    An analysis of risk factors of non-fatal drowning among children in rural areas of Guangdong Province, China: a case-control study

    <p/> <p>Background</p> <p>Drowning is a major cause of morbidity and mortality for children, yet non-fatal drowning remains poorly understood. The aim of this study was to explore potential modifiable risk factors of non-fatal drowning among children in rural areas of China.</p> <p>Methods</p> <p>A cross-sectional survey was first conducted to obtain non-fatal drowning cases, and 7432 students in grades three to eight from 17 schools participated in the cross sectional survey. Of these, 805 students reported that they experienced non-fatal drowning in the previous year. Then 368 cases were selected randomly to participate in a 1:1 matched case-control study. Each drowning case was matched by one control with the same sex and similar age (the gap less than 2 years) who was selected randomly from the same class.</p> <p>Results</p> <p>Boys were more likely to be involved in non-fatal drowning. Non-fatal drowning most often happened in the afternoon (65.1%) and natural bodies of water were the most common sites of drowning (71.1%). Swimming, diving and playing in natural waters were the leading activities that preceded non-fatal drowning. The significant risk factors for non-fatal drowning were swimming in natural waters without adult supervision (OR = 3.40, 95% CI: 1.92-6.03), playing in or beside natural waters (OR = 2.08, 95% CI: 1.17-3.70) and poor swimming skills (OR = 2.74, 95% CI: 1.14-6.62). However, the following variables were protective factors: supervisor aged 30 years or over (OR = 0.20, 95% CI: 0.09-0.49) and no water activities (OR = 0.36, 95% CI: 0.18-0.70).</p> <p>Conclusions</p> <p>The reduction in dangerous water activities, swimming training and enhancement in supervision among children might decrease the risk of non-fatal drowning.</p

    Diagnosing Death with Diatoms: A Retrospective Study of Forensic Cases in Himachal Pradesh, India

    Background: Diatoms found inside the body of a drowned victim may serve as corroborative evidence in the diagnosis of cause of death. Diatom has proved to be the only golden standard for diagnosis and confirmation of drowning deaths whether the drowning was ante-mortem or post-mortem.Methods: The study was based on the cases of death due to drowning received from three districts of Northern Range of Himachal Pradesh, India during the period of five years from Ist January, 2010 to 31st December, 2015 for diatom test. A total of 66 human cases were examined for detection of diatoms. The detailed information regarding cause of death, socio-demographic factors and other associated information was gathered. The acid digestion method accepted worldwide for diatom extraction was used.Results: Male victims predominated (75.75%). Most common affected age group was 21-40 years (53.02%). The youngest victim was a girl of 4 years age who drowned accidentally in a water tank, while the oldest victim being a 86 years old who fell accidently in a river. Married victims contributed to 33 (50.0%) and unmarried to 21 (31.81%). 13 (19.69%) drowning cases belonged to students followed by labourers (18.18%) and housewives (15.15%). The highest reported cases of drowning were from the rural areas (65.15%) followed by urban areas (16.66%). Majority (81.81%) of the victims drowned in fresh water. The incidences of drowning were more in water of natural flowing streams (khuds) (31.81%) followed by rivers (22.72%), nullahs/rivulets (12.12%), wells (9.09%), kuhls/water channels (7.57%), canals (6.06%), ponds (3.03%), water tanks (3.03%) and check dam, dam and waterfall in one each (4.54%) respectively. Accidental drowning was the most common cause of death (37.87%). Highest (51.51%) percentage of drowning cases was noticed during the months of monsoon/rainy season. Blood on mouth and nostrils was present in 14 (21.21%), froth from mouth, nose, larynx and trachea in 35 (53.03%) cases. Diatom-test was found positive in 62 (93.93%) cases. The results of the study revealed the occurrence of various varieties of diatoms in water bodies of northern region of Himachal Pradesh. The most common diatom genera detected were Navicula (86.36%).Conclusion: The study concluded that diatoms are amongst the important biological forensic evidences in diagnosing the cause and place of death due to drowning. Gender based examination revealed higher percentage of males involved in drowning fatalities and the accidental submersion was the commonest manner of death

    Obesity prevention and personal responsibility: the case of front-of-pack food labelling in Australia

    <p>Abstract</p> <p>Background</p> <p>In Australia, the food industry and public health groups are locked in serious struggle for regulatory influence over the terms of front-of-pack food labelling. Clear, unambiguous labelling of the nutritional content of pre-packaged foods and of standardized food items sold in chain restaurants is consistent with the prevailing philosophy of 'personal responsibility'. An interpretive, front-of-pack labelling scheme has the capacity to encourage healthier patterns of eating, and to be a catalyst for improvements in the nutritional quality of food products through re-formulation. On the other hand, the strength of opposition of the Australian Food and Grocery Council to 'Traffic Light Labelling', and its efforts to promote a non-interpretive, voluntary scheme, invite the interpretation that the food industry is resistant to any reforms that could destabilise current (unhealthy) purchasing patterns and the revenues they represent.</p> <p>Discussion</p> <p>This article argues that although policies that aim to educate consumers about the nutritional content of food are welcome, they are only one part of a broader basket of policies that are needed to make progress on obesity prevention and public health nutrition. However, to the extent that food labelling has the capacity to inform and empower consumers to make healthier choices - and to be a catalyst for improving the nutritional quality of commercial recipes - it has an important role to play. Furthermore, given the dietary impact of meals eaten in fast food and franchise restaurants, interpretive labelling requirements should not be restricted to pre-packaged foods.</p> <p>Summary</p> <p>Food industry resistance to an interpretive food labelling scheme is an important test for government, and a case study of how self-interest prompts industry to promote weaker, voluntary schemes that pre-empt and undermine progressive public health regulation.</p

    Statewide surveillance in California determined that the highest drowning rate from 1980 through 1989 was for the rural, desert county of Imperial (21.9 drownings per 100,000 population). To identify activities associated with drowning in this county, the authors abstracted data from the county sheriff-coroner's reports. From 1980 through 1990, there were 317 unintentional drownings; 85 percent occurred in irrigation canals. The activity prior to drowning was known for 262 persons (83 percent), and the most common activity was illegal entry into the United States. Overall, 140 persons (53 percent) were illegal entrants. Ninety-three percent of illegal entrants drowned in the All American Canal; the monthly drowning rate increased as the monthly average water velocity in the canal increased (r = 0.36; P < 0.001). Forty-eight persons (18 percent) drowned while riding in or on a land vehicle (automobile, pick-up truck, motorcycle, dune buggy, or tractor), the second most common activity associated with drowning. Seventy percent of the 23 drivers had an alcohol concentration of 100 milligrams per deciliter or more, California's limit for intoxication. To reduce drownings in Imperial County, prevention strategies should target persons engaged in at-risk activities near bodies of water.     The District of Columbia and Gun Control Laws: Homicides and Social Impacts

    Abstract The District of Columbia has one of the toughest gun laws in the United States, yet the city has one of the highest homicide rates in the nation and much of this gun violence is concentrated in the southeastern part of the city. The framework for the study was comprised of (a) the psychological theory based on many arguments, such as labeling weapons as symbols of male superiority and power, and (b) the social contract theory. Finding the perceptions about the relationship between gun control laws and homicide rates in Washington D.C. was the essential aim of the study. This qualitative case study examined the perceptions of 8 people relative to the relationship between gun control laws and homicide rates in the southeast section of the District of Columbia. Results revealed that the manufacturing and legal acquisition of firearms did not pose any problem for the participants, given the legitimate purpose of using guns. Rather, owners who use guns to perpetrate violent acts due to ineffective distribution and lack of enforcement of laws, added to gun proliferation pose a serious problem. The findings may be used by policymakers to develop policies in controlling, managing, producing, distributing, and owning guns as well as decreasing gun violence across the nation
