Variations in Links between Achievement and Health: Examining the School’s Role in Buffering the Hidden Costs of Academic Success
Thesis advisor: Rebekah Levine ColeyTheories of upward mobility argue that academic and employment success grant individuals improved health, yet emerging evidence suggests that striving for such mobility in the context of marginalization may actually dysregulate physiological stress responses and compromise health. It is still unclear whether these associations operate as a function of cumulative exposure to risk (including both socioeconomic and racial/ethnic marginalization), or whether they would emerge outside of such collective risk. Further, little is known about how the school context, one of the most central contexts in adolescent development, affects associations between mobility and health, despite evidence that opportunities for socioeconomic comparisons or for discrimination at school may further exacerbate these associations. Drawing on data from the National Longitudinal Study of Adolescent Health, a longitudinal survey of a nationally representative school-based sample of adolescents in the United States (N=14,797), the current study sought to clarify links between achievement and physiological health. Multilevel regression analyses considered prospective associations between achievement and health while attending to potential variation in links across the socioeconomic spectrum and across racial/ethnic groups. Additionally, school-level factors were taken into account and explored as potential augmenting mechanisms in these links. Findings suggested promotive links between achievement and physiological health, but also suggested that such links were not shared broadly by all youth. Although links did not vary across the socioeconomic spectrum, Asian American youth demonstrated some greater health payoffs of achievement compared to their non-Hispanic White peers, while non-Hispanic Black and Mexican American youth largely experienced reversed links. These results suggest additional evidence that striving for academic achievement while experiencing racial/ethnic marginality may engender dysregulation of the stress-response system. Thus, findings are discussed in relation to the social and historical contexts that may contribute to such divergent links. However, the school-level factors considered did not moderate links among achievement, individual characteristics, and physiological health, pointing to the importance of future research considering alternate social and contextual mechanisms in these relationships.Thesis (PhD) — Boston College, 2017.Submitted to: Boston College. Operating room nursing science learning programmes in South Africa
Operating room nurses form the corner-stone of the operating room because perioperative care of the patient rests mainly in the hands of the nursing personnel. Unique challenges face nurses functioning in the stressful surgical environment where anticipation to prevent or cope with life-threatening situations is the order of the day. The operating room nurse must be knowledgeable, skilled and alert, as he/she is held accountable for all acts of commission and omission. To ensure that nurses are appropriately educated and trained and able to keep trend with the changing technology in the operating room, it is essential that learning programmes meet the minimum standards for registration as prescribed by the South African Nursing Council. Reviewing and evaluating learning programmes on a regular basis by specialist nursing educationists, are therefore essential to ensure that the standards of education and training are maintained and upgraded if required. The aim of this study is to explore and describe the various Operating Room Nursing Science Learning Programmes offered at accredited Higher Education Institutions, utilized for the education and training of the operating room nursing students in South Africa. The proposed research is based on a qualitative paradigm and the theoretical grounding is found in Bergman’s model for professional accountability (Bergman, 1982:8). A document analysis of five approved comprehensive Operating Room Nursing Science Learning Programmes from higher education institutions in South Africa (nursing colleges and universities) was carried out, together with a sixth programme, the Operating Theatre Learning Programme, as suggested by the Standard Generating Body. Requests for permission were forwarded to the management of the selected colleges or universities for inclusion of the respective programmes in the study. The researcher formulated and utilized thirty-four essential criteria derived from three documents, the first being a document entitled “Proposed Standards for Nursing and Midwifery Qualifications” submitted to the SANC and SAQA by the SGB for Nursing and Midwifery (2001-2004). The second document entitled the Public and Private Higher Education Institutions format template for criteria for the Generation and Evaluation of Qualifications and Standards within the National Qualifications Framework was also utilized (SAQA, 1430/00) and thirdly the researcher included the most relevant criteria from the list of criteria for curriculum development as indicated by the South African Nursing Council. Various tables were compiled, to reflect the findings of the document analysis according to the thirty-three criteria indicated above, to provide a clear and broad overview of the specific data in the respective six Operating Room Nursing Science Learning Programmes utilized in the study. In conclusion recommendations for a broad macro-curriculum were made to facilitate formulation of programmes in Operating Room Nursing Science relevant to the South African context
Discourse in healthy old-elderly adults: a longitudinal study
This longitudinal study investigated discourse abilities in 16 normal adults in their 80s and 90s at two separate testing times. The discourse tasks evaluated macro-level processing of narratives as manifested on retells, summaries, gists, and morals, and in explanations of proverbs. The group results showed preservation with increased age on those discourse tasks that required global levels of processing. No significant decreases in performance were found in applying strategies of reduction, generalization, and interpretation, factors associated with preserved discourse function in normal advanced ageing are discussed. The findings have implications for differential diagnosis of dementia and aphasia
Composition-structure-property effects of antimony in soda-lime-silica glasses
Float glass-type SiO2-Na2O-CaO glasses with 0 – 10 mol% Sb2O3 were melted and
their compositional, structural, thermal and optical properties characterised. All glasses
were X-ray amorphous and increasing Sb2O3 content progressively decreased glass
transition temperature (Tg) and dilatometric softening point (Td), despite increases in
Al2O3 content from greater crucible corrosion.

that Sb was predominantly incorporated as Sb3+ (Sb3+/ΣSb ~ 0.9) and Raman
spectroscopy showed that Sb substantially decreased average (Si, Al)-O Q
n
speciation.
Both techniques confirmed that Sb3+ ions were incorporated in trigonal pyramidal [:SbO3]
polyhedra. XRF and Raman spectroscopies confirmed that SO3 content decreased with
increasing Sb2O3 content. TGA analysis showed, as a linear function of Sb2O3 content,
mass gain commencing at 700°C, reaching a maximum at 1175°C, then mass loss
above 1175°C, consistent with oxidation (Sb3+ → Sb5+) then reduction (Sb5+ → Sb3+).
The TGA samples were shown to have attained or approached Sb redox equilibrium
during measurement. Optical absorption spectroscopy (UV-Vis-nIR) showed red-shifts
of the UV absorption edge with increasing Sb2O3 content, consistent with increasing
intensity of far-UV absorption bands from Sb3+ and Sb5+ s→p transitions. UV-Vis-nIR
fluorescence spectroscopy evidenced a broad luminescence band centred at ~25,000
cm-1
, attributed to the 3P1→1S0 transition of Sb3+
, which is Stokes shifted by ~15,000
cm-1
from the 1S0→3P1 absorption at ~40,000 cm-1
. The most intense emission occurred
at 0.5 mol% Sb2O3, with concentration quenching reducing luminescence intensities at
higher Sb2O3 contents. Additions of Sb2O3 to float-type soda-lime-silica glasses could
thus enable lower melting energies and/or new solar energy applications
Bending elasticity of a curved amphiphilic film decorated anchored copolymers: a small angle neutron scattering study
Microemulsion droplets (oil in water stabilized by a surfactant film) are
progressively decorated with increasing amounts of poly ethylene- oxide (PEO)
chains anchored in the film by the short aliphatic chain grafted at one end of
the PEO chain . The evolution of the bending elasticity of the surfactant film
with increasing decoration is deduced from the evolution in size and
polydispersity of the droplets as reflected by small angle neutron scattering.
The optimum curvature radius decreases while the bending rigidity modulus
remains practically constant. The experimental results compare well with the
predictions of a model developed for the bending properties of a curved film
decorated by non-adsorbing polymer chains, which takes into account, the finite
curvature of the film and the free diffusion of the chains on the film.Comment: 30 June 200
Clinically actionable secondary findings in 130 triads from sub-Saharan African families with non-syndromic orofacial clefts
Abstract Introduction The frequency and implications of secondary findings (SFs) from genomic testing data have been extensively researched. However, little is known about the frequency or reporting of SFs in Africans, who are underrepresented in large‐scale population genomic studies. The availability of data from the first whole‐genome sequencing for orofacial clefts in an African population motivated this investigation. Methods In total, 130 case‐parent trios were analyzed for SFs within the ACMG SFv.3.0 list genes. Additionally, we filtered for four more genes (HBB, HSD32B, G6PD and ACADM). Results We identified 246 unique variants in 55 genes; five variants in four genes were classified as pathogenic or likely pathogenic (P/LP). The P/LP variants were seen in 2.3% (9/390) of the subjects, a frequency higher than ~1% reported for diverse ethnicities. On the ACMG list, pathogenic variants were observed in PRKAG (p. Glu183Lys). Variants in the PALB2 (p. Glu159Ter), RYR1 (p. Arg2163Leu) and LDLR (p. Asn564Ser) genes were predicted to be LP. Conclusion This study provides information on the frequency and pathogenicity of SFs in an African cohort. Early risk detection will help reduce disease burden and contribute to efforts to increase knowledge of the distribution and impact of actionable genomic variants in diverse populations
Pan-Cancer Analysis of lncRNA Regulation Supports Their Targeting of Cancer Genes in Each Tumor Context
Long noncoding RNAs (lncRNAs) are commonly dys-regulated in tumors, but only a handful are known toplay pathophysiological roles in cancer. We inferredlncRNAs that dysregulate cancer pathways, onco-genes, and tumor suppressors (cancer genes) bymodeling their effects on the activity of transcriptionfactors, RNA-binding proteins, and microRNAs in5,185 TCGA tumors and 1,019 ENCODE assays.Our predictions included hundreds of candidateonco- and tumor-suppressor lncRNAs (cancerlncRNAs) whose somatic alterations account for thedysregulation of dozens of cancer genes and path-ways in each of 14 tumor contexts. To demonstrateproof of concept, we showed that perturbations tar-geting OIP5-AS1 (an inferred tumor suppressor) andTUG1 and WT1-AS (inferred onco-lncRNAs) dysre-gulated cancer genes and altered proliferation ofbreast and gynecologic cancer cells. Our analysis in-dicates that, although most lncRNAs are dysregu-lated in a tumor-specific manner, some, includingOIP5-AS1, TUG1, NEAT1, MEG3, and TSIX, synergis-tically dysregulate cancer pathways in multiple tumorcontexts
Pan-cancer Alterations of the MYC Oncogene and Its Proximal Network across the Cancer Genome Atlas
Although theMYConcogene has been implicated incancer, a systematic assessment of alterations ofMYC, related transcription factors, and co-regulatoryproteins, forming the proximal MYC network (PMN),across human cancers is lacking. Using computa-tional approaches, we define genomic and proteo-mic features associated with MYC and the PMNacross the 33 cancers of The Cancer Genome Atlas.Pan-cancer, 28% of all samples had at least one ofthe MYC paralogs amplified. In contrast, the MYCantagonists MGA and MNT were the most frequentlymutated or deleted members, proposing a roleas tumor suppressors.MYCalterations were mutu-ally exclusive withPIK3CA,PTEN,APC,orBRAFalterations, suggesting that MYC is a distinct onco-genic driver. Expression analysis revealed MYC-associated pathways in tumor subtypes, such asimmune response and growth factor signaling; chro-matin, translation, and DNA replication/repair wereconserved pan-cancer. Spatial Organization and Molecular Correlation of Tumor-Infiltrating Lymphocytes Using Deep Learning on Pathology Images
Beyond sample curation and basic pathologic characterization, the digitized H&E-stained images
of TCGA samples remain underutilized. To highlight this resource, we present mappings of tumorinfiltrating lymphocytes (TILs) based on H&E images from 13 TCGA tumor types. These TIL
maps are derived through computational staining using a convolutional neural network trained to
classify patches of images. Affinity propagation revealed local spatial structure in TIL patterns and
correlation with overall survival. TIL map structural patterns were grouped using standard
histopathological parameters. These patterns are enriched in particular T cell subpopulations
derived from molecular measures. TIL densities and spatial structure were differentially enriched
among tumor types, immune subtypes, and tumor molecular subtypes, implying that spatial
infiltrate state could reflect particular tumor cell aberration states. Obtaining spatial lymphocytic
patterns linked to the rich genomic characterization of TCGA samples demonstrates one use for
the TCGA image archives with insights into the tumor-immune microenvironment
