    Complete traumatic laryngo-tracheal separation.

    Laryngotracheal separation injuries are rare and potentially fatal. Immediate respiratory signs may include dysphonia, aphonia, hemoptysis, subcutaneous emphysema and a sucking wound. Patients with this injury usually die at the site of the trauma. The absolute life saving intervention for patients with laryngotracheal injury is airway control via routine intubation or emergency tracheostomy. We present an extremely rare case of complete laryngotracheal separation in a teenager driving a quad bike in a \u27clothes line\u27 type injury with chicken wire. This case highlights the need for prompt airway evaluation, radiological imaging required, surgical management and long term injury sequelae

    A Nonstationary Negative Binomial Time Series with Time-Dependent Covariates: Enterococcus Counts in Boston Harbor

    Boston Harbor has had a history of poor water quality, including contamination by enteric pathogens. We conduct a statistical analysis of data collected by the Massachusetts Water Resources Authority (MWRA) between 1996 and 2002 to evaluate the effects of court-mandated improvements in sewage treatment. Motivated by the ineffectiveness of standard Poisson mixture models and their zero-inflated counterparts, we propose a new negative binomial model for time series of Enterococcus counts in Boston Harbor, where nonstationarity and autocorrelation are modeled using a nonparametric smooth function of time in the predictor. Without further restrictions, this function is not identifiable in the presence of time-dependent covariates; consequently we use a basis orthogonal to the space spanned by the covariates and use penalized quasi-likelihood (PQL) for estimation. We conclude that Enterococcus counts were greatly reduced near the Nut Island Treatment Plant (NITP) outfalls following the transfer of wastewaters from NITP to the Deer Island Treatment Plant (DITP) and that the transfer of wastewaters from Boston Harbor to the offshore diffusers in Massachusetts Bay reduced the Enterococcus counts near the DITP outfalls

    Estimate of the bottom compressional wave speed profile in the northeastern South China Sea using "Sources of Opportunity"

The definitive version was published in IEEE Journal of Oceanic Engineering 29 (2004): 1231-1248, doi:10.1109/JOE.2004.834681.The inversion of a broad-band "source of opportunity" signal for bottom geoacoustic parameters in the northeastern South China Sea (SCS) is presented, which supplements the towed source and chirp sonar bottom inversions that were performed as part of the Asian Seas International Acoustics Experiment (ASIAEX). This source of opportunity was most likely a "dynamite fishing" signal, which has sufficient low-frequency content (5-500 Hz) to make it complimentary to the somewhat higher frequency J-15-3 towed source (50-260 Hz) signals and the much higher frequency (1-10 kHz) chirp signals. This low frequency content will penetrate deeper into the bottom, thus extending the other inverse results. Localization of the source is discussed, using both a horizontal array for azimuthal steering and the "water wave" part of the pulse arrival for distance estimation. A linear broad-band inverse is performed, and three new variants of the broad-band inverse, based on: 1) the Airy phase; 2) the cutoff frequency; and 3) a range-dependent medium are presented. A multilayer model of the bottom compressional wave speed is obtained, and error estimates for this model are shown, both for the range-independent approximation to the waveguide and for the range-dependent waveguide.     Contaminated Turmeric Is a Potential Source of Lead Exposure for Children in Rural Bangladesh

    Background. During the conduct of a cohort study intended to study the associations between mixed metal exposures and child health outcomes, we found that 78% of 309 children aged 20–40 months evaluated in the Munshiganj District of Bangladesh had blood lead concentrations ≥5 µg/dL and 27% had concentrations ≥10 µg/dL. Hypothesis. Environmental sources such as spices (e.g., turmeric, which has already faced recalls in Bangladesh due to high lead levels) may be a potential route of lead exposure. Methods. We conducted visits to the homes of 28 children randomly selected from among high and low blood lead concentration groups. During the visits, we administered a structured questionnaire and obtained soil, dust, rice, and spice samples. We obtained water samples from community water sources, as well as environmental samples from neighborhood businesses. Results. Lead concentrations in many turmeric samples were elevated, with lead concentrations as high as 483 ppm. Analyses showed high bioaccessibility of lead. Conclusions. Contamination of turmeric powder is a potentially important source of lead exposure in this population

    Sex-biased parental care and sexual size dimorphism in a provisioning arthropod

    The diverse selection pressures driving the evolution of sexual size dimorphism (SSD) have long been debated. While the balance between fecundity selection and sexual selection has received much attention, explanations based on sex-specific ecology have proven harder to test. In ectotherms, females are typically larger than males, and this is frequently thought to be because size constrains female fecundity more than it constrains male mating success. However, SSD could additionally reflect maternal care strategies. Under this hypothesis, females are relatively larger where reproduction requires greater maximum maternal effort – for example where mothers transport heavy provisions to nests. To test this hypothesis we focussed on digger wasps (Hymenoptera: Ammophilini), a relatively homogeneous group in which only females provision offspring. In some species, a single large prey item, up to 10 times the mother’s weight, must be carried to each burrow on foot; other species provide many small prey, each flown individually to the nest. We found more pronounced female-biased SSD in species where females carry single, heavy prey. More generally, SSD was negatively correlated with numbers of prey provided per offspring. Females provisioning multiple small items had longer wings and thoraxes, probably because smaller prey are carried in flight. Despite much theorising, few empirical studies have tested how sex-biased parental care can affect SSD. Our study reveals that such costs can be associated with the evolution of dimorphism, and this should be investigated in other clades where parental care costs differ between sexes and species

    Evaluation of a simple low-cost intervention to empower people with chronic kidney disease to reduce their dietary salt intake : OxCKD1, a multi-center randomized controlled trial

    Background To evaluate the efficacy of a simple low-cost intervention to empower people with CKD to reduce their dietary salt intake. Methods A randomized controlled trial in primary and secondary care comparing the OxSalt care bundle intervention versus standard care for 1 month. Participants were people with CKD and an eGFR >20 ml/min per 1.73 m2 and were recruited from primary and secondary care. The primary outcome was a reduction in dietary salt intake, as assessed by 24-hour urinary sodium excretion, after 1 month of the intervention. Results Two hundred and one participants were recruited. Dietary salt intake, as assessed from 24-hour urine sodium excretion, fell by 1.9 (±2.9) g/d in the intervention group compared with 0.4 (±2.7) g/d in the control group (P < 0.001). Salt intake was still reduced to a lesser extent over the following year in the intervention group. Conclusions A short, low-cost, easily delivered intervention empowers people with CKD to reduce their dietary salt intake
