilities in a BI and non-BI Local government areas (LGA) of Akwa Ibom State in south-south Nigeria.Methodology: This was a cross sectional comparative descriptive study that used an interviewer administered semi-structured questionnaire as an instrument for data collection. Data collected was analyzed using SPSS software version 11.Results: A total of 154 respondents in each LGA were interviewed. Up to 135 (88.8%) opined that drugs were always available in the BI LGA as against 103 (66.9%) in the non-BI LGA (p <0.001). Similarly, 133 (86.3%) said drugs were usually available in the required formulation for BI LGA compared to 115 (74.7%) in the non-BI LGA (p <0.05). More respondents in the BI facilities, 45 (45%) reported using the facilities often compared to 39 (35.8%) in the non-BI facilities (p = 0.03). A total of 116 (75.3%) respondents in the BI LGA felt drugs were not costly compared to 50 (32.7%) in the non-BI LGA (p <0.001). Up to 11 (7.2%) respondents in the non-BI LGA were not satisfied with the drug services in the health centers, compared to 1(0.6%) in the BI LGA (p = 0.01).Conclusion: Availability of essential drugs therefore seemed to contribute to patients' usage of health facilities.Keywords: Bamako Initiative, Essential Drugs, Availability, Patronage, Health Facilities, Utilizatio

The role of primary healthcare professionals in oral cancer prevention and detection
AIM: To investigate current knowledge, examination habits and preventive practices of primary healthcare professionals in Scotland, with respect to oral cancer, and to determine any relevant training needs. SETTING: Primary care. METHOD: Questionnaires were sent to a random sample of 357 general medical practitioners (GMPs) and 331 dental practitioners throughout Scotland. Additionally, focus group research and interviews were conducted amongst primary healthcare team members. RESULTS: Whilst 58% of dental respondents reported examining regularly for signs of oral cancer, GMPs examined patients' mouths usually in response to a complaint of soreness. The majority of GMPs (85%) and dentists (63%) indicated that they felt less than confident in detecting oral cancer, with over 70% of GMPs identifying lack of training as an important barrier. Many practitioners were unclear concerning the relative importance of the presence of potentially malignant lesions in the oral cavity. A high proportion of the GMPs indicated that they should have a major role to play in oral cancer detection (66%) but many felt strongly that this should be primarily the remit of the dental team. CONCLUSION: The study revealed a need for continuing education programmes for primary care practitioners in oral cancer-related activities. This should aim to improve diagnostic skills and seek to increase practitioners' participation in preventive activities
Removal of ecotoxicity of 17α-ethinylestradiol using TAML/peroxide water treatment
17α -ethinylestradiol (EE2), a synthetic oestrogen in oral contraceptives, is one of many pharmaceuticals found in inland waterways worldwide as a result of human consumption and excretion into wastewater treatment systems. At low parts per trillion (ppt), EE2 induces feminisation of male fish, diminishing reproductive success and causing fish population collapse. Intended water quality standards for EE2 set a much needed global precedent. Ozone and activated carbon provide effective wastewater treatments, but their energy intensities and capital/operating costs are formidable barriers to adoption. Here we describe the technical and environmental performance of a fast- developing contender for mitigation of EE2 contamination of wastewater based upon smallmolecule, full-functional peroxidase enzyme replicas called “TAML activators”. From neutral to basic pH, TAML activators with H2O2 efficiently degrade EE2 in pure lab water, municipal effluents and
EE2-spiked synthetic urine. TAML/H2O2 treatment curtails estrogenicity in vitro and substantially diminishes fish feminization in vivo. Our results provide a starting point for a future process in which tens of thousands of tonnes of wastewater could be treated per kilogram of catalyst. Spatial effects, sampling errors, and task specialization in the honey bee
Task allocation patterns should depend on the spatial distribution of work within the nest, variation in task demand, and the movement patterns of workers, however, relatively little research has focused on these topics. This study uses a spatially explicit agent based model to determine whether such factors alone can generate biases in task performance at the individual level in the honey bees, Apis mellifera. Specialization (bias in task performance) is shown to result from strong sampling error due to localized task demand, relatively slow moving workers relative to nest size, and strong spatial variation in task demand. To date, specialization has been primarily interpreted with the response threshold concept, which is focused on intrinsic (typically genotypic) differences between workers. Response threshold variation and sampling error due to spatial effects are not mutually exclusive, however, and this study suggests that both contribute to patterns of task bias at the individual level. While spatial effects are strong enough to explain some documented cases of specialization; they are relatively short term and not explanatory for long term cases of specialization. In general, this study suggests that the spatial layout of tasks and fluctuations in their demand must be explicitly controlled for in studies focused on identifying genotypic specialists
On the break in the single-particle energy dispersions and the `universal' nodal Fermi velocity in the high-temperature copper-oxide superconductors
Recent data from angle-resolved photoemission experiments published by Zhou
et al. [Nature, Vol. 423, 398 (2003)] concerning a number of hole-doped
copper-oxide-based high-temperature superconductors reveal that in the nodal
directions of the underlying square Brillouin zones (i.e. the directions along
which the d-wave superconducting gap is vanishing) the Fermi velocities for
some finite range of k inside the Fermi sea and away from the nodal Fermi
wavevector k_F are to within an experimental uncertainty of approximately 20%
the same both in all the compounds investigated and over a wide range of doping
concentrations and that, in line with earlier experimental observations, at
some characteristic wavevector k_* away from k_F the Fermi velocities undergo a
sudden change, with this change (roughly speaking, a finite discontinuity)
being the greatest (smallest) in the case of underdoped (overdoped) compounds.

Spin Seebeck insulator
Thermoelectric generation is an essential function of future energy-saving
technologies. However, this generation has been an exclusive feature of
electric conductors, a situation which inflicts a heavy toll on its
application; a conduction electron often becomes a nuisance in thermal design
of devices. Here we report electric-voltage generation from heat flowing in an
insulator. We reveal that, despite the absence of conduction electrons, a
magnetic insulator LaY2Fe5O12 converts a heat flow into spin voltage. Attached
Pt films transform this spin voltage into electric voltage by the inverse spin
Hall effect. The experimental results require us to introduce thermally
activated interface spin exchange between LaY2Fe5O12 and Pt. Our findings
extend the range of potential materials for thermoelectric applications and
provide a crucial piece of information for understanding the physics of the

Effectiveness, cost-effectiveness and cost-benefit of a single annual professional intervention for the prevention of childhood dental caries in a remote rural Indigenous community
Background
The aim of the study is to reduce the high prevalence of tooth decay in children in a remote, rural Indigenous community in Australia, by application of a single annual dental preventive intervention. The study seeks to (1) assess the effectiveness of an annual oral health preventive intervention in slowing the incidence of dental caries in children in this community, (2) identify the mediating role of known risk factors for dental caries and (3) assess the cost-effectiveness and cost-benefit of the intervention.
Methods/design
The intervention is novel in that most dental preventive interventions require regular re-application, which is not possible in resource constrained communities. While tooth decay is preventable, self-care and healthy habits are lacking in these communities, placing more emphasis on health services to deliver an effective dental preventive intervention. Importantly, the study will assess cost-benefit and cost-effectiveness for broader implementation across similar communities in Australia and internationally.
Discussion
There is an urgent need to reduce the burden of dental decay in these communities, by implementing effective, cost-effective, feasible and sustainable dental prevention programs. Expected outcomes of this study include improved oral and general health of children within the community; an understanding of the costs associated with the intervention provided, and its comparison with the costs of allowing new lesions to develop, with associated treatment costs. Findings should be generalisable to similar communities around the world.
