    The safety of digital mental health interventions: Systematic review and recommendations

    Background: Evidence suggests that digital mental health interventions (DMHIs) for common mental health conditions are effective. However, digital interventions, such as face-to-face therapies, pose risks to patients. A safe intervention is considered one in which the measured benefits outweigh the identified and mitigated risks. Objective: This study aims to review the literature to assess how DMHIs assess safety, what risks are reported, and how they are mitigated in both the research and postmarket phases and building on existing recommendations for assessing, reporting, and mitigating safety in the DMHI and standardizing practice. Methods: PsycINFO, Embase, and MEDLINE databases were searched for studies that addressed the safety of DMHIs. The inclusion criteria were any study that addressed the safety of a clinical DMHI, even if not as a main outcome, in an adult population, and in English. As the outcome data were mainly qualitative in nature, a meta-analysis was not possible, and qualitative analysis was used to collate the results. Quantitative results were synthesized in the form of tables and percentages. To illustrate the use of a single common safety metric across studies, we calculated odds ratios and CIs, wherever possible. Results: Overall, 23 studies were included in this review. Although many of the included studies assessed safety by actively collecting adverse event (AE) data, over one-third (8/23, 35%) did not assess or collect any safety data. The methods and frequency of safety data collection varied widely, and very few studies have performed formal statistical analyses. The main treatment-related reported AE was symptom deterioration. The main method used to mitigate risk was exclusion of high-risk groups. A secondary web-based search found that 6 DMHIs were available for users or patients to use (postmarket phase), all of which used indications and contraindications to mitigate risk, although there was no evidence of ongoing safety review. Conclusions: The findings of this review show the need for a standardized classification of AEs, a standardized method for assessing AEs to statically analyze AE data, and evidence-based practices for mitigating risk in DMHIs, both in the research and postmarket phases. This review produced 7 specific, measurable, and achievable recommendations with the potential to have an immediate impact on the field, which were implemented across ongoing and future research. Improving the quality of DMHI safety data will allow meaningful assessment of the safety of DMHIs and confidence in whether the benefits of a new DMHI outweigh its risks

    Friendships and social acceptance of children with disabilities: the role of classroom quality, individual skills, and ECEC dosage

    Positive social experiences with peers are important for children with disabilities attending inclusive early childhood education (ECE). This study investigated the associations between classroom quality; children’s verbal, social, and Behavioral skills; and the friendships and social acceptance of children with disabilities, while testing the moderating effects of ECE dosage. Eighty-six children with disabilities (63 boys; Mage = 67.53 years, SD = 10.54 years), attending 86 inclusive ECE classrooms from the Metropolitan Area of Lisbon, Portugal, participated in the study. We found no effects of classroom quality on children’s social acceptance and friendships and ECE dosage did not moderate this association. However, ECE dosage moderated the association between children’s individual skills and their social acceptance and friendships. Findings suggest that more time in ECE is not enough to support children’s social acceptance, particularly for children with increased language and behavioral difficulties, who likely require more intensive and individualized interventions to support their peer-related social experiences.info:eu-repo/semantics/acceptedVersio

    Classroom quality and children's social skills and problem behaviors: dosage and disability status as moderators

    Multiple studies have reported associations between early childhood education (ECE) quality and dosage and children’s social and behavior development, with some suggesting that this association may be stronger for specific groups of children. In this study, we examined the association between classroom quality and children’s social skills and problem behaviors, as reported by ECE teachers, as well as the moderating effects of ECE dosage and children’s disability status. Participants were 222 children (Mage = 63.75, SD = 7.77), including 180 typically developing (90 boys) and 42 children with disabilities (29 boys), from 44 inclusive classrooms in the Metropolitan Area of Lisbon, Portugal. Our results indicated that children’s social skills and behavior problems were not directly associated with observed classroom quality domains. However, lower classroom organization predicted lower social skills and higher externalizing behavior at higher number of months with the lead teacher; and instructional support predicted increased social skills for children with disabilities. Days absent from school predicted lower social skills.     Social experiences of children with disabilities in inclusive Portuguese preschool settings

    Based on peer sociometric reports, we examined how number of friendships, social acceptance, and characteristics of social networks vary as a function of disability profile. We also investigated teachers' awareness of the sociometric status of young children with disabilities. Participants were 86 children with disabilities (63 boys) enrolled in inclusive preschool classrooms of the Metropolitan Area of Lisbon, Portugal (M-age = 67.33 months, SD = 10.54). Findings suggest that children with severe or sociobehavioral disabilities may be at increased risk of social rejection and isolation, having fewer friends and lower social network centrality than children with mild disabilities. Low agreement between teachers' classifications of the social status of children with disabilities and classifications based on peer nominations raises concerns about their awareness of processes of social rejection and neglect.     A 5GHz1.8V low power CMOS low-noise amplifier

    Wireless local-area networks (WLANs) have been deployed as office and home communications infrastructures worldwide. The diversification of the standards, such as IEEE 802.11 series demands the design of RF front-ends. Low power consumption is one of the most important design concerns in the application of those technologies. To maintain competitive hardware costs, CMOS has been used since it is the best solution for low cost and high integration processing, allowing analog circuits to be mixed with digital ones. In the receiver chain, the low noise amplifier (LNA) is one of the most critical blocks in a transceiver design. The sensitivity is mainly determined by the LNA noise figure and gain. It interfaces with the pre-select filter and the mixer. Furthermore, since it is the first gain stage, care must be taken to provide accurate input match, low-noise figure, good linearity and a sufficient gain over a wide band of operation. Several CMOS LNAs have been reported during the last decade, showing that the most research has been done at 802.11/b and GSM standards (900-2400MHz spectrum) and more recently at 802.11/a (5GHz band). One of the more significant disadvantages of 802.11/b is that the frequency band is crowded and subject to interference from other technologies, as is 2.4GHz cordless phones and Bluetooth. As the demand for radio-frequency integrated circuits, operating at higher frequency bands, increases, the IEEE 802.11/a standard becomes a very attractive option to wireless communication system developers. This paper presents the design and implementation of a low power, low noise amplifier aimed at IEEE 802.11a for WLAN applications. It was designed to be integrated with an active balun and mixer, representing the first step toward a fully integrated monolithic WLAN receiver. All the required circuits are integrated at the same die and are powered by 1.8V supply source. Preliminary experimental results (S-parameters) are shown and promise excellent results. The LNA circuit design details are illustrated in Section 2. Spectre simulation results focused at gain, noise figure (NF) and input/output matching are presented in Section 3. Finally, conclusions and comparison with other recently reported LNAs are made in Section 4, followed by future work

    The protoMIRAX Hard X-ray Imaging Balloon Experiment

    The protoMIRAX hard X-ray imaging telescope is a balloon-borne experiment developed as a pathfinder for the MIRAX satellite mission. The experiment consists essentially in a coded-aperture hard X-ray (30-200 keV) imager with a square array (13×\times13) of 2mm-thick planar CZT detectors with a total area of 169 cm2^2. The total, fully-coded field-of-view is 21∘×21∘21^{\circ}\times 21^{\circ} and the angular resolution is 1∘^{\circ}43'. In this paper we describe the protoMIRAX instrument and all the subsystems of its balloon gondola, and we show simulated results of the instrument performance. The main objective of protoMIRAX is to carry out imaging spectroscopy of selected bright sources to demonstrate the performance of a prototype of the MIRAX hard X-ray imager. Detailed background and imaging simulations have been performed for protoMIRAX balloon flights. The 3σ\sigma sensitivity for the 30-200 keV range is ~1.9 ×\times 10−5^{-5} photons cm−2^{-2} s−1^{-1} for an integration time of 8 hs at an atmospheric depth of 2.7 g cm−2^{-2} and an average zenith angle of 30∘^{\circ}. We have developed an attitude control system for the balloon gondola and new data handling and ground systems that also include prototypes for the MIRAX satellite. We present the results of Monte Carlo simulations of the camera response at balloon altitudes, showing the expected background level and the detailed sensitivity of protoMIRAX. We also present the results of imaging simulations of the Crab region. The results show that protoMIRAX is capable of making spectral and imaging observations of bright hard X-ray source fields. Furthermore, the balloon observations will carry out very important tests and demonstrations of MIRAX hardware and software in a near space environment.Comment: 9 pages, 13 figures, accepted for publication in Astronomy & Astrophysic
