Zmiany krtani i gardła dolnego w obrazie gastroskopowym
W czasie badania gastroskopowego, istniej możliwość obserwacji zmian patologicznych w obrębie krtani i gardła środkowego oraz dolnego.
Celem pracy jest przedstawienie obrazu najczęściej spotykanych patologii, o różnej etiologii: zmiany zapalne, przednowotworowe oraz nowotworowe, pourazowe, o podłożu neurologicznym; oraz stanów po leczeniu onkologicznym (chirurgia i radioterapia); praca zawiera zarówna zdjęcia wykonane w czasie gastroskopii oraz z badań laryngowideostroboskopowych krtani.
Stwierdzenie nieprawidłowość w obrębie krtani i gardła, wymaga diagnostyki laryngologicznej i/lub foniatrycznej. Należy pamiętać, że zmiany organiczne w obrębie krtani mogą być przyczyną powstawania wtórnych zmian czynnościowych, oraz na odwrót – zmiany pierwotne czynnościowe, mogą doprowadzić do powstania zmian organicznych. Zmian jakości głosu, bez widocznych zmian organicznych w obrębie krtani, wymaga pełnej oceny foniatrycznej
Experimental study of three-nucleon dynamics in proton-deuteron breakup reaction
A measurement of the differential cross sections for the proton-deuteron elastic scattering and the deuteron breakup in collision with a proton was carried out at Cyclotron Center Bronowice using the BINA detection system. An experiment to investigate the 1H(d; pp)n breakup reaction using
a deuteron beam of 300, 340, 380 and 400 MeV and the WASA detector
has been performed at the Cooler Synchrotron COSY-Jülich. As a first
step, the data collected at the beam energy of 340MeV are analysed, with a
focus on the proton–proton coincidences registered in the Forward Detector.
Elastically scattered deuterons are used for precise determination of the
luminosity. The main steps of the analysis, including energy calibration,
particle identification (PID) and efficiency studies, and their impact on the
final accuracy of the result, are discussed
Contribution of three nucleon force investigated in deuteron-proton breakup reaction
The elastic scattering and deuteron breakup data were collected in the experiment performed at KVI (Groningen) with use of unpolarized deuteron beam with energy of 80 MeV per nucleon, impinging on hydrogen target. The procedure applied to determine total integrated luminosity is presented. The result will be used for normalization of the differential cross section for the deuteron-proton breakup reaction
Assessment of Dysphagia as a Risk Factor of Chronic Cough
Background: The aim of the study was to determine the prevalence of dysphagia in patients with chronic cough and its relationship with the long-term persistence of these symptoms. Methods: Thirty consecutive patients. All patients underwent physical examination, ENT assessment, videolaryngoscopy, functional phoniatric assessment at rest and speech, Water-Swallow Test, and Fiberoptic Endoscopic Evaluation of Swallowing disorders with Reflux Finding Score. Reflux Symptom Index questionnaire was performed. The study was approved by the local Ethics Committee Review Board (KB/39/A/2016). Results: The results of the RFS and the RSI questionnaire showed the risk of reflux in participating patients. The patients presented episodes of spillage, double swallows, penetration, aspiration and residue of food at the hypopharynx. The results of functional assessment correlated with the Water-Swallow Test. The correlation between Fiberoptic Endoscopic Evaluation of Swallowing disorders and Water-Swallow Test results was found for aspiration risk, spillage, and retention of saliva. Conclusion: The results of the study showed prevalence of dysphagia in most patients with chronic chough. It seems that phoniatric assessment in those cases should be expanded and the following tests should be performed: assessment of the laryngeal elevation, Water-Swallow Test, and Fiberoptic Endoscopic Evaluation of Swallowing disorders

Dysphagia among patients after total laryngectomy: diagnostic and therapeutic procedures
Dysphagia concerns 10–89% patients after total laryngectomy; to a greater extent it regards patients receiving complementary radiotherapy. The disease mechanism is associated with anatomical changes after surgery (typeof surgery) or complications of adjuvant therapy (xerostomia, neuropathy, swelling of tissue, etc.). The above changes lead to: decreased mobility of the lateral walls of the pharynx and tongue retraction, the occurrence of tounge pumping movements, decreased swallowing reflex, weakening of the upper esophageal sphincter opening, contraction of the cricopharyngeal muscle, tissue fibrosis, formation of pharyngeal pseudodiverticulum, etc. As a result: regurgitation of food through the nose and oral cavity, food sticking in middle and lower pharynx, prolongation of bolus transit time. Upon the formation of tracheoesophageal fistula, there may be aspiration of gastric contents. The above changes considerably reduce patients’ quality of life after surgery. The diagnostic protocol includes: medical interview (questionnaires such as EAT 10, SSQ, MDADI, DHI can be helpful), clinical swallowing assessment and instrumental examinations: primarily videofluoroscopy but also endoscopic evaluation of swallowing. Selected cases also require high frequency manometry. The treatment options include: surgical methods (e.g. balloon dilatation of the upper esophageal sphincter, cricopharyngeal myotomy, pharyngeal plexus neurectomy, removal of the pharyngeal pseudodiverticulum), pharmacological treatment or conservative methods (e.g. botulinum toxin injection of the upper esophageal sphincter, speech therapy, nutritional treatment) and supportive methods such as consultation with a psychologist, physiotherapist, clinical dietitian). The selection of a specific treatment method should be preceded by a diagnostic process in which the mechanism of functional disorders related to voice formation and swallowing will be established
Zaburzenia połykania u pacjentów po całkowitym usunięciu krtani: postępowanie diagnostyczno-terapeutyczne
Zaburzenia połykania występują u 10–89% chorych po całkowitym usunięciu krtani; w większym odsetku u pacjentów leczonych uzupełniającą radioterapią. Mechanizm zaburzeń związany jest ze zmianami anatomicznymi po operacji (zakres operacji) lub powikłaniami terapii adiuwantowej (kserostomia, neuropatia, obrzęk tkanek itp.). Powyższe zmiany prowadzą do: obniżenie ruchomości ścian bocznych gardła i retrakcji języka, pojawienia się ruchów pompujących języka, obniżenia odruchu do połykania, osłabienia otwarcia górnego zwieracza przełyku, skurczu mięśnia pierścienno-gardłowego, włóknienia tkanek, powstania pseudouchyłka gardła itp. W efekcie dochodzi do: regurgitacji treści pokarmowych do nosa i jamy ustnej, ich zalegania na poziomie gardła środkowego i dolnego, wydłużenia transportu bolusa. W przypadku powstania przetoki tchawiczo-przełykowej może dochodzić do aspiracji treści pokarmowych. Powyższe zmiany znacznie obniżają jakość życia chorych po operacji. Protokół diagnostyczny obejmuje: zebranie wywiadu (pomocne bywają kwestionariusze, np.: EAT 10, SSQ, MDADI, DHI), wykonanie oceny klinicznej połykania oraz badań instrumentalnych: przede wszystkim wideofluoroskopii, ale także badania endoskopowego połykania. W wybranych przypadkach konieczne jest wykonanie manometrii wieloczęstotliwościowej. Wśród metod leczenia można wyróżnić: metody chirurgiczne (np.: dylatacja balonem górnego zwieracza przełyku, miotomia mięśnia pierścienno-gardłowego, neurektomia splotu gardłowego, usunięcie pseudouchyłka gardła), metody zachowawcze (np.: ostrzyknięcie toksyną botulinową zwieracza górnego przełyku, rehabilitacja logopedyczna, leczenie żywieniowe) oraz metody wspomagające (np. konsultacja psychologa, fizjoterapeuty, dietetyka klinicznego). Wybór konkretnego sposobu leczenia powinien być poprzedzony procesem diagnostycznym, w którym zostanie ustalony mechanizm zaburzeń czynnościowych związanych z tworzeniem głosu i połykaniem
