    Impact of Genetic Discoveries on the Classification of Lone Atrial Fibrillation

    Atrial fibrillation (AF), the most common sustained cardiac arrhythmia, represents a major burden to patients and health care systems through its sequelae of heart failure and stroke. Its age-dependent increase in prevalence has led to worrisome predictions of an expanding burden secondary to our aging population. This growing epidemic is further exacerbated by a current lack of highly effective therapies for the arrhythmia stemming from our incomplete understanding of its complex pathophysiology. Recent genetic studies, triggered in part by evidence of a hereditary component of AF, have begun to identify predisposing genes and offer further insights into the mechanisms of lone AF. A variety of ion channels and most recently a circulating hormone have been implicated. The apparent genetic diversity underlying the arrhythmia has served to emphasize the heterogeneity of factors that govern its initiation and maintenance. The different causative genes seem to predispose to AF through distinct putative mechanisms, including enhanced and delayed atrial action potential repolarization, cellular hyperexcitability, and conduction velocity heterogeneity. Classification of lone AF into mechanistic subgroups serves to emphasize its heterogeneity and has the potential to guide developmental and clinical treatment strategies. The frequent recalcitrant nature of the arrhythmia to contemporary pharmacological and invasive therapies may be overcome through an ability to identify, through genetics, the mechanistic subclass of AF for an individual patient. Proper identification of the culprit pathophysiology may permit administration of a targeted form of therapy that carries maximal efficacy and minimal risk in a manner consistent with the vision of pharmacogenomics

    The Short QT Syndrome Proposed Diagnostic Criteria

    ObjectivesWe aimed to develop diagnostic criteria for the short QT syndrome (SQTS) to facilitate clinical evaluation of suspected cases.BackgroundThe SQTS is a cardiac channelopathy associated with atrial fibrillation and sudden cardiac death. Ten years after its original description, a consensus regarding an appropriate QT interval cutoff and specific diagnostic criteria have yet to be established.MethodsThe MEDLINE database was searched for all reported cases of SQTS in the English language, and all relevant data were extracted. The distribution of QT intervals and electrocardiographic (ECG) features in affected cases were analyzed and compared to data derived from ECG analysis from general population studies.ResultsA total of 61 reported cases of SQTS were identified. Index events, including sudden cardiac death, aborted cardiac arrest, syncope, and/or atrial fibrillation occurred in 35 of 61 (57.4%) cases. The cohort was predominantly male (75.4%) and had a mean QTc value of 306.7 ms with values ranging from 248 to 381 ms in symptomatic cases. In reference to the ECG characteristics of the general population, and in consideration of clinical presentation, family history, and genetic findings, a highly sensitive diagnostic scoring system was developed.ConclusionsBased on a comprehensive review of 61 reported cases of the SQTS, formal diagnostic criteria have been proposed that will facilitate diagnostic evaluation in suspected cases of SQTS. Diagnostic criteria may lead to a greater recognition of this condition and provoke screening of at-risk family members

    LunaNet: a Flexible and Extensible Lunar Exploration Communications and Navigation Infrastructure

    NASA has set the ambitious goal of establishing a sustainable human presence on the Moon. Diverse commercial and international partners are engaged in this effort to catalyze scientific discovery, lunar resource utilization and economic development on both the Earth and at the Moon. Lunar development will serve as a critical proving ground for deeper exploration into the solar system. Space communications and navigation infrastructure will play an integral part in realizing this goal. This paper provides a high-level description of an extensible and scalable lunar communications and navigation architecture, known as LunaNet. LunaNet is a services network to enable lunar operations. Three LunaNet service types are defined: networking services, position, navigation and timing services, and science utilization services. The LunaNet architecture encompasses a wide variety of topology implementations, including surface and orbiting provider nodes. In this paper several systems engineering considerations within the service architecture are highlighted. Additionally, several alternative LunaNet instantiations are presented. Extensibility of the LunaNet architecture to the solar system internet is discussed

    BackgroundMahi-mahi (Coryphaena hippurus) is a commercially and ecologically important fish species that is widely distributed in tropical and subtropical waters. Biological attributes and reproductive capacities of mahi-mahi make it a tractable model for experimental studies. In this study, life development of cultured mahi-mahi from the zygote stage to adult has been described.ResultsA comprehensive developmental table has been created reporting development as primarily detailed observations of morphology. Additionally, physiological, behavioral, and molecular landmarks have been described to significantly contribute in the understanding of mahi life development.ConclusionRemarkably, despite the vast difference in adult size, many developmental landmarks of mahi map quite closely onto the development and growth of Zebrafish and other warm-water, active Teleost fishes

    Alcohol consumption and leukocyte telomere length.

    The relationship between alcohol consumption and mortality generally exhibits a U-shaped curve. The longevity observed with moderate alcohol consumption may be explained by other confounding factors, and, if such a relationship is present, the mechanism is not well understood. Indeed, the optimal amount of alcohol consumption for health has yet to be determined. Leukocyte telomere length is an emerging quantifiable marker of biological age and health, and a shorter telomere length is a predictor of increased mortality. Because leukocyte telomere length is a quantifiable and objectively measurable biomarker of aging, we sought to identify the amount of alcohol consumption associated with the longest telomere length and least telomere length attrition. Among over 2,000 participants from two distinct cohort studies, we found no pattern of alcohol consumption that was associated with longer telomere length or less telomere length attrition over time. Binge drinking may reduce telomere length. Using telomere length as a marker of age and health, these data fail to demonstrate any benefits of alcohol consumption, even when consumed in moderation

    Use of qualitative research in foodservice organizations: a review of challenges, strategies, and applications

    Purpose – The purpose of this paper is to present the challenges encountered when conducting qualitative research in foodservice operations and to discuss the strategies to overcome the identified challenges. Design/methodology/approach – The researchers conducted food safety observations, interviews, and focus groups with more than 600 foodservice employees and managers. The researchers encountered multiple challenges including institutional review board approvals, managements’ willingness to participate, and organizational and cultural barriers. Findings – Obtaining in-depth, credible information through observations, interviews, and focus groups adds depth and breadth to hospitality studies. However, given high industry turnover, recruitment and retention throughout a study is problematic. Moreover, researchers encounter many barriers as they obtain data, such as establishing authenticity and overcoming Hawthorne and halo effects. Originality/value – Strategies to increase participation and thereby improve qualitative research have not been previously addressed in the hospitality literatur
