    Accessing elite nurses for research: reflections on the theoretical and practical issues of telephone interviewing

    Elite groups are interesting as they frequently are powerful (in terms of position, knowledge and influence) and enjoy considerable authority. It is important, therefore, to involve them in research concerned with understanding social contexts and processes. This is particularly pertinent in healthcare, where considerable strategic development and change are features of everyday practice that may be guided or perceived as being guided, by elites. This paper evolved from a study investigating the availability and role of nurses whose remit involved leading nursing research and development within acute NHS Trusts in two health regions in Southern England. The study design included telephone interviews with Directors of Nursing Services during which time the researchers engaged in a reflective analysis of their experiences of conducting research with an `elite' group. Important issues identified were the role of gatekeepers, engagement with elites and the use of the telephone interview method in this context. The paper examines these issues and makes a case for involving executive nurses in further research. The paper also offers strategies to help researchers design and implement telephone interview studies successfully to maximise access to the views and experiences of `hard to reach groups', such as elites, while minimising the associated disruption

    Fluctuation-dissipation relations in the non-equilibrium critical dynamics of Ising models

    We investigate the relation between two-time, multi-spin, correlation and response functions in the non-equilibrium critical dynamics of Ising models in d=1 and d=2 spatial dimensions. In these non-equilibrium situations, the fluctuation-dissipation theorem (FDT) is not satisfied. We find FDT `violations' qualitatively similar to those reported in various glassy materials, but quantitatively dependent on the chosen observable, in contrast to the results obtained in infinite-range glass models. Nevertheless, all FDT violations can be understood by considering separately the contributions from large wavevectors, which are at quasi-equilibrium and obey FDT, and from small wavevectors where a generalized FDT holds with a non-trivial limit fluctuation-dissipation ratio X. In d=1, we get X = 1/2 for spin observables, which measure the orientation of domains, while X = 0 for observables that are sensitive to the domain-wall motion. Numerical simulations in d=2 reveal a unique X = 0.34 for all observables. Measurement protocols for X are discussed in detail. Our results suggest that the definition of an effective temperature Teff = T / X for large length scales is generically possible in non-equilibrium critical dynamics.Comment: 26 pages, 10 figure

    Early transmission characteristics of influenza A(H1N1)v in Australia: Victorian state, 16 May – 3 June 2009

    Australia was one of the first countries of the southern hemisphere to experience influenza A(H1N1)v with community transmission apparent in Victoria, Australia, by 22 May 2009. With few identified imported cases, the epidemic spread through schools and communities leading to 897 confirmed cases by 3 June 2009. The estimated reproduction ratio up to 31 May 2009 was 2.4 (95% credible interval (CI): 2.1-2.6). Methods designed to account for undetected transmission reduce this estimate to 1.6 (95% CI: 1.5-1.8). Time varying reproduction ratio estimates show a steady decline in observed transmission over the first 14 days of the epidemic. This could be accounted for by ascertainment bias or a true impact of interventions including antiviral prophylaxis, treatment and school closure. Most cases (78%) in the first 19 days in Victoria were under the age of 20 years-old. Estimates suggest that the average youth primary case infected at least two other youths in the early growth phase, which was sufficient to drive the epidemic
