Atención prequirúrgica del hipertenso eventual

Se investigaron pacientes con antecedentes de haber elevado sus cifras tensionales ante situaciones estresantes, pero que habitualmente eran normotensos (hipertensos eventuales). Este tipo de paciente constituye una causa frecuente de suspensión del acto quirúrgico por hipertensión arterial. Se propuso utilizar la prueba de estimulación por frío, ante la necesidad de identificar el comportamiento tensional operatorio de estos pacientes y por su probada sensibilidad y especificidad como prueba de provocación de hipertensión arterial y equivalente al estrés quirúrgico, para formular un pronóstico de comportamiento tensional transoperatorio. Se trabajó sobre una muestra de 30 sujetos, a todos se les realizó la prueba, 24 tuvieron resultados positivos y se les indicó tratamiento individualizado, 6 fueron negativas, no se prescribió tratamiento por no expresar hiperreactividad. No se produjo hipertensión perioperatoria, ni suspensiones; se concluyó que la prueba es un valioso instrumento para la predicción tensional transoperatoria y auxiliar en la desición de prescribir hipotensores.Some patients who had previously had elevated arterial tension resulting from stressing situations, but who were usually normotensive (occasional hypertensives) were studied. This type of patient is the most frequent cause of suspension of the surgical procedure as a result of arterial hypertension. It was proposed to use the cold pressure test due to the need of identifying the tensional operative behaviour among these patients and to its sensitivity and specificity capable of producing an arterial hypertension similar to the one caused by surgical stress. It allows to have a prognosis of the tensional transoperative behavior. 30 individuals were included in the sample and all of them underwent this test. 24 had positive results and were indicated individualized treatment. The other 6 were negative and no treatment was prescribed since no hyperreactivity was observed. There was neither perioperative hypertension nor suspensions. It was concluded that the test is a valuable tool for tensional transoperative prediction and that it is also useful at the time of prescribing hypotensors

AuI-Catalyzed Hydroalkynylation of Haloalkynes

The AuI-catalyzed reaction between terminal alkynes and aromatic haloalkynes proceeds through divergent pathways depending on the nature of the catalyst counteranion. Thus, cationic complexes containing strongly basic NHC ligands and noncoordinating anions such as BArF4 catalyze the cis haloalkynylation of the terminal alkyne, whereas introduction of a weakly basic triflate counteranion results in the stereoselective hydroalkynylation of the haloalkyne, yielding haloenyne products in good yields and complete trans selectivity. Experimental and computational studies suggest that the hydroalkynylation reaction takes place via nucleophilic attack of the terminal alkyne to the C2 carbon of the activated haloalkyne, assisted by a concerted proton abstraction by the triflate, and that the protodeauration is the turnover-limiting step, in agreement with an observed primary kinetic isotope effect

Studies of Acute and Chronic Toxicity of Commercial Herbicides with Glyphosate against Danio rerio

Glyphosate or N-(phosphonomethyl) glycine is a broad spectrum non-selective systemic herbicide, used to kill weeds, mainly in soybean crops. Nowadays there are many controversies about the intensive use of these herbicides due to the potential environmental impact and the effects on human health. The environmental impacts of commercial Glyphosate formulation Roundup were assessed by evaluation of acute and chronic toxicity of Danio rerio fish.The effects of glyphosate commercial formulations and glyphosate isopropylamine salt solutions were evaluated in different steps. First the lethal doses of Roundup herbicide toward the experimental models were determined. Subsequently the specie was exposed to sublethal concentrations of both, the commercial preparations and pure glyphosate salt in order to evaluate the chronic toxicity.Acute toxicity was assessed by calculating the mortality indexes and chronic toxicity by measuring several biochemical parameters such as the activity of marker enzymes like aspartate aminotransferase (AST), alanine aminotransferase (ALT) and acetylcholinesterase (AchE). The registers of histological alterations in liver tissue sections were also considered in this study.Commercial herbicides produce mortality of D. rerio and sublethal doses of these herbicides and the salt of the pure compound produce effects of chronic toxicity at the liver and muscle level; such are enough causes to limit the potential survival of these organisms in the medium.Fil: Jofré, Diego Martín. CINCO AÑOS DE EXPERIENCIA CON MISOPROSTOL INTRAVAGINAL PARA LA INDUCCION DE PARTO. USO DE UNA NUEVA PRESENTACION FARMACEUTICA EN SUPOSITORIOS

Se presentan los resultados generales de 1534 casos con diversas patologías inducidos con misoprostol en tabletas, 50 µg intravaginal cada 8 horas por dos veces al día, durante los años 1997 al 2001 en el Servicio de Obstetricia y Ginecología del Hospital Las Higueras de Talcahuano, destacando la baja incidencia de cesárea (6,3%) y de Apgar bajo a los 5 minutos (0,5%). En 47 casos se utilizó en forma prospectiva y randomizada para la inducción (enero a marzo de 2002), una nueva presentación farmacéutica (supositorios) aún no comercializada, la cual produjo un alto porcentaje de éxito a las dosis de 50 µg cada 8 horas y 100 µg cada 12 horas, hasta por un período de 48 horas: 96% y 81,8% respectivamente, con un bajo porcentaje de cesáreas (7,1%) y de polisistolia (6,4%) y ningún recién nacido deprimid
