Efficacy of confrontational counselling for smoking cessation in smokers with previously undiagnosed mild to moderate airflow limitation: study protocol of a randomized controlled trial
<p>Abstract</p> <p>Background</p> <p>The use of spirometry for early detection of chronic obstructive pulmonary disease (COPD) is still an issue of debate, particularly because of a lack of convincing evidence that spirometry has an added positive effect on smoking cessation. We hypothesise that early detection of COPD and confrontation with spirometry for smoking cessation may be effective when applying an approach we have termed "confrontational counselling"; a patient-centred approach which involves specific communication skills and elements of cognitive therapy. An important aspect is to confront the smoker with his/her airflow limitation during the counselling sessions. The primary objective of this study is to test the efficacy of confrontational counselling in comparison to regular health education and promotion for smoking cessation delivered by specialized respiratory nurses in current smokers with previously undiagnosed mild to moderate airflow limitation.</p> <p>Methods/Design</p> <p>The study design is a randomized controlled trial comparing confrontational counselling delivered by a respiratory nurse combined with nortriptyline for smoking cessation (experimental group), health education and promotion delivered by a respiratory nurse combined with nortriptyline for smoking cessation (control group 1), and "care as usual" delivered by the GP (control group 2). Early detection of smokers with mild to moderate airflow limitation is achieved by means of a telephone interview in combination with spirometry. Due to a comparable baseline risk of airflow limitation and motivation to quit smoking, and because of the standardization of number, duration, and scheduling of counselling sessions between the experimental group and control group 1, the study enables to assess the "net" effect of confrontational counselling. The study has been ethically approved and registered.</p> <p>Discussion</p> <p>Ethical as well as methodological considerations of the study are discussed in this protocol. A significant and relevant effect of confrontational counselling would provide an argument in favour of early detection of current smokers with airflow limitation. Successful treatment of tobacco dependence in respiratory patients requires repeated intensive interventions. The results of this study may also show that respiratory nurses are able to deliver this treatment and that intensive smoking cessation counselling is more feasible.</p> <p>Trial registration:</p> <p>Netherlands Trial Register (ISRCTN 64481813).</p
Effectiveness, relapse prevention and mechanisms of change of cognitive therapy vs. interpersonal therapy for depression: Study protocol for a randomised controlled trial
<p>Abstract</p> <p>Background</p> <p>Major depression is a common mental disorder that substantially impairs quality of life and has high societal costs. Although psychotherapies have proven to be effective antidepressant treatments, initial response rates are insufficient and the risk of relapse and recurrence is high. Improvement of treatments is badly needed. Studying the mechanisms of change in treatment might be a good investment for improving everyday mental health care. However, the mechanisms underlying therapeutic change remain largely unknown. The objective of the current study is to assess both the effectiveness of two commonly used psychotherapies for depression in terms of reduction of symptoms and prevention of relapse on short and long term, as well as identifying underlying mechanisms of change.</p> <p>Methods</p> <p>In a randomised trial we will compare (a) Cognitive Therapy (CT) with (b) Interpersonal therapy (IPT), and (c) an 8-week waiting list condition followed by treatment of choice. One hundred eighty depressed patients (aged 18-65) will be recruited in a mental health care centre in Maastricht (the Netherlands). Eligible patients will be randomly allocated to one of the three intervention groups. The primary outcome measure of the clinical evaluation is depression severity measured by the Beck Depression Intenvory-II (BDI-II). Other outcomes include process variables such as dysfunctional beliefs, negative attributions, and interpersonal problems. All self-report outcome assessments will take place on the internet at baseline, three, seven, eight, nine, ten, eleven, twelve and twenty-four months. At 24 months a retrospective telephone interview will be administered. Furthermore, a rudimentary analysis of the cost-effectiveness will be embedded. The study has been ethically approved and registered.</p> <p>Discussion</p> <p>By comparing CT and IPT head-to-head and by investigating multiple potential mediators and outcomes at multiple time points during and after therapy, we hope to provide new insights in the effectiveness and mechanisms of change of CT and IPT for depression, and contribute to the improvement of mental health care for adults suffering from depression.</p> <p>Trial registration</p> <p>The study has been registered at the Netherlands Trial Register, part of the Dutch Cochrane Centre (<a href="http://www.controlled-trials.com/ISRCTN67561918">ISRCTN67561918</a>)</p
Constraints on Automorphic Forms of Higher Derivative Terms from Compactification
By dimensionally reducing the higher derivative corrections of
ten-dimensional IIB theory on a torus we deduce constraints on the E_{n+1}
automorphic forms that occur in d=10-n dimensions. In particular we argue that
these automorphic forms involve the representation of E_{n+1} with fundamental
weight \lambda^{n+1}, which is also the representation to which the string
charges in d dimensions belong. We also consider a similar calculation for the
reduction of higher derivative terms in eleven-dimensional M-theory.Comment: Minor corrections, to appear in JHE

By dimensionally reducing the ten-dimensional higher derivative type IIA
string theory effective action we place constraints on the automorphic forms
that appear in the effective action in lower dimensions. We propose a number of
properties of such automorphic forms and consider the prospects that E11 can
play a role in the formulation of the higher derivative string theory effective

The new IL-1 family member IL-1F8 stimulates production of inflammatory mediators by synovial fibroblasts and articular chondrocytes
Six novel members of the IL-1 family of cytokines were recently identified, primarily through the use of DNA database searches for IL-1 homologues, and were named IL-1F5 to IL-1F10. In the present study, we investigated the effect of IL-1F8 on primary human joint cells, and examined the expression of the new IL-1 family members in human and mouse joints. Human synovial fibroblasts (hSFs) and human articular chondrocytes (hACs) expressed the IL-1F8 receptor (IL-1Rrp2) and produced pro-inflammatory mediators in response to recombinant IL-1F8. IL-1F8 mRNA expression was increased in hSFs upon stimulation with proinflammatory cytokines, whereas in hACs IL-1F8 mRNA expression was constitutive. However, IL-1F8 protein was undetectable in hSF and hAC culture supernatants. Furthermore, although IL-1β protein levels were increased in inflamed human and mouse joint tissue, IL-1F8 protein levels were not. IL-1F8 levels in synovial fluids were similar to or lower than those in matched serum samples, suggesting that the joint itself is not a major source of IL-1F8. Serum levels of IL-1F8 were similar in healthy donors, and patients with rheumatoid arthritis, osteoarthritis and septic shock, and did not correlate with inflammatory status. Interestingly however, we observed high IL-1F8 levels in several serum samples in all groups. In conclusion, IL-1F8 exerts proinflammatory effects in primary human joint cells. Joint and serum IL-1F8 protein levels did not correlate with inflammation, but they were high in some human serum samples tested, including samples from patients with rheumatoid arthritis. It remains to be determined whether circulating IL-1F8 can contribute to joint inflammation in rheumatoid arthritis

Non-overshooting stabilisation via state and output feedback
The concept of “strong stability” of LTI systems has been introduced in a recent paper [KHP2]. This is a stronger notion of stability compared to alternative definitions (e.g. stability in the sense of Lyapunov, asymptotic stability), which allows the analysis and design of control systems with non-overshooting response in the state-space for arbitrary initial conditions. The paper reviews the notion of “strong stability” [KHP2] and introduces the problem of non-overshooting stabilization. It is shown that non-overshooting stabilization under dynamic and static output feedback are, in a certain sense, equivalent problems. Thus, we turn our attention to static non-overshooting stabilization problems under state-feedback, output injection and output feedback. After developing a number of preliminary results, we give a geometric interpretation to the problem in terms of the intersection of an affine hyperplane and the interior of an open convex cone. A solution to the problem is finally obtained via Linear Matrix Inequalities, along with the complete parametrization of the optimal solution set
Triplet-Singlet Spin Relaxation via Nuclei in a Double Quantum Dot
The spin of a confined electron, when oriented originally in some direction,
will lose memory of that orientation after some time. Physical mechanisms
leading to this relaxation of spin memory typically involve either coupling of
the electron spin to its orbital motion or to nuclear spins. Relaxation of
confined electron spin has been previously measured only for Zeeman or exchange
split spin states, where spin-orbit effects dominate relaxation, while spin
flips due to nuclei have been observed in optical spectroscopy studies. Using
an isolated GaAs double quantum dot defined by electrostatic gates and direct
time domain measurements, we investigate in detail spin relaxation for
arbitrary splitting of spin states. Results demonstrate that electron spin
flips are dominated by nuclear interactions and are slowed by several orders of
magnitude when a magnetic field of a few millitesla is applied. These results
have significant implications for spin-based information processing
Photoconductivity of biased graphene
Graphene is a promising candidate for optoelectronic applications such as
photodetectors, terahertz imagers, and plasmonic devices. The origin of
photoresponse in graphene junctions has been studied extensively and is
attributed to either thermoelectric or photovoltaic effects. In addition, hot
carrier transport and carrier multiplication are thought to play an important
role. Here we report the intrinsic photoresponse in biased but otherwise
homogeneous graphene. In this classic photoconductivity experiment, the
thermoelectric effects are insignificant. Instead, the photovoltaic and a
photo-induced bolometric effect dominate the photoresponse due to hot
photocarrier generation and subsequent lattice heating through electron-phonon
cooling channels respectively. The measured photocurrent displays polarity
reversal as it alternates between these two mechanisms in a backgate voltage
sweep. Our analysis yields elevated electron and phonon temperatures, with the
former an order higher than the latter, confirming that hot electrons drive the
photovoltaic response of homogeneous graphene near the Dirac point
E11, generalised space-time and equations of motion in four dimensions
We construct the non-linear realisation of the semi-direct product of E11 and
its first fundamental representation at low levels in four dimensions. We
include the fields for gravity, the scalars and the gauge fields as well as the
duals of these fields. The generalised space-time, upon which the fields
depend, consists of the usual coordinates of four dimensional space-time and
Lorentz scalar coordinates which belong to the 56-dimensional representation of
E7. We demand that the equations of motion are first order in derivatives of
the generalised space-time and then show that they are essentially uniquely
determined by the properties of the E11 Kac-Moody algebra and its first
fundamental representation. The two lowest equations correctly describe the
equations of motion of the scalars and the gauge fields once one takes the
fields to depend only on the usual four dimensional space-time
