    A Model for HIV Disclosure of a Parent\u27s And/Or a Child\u27s Illness

    HIV prevalence in Kenya remains steady at 5.6% for adults 15 years and older, and 0.9% among children aged below 14 years. Parents and children are known to practice unprotected sex, which has implications for continued HIV spread within the country. Additionally, due to increased accessibility of antiretroviral therapy, more HIV-positive persons are living longer. Therefore, the need for HIV disclosure of a parent\u27s and/or a child\u27s HIV status within the country will continue for years to come. We conducted a qualitative phenomenological study to understand the entire process of disclosure from the time of initial HIV diagnosis of an index person within an HIV-affected family, to the time of full disclosure of a parent\u27s and/or a child\u27s HIV status to one or more HIV-positive, negative, or untested children within these households. Participants were purposively selected and included 16 HIV-positive parents, seven HIV-positive children, six healthcare professionals (physician, clinical officer, psychologist, registered nurse, social worker, and a peer educator), and five HIV-negative children. All participants underwent an in-depth individualized semistructured interview that was digitally recorded. Interviews were transcribed and analyzed in NVivo 8 using the modified Van Kaam method. Six themes emerged from the data indicating that factors such as HIV testing, living with HIV, evolution of disclosure, questions, emotions, benefits, and consequences of disclosure interact with each other and either impede or facilitate the HIV disclosure process. Kenya currently does not have guidelines for HIV disclosure of a parent\u27s and/or a child\u27s HIV status. HIV disclosure is a process that may result in poor outcomes in both parents and children. Therefore, understanding how these factors affect the disclosure process is key to achieving optimal disclosure outcomes in both parents and children. To this end, we propose an HIV disclosure model incorporating these six themes that is geared at helping healthcare professionals provide routine, clinic-based, targeted, disclosure-related counseling/advice and services to HIV- positive parents and their HIV-positive, HIV-negative, and untested children during the HIV disclosure process.     A Racial Impact Analysis of HB 1075/SB 201

    The economic and social consequences of untreated (or under-treated) substance abuse among minors are significant. This report provides a racial impact analysis of HB 1075/SB 201, legislation approved in the 2012 General Assembly session that seeks to improve access and use of substance and alcohol services by minors. In short, this policy could go a long way to ensure that families are properly educated about these life-changing (and life-saving) programs; however our analysis raises concerns related to cultural competency that may serve to undermine the legislation’s goal. Virginia is incredibly diverse and its communities vary widely with its assets and risks. In this vein, we offer concrete recommendations to maximize the policy’s racial equity. Our analysis also sheds light on the ongoing challenge Virginia’s state agencies have had to address cultural competency within its services. Additional research is necessary to determine what service gaps may exist, which would increase or decrease the racial equity impact. By answering these questions, Virginia will be better prepared to further reduce alcohol and substance abuse by all minors

The purpose of the study was to understand the lived experiences of HIV-positive parents and their children during the disclosure process in Kenya. Sixteen HIV-positive parents, seven HIV-positive children, and five HIV-negative children completed semistructured, in-depth interviews. Data were analyzed using the Van Kaam method; NVivo 8 software was used to assist data analysis. We present data on the process of disclosure based on how participants recommended full disclosure be approached to HIV-positive and negative children. Participants recommended disclosure as a process starting at five years with full disclosure delivered at 10 years when the child was capable of understanding the illness, or by 14 years when the child was mature enough to receive the news if full disclosure had not been conducted earlier. Important considerations at the time of full disclosure included the parent’s and/or child’s health statuses, number of infected family members’ illnesses to be disclosed to the child, child’s maturity and understanding level, and the person best suited to deliver full disclosure to the child. The results also revealed it was important to address important life events such as taking a national school examination during disclosure planning and delivery. Recommendations are made for inclusion into HIV disclosure guidelines, manuals, and programs in resource-poor nations with high HIV prevalence

    The aim of this study is to describe what an English teacher can do to build high school students’ English learning motivation. The researcher hopes that this research will help teachers realize what things they can do to maintain their students' motivation in learning English by giving broader explanations about motivation, especially to high school English teachers. The researcher used an observation protocol in this research. The technique that was used to collect the data was the classroom observation. After that, the researcher analyzed and described the calculation table and also the transcription data that was used as the Excerpt in findings and discussions. It can be concluded that positive feedback such as the way the teacher give encouragement, compliment, or appreciation can maintain or even gain the students’ English learning motivation. Teacher needs to learn to give students more positive feedback to the students in order to maintain their motivation English classroom. It can encourage them be more active and participate more in the learning process. Teacher building a good relationship with the students is also necessary since the teacher can know more about the students' preferences and can relate it to the learning material. However, this research result cannot be generalized since the data were collected from one teacher and one classroom’s observations

    Structural and wetting properties of nature\u27s finest silks (order Embioptera)

    Insects from the order Embioptera (webspinners) spin silk fibres which are less than 200 nm in diameter. In this work, we characterized and compared the diameters of single silk fibres from nine species—Antipaluria urichi, Pararhagadochir trinitatis, Saussurembia calypso, Diradius vandykei, Aposthonia ceylonica, Haploembia solieri, H. tarsalis, Oligotoma nigra and O. saundersii. Silk from seven of these species have not been previously quantified. Our studies cover five of the 10 named taxonomic families and represent about one third of the known taxonomic family-level diversity in the order Embioptera. Naturally spun silk varied in diameter from 43.6 ± 1.7 nm for D. vandykei to 122.4 ± 3.2 nm for An. urichi. Mean fibre diameter did not correlate with adult female body length. Fibre diameter is more similar in closely related species than in more distantly related species. Field observations indicated that silk appears shiny and smooth when exposed to rainwater. We therefore measured contact angles to learn more about interactions between silk and water. Higher contact angles were measured for silks with wider fibre diameter and higher quantity of hydrophobic amino acids. High static contact angles (ranging up to 122° ± 3° for An. urichi) indicated that silken sheets spun by four arboreal, webspinner species were hydrophobic. A second contact angle measurement made on a previously wetted patch of silk resulted in a lower contact angle (average difference was greater than 27°) for all four species. Our studies suggest that silk fibres which had been previously exposed to water exhibited irreversible changes in hydrophobicity and water adhesion properties. Our results are in alignment with the ‘super-pinning’ site hypothesis by Yarger and co-workers to describe the hydrophobic, yet water adhesive, properties exhibited by webspinner silk fibres. The physical and chemical insights gained here may inform the synthesis and development of smaller diameter silk fibres with unique water adhesion properties
