Climate change increases the risk of fisheries conflict
The effects of climate change on the ocean environment â especially ocean warming, acidification, and sea level rise â will impact fish stocks and fishers in important ways. Likely impacts include changes in fish stocksâ productivity and distribution, human migration to and away from coastal areas, stresses on coastal fisheries infrastructure, and challenges to prevailing maritime boundaries. In this paper, we explore these and other related phenomena, in order to assess whether and how the impacts of climate change on fisheries will contribute to the risk of fisheries conflict. We argue that climate change will entail an increase in the conditions that may precipitate fisheries conflict, and thereby create new challenges for existing fisheries management institutions. Several potential changes in fisheries management policy are recommended to avert the growing risk of fisheries-related conflicts
Liver transplantation for alcoholic cirrhosis: Long term follow-up and impact of disease recurrence
Background. Alcoholic liver disease has emerged as a leading indication for hepatic transplantation, although it is a controversial use of resources. We aimed to examine all aspects of liver transplantation associated with alcohol abuse. Methods. Retrospective cohort analysis of 123 alcoholic patients with a median of 7 years follow-up at one center. Results. In addition to alcohol, 43 (35%) patients had another possible factor contributing to cirrhosis. Actuarial patient and graft survival rates were, respectively, 84% and 81% (1 year); 72% and 66% (5 years); and 63% and 59% (7 years). After transplantation, 18 patients (15%) manifested 21 noncutaneous de novo malignancies, which is significantly more than controls (P=0.0001); upper aerodigestive squamous carcinomas were over-represented (P=0.03). Thirteen patients had definitely relapsed and three others were suspected to have relapsed. Relapse was predicted by daily ethanol consumption (P=0.0314), but not by duration of pretransplant sobriety or explant histology. No patient had alcoholic hepatitis after transplantation and neither late onset acute nor chronic rejection was significantly increased. Multiple regression analyses for predictors of graft failure identified major biliary/vascular complications (P=0.01), chronic bile duct injury on biopsy (P=0.002), and pericellular fibrosis on biopsy (P=0.05); graft viral hepatitis was marginally significant (P=0.07) on univariate analysis. Conclusions. Alcoholic liver disease is an excellent indication for liver transplantation in those without coexistent conditions. Recurrent alcoholic liver disease alone is not an important cause of graft pathology or failure. Potential recipients should be heavily screened before transplantation for coexistent conditions (e.g., hepatitis C, metabolic diseases) and other target-organ damage, especially aerodigestive malignancy, which are greater causes of morbidity and mortality than is recurrent alcohol liver disease
ACR Appropriateness CriteriaÂź Hodgkin Lymphoma-Favorable Prognosis Stage I and II
This topic addresses the treatment of newly diagnosed patients with favorable prognosis stage I and II Hodgkin lymphoma. In most cases, combined modality therapy (chemotherapy followed by involved site radiation therapy) constitutes the current standard of care. The American College of Radiology Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed annually by a multidisciplinary expert panel. The guideline development and revision include an extensive analysis of current medical literature from peer-reviewed journals and the application of well-established methodologies (RAND/UCLA Appropriateness Method and Grading of Recommendations Assessment, Development, and Evaluation or GRADE) to rate the appropriateness of imaging and treatment procedures for specific clinical scenarios. In those instances where evidence is lacking or equivocal, expert opinion may supplement the available evidence to recommend imaging or treatment. By combining the most recent medical literature and expert opinion, this revised guideline can aid clinicians in the appropriate use of combined modality therapy for favorable prognosis stage I and II Hodgkin lymphoma. Increasing information about the late effects of treatment has led to attempts to decrease toxicity by using less chemotherapy (decreased duration and/or intensity or different agents) and less radiation therapy (reduced volume and/or dose) while maintaining excellent efficacy

Improving Situational Awareness in the Arctic Ocean
To successfully operate in a harsh environment like the Arctic Ocean, one must be able to understand and predict how that environment will evolve over different spatial and temporal scales. This is particularly challenging given the on-going and significant environmental changes that are occurring in the region. Access to the most recent environmental information provides timely knowledge that enables ship-based operations to proceed efficiently, effectively and safely in this difficult arena. Knowledge of the evolving environmental conditions during a field campaign is critical for effective planning, optimal execution of sampling strategies, and to provide a broader context to data collected at specific times and places. We describe the collaborations and processes that enabled an operational system to be developed to provide a remote field-team, located on USCGC Healy in the Beaufort Sea, with near real-time situational awareness information regarding the weather, sea ice conditions, and oceanographic processes. The developed system included the punctual throughput of near real-time products such as satellite imagery, meteorological forecasts, ice charts, model outputs, and up to date locations of key sea ice and ocean-based assets. Science and operational users, as well as onshore personnel, used this system for real-time practical considerations such as ship navigation, and to time scientific operations to ensure the appropriate sea ice and weather conditions prevailed. By presenting the outputs of the system within the context of case studies our results clearly demonstrate the benefits that improved situational awareness brings to ship-based operations in the Arctic Ocean, both today and in the future
Information Transmission in Cercal Giant Interneurons Is Unaffected by Axonal Conduction Noise
What are the fundamental constraints on the precision and accuracy with which nervous systems can process information? One constraint must reflect the intrinsic ânoisinessâ of the mechanisms that transmit information between nerve cells. Most neurons transmit information through the probabilistic generation and propagation of spikes along axons, and recent modeling studies suggest that noise from spike propagation might pose a significant constraint on the rate at which information could be transmitted between neurons. However, the magnitude and functional significance of this noise source in actual cells remains poorly understood. We measured variability in conduction time along the axons of identified neurons in the cercal sensory system of the cricket Acheta domesticus, and used information theory to calculate the effects of this variability on sensory coding. We found that the variability in spike propagation speed is not large enough to constrain the accuracy of neural encoding in this system

Additionally, innovative targeted therapies provide newer salvage options to consider.
