    The role of families in the stratification of attainment: Parental occupations, parental education and family structure in the 1990s.

    The closing decades of the 20th century have witnessed a large increase in the numbers of young people remaining in education post-16 rather than entering the labour market. Concurrently, overall educational attainment in General Certificate of Secondary Education (GCSE) qualifications in England and Wales has steadily increased since their introduction in 1988. The 1990s represent a key period of change in these trends. Some sociologists argue that processes of detraditionalisation have occurred whereby previous indicators of social inequality, such as social class, are less relevant to the transitions of young people from school to work. Sociologists from other traditions argue that inequalities persist in the stratification of educational attainment by the family backgrounds of young people but that these factors have changed during this period. This thesis is an investigation of the influence of family background factors upon GCSE attainment during the 1990s. This includes extensive statistical analysis of measures of parental occupation, parental education and family structure with gender, ethnicity, school type and housing tenure type within the Youth Cohort Study of England and Wales. These analyses include over 100,000 respondents in 6 cohorts of school leavers with the harmonisation of data from cohort 6 (1992) to the Youth Cohort Time Series for England, Wales and Scotland 1984-2002 (Croxford, Ianelli and Shapira 2007). By adding the 1992 data to existing 1990s cohorts, the statistical models fitted apply to the complete set of 1990s cohorts and are therefore able to provide insight for the whole of this period. Strong differentials by parental occupation persist throughout the 1990s and do not diminish despite the overall context of rising attainment. This relationship remains net of the other factors listed, irrespective of the measure of parental occupation or the GCSE attainment outcome measure used. This builds upon and supports previous work conducted using the Youth Cohort Study and suggests that stratification in educational attainment remains a significant factor. Gender and ethnicity remain further sources of persistent stratification in GCSE attainment. Following a discussion of the weighting system and features of the Youth Cohort Study as a dataset, a thorough investigation of missing data is included, with the results of multiply imputed datasets used to examine the potential for missing data to bias estimates. This includes a critique of these approaches in the context of survey data analysis. The findings from this investigation suggest the importance of survey data collection methods, the limitations of post-survey bias correction methods and provide a thorough investigation of the data. The analysis then develops and expands previous work by investigating variation in GCSE attainment by subjects studied, through Latent Class Analysis of YCS cohort 6 (1992). Of the four groups identified in the model, a clear division is noted between those middle-attaining groups with respect to attainment in Science and Mathematics. GCSE attainment in combinations of subjects studied is stratified particularly with respect to gender and ethnicity.     COMPOSITION AND OCCURRENCE OF THE GRANDISPORA MACULOSA ZONAL ASSEMBLAGE (MISSISSIPPIAN) IN THE SUBSURFACE OF THE CARNARVON BASIN AND THE COOLCALALAYA SUB-BASIN OF WESTERN AUSTRALIA, AND ITS GONDWANAN DISTRIBUTION

    The Grandispora maculosa miospore assemblage – initially described in 1968 from Middle-Late Mississippian strata of New South Wales (eastern Australia) – is well represented in samples examined herein from 10 Western Australian subsurface sections located in the northern Perth Basin (Coolcalalaya Sub-basin) and, to its immediate north, in several sub-basins of the southern and northern sectors of the Carnarvon Basin. Of particular stratigraphic-correlative importance is the presence of the eponymous G. maculosa together with, inter alia, Reticulatisporites magnidictyus, Verrucosisporites quasigobbettii, V. gregatus, Apiculiretusispora tersa, Raistrickia accinta, R. radiosa, Foveosporites pellucidus, and Cordylosporites asperidictyus. Four species are newly described herein: Apiculatasporites spiculatus, Dibolisporites sejunctus, Raistrickia corymbiata, and Vallatisporites valentulus. Published accounts from elsewhere in Gondwana collectively signify the widespread dissemination of the G. maculosa palynoflora, particularly through northern and western regions of the supercontinent, thus affording an effective means of intra-Gondwanan stratal correlation. Limited absolute dating and stratigraphic-successional considerations across Gondwana indicate that the age of the G. maculosa Assemblage can be bracketed within the middle Visean-early Serpukhovian of the Middle-Late Mississippian. This age is supported by the complete absence of bilaterally symmetrical, non-striate, saccate pollen grains, produced by walchian conifers, which were introduced globally (including in Australia) and near-synchronously late in the Serpukhovian.     Machine Learning in Falls Prediction; A cognition-based predictor of falls for the acute neurological in-patient population

    Background Information: Falls are associated with high direct and indirect costs, and significant morbidity and mortality for patients. Pathological falls are usually a result of a compromised motor system, and/or cognition. Very little research has been conducted on predicting falls based on this premise. Aims: To demonstrate that cognitive and motor tests can be used to create a robust predictive tool for falls. Methods: Three tests of attention and executive function (Stroop, Trail Making, and Semantic Fluency), a measure of physical function (Walk-12), a series of questions (concerning recent falls, surgery and physical function) and demographic information were collected from a cohort of 323 patients at a tertiary neurological center. The principal outcome was a fall during the in-patient stay (n = 54). Data-driven, predictive modelling was employed to identify the statistical modelling strategies which are most accurate in predicting falls, and which yield the most parsimonious models of clinical relevance. Results: The Trail test was identified as the best predictor of falls. Moreover, addition of any others variables, to the results of the Trail test did not improve the prediction (Wilcoxon signed-rank p < .001). The best statistical strategy for predicting falls was the random forest (Wilcoxon signed-rank p < .001), based solely on results of the Trail test. Tuning of the model results in the following optimized values: 68% (+- 7.7) sensitivity, 90% (+- 2.3) specificity, with a positive predictive value of 60%, when the relevant data is available. Conclusion: Predictive modelling has identified a simple yet powerful machine learning prediction strategy based on a single clinical test, the Trail test. Predictive evaluation shows this strategy to be robust, suggesting predictive modelling and machine learning as the standard for future predictive tools

    Cryptococcal infections over a 15 year period at a tertiary facility & impact of guideline management

    Introduction: The aim of this research paper is to determine the incidence, risk factors and clinical outcome of all patients diagnosed and treated for cryptococcosis at our institution. We also aim to compare treatment outcome following introduction of the 2010 Infectious Disease Society of America (IDSA) guidelines. Methods: Retrospective analysis of all patients diagnosed and treated for cryptococcal infection occurring between January 2001 and December 2015. Results: Of 102 patients diagnosed with cryptococcal infection, 97 were eligible for study inclusion. There appears to be an overall increased incidence of cryptococcosis in both transplant and non-transplant cohorts with a peak in 2015 of 6 transplant and 13 non-transplant cases. 38/52 (73%) of identified isolates were C. neoformans, and 14/52 (27%) were C. gattii. Notably, 14/14 (100%) of C. gattii isolates were associated with meningitis, as compared to only 38/64 (59%) C. neoformans associated with meningitis (p: 0.003). It appears that patients presenting with cough are less likely to have meningitis, 17/27 (63%), (p: 0.005). When stratifying for culture positive meningitis lumbar puncture opening pressure, the median in the culture positive cohort was 31.5cmH2 O compared with 15.5cmH2O (p:0.036). Multiple admissions were required prior to diagnosis in the majority of cases with only 18/72 (25%) diagnosed on 1st presentation. Post-guideline mortality has improved from 17.1% to 6.1% (p: 0.046). Conclusion: Cryptococcal infection remains relatively uncommon, but there appears to be an increasing trend in incidence. Overall mortality is relatively low and has improved since introduction of the 2010 IDSA guidelines

    Structure–property insights into nanostructured electrodes for Li-ion batteries from local structural and diffusional probes

    Microwave heating presents a faster, lower energy synthetic methodology for the realization of functional materials. Here, we demonstrate for the first time that employing this method also leads to a decrease in the occurrence of defects in olivine structured LiFe1−xMnxPO4. For example, the presence of antisite defects in this structure precludes Li+ diffusion along the b-axis leading to a significant decrease in reversible capacities. Total scattering measurements, in combination with Li+ diffusion studies using muon spin relaxation (μ+SR) spectroscopy, reveal that this synthetic method generates fewer defects in the nanostructures compared to traditional solvothermal routes. Our interest in developing these routes to mixed-metal phosphate LiFe1−xMnxPO4 olivines is due to the higher Mn2+/3+ redox potential in comparison to the Fe2+/3+ pair. Here, single-phase LiFe1−xMnxPO4 (x = 0, 0.25, 0.5, 0.75 and 1) olivines have been prepared following a microwave-assisted approach which allows for up to 4 times faster reaction times compared to traditional solvothermal methods. Interestingly, the resulting particle morphology is dependent on the Mn content. We also examine their electrochemical performance as active electrodes in Li-ion batteries. These results present microwave routes as highly attractive for reproducible, gram-scale syntheses of high quality nanostructured electrodes which display close to theoretical capacity for the full iron phase

    Effect of medical student preference on rural clinical school experience and rural career intentions

    Introduction: The key parameter for rural clinical schools (RCSs) is to provide at least 1 year of clinical training in rural areas for 25% of Australian Commonwealth supported medical students with the intent to influence future rural medical workforce outcomes. The objective of this study was to describe the association between a medical student’s selection preference and their RCS experience and rural career intent. Methods: Medical students completing an RCS placement in 2012 and 2013 were encouraged to complete a survey regarding their experience and future career intent. Data were analysed to compare medical students for whom the RCS was their first choice with students who described the RCS as other than their first preference. Results: Students for whom RCS was their first choice (724/1092) were significantly more likely to be female, come from a rural background and be from an undergraduate program. These students reported more positive experiences of all aspects of the RCS program (costs, access, support and networks, safety) and were 2.36 times more likely to report intentions to practice in a non-metropolitan area (odds ratio(OR)=2.36 (95% confidence interval(CI)=1.82–3.06), p\u3c0.001). This was true for students of rural (OR=3.11 (95%CI=1.93–5.02), p\u3c0.001) and metropolitan backgrounds (OR=2.07 (95%CI=1.48–2.89), p\u3c0.001). More students in the first-choice group (68.8%) intended to practice in a regional area (not a capital or major city), significantly higher than the 48.4% of participants in the other-preference group (χ2 (1) 42.79, p\u3c0.001). Conclusions: The decision to choose an RCS placement is a marker of rural career intention and a positive rural training experience for students of both rural and metropolitan backgrounds. It may be important to identify other-preference students and their specific social support needs to ensure a positive perception of a future rural career
