    Evaluation of industry payments and financial conflict of interest disclosures among task force authors of Endocrine Society clinical practice guidelines

    Introduction: Clinical practice guidelines are considered the gold standard for disease management and treatment. Industry payments to guideline authors may influence their clinical recommendations, potentially resulting in medical and/or financial consequences to patients.Research Question: Determine the extent Endocrine Society guideline authors receive industry payments and report financial conflicts of interest in adherence to the Physician Payments Sunshine Provision of the Affordable Care Act.Study Design: Cross-sectional analysis of all clinical practice guidelines published by the Endocrine Society since the Sunshine Provision mandate.Methods: We searched the Endocrine Society's website for clinical guidelines published between January 2014 and December 2017. Identified guideline authors were independently searched by two investigators on the Open Payments Database. Received payments were extracted and statistically analyzed (excluding food/beverage payments). Payments were cross-referenced with corresponding author disclosure statements.Results: Of the 57 evaluable guideline authors, 34 authors (59.6%) received at least one industry payment. Of these authors, thirty-three (57.89%) received ≥ 1,000,twentysix(45.611,000, twenty-six (45.61%) ≥ 10,000, twenty-two (38.60%) ≥ 50,000,andtwentyone(36.8450,000, and twenty-one (36.84%) ≥ 100,000. Sixteen authors (28.07%) received ≥ 250,000inindustrypayments.Mediantotalpaymentswere250,000 in industry payments. Median total payments were 4,060 (interquartile range [IQR] 0263,264.23).Twentyseven(47.370-263,264.23). Twenty-seven (47.37%) financial disclosure statements were inaccurate. Median payment (minus food/beverage) for inaccurate disclosures were 28,523.93 (IQR 5,71494,418.02),withapaymenttotalof5,714-94,418.02), with a payment total of 2,870,485.27.Conclusion: Industry payments among Endocrine Society clinical practice guideline authors were widespread, with several exceeding $250,000. Nearly half of author disclosure statements were inaccurate. The Endocrine Society's disclosure policy should be more strictly enforced for future guideline authors

    Technical Basis for Gas-Phase Vadose Zone Remediation Technologies at Hanford: A Review -12186

    ABSTRACT In situ vadose zone remediation approaches are being evaluated as potential options to mitigate the transport of inorganic and radionuclide contaminants from the vadose zone to the groundwater. Some of the candidate approaches are based on changing the contaminant or subsurface conditions in a way that slows downward migration of the contaminants through the vadose zone using amendments delivered in the gas-phase. Two promising approaches that have undergone testing at Hanford include soil desiccation to address technetium-99 contamination and ammonia-induced sequestration of uranium. For soil desiccation, a dry gas is injected to desiccate a targeted portion of the subsurface and thereby decrease contaminant movement by removing moisture and decreasing the hydraulic conductivity of the desiccated zone. Ammonia-induced sequestration of uranium relies on changing the pore water chemistry, primarily through pH changes, to induce dissolution and precipitation processes that decrease the amount of mobile uranium in the vadose zone

    Vacancy Control in Acene Blends Links Exothermic Singlet Fission to Coherence

    The fission of singlet excitons into triplet pairs in organic materials holds great technological promise, but the rational application of this phenomenon is hampered by a lack of understanding of its complex photophysics. Here, we use the controlled introduction of vacancies by means of spacer molecules in tetracene and pentacene thin films as a tuning parameter complementing experimental observables to identify the operating principles of different singlet fission pathways. Time-resolved spectroscopic measurements in combination with microscopic modelling enables us to demonstrate distinct scenarios, resulting from different singlet-to-triplet pair energy alignments. For pentacene, where fission is exothermic, coherent mixing between the photoexcited singlet and triplet-pair states is promoted by vibronic resonances, which drives the fission process with little sensitivity to the vacancy concentration. Such vibronic resonances do not occur for endothermic materials such as tetracene, for which we find fission to be fully incoherent; a process that is shown to slow down with increasing vacancy concentration

    OSCE best practice guidelines—applicability for nursing simulations

    Background: Objective structured clinical examinations (OSCEs) have been used for many years within healthcare programmes as a measure of students’ and clinicians’ clinical performance. OSCEs are a form of simulation and are often summative but may be formative. This educational approach requires robust design based on sound pedagogy to assure practice and assessment of holistic nursing care. As part of a project testing seven OSCE best practice guidelines (BPGs) across three sites, the BPGs were applied to an existing simulation activity. The aim of this study was to determine the applicability and value of the OSCE BPGs in an existing formative simulation. Methods: A mixed methods approach was used to address the research question: in what ways do OSCE BPGs align with simulations. The BPGs were aligned and compared with all aspects of an existing simulation activity offered to first-year nursing students at a large city-based university, prior to their first clinical placement in an Australian healthcare setting. Survey questions, comprised of Likert scales and free-text responses, used at other sites were slightly modified for reference to simulation. Students’ opinions about the refined simulation activity were collected via electronic survey immediately following the simulation and from focus groups. Template analysis, using the BPGs as existing or a priori thematic codes, enabled interpretation and illumination of the data from both sources.Results: Few changes were made to the existing simulation plan and format. Students’ responses from surveys (n = 367) and four focus groups indicated that all seven BPGs were applicable for simulations in guiding their learning, particularly in the affective domain, and assisting their perceived needs in preparing for upcoming clinical practice. Discussion: Similarities were found in the intent of simulation and OSCEs informed by the BPGs to enable feedback to students about holistic practice across affective, cognitive and psychomotor domains. The similarities in this study are consistent with findings from exploring the applicability of the BPGs for OSCEs in other nursing education settings, contexts, universities and jurisdictions. The BPGs also aligned with other frameworks and standards often used to develop and deliver simulations. Conclusions: Findings from this study provide further evidence of the applicability of the seven OSCE BPGs to inform the development and delivery of, in this context, simulation activities for nurses. The manner in which simulation is offered to large cohorts requires further consideration to meet students’ needs in rehearsing the registered nurse role

    Striatal vs extrastriatal dopamine D2 receptors in antipsychotic response - a double-blind PET study in schizophrenia

    Blockade of dopamine D2 receptors remains a common feature of all antipsychotics. It has been hypothesized that the extrastriatal (cortical, thalamic) dopamine D2 receptors may be more critical to antipsychotic response than the striatal dopamine D2 receptors. This is the first double-blind controlled study to examine the relationship between striatal and extrastriatal D2 occupancy and clinical effects. Fourteen patients with recent onset psychosis were assigned to low or high doses of risperidone (1 mg vs 4 mg/day) or olanzapine (2.5 mg vs 15 mg/day) in order to achieve a broad range of D2 occupancy levels across subjects. Clinical response, side effects, striatal ([11C]-raclopride-positron emission tomography (PET)), and extrastriatal ([11C]-FLB 457-PET) D2 receptors were evaluated after treatment. The measured D2 occupancies ranged from 50 to 92% in striatal and 4 to 95% in the different extrastriatal (frontal, temporal, thalamic) regions. Striatal and extrastriatal occupancies were correlated with dose, drug plasma levels, and with each other. Striatal D2 occupancy predicted response in positive psychotic symptoms (r=0.62, p=0.01), but not for negative symptoms (r=0.2, p=0.5). Extrastriatal D2 occupancy did not predict response in positive or negative symptoms. The two subjects who experienced motor side effects had the highest striatal occupancies in the cohort. Striatal D2 blockade predicted antipsychotic response better than frontal, temporal, and thalamic occupancy. These results, when combined with the preclinical data implicating the mesolimbic striatum in antipsychotic response, suggest that dopamine D2 blockade within specific regions of the striatum may be most critical for ameliorating psychosis in schizophrenia.peer-reviewe

    Social support and Quality of Life: a cross-sectional study on survivors eight months after the 2008 Wenchuan earthquake

    <p>Abstract</p> <p>Background</p> <p>The 2008 Wenchuan earthquake resulted in extensive loss of life and physical and psychological injuries for survivors. This research examines the relationship between social support and health-related quality of life for the earthquake survivors.</p> <p>Methods</p> <p>A multistage cluster sampling strategy was employed to select participants from 11 shelters in nine counties exposed to different degrees of earthquake damage, for a questionnaire survey. The participants were asked to complete the Short Form 36 and the Social Support Rating Scale eight months after the earthquake struck. A total of 1617 participants returned the questionnaires. The quality of life of the survivors (in the four weeks preceding the survey) was compared with that of the general population in the region. Multivariate logistic regression analysis and canonical correlation analysis were performed to determine the association between social support and quality of life.</p> <p>Results</p> <p>The earthquake survivors reported poorer quality of life than the general population, with an average of 4.8% to 19.62% reduction in scores of the SF-36 (p < 0.001). The multivariate logistic regression analysis showed that those with stronger social support were more likely to have better quality of life. The canonical correlation analysis found that there was a discrepancy between actual social support received and perceived social support available, and the magnitude of this discrepancy was inversely related to perceived general health (rs = 0.467), and positively related to mental health (rs = 0.395).</p> <p>Conclusion</p> <p>Social support is associated with quality of life in the survivors of the earthquake. More attention needs to be paid to increasing social support for those with poorer mental health.</p
