    MANAGEMENT OF SCIATICA THROUGH AYURVEDA: A CASE STUDY

    In India low back ache is one of the most common emerging diseases which are affecting day to day life of people physically as well as psychologically. Low back ache is the second most common disorder causing disability. Among its various causes the most commonly appearing one is Sciatica. Sciatica is a condition in which pain starts from low back and radiates to leg mostly unilateral but in some, it may be bilateral also. In Ayurvedic perspective its features resemble to those of Gridhrasi. In Ayurvedic classics two types of Gridhrasi have been described namely Vataja and Vata-kaphaja. The case study being presented is of a 44yr old male patient suffering from Vataja gridharsi. Basti karma is the choice of treatment in cases of Gridhrasi where as Snehana/ Snigdha svedana is choice of treatment for any Vataja disorder. So, Panchakarma therapy in this case includes Patrapindasveda, Sarvang vashpasveda and Basti therapy along with oral medication assessed on the basis of Oswestry disability index. The results indicate that the patient improved in presenting features up to 68.75% as well as day to day activities were also performed effectively by him

    Clinical profile of patients with pancytopenia in a tertiary care centre

    Background: Pancytopenia is a condition in which all three cell lines i.e. erythroid, myeloid and megakaryocytic are affected resulting in anaemia, leukopenia and thrombocytopenia. It can be a manifestation of a wide variety of disorders which primarily or secondarily affect the bone marrow. Early diagnosis of these conditions will help in implementing the appropriate treatment. The objective of the study was to know the clinical presentation and etiology of patients presenting with pancytopenia.Methods: A one year study from April 2016 to March 2017 was conducted in the department of medicine at a tertiary care centre. Total 32 patients with pancytopenia were studied to determine their clinical features and etiology through relevant investigations.Results: Our study revealed megaloblastic anaemia (46.87%) as the most common cause of pancytopenia, followed by malaria (12.5%). Other causes included hypothyroidism (6.2%), hypersplenism (6.2%), drugs (12.5%) and miscellaneous (9.43%).Conclusions: Megaloblastic anaemia was found to be the major cause of pancytopenia in patients. Other causes of pancytopenia were malaria, drugs, hypersplenism and hypothyroidism. These should be kept in mind while evaluating patients with pancytopenia

    Gender-Associated Oral and Periodontal Health Based on Retrospective Panoramic Radiographic Analysis of Alveolar Bone Loss

    Gender-based heterogeneity in periodontal disease has been witnessed in the recent past with huge mounting evidence. The composite effect of sex-based genetic structure and the sex steroid hormones runs in line with the corresponding gender-related differences in risk for chronic periodontitis. Since estrogens, the predominant sex hormones in women, show immune protective and anti-inflammatory effects in hormonally active premenopausal women, they show better periodontal status compared to age-matched men. Conversely, after menopause with a weakening estrogen signal, women may show an equal or even more serious periodontal status compared to men. Periodontal status of postmenopausal women may be improved by menopausal hormone therapy. Alveolar bone loss, an irreversible sign of past periodontal disease activity can be easily observed on radiographs in an objective manner. Orthopantomographs provide a fairly accurate assessment of the status of alveolar bone in the whole mouth. A cross-sectional retrospective panoramic radiographic analysis has been carried out in a north Indian dental institute to decipher the gender-based distribution of periodontal bone loss. The current chapter shall provide an update on gender-based differences in oral health, underlying mechanisms, differences in patterns and distribution of alveolar bone loss (case study), and potential gender-specific disease protection and management strategies

    Literature Review of Omicron: A Grim Reality Amidst COVID-19

    Coronavirus disease 2019 (COVID-19) first emerged in Wuhan city in December 2019, and became a grave global concern due to its highly infectious nature. The Severe Acute Respiratory Coronavirus-2, with its predecessors (i.e., MERS-CoV and SARS-CoV) belong to the family of Coronaviridae. Reportedly, COVID-19 has infected 344,710,576 people around the globe and killed nearly 5,598,511 persons in the short span of two years. On November 24, 2021, B.1.1.529 strain, later named Omicron, was classified as a Variant of Concern (VOC). SARS-CoV-2 has continuously undergone a series of unprecedented mutations and evolved to exhibit varying characteristics. These mutations have largely occurred in the spike (S) protein (site for antibody binding), which attribute high infectivity and transmissibility characteristics to the Omicron strain. Although many studies have attempted to understand this new challenge in the COVID-19 strains race, there is still a lot to be demystified.     La priorización de nuevos requerimientos a implementar en un software propietario es un punto fundamental para su mantenimiento, la conservación de la calidad, observación de las reglas de negocio y los estándares de la empresa. Aunque existen herramientas de priorización basadas en técnicas probadas y reconocidas, las mismas requieren una calificación previa de cada requerimiento. Si la empresa cuenta con solicitudes provenientes de varios clientes de un mismo producto, aumentan los factores que afectan a la empresa, las herramientas disponibles no contemplan estos aspectos y hacen mucho más compleja la tarea de calificación. Este trabajo de investigación abarca la realización de un relevamiento de los métodos de priorización y selección de nuevos requerimientos utilizados por empresas de la zona de Rosario, y la definición de una metodología para la selección un nuevo requerimiento, que implica el análisis y evaluación de todas las implicaciones sobre el producto de software y la empresa, respetando sus reglas de negocio.     Visiting a “frail elderly”: A review on model of care and treatment strategies

    Frail elderly are those who have less than normal physiological reserve which predisposes them to more severe adverse outcomes following a stressor compared to normal. As the number of elderly is increasing worldwide, there is increase in the problem of frailty. Frailty is a dynamic state in the sense that if interventions are directed to correct the state of frailty, then an elderly starts to shift toward normal side and if not then deteriorated even more. When it comes to managing a frail elderly, then the place of care can be patientfs home as well as health care facility. Comprehensive geriatric assessment and program for all.-inclusive care for the elderly are the concepts that advocate multidisciplinary approach for the management of frail elderly. There is increasing evidence in favor of the management of these patients at their home and delaying the transfer to health care facility till these patients cannot be managed at home. Apart from the specific treatment of the acute illness and the underlying chronic illness, there are other interventions cum life style modification which can help in improving a frail state. These includes increasing content of whey protein, essential amino acids in the diet, combination of resistance and endurance exercises, Vitamin D supplementation, and pharmacotherapy such as testosterone, ghrelin, and growth hormone

    Amit Jain’s surgical scoring system and its ability in predicting the major amputation in diabetic foot complications

    There are numerous scoring system used in different parts of the world and most of them are for diabetic foot ulcers only with Amit Jain’s surgical scoring system being the first such scoring for diabetic foot complications. This study aims to validate the Amit Jain’s scoring system in predicting the risk of major amputation in diabetic foot complications. A retrospective analysis was done in Department of General Surgery of Raja Rajeswari medical college, Bengaluru, India. The study period was from January 2018 to December 2019. All the patients who underwent surgeries for diabetic foot complications in our department were included in the study. A total of 47 patients were included in this study. Majority of patients (76.6%) were males 61.7% of patients had diabetes mellitus of less than 10 years duration. Abscess was the most common pathological lesion seen in the foot affecting 36.17%. Most of the patients (59.6%) with diabetic foot complications had Amit Jain’s surgical score of 6-10 and were in low risk category.  12 patients (25.5%) underwent major amputation in this study and a significant association (P<0.001) was noted between Amit Jain’s surgical scoring and major amputation.&nbsp

Insulin degludec is a second-generation, ultra-long-acting acylated basal insulin analogue that possesses a flat, stable glucose-lowering effect in patients with type 1 or type 2 diabetes mellitus. Findings from clinical trials have demonstrated that new-generation, once-daily basal insulin degludec provides similar A1C control as compared to insulin glargine, both administered as basal-oral therapy or in combination with insulin aspart, with the added benefits of lower rates of hypoglycemia, particularly nocturnal hypoglycemia
