Perceived consequences of the COVID-19 pandemic and childbearing intentions in Poland
Objective: We aim to investigate how the perceived consequences of COVID-19 affect people's childbearing intentions in Poland. Background: With the pandemic having an impact on virtually all spheres of people's lives, some evidence already exists that it will lead to fertility postponement, as people are reluctant to make their reproductive choices in such uncertain times. Method: We analyse a nationally representative sample of 1000 respondents aged 18-49. In the sample, 234 respondents declared that they had intended to have a child before the outbreak of the pandemic and about 20% of them stated having postponed or foregone their intention because of COVID-19. We perform logistic regression analyses to verify which perceived consequences of the pandemic are most decisive for this choice. We supplement our analyses with insights from qualitative, open-ended question on the effects of the pandemic. Results: We find that people’s decision to postpone childbearing is related to respondents' perceived lower sense of financial security and worse mental well-being in the pandemic. In the model, where both factors are included, only mental well-being remains significant. Qualitative analyses point to several other factors important to fertility decisions in the pandemic, e.g., women's fear of lonely childbirth. Conclusion: People’s decision to postpone childbearing because of COVID-19 is mostly related to pandemic-induced financial insecurity and this effect seems to be mediated by the psychological reaction to the situation.Fragestellung: Diese Studie untersucht, wie die wahrgenommenen Konsequenzen von COVID-19 die Fortpflanzungsabsichten der Polen beeinflussen. Hintergrund: Die Pandemie betrifft praktisch alle Lebensbereiche der Menschen. Es gibt bereits einige Hinweise darauf, dass sie zu einer aufgeschobenen Fertilität führen wird, da die Menschen in solch unsicheren Zeiten zögern, reproduktive Entscheidungen zu treffen. Methode: Wir analysieren eine bundesweit repräsentative Stichprobe von 1000 Befragten im Alter von 18-49 Jahren. In der Stichprobe der Studie gaben 234 Befragte an, dass sie vor dem Ausbruch der Pandemie beabsichtigten, ein Kind zu bekommen, und etwa 20% gaben an, dass sie diese Absicht aufgrund von COVID-19 verschoben oder aufgegeben haben. Wir führen logistische Regressionsanalysen durch, um zu testen, welche wahrgenommenen Folgen der Pandemie für diese Wahl entscheidend sind. Wir ergänzen unsere Analysen mit Erkenntnissen aus qualitativen, offenen Fragen zu den Auswirkungen der Pandemie. Ergebnisse: Wir finden, dass die Entscheidung der Menschen, ihre Pläne für Kinder zu verschieben, mit dem wahrgenommenen geringeren Gefühl der finanziellen Sicherheit und dem schlechteren psychologischen Wohlbefinden der Befragten während einer Pandemie zusammenhängt. In einem Modell, in dem beide Faktoren enthalten sind, bleibt nur das psychische Wohlbefinden signifikant. Qualitative Analysen weisen auf mehrere andere Faktoren hin, die für Fertilitätsentscheidungen in der Pandemie relevant sind, z.B. die Angst der Frauen, allein zu gebären. Schlussfolgerung: Die Entscheidung, die Empfängnis aufgrund von COVID-19 zu verschieben, hängt hauptsächlich mit der pandemiebedingten finanziellen Unsicherheit zusammen und scheint durch die psychologische Reaktion auf die Situation vermittelt zu werden
Effect of overexpression of β- and γ-actin isoforms on actin cytoskeleton organization and migration of human colon cancer cells
Actins are eukaryotic proteins, which are involved in diverse cellular functions including muscle contraction, cell motility, adhesion and maintenance of cell shape. Cytoplasmic actin isoforms β and γ are ubiquitously expressed and essential for cell functioning. However, their unique contributions are not very well understood. The aim of this study was to determine the effect of β- and γ-actin overexpression on the migration capacity and actin cytoskeleton organization of human colon adenocarcinoma BE cells. In cells overexpressing β- or γ-actin, distinct cytoskeletal actin rearrangements were observed under the laser scanning confocal microscope. Overexpressed actins localized at the submembranous region of the cell body, especially near to the leading edge and on the tips of pseudopodia. The cells transfected with plasmids containing cDNA for β- or γ-actin were characterized by increased migration and invasion capacities. However, the migration velocity was statistically significantly higher only in the case of γ-actin overexpressing cells. In conclusion, the increased level of β- or γ-actin leads to actin cytoskeletal remodeling followed by an increase in migration and invasion capacities of human colon BE cells. These data suggest that expression of both actin isoforms has an impact on cancer cell motility, with the subtle predominance of γ-actin, and may influence invasiveness of human colon cancer
Acromegaly in a patient with normal pituitary gland and somatotropic adenoma located in the sphenoid sinus
Ectopic acromegaly is a very rare clinical entity occurring in less than 1% of acromegalic patients. In most cases it is caused by GHRH or
rarely GH–secreting neoplasms. Even rarer are ectopic pituitary adenomas located in the sphenoid sinus or nasopharynx that originate
from pituitary remnants in the craniopharyngeal duct. This dissertation presents the difficulties in visualizing GH-secreting adenoma
located in the sphenoid sinus.
A 55-year-old man had somatic features of acromegaly for several years. MRI imaging revealed a slightly asymmetric pituitary gland
(14 ¥ 4 mm) without focal lesions. Simultaneously, a spherical mass, 10 mm in diameter, corresponding with ectopic microadenoma was
demonstrated on the upper wall of the sphenoid sinus. The serum GH level was 4.3 mg/l, IGF-1 = 615 mg/l, and a lack of GH suppression
with oral glucose was proven. After preliminary treatment with a long-acting somatostatin analogue, transsphenoidal pituitary tumour
removal was performed. Histopathological, electron microscopical and immunohistochemical analysis revealed densely granulated somatotropic
pituitary adenoma: GH(+), PRL(-), ACTH(-), TSH(-), FSH(-), LH(-), MIB1 < 1%, SSTR3(+) and SSTR5(+). Post-surgical evaluation
showed normal pituitary MRI scans, GH and IGF-1 levels 0.18 μg/l and 140 mg/l, respectively, as well as normal GH suppression with
oral glucose.
The careful analysis of possible pituitary embryonic malformations points out their significance for proper localization of extrapituitary
adenomas.Ektopowa postać akromegalii występuje u mniej niż 1% chorych. W większości przypadków jest ona spowodowana wydzielaniem GHRH
lub sporadycznie GH przez guz nowotworowy. Jeszcze rzadsze są ektopowo położone gruczolaki przysadki. Zlokalizowane najczęściej w
zatoce klinowej lub nosogardzieli, rozwijają się z fragmentu przysadki pozostałego w przewodzie czaszkowo-gardłowym. Prezentowana
praca ukazuje trudności w wizualizacji gruczolaka somatotropowego położonego w zatoce klinowej.
Mężczyzna w wieku 55 lat z somatycznymi cechami akromegalii od kilkunastu lat. W MRI przysadka nieco asymetryczna, 14 ¥ 4 mm, bez
zmian ogniskowych. Na górnej ścianie zatoki klinowej obecny kulisty obszar o średnicy 10 mm odpowiadający mikrogruczolakowi. Stężenie
GH = 4,3 μg/l, IGF-1 = 615 μg/l, brak hamowania GH glukozą. Po przygotowaniu długodziałającym analogiem somatostatyny guz
usunięto z dostępu przez zatokę klinową. Badania histopatologiczne, elektronomikroskopowe oraz immunohistochemiczne wykazały
obecność bogatoziarnistego somatotropowego gruczolaka przysadki: GH(+), PRL(-), ACTH(-), TSH(-), FSH(-), LH(-), MIB1 < 1%,
SSTR3(+), SSTR5(+). W ocenie pooperacyjnej: prawidłowy obraz przysadki w MRI, GH = 0,18 μg/l; IGF-1 = 140 μg/l, obecne hamowanie
GH glukozą.
Staranna analiza możliwych zaburzeń rozwoju embrionalnego przysadki potwierdza ich znaczenie dla prawidłowej lokalizacji pozaprzysadkowo
położonych gruczolaków
Disorders of carbohydrate metabolism and their relationship with the prevalence of cancer in patients with acromegaly – authors’ own observations
Background . Acromegaly is a rare chronic disease with an excessive secretion of growth hormone, which leads to characteristic changes in appearance, organ and metabolic complications and neoplasms, which are a significant cause of shortened survival time. One of the most common consequences of acromegaly is disturbed carbohydrate metabolism.
Objectives . The aim of this study was to assess the prevalence of disorders of carbohydrate tolerance and their relationship with the prevalence of malignant tumours in subjects with acromegaly.
Material and methods. The study group comprised 69 patients with confirmed acromegaly (26 M, 43 F), aged 26–83 (mean 58.9 ± 11.0). Medical histories and the results of laboratory tests – plasma fasting glucose and oral glucose tolerance test, as well as the results of imaging and histopathological examinations performed in cases of suspected cancer lesions were analyzed.
Results Disorders of carbohydrate tolerance were confirmed in a total of 46 patients (66.7%). 23 patients (33.3%) had normal glucose tolerance. Diabetes was diagnosed in 22 patients (31.9%), while prediabetes was diagnosed in 24 (34.8%) patients. In the analyzed group there were 6 cases of cancers, including 5 patients with concomitant diabetes, which represented a value significantly higher (p = 0.01) compared to patients without diabetes. The sixth case of cancer was detected in a woman with IGT . In patients with normal glucose tolerance there was no case of cancer.
Conclusions . Among patients with acromegaly, disorders of carbohydrate tolerance occur in more than half of the cases, which confirms the need for active screening in this group of patients, in which a significant role is played by General Practitioners. General Practitioners should also consider the possibility of acromegaly in the differential diagnosis of glucose tolerance disorders detected in their patients. All patients with acromegaly, especially with concomitant diabetes or prediabetes, should be screened for cancers
Type and frequency of autoimmune diseases coexisting with LADA
Wstęp. Liczne doniesienia dowodzą częstego współistnienia cukrzycy typu 1 i chorób z autoagresji (AD).
Cel pracy. Ustalenie rodzaju chorób autoimmunizacyjnych i częstości ich współwystępowania z cukrzycą LADA w materiale własnym.
Materiał i metody. 32 chorych (17 K; 15 M), w wieku 28–77 lat (56,3 ± 12,4) z rozpoznaniem L ADA potwierdzonym obecnością przeciwciał anty-GAD. Retrospektywna analiza obrazu klinicznego, chorób współistniejących, badań hormonalnych i immunologicznych oraz leczenia.
Wyniki. Współistnienie AD i LADA stwierdzono u 46,9% chorych (n = 15; 11 K i 4 M). U 12 osób z LADA (37,5%) rozpoznano jedną z chorób autoimmunizacyjnych tarczycy: u ośmiu chorobę Hashimoto (25%), u czworga chorobę Gravesa (12,5%). U czworga chorych (12,5%) cukrzycy towarzyszyło bielactwo, u jednej łuszczyca, u dwojga niedokrwistość A–B. U 11 chorych (34,4%) współistniała z LADA tylko jedna choroba z autoagresji: u pięciu choroba Hashimoto, u trojga choroba Gravesa, u pozostałych trojga odpowiednio łuszczyca, bielactwo i niedokrwistość A–B. Równoczesne współistnienie trzech chorób z autoagresji stwierdzono u dwojga chorych, cztery choroby z autoagresji: u jednego chorego; i pięć AD u jednego chorego. Autoimmunologiczny zespół wielogruczołowy (AZW) rozpoznano u 12 chorych, u 11 chorych — AZW typu 3 (LADA + wole Hashimoto u siedmiorga i L ADA wraz z chorobą Gravesa u czterech chorych). U jednej chorej L ADA była składową APS typu 2 i współistniała z chorobą Addisona, wolem Hashimoto, niedoczynnością przytarczyc i przedwczesnym wygasaniem jajników. Porównano częstość współwystępowania LADA i AD w dwóch grupach chorych, różniących się mianem przeciwciał anty-GAD.
Wniosek. Współistnienie chorób z autoagresji, najczęściej chorób tarczycy, stwierdzano u około połowy chorych z LADA, częściej u kobiet i osób z wyższym mianem anty-GAD.Introduction. Several data confirm frequent coexistence of type 1 diabetes and autoimmune diseases (AD).
Aim of the study. Aim of the study was to determine the type and frequency of AD coexisting with LADA.
Material and methods. 32 patients (17 F; 15 M) aged 28–77 y (mean 56.3 ± 12.4) with LADA, confirmed by the presence of anti-GAD antibodies. A retrospective analysis of the clinical picture, medical history, hormonal and immunological tests and treatment.
Results. The coexistence of LADA and AD was found in 46.9% of cases (n = 15; 11 F, 4 M). In 12 patients with LADA (37.5%) it was one of autoimmune thyroid diseases: in 8 (25%) Hashimoto’s disease and in 4 (12.5%) — Graves’ disease. In 4 pts (12.5%) diabetes was accompanied by vitiligo, in 1 — by psoriasis and in 2 — by anemia A–B. In 11 pts (34.4%) LADA coexisted with only one AD. In 5 cases it was Hashimoto’s thyroiditis, in 3 — Graves’ disease and in the remaining 3 respectively: psoriasis, vitiligo and anemia A–B. The coexistence of 3 AD was found in 2 pts, 4 AD — in 1 patient and 5 AD — in 1 patient. Autoimmune polyglandular syndrome (APS) was diagnosed in 12 pts: APS type 3 in 11 cases (LADA + Hashimoto in 7 pts and LADA + Graves’ disease in 4 pts.). In one case LADA was a part of APS type 2 and coexisted with Addison’s disease, Hashimoto goiter, hypoparathyroidism and premature ovarian failure. We compared the frequency of co-occurrence of LADA and AD in 2 groups of pts, depending on the anti-GAD.
Conclusion. The incidence of AD, most common thyroid disease, was found in about half of the patients with LADA, more frequently in women and people with higher anti-GAD antibodies
Jakość życia pacjentów z cukrzycą typu 2 po wdrożeniu insulinoterapii — badanie prospektywne
Introduction. In case of many patients with type 2 diabetes, despite deterioration of glycaemic control the moment of intensification of treatment is delayed because of barriers to initiating insulin therapy. The aim of the study was an evaluation of changes on health related quality of life (HRQoL) after intensification of the treatment among group of type 2 diabetes patients.
Methods. The study involved 52 patients, treated so far by oral anti-diabetic drugs (OAD), for whom insulin was introduced. This group was exposed to 8 (± 1.5) months long observation. HRQoL was measured with questionnaires: EQ-5D, DQL-BCI and DSC-R.
Results. After prospective observation, patients described significant improvement in QoL (EQ-VAS, DSC-R). DQL-BCI score remained unchanged, although detailed analysis showed improvement in treatment satisfaction, improvement in the level of knowledge about diabetes and increase in diabetes-related pain.
Conclusions. Our study confirms the improvement in HRQoL after insulin therapy introduction. The results should encourage to early implementation of intensification of treatment. Wstęp. W przypadku wielu pacjentów z cukrzycą typu 2, pomimo obserwowanego pogorszenia kontroli glikemii, dochodzi do opóźnienia intensyfikacji leczenia ze względu na bariery przed wdrożeniem insulinoterapii. Celem badania była ocena zmian w zakresie zależnej od zdrowia jakości życia wśród pacjentów z cukrzycą typu 2 po wdrożeniu insulinoterapii.
Metody. Badaniem objęto 52 chorych z cukrzycą typu 2, dotychczas leczonych doustnymi lekami przeciwcukrzycowymi, u których zintensyfikowano leczenie poprzez dołączenie insuliny do dotychczas podawanych leków doustnych lub zastosowano monoterapię insuliną. Chorych poddano 8 (± 1,5)-miesięcznej obserwacji. Do pomiaru jakości życia zastosowano kwestionariusze: EQ-5D, DQL-BCI oraz DSC-R.
Wyniki. Po okresie prospektywnej obserwacji stwierdzono istotną poprawę w zakresie jakości życia (EQ- -VAS, DSC-R). Wynik oceniany za pomocą kwestionariusza DQL-BCI nie uległ zmianie, jednakże szczegółowa analiza wykazała poprawę w zakresie satysfakcji z leczenia, wzrost poziomu wiedzy na temat cukrzycy, a także wzrost częstości odczuwania bólu związanego z leczeniem cukrzycy.
Wnioski. Niniejsze badanie potwierdziło poprawę w zakresie zależnej od zdrowia jakości życia po wdrożeniu insulinoterapii, co powinno stanowić zachętę do podejmowania wczesnej intensyfikacji leczenia u pacjentów z pogorszeniem kontroli glikemii
Kontrola metaboliczna i realizacja zaleceń PTD przed wdrożeniem i po wdrożeniu insulinoterapii u chorych na cukrzycę typu 2
INTRODUCTION. Gradual exhaustion of secretion
abilities of b cell in diabetes type 2 is inexorably connected
with worse metabolic control and necessity
to incorporate insulin therapy. The aim was to assess
the improvement of metabolic control after insulin
treatment initiation.
MATERIAL AND METHODS. The research was conducted
on a group of 52 patients with diabetes type 2
— 30 female and 22 male, aged 44–79 (mean 63.1
± 10.1). The treatment of patients in mentioned group
was intensified by administration of insulin along
with so far administered oral medicines, and exposing
them to 8 ± 1.5 months long observation.
RESULTS. After administration of insulin along with
so far administered oral medicines a significant average
reduction of HbA1c level was achieved: from
7.9% to 7.5%, which means that percentage of patients
with HbA1c £ 7.0% increased from 28.8% (n =
= 15) to 44.3% (n = 23). The percentage of patients
with HbA1c £ 6.5% remained unchanged and amounted
to 19.2% (n = 10). There was observed an increase
of percentage of patients with BP systolic
< 130 mm Hg from 19.2% (n = 10) to 30.8% (n = 16)
and BP diastolic < 80 mm Hg from 15.4% (n = 8) to
26.9% (n = 14). When it comes to lipid management
the researchers observed slight increase of percentage
of patients who met compensation criteria: total
cholesterol concentration < 175 mg/dL from 34.6%
(n = 18) to 40.4% (n = 21), HDL > 40 mg/dL from
55.8% (n = 29) to 67.3% (n = 35), LDL < 100 mg/dL
from 38.5% (n = 20) to 40.4% (n = 21) and TG < 150
mg/dL from 50% (n = 26) to 51.9% (n = 27).
CONCLUSIONS. The results of the study confirmed
that insulin therapy had a positive impact on the
level of metabolic control in patients with unstable
type 2 diabetes, which was visible after only a few
months of treatment. These results should encourage
both patients and their doctors to opt for insulin

sekrecyjnych komórki b w cukrzycy typu 2 wiąże się
z pogorszeniem kontroli metabolicznej oraz potrzebą
rozpoczęcia insulinoterapii. Celem badania była ocena
poprawy kontroli metabolicznej po włączeniu leczenia
insuliną.
MATERIAŁ I METODY. Badaniem objęto 52 chorych
na cukrzycę typu 2 — 30 kobiet i 22 mężczyzn
w wieku 44–79 lat (średnio 63,1 ± 10,1 roku). Leczenie
zintensyfikowano poprzez dołączenie insuliny do
dotychczas stosowanych leków doustnych lub zastosowano
monoterapię insuliną. Badanych poddano
8 ± 1,5-miesięcznej obserwacji.
WYNIKI. Osiągnięto istotną redukcję średniego stężenia
hemoglobiny glikowanej (HbA1c) z 7,9% do
7,5%, w związku z czym odsetek osób z HbA1c £ 7,0%
wzrósł z 28,8% (n = 15) do 44,3% (n = 23). Odsetek
osób z HbA1c £ 6,5% nie zmienił się i wynosił 19,2%
(n = 10). Zaobserwowano wzrost odsetka osób
z ciśnieniem skurczowym < 130 mm Hg z 19,2%
(n = 10) do 30,8% (n = 16) oraz ciśnieniem rozkurczowym
< 80 mm Hg z 15,4% (n = 8) do 26,9%
(n = 14). W zakresie kontroli gospodarki lipidowej
zanotowano niewielki wzrost odsetka osób spełniających
kryteria wyrównania: stężenie cholesterolu całkowitego
< 175 mg/dl z 34,6% (n = 18) do 40,4%
(n = 21), cholesterolu frakcji HDL > 40 mg/dl z 55,8%
(n = 29) do 67,3% (n = 35), cholesterolu frakcji LDL
< 100 mg/dl z 38,5% (n = 20) do 40,4% (n = 21)
oraz triglicerydów < 150 mg/dl z 50% (n = 26) do
51,9% (n = 27).
WNIOSKI. Uzyskane wyniki potwierdzają korzystny,
widoczny już po kilku miesiącach leczenia wpływ insulinoterapii
na poziom wyrównania metabolicznego
u chorych na niewyrównaną cukrzycę typu 2.
Powinny one stanowić zachętę do przełamania oporów
przed zastosowaniem leczenia insuliną zarówno
u samych pacjentów, jak i wśród prowadzących

Thyroid diseases in patients with acromegaly — own observations
Wstęp. Nadmiar hormonu wzrostu (GH) w przebiegu akromegalii prowadzi do charakterystycznych zmian wyglądu zewnętrznego, jak też rozwoju powikłań narządowych i metabolicznych oraz nowotworów. Jednym z najczęstszych następstw jest powiększenie tarczycy, a zwłaszcza wole guzowate.
Cel pracy. Ocena częstości występowania chorób tarczycy u chorych na akromegalię.
Materiał i metody. Badaną grupę stanowiło 62 chorych (23 M, 39 K), w wieku 25–82 lata. Analizowano dokumentację medyczną, wyniki badań obrazowych i hormonalnych tarczycy oraz obecność przeciwciał.
Wyniki. Patologię tarczycy wykryto u 48 chorych (77,42%), w tym u 89,74% spośród 39 kobiet i u 56,52% spośród 23 mężczyzn. Najczęściej rozpoznawano wole guzowate z eutyreozą (50%). Wole guzowate nadczynne stwierdzono u 19,35%. Przewlekłe autoimmunologiczne zapalenie tarczycy i wole proste wykryto w pojedynczych przypadkach. Nie potwierdzono żadnego przypadku raka tarczycy.
Wnioski. Patologia tarczycy jest bardzo częstym objawem akromegalii i może dotyczyć zarówno zmian w jej morfologii, jak i czynności. Każdy chory w chwili rozpoznania akromegalii powinien przejść wnikliwą diagnostykę tyreologiczną i być objęty odpowiednim leczeniem lub obserwacją, w czym duża rola lekarza podstawowej opieki zdrowotnej (POZ). W każdym przypadku należy dążyć do wykluczenia raka tarczycy.Introduction. One of the most common consequences of the disease is the enlargement of the thyroid gland, especially nodular goiter.
Aim of the study. Evaluate the prevalence of various diseases of the thyroid in patients with acromegaly.
Material and methods. The study group covered 62 patients (23 M, 39 F), aged 25–82 years. We analyzed the medical histories, results of imaging and hormonal studies and the presence of antibodies.
Results. Thyroid pathology was detected in 48 patients (77.42%), including 89.74% out of 39 female and 56.52% out of 23 male. Most often we observed euthyroid nodular goiter (50%). Toxic nodular goiter was found in 19.35%. Chronic autoimmune thyroiditis and simple goiter were detected in few cases. There wasn’t any case of thyroid cancer.
Conclusions. The pathology of the thyroid is a very common symptom of acromegaly and may involve changes in the morphology and function. At the moment of the diagnosis of acromegaly each patient should undergo a thyroid evaluation and should be treated or followed not only by a specialist but also by GP. In any case, efforts should be made to exclude thyroid cancer

Celem badania jest przedstawienie dwóch przypadków pacjentów z orbitopatią IgG4.Materiał i metody: Analizy dokonano w oparciu o dokumentację medyczną pacjentówPrzypadek 1: 68-mężczyzna został przyjęty do poradni endokrynologicznej z powodu narastającego wytrzeszczu obu gałek ocznych. Badanie MRI potwierdziło powiększenie i obrzęk wszystkich mięśni okoruchowych. Inne choroby, w przebiegu których dochodzić może do orbitopatii, w tym choroba Graves’a, zostały wykluczone. W oparciu o podwyższony poziom przeciwciał IgG4 w surowicy krwi, rozpoznano chorobę IgG4-zależną. Rozpoczęto leczenie glikokortykosteroidami, w wyniku czego doszło do znacznej regresji zmian w oczodołach. Leczenie powtórzono po upływie roku z powodu nawrotu objawów, z następowym leczeniem metotreksatem. Do chwili obecnej pacjent pozostaje w całkowitej remisji klinicznej, kontynuuje leczenie metotreksatem z dobrą tolerancją.Przypadek 2: 62-letnia kobieta, z chorobą Graves’a, po operacji usunięcia guza z prawego oczodołu (hist-pat: sclerosing inflammatory pseudotumor), została przyjęta do Kliniki Endokrynologii z powodu nawrotu wytrzeszczu oka prawego. Wykluczono orbitopatię w przebiegu choroby Graves’a. Na podstawie podwyższonego poziomu przeciwciał IgG4 w surowicy oraz obrazu histopatologicznego usuniętej uprzednio zmiany oczodołu rozpoznano chorobę IgG4-zależną. Rozpoczęto leczenie glikokortykosteroidami, z następową terapią metotrexatem oraz azatiopryną. Pacjentka do chwili obecnej pozostaje w remisji choroby.Wnioski: W przypadku cech orbitopatii w diagnostyce różnicowej należy brać pod uwagę możliwość rzadko występującej patologii, jaką są choroby IgG4- zależne. Słowa kluczowe: choroby IgG4-zależne, orbitopatia, glikokortykosteroidy Abstract Introduction: Immunoglobulin-G4-related disease (IgG4-RD) is a multidisciplinary problem due to plasmatic cells infiltration and areas of fibrosis in the affected tissues, including the eye.The aim of the study was to present two patients with orbitopathy due to IgG4-related eye disease.Material and methods: Analysis of medical records of patients.Case 1: A 68-year old man, was admitted to the endocrinology clinic because of increased protrusion of both eyeballs. The MRI scans revealed enlargement and swallowing of all periorbital muscles. Common diseases of the eyeballs, including Graves disease were excluded. Based on elevated serum IgG4, IgG4-related disease was stated. The glicocorticosteroid therapy was introduced. Evident regression of protrusion of eyeballs was observed. The therapy was repeated one year later because of recurrence of symptoms and methotrexate therapy was added to this. The patient continues now methotrexate therapy only and does not present any symptoms of orbitopathy. Case 2: A 62-years old woman with Graves disease, after surgery of the tumor of the right orbit, was admitted to the Endocrinology Clinic because of right eye protrusion rebound. The Graves orbitopathy was ruled out. Since IgG4 level in plasma was elevated and considering histopathological recognition of the removed tumor of the orbit (sclerosing inflammatory pseudotumor), IgG4-related disease of the eye was stated. Glucocorticosteroids were introduced, followed by methotrexate and azathioprine therapy. The patient remains in remission now.Conclusion: Presented cases highlight the importance of considering IgG4-RD in the differential diagnosis of orbitopathy.
