Kalcyfikacja naczyń wieńcowych u chorych ze schyłkową niewydolnością nerek
Kalcyfikacja naczyniowa (VC), a zwłaszcza kalcyfikacja naczyń wieńcowych, stanowi ważny problem kliniczny chorych dializowanych. Uważa się, że VC odpowiada za znaczną część zwiększonej śmiertelności obserwowanej w tej populacji chorych. Do rozwoju VC dochodzi we wczesnych okresach przewlekłej choroby nerek. Patogeneza VC jest złożona
i nie do końca poznana. Obecnie nie ma skutecznych metod leczenia VC, a podstawą postępowania jest przede wszystkim profilaktyka, polegająca na
biochemicznym wyrównaniu gospodarki wapniowofosforanowej oraz kontroli stężenia parathormonu.

Nerkopochodne włóknienie układowe
Nerkopochodne włóknienie układowe (NSF) jest nową jednostką chorobową, dotyczącąwyłącznie pacjentów z upośledzoną funkcją nerek, w większości przypadków eksponowanychna działanie gadolinowego środka cieniującego. Nerkopochodne włóknienieukładowe charakteryzuje się występowaniem objawów skórnych zlokalizowanychgłównie w obrębie kończyn (podudzia, przedramiona), rzadziej tułowia. Zmienionachorobowo skóra pacjentów z NSF jest pogrubiała, stwardniała i bolesna. Proceswłóknienia nie ogranicza się jedynie do powłok ciała, może obejmować narządywewnętrzne, prowadząc do rozwoju groźnych powikłań oraz zgonu chorego. LeczenieNSF jak dotychczas ma charakter wyłącznie objawowy, a zasadniczym celem terapiijest poprawa funkcji nerek, która może prowadzić do złagodzenia objawów, a nawetcałkowitego ich ustąpienia. Zgodnie z zaleceniami FDA należy unikać stosowaniakontrastu gadolinowego u chorych z GFR poniżej 30 ml/min/1,73 m2 (MDRD)
Nefropatia indukowana kontrastem radiologicznym
Nefropatia indukowana kontrastem (CIN) jest częstą przyczyną ostrego uszkodzenianerek i stanowi uznany czynnik zwiększonej chorobowości oraz śmiertelności zwłaszczau osób obciążonych chorobami układu sercowo-naczyniowego. Patomechanizmpowstawania CIN jest złożony i nie został do końca poznany. Powszechnie wiadomo, iżobecność niektórych stanów chorobowych, takich jak wyjściowo pogorszona funkcjanerek, cukrzyca, odwodnienie sprzyjają rozwojowi CIN. Od wielu lat podejmowane sąliczne próby ustalenia sposobu postępowania, który zapobiegałby rozwojowi CIN. Pomimoprzeprowadzenia licznych badań wątpliwości nie budzi jedynie właściwe nawodnieniepacjenta, podczas gdy inne metody są nadal dyskutowane. OGLE-2005-BLG-071Lb, the Most Massive M-Dwarf Planetary Companion?
We combine all available information to constrain the nature of
OGLE-2005-BLG-071Lb, the second planet discovered by microlensing and the first
in a high-magnification event. These include photometric and astrometric
measurements from Hubble Space Telescope, as well as constraints from higher
order effects extracted from the ground-based light curve, such as microlens
parallax, planetary orbital motion and finite-source effects. Our primary
analysis leads to the conclusion that the host of Jovian planet
OGLE-2005-BLG-071Lb is an M dwarf in the foreground disk with mass M= 0.46 +/-
0.04 Msun, distance D_l = 3.3 +/- 0.4 kpc, and thick-disk kinematics v_LSR ~
103 km/s. From the best-fit model, the planet has mass M_p = 3.8 +/- 0.4 M_Jup,
lies at a projected separation r_perp = 3.6 +/- 0.2 AU from its host and so has
an equilibrium temperature of T ~ 55 K, i.e., similar to Neptune. A degenerate
model less favored by \Delta\chi^2 = 2.1 (or 2.2, depending on the sign of the
impact parameter) gives similar planetary mass M_p = 3.4 +/- 0.4 M_Jup with a
smaller projected separation, r_\perp = 2.1 +/- 0.1 AU, and higher equilibrium
temperature T ~ 71 K. These results from the primary analysis suggest that
OGLE-2005-BLG-071Lb is likely to be the most massive planet yet discovered that
is hosted by an M dwarf. However, the formation of such high-mass planetary
companions in the outer regions of M-dwarf planetary systems is predicted to be
unlikely within the core-accretion scenario. There are a number of caveats to
this primary analysis, which assumes (based on real but limited evidence) that
the unlensed light coincident with the source is actually due to the lens, that
is, the planetary host. However, these caveats could mostly be resolved by a

Syndecan-4 Is an Independent Predictor of All-Cause as Well as Cardiovascular Mortality in Hemodialysis Patients.
BACKGROUND:Left ventricular hypertrophy is associated withincreased mortality in hemodialysis (HD) patients.Syndecan-4 plays a role in many processes that are involved in the heart fibrosis and hypertrophy.We designed this study to prospectively determine whether syndecan-4 was predictive of mortality in a group of HD patients. METHODS:In total, 191 HD patients were included. Clinical, biochemical and echocardiographic parameters were recorded. HD patients were followed-up for 23.18 ± 4.02 months. RESULTS:Syndecan-4 levels correlated strongly with geometrical echocardiographic parameters and ejection fraction. Relations with pressure-related parameters were weak and only marginally significant. Using the receiver operating characteristics the optimal cut-off points in predicting all-cause as well as cardiovascular (CV) mortality were evaluated and patients were divided into low and high syndecan-4 groups. A Kaplan-Meier analysis showed that the cumulative incidences of all-cause as well as CV mortality were higher in high serum syndecan-4 group compared with those with low serum syndecan-4 (p<0.001 in both cases).A multivariate Cox proportional hazards regression analysis revealed syndecan-4 concentration to be an independent and significant predictor of all-cause (hazard ratio, 2.99; confidence interval, 2.34 to 3.113; p<0.001)as well as CV mortality (hazard ratio, 2.81;confidence interval, 2.28to3.02; p<0.001). CONCLUSIONS:Serum syndecan-4 concentration reflects predominantly geometrical echocardiographic parameters. In HD patients serum syndecan-4 concentration is independently associated with all-cause as well as CV mortality
Reliability of the Epworth Sleepiness Scale and the Berlin Questionnaire for screening obstructive sleep apnea syndrome in the context of the examination of candidates for drivers
Background: The aim of the study has been to assess the usefulness of the Epworth Sleepiness Scale (ESS) and the Berlin Questionnaire (BQ) for obstructive sleep apnea syndrome (OSAS) screening. The capacity of both tests to discriminate between healthy individuals or with mild OSAS (apnea-hypopnea index (AHI) 0.05). Sensitivity of the ESS and BQ was 53.2% and 93.1%, respectively while specificity was 58.8% and 16.2%, respectively. Poor correlation between the ESS score and AHI and apnea index were noticed (r = 0.22, p = 0.001 and r = 0.24, p < 0.001, respectively). Conclusions: Considering its low sensitivity, the ESS should not be used as a screening test for the OSAS diagnosis amongst candidates for drivers. The BQ is characterised by high sensitivity for the OSAS diagnosis with AHI ≥ 15/h, however, due to low specificity, the questionnaire may increase the number of healthy individuals referred for needless diagnostic procedures.
