In many developed countries around the world residential care interventions for children and adolescents have come under increasing scrutiny. Against this background an international summit was organised in England (spring 2016) with experts from 13 countries to reflect on therapeutic residential care (TRC). The following working definition of TRC was leading: "Therapeutic residential care involves the planful use of a purposefully constructed, multi-dimensional living environment designed to enhance or provide treatment, education, socialization, support, and protection to children and youth with identified mental health or behavioural needs in partnership with their families and in collaboration with a full spectrum of community based formal and informal helping resources". The meeting was characterised by exchange of information and evidence, and by preparing an international research agenda. In addition, the outlines of a consensus statement on TRC were discussed. This statement, originally published in English and now reproduced in a Dutch translation, comprises inter alia five basic principles of care that according to the Work Group on Therapeutic Residential Care should be guiding for residential youth care provided at any time

AI-Enabled Medical Education: Threads of Change, Promising Futures, and Risky Realities Across Four Potential Future Worlds

BackgroundThe rapid trajectory of artificial intelligence (AI) development and advancement is quickly outpacing society's ability to determine its future role. As AI continues to transform various aspects of our lives, one critical question arises for medical education: what will be the nature of education, teaching, and learning in a future world where the acquisition, retention, and application of knowledge in the traditional sense are fundamentally altered by AI? ObjectiveThe purpose of this perspective is to plan for the intersection of health care and medical education in the future. MethodsWe used GPT-4 and scenario-based strategic planning techniques to craft 4 hypothetical future worlds influenced by AI's integration into health care and medical education. This method, used by organizations such as Shell and the Accreditation Council for Graduate Medical Education, assesses readiness for alternative futures and effectively manages uncertainty, risk, and opportunity. The detailed scenarios provide insights into potential environments the medical profession may face and lay the foundation for hypothesis generation and idea-building regarding responsible AI implementation. ResultsThe following 4 worlds were created using OpenAI’s GPT model: AI Harmony, AI conflict, The world of Ecological Balance, and Existential Risk. Risks include disinformation and misinformation, loss of privacy, widening inequity, erosion of human autonomy, and ethical dilemmas. Benefits involve improved efficiency, personalized interventions, enhanced collaboration, early detection, and accelerated research. ConclusionsTo ensure responsible AI use, the authors suggest focusing on 3 key areas: developing a robust ethical framework, fostering interdisciplinary collaboration, and investing in education and training. A strong ethical framework emphasizes patient safety, privacy, and autonomy while promoting equity and inclusivity. Interdisciplinary collaboration encourages cooperation among various experts in developing and implementing AI technologies, ensuring that they address the complex needs and challenges in health care and medical education. Investing in education and training prepares professionals and trainees with necessary skills and knowledge to effectively use and critically evaluate AI technologies. The integration of AI in health care and medical education presents a critical juncture between transformative advancements and significant risks. By working together to address both immediate and long-term risks and consequences, we can ensure that AI integration leads to a more equitable, sustainable, and prosperous future for both health care and medical education. As we engage with AI technologies, our collective actions will ultimately determine the state of the future of health care and medical education to harness AI's power while ensuring the safety and well-being of humanity

Tämä opinnäytetyö käsittelee lehtikuusen kasvatusmahdollisuuksia Tiilään koulutilalla Itä-Uudellamaalla. Nimestään huolimatta tila on yksityisomistuksessa. Työn tarkoituksena on selvittää, löytyykö tilalta lehtikuusen kasvatukseen sopivia metsikkökuvioita, ja onko lehtikuusen kasvatus taloudellisesti kilpailukykyinen vaihtoehto muille puulajeille. Opinnäytetyön teoriaosuudessa käsitellään lehtikuusen ominaisuuksia, käyttömahdollisuuksia ja lehtikuusen kasvatusta Suomessa. Koska Suomessa kasvatetaan puuntuotostarkoituksessa lähes ainoastaan siperianlehtikuusta, myös tässä opinnäytetyössä tarkastellaan nimenomaan siperianlehtikuusen kasvatuksen kannattavuutta. Opinnäytetyössä on selvitetty lehtikuusen kasvatuksesta aiheutuvia kustannuksia sekä lehtikuusen puuntuotospotentiaalia erilaisilla kasvupaikoilla ja puun myynnistä saatavia tuloja. Lehtikuusen kasvatuksen kannattavuutta on vertailtu kuusen ja männyn kasvatuksen kannattavuuteen. Tiilään koulutilalla lehtikuusen kasvatukseen sopivat metsikkökuviot on selvitetty tilan metsä-suunnitelman tietojen pohjalta. The cultivation possibilities of larch in agricultural training farm of Tiilää This thesis introduced the cultivation possibilities of larch in the agricultural training farm of Tiilää in Eastern Uusimaa. The farm is in private ownership. The purpose of this thesis was to find out if there were copse areas which are convenient for cultivating larch on the farm, and if cultivating larch was economically competitive comparing to other tree species. The theoretical part of the thesis introduced the features, use and cultivation of larch in Finland. Because it is the Siberian larch that is cultivated in Finland, the focus of the present thesis was explicitly on the profitability of cultivating Siberian larch. This thesis introduced costs of cultivating larch, incomes from selling timber and the growth of larch in different habitats. Profitability of cultivating larch was compared to cultivating spruce or pine. Copse areas which are convenient for cultivating larch in agricultural training farm of Tiilää were found on basis of the forest management plan. Information about cost caused by management and yield estimations were obtained, for example, from Forest Management Association Uusimaa. Estimations of tree growth were founded on secondary literature, models of felling trees and cubic tables of trees and also MOTTI- computer program formulated by Finnish Forest Research Institute. The result showed that growth of larch, spruce and pine were quite similar in the habitats that were compared. The economical profitability of cultivation of larch depends on incomes obtained from selling timber
