Occupational health professionals’ knowledge, understanding and use of work ability
BACKGROUND:
The concept of work ability (WA) has a 30-year history in Finland, where it has been used extensively in occupational health (OH) research and practice. The extent to which WA has been integrated into UK OH practice is unclear.
AIMS:
(i) To compare knowledge, understanding and use of WA among OH nurses and physicians in the UK and Finland and (ii) to identify factors that influence the use of WA in Finnish OH practice.
METHODS:
An online questionnaire administered to OH practitioners in the UK and Finland.
RESULTS:
A total of 436 UK and 97 Finnish OH practitioners completed the questionnaire. Though familiarity with the term 'work ability' was similar among Finnish and UK respondents, substantial differences were found in understanding of the term. Ninety-five per cent (Finland) and 7% (UK) of respondents reported using the Work Ability Index (WAI), a validated measure of WA, in their practice. Finnish respondents indicated that they used the WAI results primarily for individual case management, understanding population health trends, health promotion and determining WA across age groups. UK respondents primarily attributed failure to use the WAI to lack of training. Primary factors influencing use of WA in Finland included it being considered common practice and an effect ive system by which to conduct individual assessments.
CONCLUSIONS:
There are large differences between Finland and the UK in the assessment of WA in OH practice. Differences may reflect contrasting OH legislative frameworks
Molecular Aspects of Secretory Granule Exocytosis by Neurons and Endocrine Cells
Neuronal communication and endocrine signaling are fundamental for integrating
the function of tissues and cells in the body. Hormones released by endocrine
cells are transported to the target cells through the circulation. By contrast, transmitter
release from neurons occurs at specialized intercellular junctions, the synapses.
Nevertheless, the mechanisms by which signal molecules are synthesized,
stored, and eventually secreted by neurons and endocrine cells are very similar.
Neurons and endocrine cells have in common two different types of secretory
organelles, indicating the presence of two distinct secretory pathways. The synaptic
vesicles of neurons contain excitatory or inhibitory neurotransmitters, whereas the
secretory granules (also referred to as dense core vesicles, because of their electron
dense content) are filled with neuropeptides and amines. In endocrine cells, peptide
hormones and amines predominate in secretory granules. The function and content
of vesicles, which share antigens with synaptic vesicles, are unknown for most
endocrine cells. However, in B cells of the pancreatic islet, these vesicles contain
GABA, which may be involved in intrainsular signaling.'
Exocytosis of both synaptic vesicles and secretory granules is controlled by
cytoplasmic calcium. However, the precise mechanisms of the subsequent steps,
such as docking of vesicles and fusion of their membranes with the plasma membrane,
are still incompletely understood. This contribution summarizes recent observations
that elucidate components in neurons and endocrine cells involved in
exocytosis. Emphasis is put on the intracellular aspects of the release of secretory
granules that recently have been analyzed in detail
The carboxy-terminal fragment of α1A calcium channel preferentially aggregates in the cytoplasm of human spinocerebellar ataxia type 6 Purkinje cells
Spinocerebellar ataxia type 6 (SCA6) is an autosomal dominant neurodegenerative disease caused by a small polyglutamine (polyQ) expansion (control: 4–20Q; SCA6: 20–33Q) in the carboxyl(C)-terminal cytoplasmic domain of the α1A voltage-dependent calcium channel (Cav2.1). Although a 75–85-kDa Cav2.1 C-terminal fragment (CTF) is toxic in cultured cells, its existence in human brains and its role in SCA6 pathogenesis remains unknown. Here, we investigated whether the small polyQ expansion alters the expression pattern and intracellular distribution of Cav2.1 in human SCA6 brains. New antibodies against the Cav2.1 C-terminus were used in immunoblotting and immunohistochemistry. In the cerebella of six control individuals, the CTF was detected in sucrose- and SDS-soluble cytosolic fractions; in the cerebella of two SCA6 patients, it was additionally detected in SDS-insoluble cytosolic and sucrose-soluble nuclear fractions. In contrast, however, the CTF was not detected either in the nuclear fraction or in the SDS-insoluble cytosolic fraction of SCA6 extracerebellar tissues, indicating that the CTF being insoluble in the cytoplasm or mislocalized to the nucleus only in the SCA6 cerebellum. Immunohistochemistry revealed abundant aggregates in cell bodies and dendrites of SCA6 Purkinje cells (seven patients) but not in controls (n = 6). Recombinant CTF with a small polyQ expansion (rCTF-Q28) aggregated in cultured PC12 cells, but neither rCTF-Q13 (normal-length polyQ) nor full-length Cav2.1 with Q28 did. We conclude that SCA6 pathogenesis may be associated with the CTF, normally found in the cytoplasm, being aggregated in the cytoplasm and additionally distributed in the nucleus
Natural Light Exposure, Sleep and Depression among Day Workers and Shiftworkers at Arctic and Equatorial Latitudes
OBJECTIVES: This study aimed to investigate the relationship between individual natural light exposure, sleep need, and depression at two latitudes, one extreme with a few hours of light per day during winter, and the other with equal hours of light and darkness throughout the year. METHODS: This cross-sectional study included a sample of Brazilian workers (Equatorial, n = 488 workers) and a Swedish sample (Arctic, n = 1,273). RESULTS: The reported mean total natural light exposure per 4-week cycle differed significantly between the Equatorial and Arctic regions. However, shiftworkers from both sites reported similar hours of natural light exposure. Short light exposure was a predictor for insufficient sleep. CONCLUSION: Reduced exposure to natural light appears to increase the perception of obtaining insufficient sleep. Arctic workers were more prone to develop depression than Equatorial workers
