    Treatment for inclusion body myositis

    Background Inclusion body myositis (IBM) is a late-onset inflammatory muscle disease (myopathy) associated with progressive proximal and distal limb muscle atrophy and weakness. Treatment options have attempted to target inflammatory and atrophic features of this condition (for example with immunosuppressive and immunomodulating drugs, anabolic steroids, and antioxidant treatments), although as yet there is no known effective treatment for reversing or minimising the progression of inclusion body myositis. In this review we have considered the benefits, adverse effects, and costs of treatment in targeting cardinal effects of the condition, namely muscle atrophy, weakness, and functional impairment. Objectives To assess the effects of treatment for IBM. Search methods On 7 October 2014 we search ed the Cochrane Neuromuscular Disease Group Specialized Register, the Cochrane Central Register for Controlled Trials (CENTRAL), MEDLINE, and EMBASE. Additionally in November 2014 we searched clinical trials registries for ongoing or completed but unpublished trials. Selection criteria We considered randomised or quasi-randomised trials, including cross-over trials, of treatment for IBM in adults compared to placebo or any other treatment for inclusion in the review. We specifically excluded people with familial IBM and hereditary inclusion body myopathy, but we included people who had connective tissue and autoimmune diseases associated with IBM, which may or may not be identified in trials. We did not include studies of exercise therapy or dysphagia management, which are topics of other Cochrane systematic reviews. Data collection and analysis We used standard Cochrane methodological procedures. Main results The review included 10 trials (249 participants) using different treatment regimens. Seven of the 10 trials assessed single agents, and 3 assessed combined agents. Many of the studies did not present adequate data for the reporting of the primary outcome of the review, which was the percentage change in muscle strength score at six months. Pooled data from two trials of interferon beta-1a (n = 58) identified no important difference in normalised manual muscle strength sum scores from baseline to six months (mean difference (MD) -0.06, 95% CI -0.15 to 0.03) between IFN beta-1a and placebo (moderate-quality evidence). A single trial of methotrexate (MTX) (n = 44) provided moderate-quality evidence that MTX did not arrest or slow disease progression, based on reported percentage change in manual muscle strength sum scores at 12 months. None of the fully published trials were adequately powered to detect a treatment effect. We assessed six of the nine fully published trials as providing very low-quality evidence in relation to the primary outcome measure. Three trials (n = 78) compared intravenous immunoglobulin (combined in one trial with prednisone) to a placebo, but we were unable to perform meta-analysis because of variations in study analysis and presentation of trial data, with no access to the primary data for re-analysis. Other comparisons were also reported in single trials. An open trial of anti-T lymphocyte immunoglobulin (ATG) combined with MTX versus MTX provided very low-quality evidence in favour of the combined therapy, based on percentage change in quantitative muscle strength sum scores at 12 months (MD 12.50%, 95% CI 2.43 to 22.57). Data from trials of oxandrolone versus placebo, azathioprine (AZA) combined with MTX versus MTX, and arimoclomol versus placebo did not allow us to report either normalised or percentage change in muscle strength sum scores. A complete analysis of the effects of arimoclomol is pending data publication. Studies of simvastatin and bimagrumab (BYM338) are ongoing. All analysed trials reported adverse events. Only 1 of the 10 trials interpreted these for statistical significance. None of the trials included prespecified criteria for significant adverse events. Authors\u27 conclusions Trials of interferon beta-1a and MTX provided moderate-quality evidence of having no effect on the progression of IBM. Overall trial design limitations including risk of bias, low numbers of participants, and short duration make it difficult to say whether or not any of the drug treatments included in this review were effective. An open trial of ATG combined with MTX versus MTX provided very low-quality evidence in favour of the combined therapy based on the percentage change data given. We were unable to draw conclusions from trials of IVIg, oxandrolone, and AZA plus MTX versus MTX. We need more randomised controlled trials that are larger, of longer duration, and that use fully validated, standardised, and responsive outcome measures

    Performance of Task Functions by Dietetic Technicians

    The effective use of professional dietitians is predicated on the effective delegation of less demanding responsibilities and routine tasks to educationally qualified dietetic technicians. This is essential to effect optimal nutrition care at lowest cost. The purpose of this research was to determine the degree to which dietetic technicians were performing identified tasks of the role as defined by the American Dietetic Association. A questionnaire was the survey instrument used to collect data from the 130 dietetic technicians who were members of the American Dietetic Association. Task scores were derived by determining the average per cent of listed functions the dietetic technicians reported having had performed, For the clinical, generalist, and administrative technician groups, the mean task score was approximately 75 per cent. Internal comparisons indicated that for clinical technicians, those employed in long term care facilities perform significantly more task functions than those employed in other facilities. Comparisons with previous researchers indicated that technicians surveyed in this study performed more of the listed task functions than dietitians had indicated a willingness to delegate in either 1976 or 1978. Demographic profiles indicated similar characteristics for the clinical, administrative and generalist technician. However, the clinical technician was employed in relatively larger facilities, primarily acute care teaching hospitals. Administrative technicians were employed in primarily either long term care facilities or acute care hospitals of relatively smaller capacities. The generalist was employed most infrequently in acute (or teaching) facilities and most frequently in long term care facilities or in other facilities, primarily of less than 150 beds in capacity. This study did indicate a need for a more detailed delineation of the role of the dietetic technician

    Mycobacterium abscessus: Environmental Bacterium Turned Clinical Nightmare

    Mycobacteria are a large family of over 100 species, most of which do not cause diseases in humans. The majority of the mycobacterial species are referred to as nontuberculous mycobacteria (NTM), meaning they are not the causative agent of tuberculous (TB) or leprosy, i.e., Mycobacterium tuberculous complex and Mycobacterium leprae, respectively. The latter group is undoubtedly the most infamous, with TB infecting an estimated 10 million people and causing over 1.2 million deaths in 2017 alone TB and leprosy also differ from NTM in that they are only transmitted from person to person and have no environmental reservoir, whereas NTM infections are commonly acquired from the environment. It took until the 1950′s for NTM to be recognised as a potential lung pathogen in people with underlying pulmonary disease and another three decades for NTM to be widely regarded by the medical community when Mycobacterium avium complex was identified as the most common group of opportunistic pathogens in AIDS patients. This review focuses on an emerging NTM called Mycobacterium abscessus (M. abs). M. abs is a rapidly growing NTM that is responsible for opportunistic pulmonary infections in patients with structural lung disorders such as cystic fibrosis and bronchiectasis, as well as a wide range of skin and soft tissue infections in humans. In this review, we discuss how we came to understand the pathogen, how it is currently treated and examine drug resistance mechanisms and novel treatments currently in development. We highlight the urgent need for new and effective treatments for M. abs infection as well as improved in vivo methods of efficacy testing

    Time-Gating of Pulsed Eddy Current Signals for Defect Characterization and Discrimination in Aircraft Lap-Joints

    Pulsed eddy current (PEC) nondestructive testing differs from conventional eddy current techniques in that the probe coil is excited by a pulse, rather than continuous excitation at a single frequency. Reviews of early work on pulsed eddy currents are given by Waidelich1 and by Renkin.2 Pulsed excitation causes the propagation of a highly attenuated traveling wave, which is governed by the diffusion equation.3 The diffusive propagation of the eddy current pulse results in spatial broadening and a delay, or travel time, proportional to the square of the distance traveled. It was realized in early work on pulsed eddy current systems that this time dependence offered certain advantages over conventional eddy currents.4 In the current study we demonstrate the ability of a prototype pulsed eddy current instrument, described elsewhere,5,6 to take advantage of this time dependence to discriminate flaws from such interfering signals as probe liftoff, air gaps, and fasteners

    The aging commercial aviation fleet requires new methods for detecting and characterizing corrosion. In particular, the need exists for a rapid and reliable method of nondestructively detecting and characterizing corrosion in layered aircraft skins. Aircraft skin consists of thin aluminum sheet. There are many joints at which these sheets are overlapped and attached to the airframe by fasteners that extend through the sheets and into the substructure. The overlapping aluminum sheets are generally separated by a thin gap, which may be filled with a sealant. Because of the overlapping nature of such joints, the second sheet and the substructure are not directly accessible from the surface
