    The value of D-dimer in the detection of early deep-vein thrombosis after total knee arthroplasty in Asian patients: a cohort study

    <p>Abstract</p> <p>Background and purpose</p> <p>The relationship of D-dimer and deep-vein thrombosis (DVT) after total knee arthroplasty (TKA) remains controversial. The purpose of this study was to assess the value of D-dimer in the detection of early DVT after TKA.</p> <p>Methods</p> <p>The measurements of plasma D-dimer level were obtained preoperatively and at day 7 postoperatively in 78 patients undergoing TKA. Ascending venography was performed in 7 to 10 days after surgery. The plasma D-dimer levels were correlated statistically with the venographic DVT.</p> <p>Results</p> <p>Venographic DVT was identified in 40% of patients. High plasma D-dimer level >2.0 ÎŒg/ml was found in 68% of patients with DVT and 45% without DVT (P < 0.05). Therefore, high D-dimer level greater than 2.0 ÎŒg/ml showed 68% sensitivity, 55% specificity, 60% accuracy, 50% positive predictive rate and 72% negative predictive rate in the detection of early DVT after TKA.</p> <p>Conclusion</p> <p>High plasma D-dimer level is a moderately sensitive, but less specific marker in the detection of early of DVT after TKA. Measurement of serum D-dimer alone is not accurate enough to detect DVT after TKA. Venography is recommended in patients with elevated D-dimer and clinically suspected but asymptomatic DVT after TKA.</p

    Why Do Players Stick to a Specific Online Game? The Users and Gratifications Perspective

    Driven by the dominant Internet usage and the prospective profits from the game industry, especially from the thriving and robust free-to-play model of online games, there is a need to realize players’ behaviors. Playing online games is experienceoriented but rare studies further explore what reactions of initial (trial) experiences in game playing are and how they will further influence players’ behaviors. Uses and gratification theory can be seen in cases such as online games selection. Players select an online game not only to fit particular interests but also to attempt to show empowerment or other socially conscience motives. This study, therefore, seeks to explore the important antecedents (i.e. gratifications, presence, service mechanisms, and continuance motivation) of stickiness intention on the online game and examine the associated relationships among them. The implications of findings to both researchers and practitioners are also discussed

    Toona Sinensis Extracts Induced Cell Cycle Arrest and Apoptosis in the Human Lung Large Cell Carcinoma

    Toona sinensis extracts have been shown to exhibit anti-cancer effects in human ovarian cancer cell lines, human promyelocytic leukemia cells and human lung adenocarcinoma. Its safety has also been confirmed in animal studies. However, its anti-cancer properties in human lung large cell carcinoma have not been studied. Here, we used a powder obtained by freeze-drying the super-natant of centrifuged crude extract from Toona sinensis leaves (TSL-1) to treat the human lung carcinoma cell line H661. Cell viability was evaluated by the 3-(4-,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide assay. Flow cytometry analysis revealed that TSL-1 blocked H661 cell cycle progression. Western blot analysis showed decreased expression of cell cycle proteins that promote cell cycle progression, including cyclin-dependent kinase 4 and cyclin D1, and increased the expression of proteins that inhibit cell cycle progression, including p27. Furthermore, flow cytometry analysis showed that TSL-1 induced H661 cell apoptosis. Western blot analysis showed that TSL-1 reduced the expression of the anti-apoptotic protein B-cell lymphoma 2, and degraded the DNA repair protein, poly(ADP-ribose) polymerase. TSL-1 shows potential as a novel therapeutic agent or for use as an adjuvant for treating human lung large cell carcinoma

    Cross-Layer Optimization for Efficient Delivery of Scalable Video over WiMAX

    Abstract. Video delivery application over wireless network has become increasingly important in recent years. For achieving more flexible video streams transmission, the scalable extension of the H.264/AVC standard has been developed, as well as which is combined with temporal, spatial and quality scalabilities to adapt efficiently the wireless bandwidth resource. In this paper, a cross-layer optimization for robust H.264 scalable video delivery over WiMAX network is proposed. In order to obtain a better delivery performance, the proposed method is used the hierarchical B-picture approach to classify video packets into different priorities. In the cross-layer optimization, the scalable video and packet classifier are proposed at the application layer, and the packet priority and queue allocation are used at the MAC layer. Finally, it is shown by NS2 simulations that the proposed method provides more stable wireless bandwidth and also achieves a better quality of the video streaming delivery

    Is ChatGPT A Good Translator? Yes With GPT-4 As The Engine

    This report provides a preliminary evaluation of ChatGPT for machine translation, including translation prompt, multilingual translation, and translation robustness. We adopt the prompts advised by ChatGPT to trigger its translation ability and find that the candidate prompts generally work well with minor performance differences. By evaluating on a number of benchmark test sets, we find that ChatGPT performs competitively with commercial translation products (e.g., Google Translate) on high-resource European languages but lags behind significantly on low-resource or distant languages. As for the translation robustness, ChatGPT does not perform as well as the commercial systems on biomedical abstracts or Reddit comments but exhibits good results on spoken language. Further, we explore an interesting strategy named pivot prompting\mathbf{pivot~prompting} for distant languages, which asks ChatGPT to translate the source sentence into a high-resource pivot language before into the target language, improving the translation performance noticeably. With the launch of the GPT-4 engine, the translation performance of ChatGPT is significantly boosted, becoming comparable to commercial translation products, even for distant languages. Human analysis on Google Translate and ChatGPT suggests that ChatGPT with GPT-3.5 tends to generate more hallucinations and mis-translation errors while that with GPT-4 makes the least errors.     Utilization of Western Medicine and Traditional Chinese Medicine Services by Physicians and Their Relatives: The Role of Training Background

    Despite extensive efforts to improve the attitude and practice of physicians with respect to complementary and alternative medicine (CAM), the role of training background on physician's own utilization of mainstream Western medicine (WM) and CAM remains unclear. We aimed to compare personal utilizations of WM and traditional Chinese medicine (TCM) among doctors trained in WM only, TCM only or both. A retrospective population-based study was conducted using the 2004 Taiwan's National Health Insurance data. A total of 103 879 doctors and their relatives and 2 623 658 other adults with equivalent socioeconomic status were analyzed. Ambulatory care utilization of WM and TCM services was compared using the following three measures: probability of any use, number of visits and total annual expenditure. Doctors who were trained in Western medicine only (WMDs) had the highest WM use, followed by doctors who were trained in both (WMD-CMDs), while Chinese medicine-trained doctors (CMDs) had the lowest use. For TCM use, a reverse pattern was observed. Similar patterns were found among doctors' relatives. Compared with other adults with equivalent socioeconomic status, both the CMDs and WMD-CMDs had a greater use of TCM services. For WM, although the WMDs' probability and frequency of usage were similar to other adults, they incurred considerably higher expenditure. The use of WM and TCM by doctors and their relatives was significantly associated with the training background of the doctors. This highlights the importance of how increasing knowledge and understanding of other medical discipline may influence a practitioner's care-providing behaviors

    A SEEMINGLY UNRELETED REGRESSION ANALYSIS ON THE TRADING BEHAVIOR OF MUTUAL FUND INVESTORS

    This paper provides a comprehensive investigation on the causality relationship between fund performance and trading flows. We analyze if investors behave asymmetrically in fund purchasing and selling by seemingly unrelated regression which comprises several individual relationships that are linked by the fact that their disturbances or the error terms are correlated. The empirical result shows a significantly negative relationship between fund performance and purchase flows for domestic funds. The magnitude of domestic funds redemption negatively affects current return, but not for international funds. As previousfund return positively affects current net flows,the further lagged performances have no significant impact on the trading flows, revealing that fund investors are sensitive only to short-term past performance. Most importantly, while negative fund performance leads to the increases in redemption, positive performance contrarily leads to the decreases in purchase. The evidences strongly indicate an asymmetry behavior of fund investors in the return-purchase causality relations

    Fungal members of Colletotrichum (Ascomycota) were found to be associated with Chordodes formosanus, one of the three currently known horsehair worm (Nematomorpha) species in Taiwan. The fungi were identified as Colletotrichum fructicola, which is mostly known as a plant pathogen, through the use of the nuclear ribosomal internal transcribed spacer and partial large subunit (nrITS + nrLSU) and glyceraldehyde-3-phosphate dehydrogenase (GAPDH) DNA sequences. To our knowledge, this report represents both the first records for Colletotrichum associated with hairworms and for fungi on Nematomorpha. These findings expand the knowledge on the ecological relationships of both clades

    A solar cell powered Li-ion battery charger in CMOS 0.25um is proposed. The solar battery charger consists of a DC/DC boost converter and a battery charger. The voltage generated by a solar cell is up converted from 0.65V to 1.8V, which is used as the VDD of the battery charger.  In this way, the solar battery charger automatically converts solar energy to electricity and stores it directly to a Li-ion rechargeable battery. In this system, a super capacitor is needed as a charge buffer between the boost converter and the charger.<br /
