    Evaluating Quality Interactions in Preschool Math: Validation of a Video-based Observation Protocol

    Students who receive higher quality math instruction have better outcomes in math (Klibanoff et al. 2006; Kyriakides et al., 2008; Nye et al., 2004; Rockoff, 2004). However, examining math teaching quality, particularly in preschool classrooms, is complicated by the lack of validated measures. To fill this gap, we have validated a video-based observation protocol known as Evaluating Quality Interactions in Preschool-Math (EQUIP-M). EQUIP-M measures quality math instruction and focuses on three interactions: those between the teacher and the mathematics (Teacher Intentionality), those between the teacher and the students around the mathematics (Teacher Responsiveness), and those between the students and the mathematics (Student Mathematical Sense-making). The sample was comprised of preschool teachers (n=179) participating in a multi-year, professional development program evaluation. Teachers planned activities to engage students in mathematics and videographers recorded those activities in their entirety. Teachers were video-recorded up to three times within a two-week period at up to three timepoints (pre-intervention, post-intervention, one-year follow-up). Each video was scored by at least two certified raters. Raters watched each video in its entirety and then assigned a score for the eight EQUIP-M dimensions. Preliminary results indicate that raters can be trained to apply scoring rubrics accurately and consistently. To assess inter-rater reliability, we calculated an intraclass correlation coefficient of 0.84 using a two-way random effects model with absolute agreement. With further development and evaluation, EQUIP-M has the potential to identify strengths and weaknesses in teaching math and, ultimately, to inform educational professionals about how to improve teacher-child interactions around math

    A Behavioural Assessment of Social Anxiety and Social Motivation in Fragile X, Cornelia de Lange and Rubinstein-Taybi Syndromes

    Unique socio-behavioural phenotypes are reported for individuals with different neurodevelopmental disorders. Here, the effects of adult familiarity and nature of interaction on social anxiety and social motivation were investigated in individuals with fragile X (FXS; n = 20), Cornelia de Lange (CdLS; n = 20) and Rubinstein-Taybi (RTS; n = 20) syndromes, compared to individuals with Down syndrome (DS; n = 20). The Social Anxiety and Motivation Rating Scale was employed whilst participants completed four social tasks, each administered separately by a familiar adult, and also by an unfamiliar adult. Compared to participants with DS, those with FXS and RTS exhibited high levels of social anxiety but similar levels of social motivation. Participants with CdLS showed heightened social anxiety and reduced social motivation only during interactions with an unfamiliar adult when active participation was voluntary

    Implementing performance improvement in New Zealand emergency departments: the six hour time target policy national research project protocol

    <p>Abstract</p> <p>Background</p> <p>In May 2009, the New Zealand government announced a new policy aimed at improving the quality of Emergency Department care and whole hospital performance. Governments have increasingly looked to time targets as a mechanism for improving hospital performance and from a whole system perspective, using the Emergency Department waiting time as a performance measure has the potential to see improvements in the wider health system. However, the imposition of targets may have significant adverse consequences. There is little empirical work examining how the performance of the wider hospital system is affected by such a target. This project aims to answer the following questions: How has the introduction of the target affected broader hospital performance over time, and what accounts for these changes? Which initiatives and strategies have been successful in moving hospitals towards the target without compromising the quality of other care processes and patient outcomes? Is there a difference in outcomes between different ethnic and age groups? Which initiatives and strategies have the greatest potential to be transferred across organisational contexts?</p> <p>Methods/design</p> <p>The study design is mixed methods; combining qualitative research into the behaviour and practices of specific case study hospitals with quantitative data on clinical outcomes and process measures of performance over the period 2006-2012. All research activity is guided by a Kaupapa Māori Research methodological approach. A dynamic systems model of acute patient flows was created to frame the study. Consequences of the target (positive and negative) will be explored by integrating analyses and insights gained from the quantitative and qualitative streams of the study.</p> <p>Discussion</p> <p>At the time of submission of this protocol, the project has been underway for 12 months. This time was necessary to finalise both the case study sites and the secondary outcomes through key stakeholder consultation. We believe that this is an appropriate juncture to publish the protocol, now that the sites and final outcomes to be measured have been determined.</p

    Evolution of the Symbiotic Nova PU Vul -- Outbursting White Dwarf, Nebulae, and Pulsating Red Giant Companion

    We present a composite light-curve model of the symbiotic nova PU Vul (Nova Vulpeculae 1979) that shows a long-lasted flat optical peak followed by a slow decline. Our model light-curve consists of three components of emission, i.e., an outbursting white dwarf (WD), its M-giant companion, and nebulae. The WD component dominates in the flat peak while the nebulae dominate after the photospheric temperature of the WD rises to log T (K) >~ 4.5, suggesting its WD origin. We analyze the 1980 and 1994 eclipses to be total eclipses of the WD occulted by the pulsating M-giant companion with two sources of the nebular emission; one is an unocculted nebula of the M-giant's cool-wind origin and the other is a partially occulted nebula associated to the WD. We confirmed our theoretical outburst model of PU Vul by new observational estimates, that spanned 32 yr, of the temperature and radius. Also our eclipse analysis confirmed that the WD photosphere decreased by two orders of magnitude between the 1980 and 1994 eclipses. We obtain the reddening E(B-V) ~ 0.3 and distance to PU Vul d ~ 4.7 kpc. We interpret the recent recovery of brightness in terms of eclipse of the hot nebula surrounding the WD, suggesting that hydrogen burning is still going on.     Efficacy of autologous mesenchymal stromal cell treatment for chronic degenerative musculoskeletal conditions in dogs: a retrospective study

    Introduction: The objective of this study was to retrospectively analyze clinical data from a referral regenerative medicine practice, to investigate the efficacy of autologous mesenchymal stromal cells (MSC) in 245 dogs deemed unresponsive to conventional treatment by their referring vet. Methods: Diagnostic imaging [radiology and musculoskeletal ultrasound (MSK-US)] identified musculoskeletal pathology holistically. MSCs, produced according to current guidelines, were initially administered with PRP by targeted injection to joints and/or tendons, with a second MSC monotherapy administered 12 weeks later to dogs with severe pathology and/or previous elbow arthroscopic interventions. Dogs with lumbosacral disease received epidural MSCs with additional intravenous MSCs administered to dogs with spondylosis of the cervical, thoracic and lumbar spine. All dogs received laser therapy at 10 J/cm2 at the time of treatment and for 5 sessions thereafter. Objective outcome measures (stance analysis, range of joint motion, pressure algometry) and validated subjective outcome measures (owner reported VetMetrica HRQL™ and veterinary pain and quality of life impact scores) were used to investigate short and long-term (6–104 weeks) efficacy. Outcome data were collected at predetermined time windows (0–6, 7–12, 13–18, 19–24, 25–48, 49–78, 79–104) weeks after initial treatment. Results: There were statistically significant improvements in post compared with pre-treatment measures at all time windows in stance analysis, shoulder and hip range of motion, lumbosacral pressure algometry, and to 49–78 weeks in carpus and elbow range of motion. Improvements in 4 domains of quality of life as measured by VetMetricaTM were statistically significant, as were scores in vet-assessed pain and quality of life impact. In dogs receiving one initial treatment the mean time before a second treatment was required to maintain improvements in objective measures was 451 days. Diagnostic imaging confirmed the regenerative effects of MSCs in tendinopathies by demonstrating resolution of abnormal mineralization and restoration of normal fiber patterns. Discussion: This represents the first study using “real-world” data to show that cell-based therapies, injected into multiple areas of musculoskeletal pathology in a targeted holistic approach, resulted in rapid and profound positive effects on the patient's pain state and quality of life which was maintained with repeat treatment for up to 2 years
