Quenched invariance principle for random walks in balanced random environment
We consider random walks in a balanced random environment in ,
. We first prove an invariance principle (for ) and the
transience of the random walks when (recurrence when ) in an
ergodic environment which is not uniformly elliptic but satisfies certain
moment condition. Then, using percolation arguments, we show that under mere
ellipticity, the above results hold for random walks in i.i.d. balanced

Expression Levels of Thymidylate Synthase, Thymidylate Phosphorylase and Dihydropyrimidine Dehydrogenase in Head and Neck Squamous Cell Carcinoma: Preliminary Study
Introduction Pharyngo-laryngeal tumors classified as T3-4, N0-3, M0, are conventionally treated by mutilating surgery (total (pharyngo)-laryngectomy). Neo-adjuvant chemotherapy with 5-FU/platinum salt can be proposed in an attempt to preserve the larynx. The level of the response to chemotherapy ranges from 36 to 54% of cases. Thus, a large number of patients receive chemotherapy that is ineffective and not free from adverse effects. Three main enzymes are involved in the metabolism of 5-FU: thymidylate synthase (TS), thymidylate phosphorylase (TP) and dihydropyrimidine dehydrogenase (DPD). Several studies suggest that a high level of expression of these three genes correlates with a poor clinical response to 5-FU. The main purpose of our study was to look for a correlation between the levels of expression of the genes for sensitivity to 5-FU (TS, TP, DPD) within the tumor and the clinical response observed after three courses of chemotherapy combining 5-FU/platinum salt in patients presenting with advanced cancer of the pharyngolarynx. Methods This was a prospective genetic study that had required approval from the Ethics Committee. The main assessment criterion was based on the assessment of the clinical response by an ENT panendoscopy and a cervical CT scan, after three courses of chemotherapy. The expression of the genes was determined by quantitative RT-PCR, using total RNA extracted from tumor biopsies taken during the initial panendoscopy. Results The means calculated, in our study, for the three genes of interest (TS, TP, DPD) were lower in the responder group than those in the non-responder group. Discussion Our preliminary findings reveal trends that confirm the hypothesis that the lower the level of expression of the sensitivity genes, the better the clinical response to chemotherapy. They now form part of a larger study that is currently in progress
Cooling rate dependence of the antiferromagnetic domain structure of a single crystalline charge ordered manganite
The low temperature phase of single crystals of NdCaMnO
and GdCaMnO manganites is investigated by squid
magnetometry. NdCaMnO undergoes a charge-ordering
transition at =245K, and a long range CE-type antiferromagnetic state
is established at =145K. The dc-magnetization shows a cooling rate
dependence below , associated with a weak spontaneous moment. The
associated excess magnetization is related to uncompensated spins in the
CE-type antiferromagnetic structure, and to the presence in this state of
fully orbital ordered regions separated by orbital domain walls. The observed
cooling rate dependence is interpreted to be a consequence of the rearrangement
of the orbital domain state induced by the large structural changes occurring

Inelastic x-ray scattering investigations of lattice dynamics in SmFeAsOF superconductors
We report measurements of the phonon density of states as measured with
inelastic x-ray scattering in SmFeAsOF powders. An unexpected
strong renormalization of phonon branches around 23 meV is observed as fluorine
is substituted for oxygen. Phonon dispersion measurements on
SmFeAsOF single crystals allow us to identify the 21 meV A
in-phase (Sm,As) and the 26 meV B (Fe,O) modes to be responsible for
this renormalization, and may reveal unusual electron-phonon coupling through

Renormalization and asymptotic expansion of Dirac's polarized vacuum
We perform rigorously the charge renormalization of the so-called reduced
Bogoliubov-Dirac-Fock (rBDF) model. This nonlinear theory, based on the Dirac
operator, describes atoms and molecules while taking into account vacuum
polarization effects. We consider the total physical density including both the
external density of a nucleus and the self-consistent polarization of the Dirac
sea, but no `real' electron. We show that it admits an asymptotic expansion to
any order in powers of the physical coupling constant \alphaph, provided that
the ultraviolet cut-off behaves as \Lambda\sim e^{3\pi(1-Z_3)/2\alphaph}\gg1.

A new approach to the modelling of local defects in crystals: the reduced Hartree-Fock case
This article is concerned with the derivation and the mathematical study of a
new mean-field model for the description of interacting electrons in crystals
with local defects. We work with a reduced Hartree-Fock model, obtained from
the usual Hartree-Fock model by neglecting the exchange term. First, we recall
the definition of the self-consistent Fermi sea of the perfect crystal, which
is obtained as a minimizer of some periodic problem, as was shown by Catto, Le
Bris and Lions. We also prove some of its properties which were not mentioned
before. Then, we define and study in details a nonlinear model for the
electrons of the crystal in the presence of a defect. We use formal analogies
between the Fermi sea of a perturbed crystal and the Dirac sea in Quantum
Electrodynamics in the presence of an external electrostatic field. The latter
was recently studied by Hainzl, Lewin, S\'er\'e and Solovej, based on ideas
from Chaix and Iracane. This enables us to define the ground state of the
self-consistent Fermi sea in the presence of a defect. We end the paper by
proving that our model is in fact the thermodynamic limit of the so-called

Stereotactic radiosurgery for secretory pituitary adenomas: systematic review and International Stereotactic Radiosurgery Society practice recommendations.
A systematic review was performed to provide objective evidence on the use of stereotactic radiosurgery (SRS) in the management of secretory pituitary adenomas and develop consensus recommendations.
The authors performed a systematic review of the English-language literature up until June 2018 using the PRISMA guidelines. The PubMed (Medline), Embase, and Cochrane databases were searched. A total of 45 articles reporting single-institution outcomes of SRS for acromegaly, Cushing's disease, and prolactinomas were selected and included in the analysis.
For acromegaly, random effects meta-analysis estimates for crude tumor control rate, crude endocrine remission rate, and any new hypopituitarism rates were 97.0% (95% CI 96.0%-98.0%), 44.0% (95% CI 35.0%-53.0%), and 17.0% (95% CI 13.0%-23.0%), respectively. For Cushing's disease, random effects estimates for crude tumor control rate, crude endocrine remission rate, and any new hypopituitarism rate were 92.0% (95% CI 87.0%-95.0%), 48.0% (95% CI 35.0%-61.0%), and 21.0% (95% CI 13.0%-31.0%), respectively. For prolactinomas, random effects estimates for crude tumor control rate, crude endocrine remission rate, and any new hypopituitarism rate were 93.0% (95% CI 90.0%-95.0%), 28.0% (95% CI 19.0%-39.0%), and 12.0% (95% CI 6.0%-24.0%), respectively. Meta-regression analysis did not show a statistically significant association between mean margin dose with crude endocrine remission rate or mean margin dose with development of any new hypopituitarism rate for any of the secretory subtypes.
SRS offers effective tumor control of hormone-producing pituitary adenomas in the majority of patients but a lower rate of endocrine improvement or remission
Glueballs, gluon condensate, and pure glue QCD below T_c
A quasiparticle description of pure glue QCD thermodynamics at T<T_c is
proposed and compared to recent lattice data. Given that a gas of glueballs
with constant mass cannot quantitatively reproduce the early stages of the
deconfinement phase transition, the problem is to identify a relevant mechanism
leading to the observed sudden increase of the pressure, trace anomaly, etc. It
is shown that the strong decrease of the gluon condensate near T_c combined
with the increasing thermal width of the lightest glueballs might be the

Early response to immunosuppressive therapy predicts good renal outcome in lupus nephritis: lessons from long-term followup of patients in the Euro-Lupus Nephritis Trial.

Abstract
OBJECTIVE: In the Euro-Lupus Nephritis Trial (ELNT), 90 patients with lupus nephritis were randomly assigned to a high-dose intravenous cyclophosphamide (IV CYC) regimen (6 monthly pulses and 2 quarterly pulses with escalating doses) or a low-dose IV CYC regimen (6 pulses of 500 mg given at intervals of 2 weeks), each of which was followed by azathioprine (AZA). After a median followup of 41 months, a difference in efficacy between the 2 regimens was not observed. The present analysis was undertaken to extend the followup and to identify prognostic factors.
METHODS: Renal function was prospectively assessed quarterly in all 90 patients except 5 who were lost to followup. Survival curves were derived using the Kaplan-Meier method.
RESULTS: After a median followup of 73 months, there was no significant difference in the cumulative probability of end-stage renal disease or doubling of the serum creatinine level in patients who received the low-dose IV CYC regimen versus those who received the high-dose regimen. At long-term followup, 18 patients (8 receiving low-dose and 10 receiving high-dose treatment) had developed permanent renal impairment and were classified as having poor long-term renal outcome. We demonstrated by multivariate analysis that early response to therapy at 6 months (defined as a decrease in serum creatinine level and proteinuria <1 g/24 hours) was the best predictor of good long-term renal outcome.
CONCLUSION: Long-term followup of patients from the ELNT confirms that, in lupus nephritis, a remission-inducing regimen of low-dose IV CYC followed by AZA achieves clinical results comparable with those obtained with a high-dose regimen. Early response to therapy is predictive of good long-term renal outcome.

Ground State and Charge Renormalization in a Nonlinear Model of Relativistic Atoms
We study the reduced Bogoliubov-Dirac-Fock (BDF) energy which allows to
describe relativistic electrons interacting with the Dirac sea, in an external
electrostatic potential. The model can be seen as a mean-field approximation of
Quantum Electrodynamics (QED) where photons and the so-called exchange term are
neglected. A state of the system is described by its one-body density matrix,
an infinite rank self-adjoint operator which is a compact perturbation of the
negative spectral projector of the free Dirac operator (the Dirac sea).
We study the minimization of the reduced BDF energy under a charge
constraint. We prove the existence of minimizers for a large range of values of
the charge, and any positive value of the coupling constant . Our
result covers neutral and positively charged molecules, provided that the
positive charge is not large enough to create electron-positron pairs. We also
prove that the density of any minimizer is an function and compute the
effective charge of the system, recovering the usual renormalization of charge:
the physical coupling constant is related to by the formula

is the ultraviolet cut-off. We eventually prove an estimate on the
highest number of electrons which can be bound by a nucleus of charge . In
the nonrelativistic limit, we obtain that this number is , recovering
a result of Lieb.
