    Fast Ion Effects During Test Blanket Module Simulation Experiments in DIII-D

    Fast beam-ion losses were studied in DIII-D in the presence of a scaled mockup of two Test Blanket Modules (TBM) for ITER. Heating of the protective tiles on the front of the TBM surface was found when neutral beams were injected and the TBM fields were engaged. The fast-ion core confinement was not significantly affected. Different orbit-following codes predict the formation of a hot spot on the TBM surface arising from beam-ions deposited near the edge of the plasma. The codes are in good agreement with each other on the total power deposited at the hot spot predicting an increase in power with decreasing separation between the plasma edge and the TBM surface. A thermal analysis of the heat flow through the tiles shows that the simulated power can account for the measured tile temperature rise. The thermal analysis, however, is very sensitive to the details of the localization of the hot spot which is predicted to be different among the various codes

    Is race medically relevant? A qualitative study of physicians' attitudes about the role of race in treatment decision-making

    <p>Abstract</p> <p>Background</p> <p>The role of patient race in medical decision-making is heavily debated. While some evidence suggests that patient race can be used by physicians to predict disease risk and determine drug therapy, other studies document bias and stereotyping by physicians based on patient race. It is critical, then, to explore physicians' attitudes regarding the medical relevance of patient race.</p> <p>Methods</p> <p>We conducted a qualitative study in the United States using ten focus groups of physicians stratified by self-identified race (black or white) and led by race-concordant moderators. Physicians were presented with a medical vignette about a patient (whose race was unknown) with Type 2 diabetes and untreated hypertension, who was also a current smoker. Participants were first asked to discuss what medical information they would need to treat the patient. Then physicians were asked to explicitly discuss the importance of race to the hypothetical patient's treatment. To identify common themes, codes, key words and physician demographics were compiled into a comprehensive table that allowed for examination of similarities and differences by physician race. Common themes were identified using the software package NVivo (QSR International, v7).</p> <p>Results</p> <p>Forty self-identified black and 50 self-identified white physicians participated in the study. All physicians - regardless of their own race - believed that medical history, family history, and weight were important for making treatment decisions for the patient. However, black and white physicians reported differences in their views about the relevance of race. Several black physicians indicated that patient race is a central factor for choosing treatment options such as aggressive therapies, patient medication and understanding disease risk. Moreover, many black physicians considered patient race important to understand the patient's views, such as alternative medicine preferences and cultural beliefs about illness. However, few white physicians explicitly indicated that the patient's race was important over-and-above medical history. Instead, white physicians reported that the patient should be treated aggressively regardless of race.</p> <p>Conclusions</p> <p>This investigation adds to our understanding about how physicians in the United States consider race when treating patients, and sheds light on issues physicians face when deciding the importance of race in medical decision-making.</p

    The new TAE - Alfvén Wave Active Excitation System at JET

    After many years of successful operation, the JET saddle coil system will be dismantled during the 2004-2005 shutdown. A new antenna system has been designed and is being constructed to replace it and excite magneto-hydrodynamics modes in the Alfvén frequency range (10500kHz), keeping similar operational capabilities (IANT~30A, VANT~1kV, maximum power ~5kW). In addition to the constraints imposed by halo current and disruption-induced voltages and currents, the design must comply with the requirements of a remote handling installation. The physics basis, design principles and constraints will be presented along with the results of the coupling and engineering analysis, and a discussion of the possible extrapolation of such a system to ITER

    Extensive Genetic Diversity, Unique Population Structure and Evidence of Genetic Exchange in the Sexually Transmitted Parasite Trichomonas vaginalis

    The human parasite Trichomonas vaginalis causes trichomoniasis, the world's most common non-viral sexually transmitted infection. Research on T. vaginalis genetic diversity has been limited by a lack of appropriate genotyping tools. To address this problem, we recently published a panel of T. vaginalis-specific genetic markers; here we use these markers to genotype isolates collected from ten regions around the globe. We detect high levels of genetic diversity, infer a two-type population structure, identify clinically relevant differences between the two types, and uncover evidence of genetic exchange in what was believed to be a clonal organism. Together, these results greatly improve our understanding of the population genetics of T. vaginalis and provide insights into the possibility of genetic exchange in the parasite, with implications for the epidemiology and control of the disease. By taking into account the existence of different types and their unique characteristics, we can improve understanding of the wide range of symptoms that patients manifest and better implement appropriate drug treatment. In addition, by recognizing the possibility of genetic exchange, we are more equipped to address the growing concern of drug resistance and the mechanisms by which it may spread within parasite populations

    Local variables affecting H-mode threshold on alcator C-mod

    An edge temperature threshold for the LH transition is found on the Alcator C-Mod tokamak. The critical temperature depends weakly on density and increases with B-T and with unfavourable drift direction. T-e at the H-L transition can be greater than or equal to the L-H threshold. Magnetic fluctuations are observed at some H-L transitions. Measured parameters are compared with H-mode theories, including nonlinear drift-ballooning code simulations

    Plasma vertical stabilisation in ITER

    This paper describes the progress in analysis of the ITER plasma vertical stabilisation (VS) system since its design review in 2007–2008. Two indices characterising plasma VS were studied. These are (1) the maximum value of plasma vertical displacement due to free drift that can be stopped by the VS system and (2) the maximum root mean square value of low frequency noise in the dZ/dt measurement signal used in the VS feedback loop. The first VS index was calculated using the PET code for 15 MA plasmas with the nominal position and shape. The second VS index was studied with the DINA code in the most demanding simulations for plasma magnetic control of 15 MA scenarios with the fastest plasma current ramp-up and early X-point formation, the fastest plasma current ramp-down in a divertor configuration, and an H to L mode transition at the current flattop. The studies performed demonstrate that the VS in-vessel coils, adopted recently in the baseline design, significantly increase the range of plasma controllability in comparison with the stabilising systems VS1 and VS2, providing operating margins sufficient to achieve ITER's goals specified in the project requirements. Additionally two sets of the DINA code simulations were performed with the goal of assessment of the capability of the PF system with the VS in-vessel coils: (i) to control the position of runaway electrons generated during disruptions in 15 MA scenarios and (ii) to trigger ELMs in H-mode plasmas of 7.5 MA/2.65 T scenarios planned for the early phase of ITER operation. It was also shown that ferromagnetic structures of the vacuum vessel (ferromagnetic inserts) and test blanket modules insignificantly affect the plasma VS
