    Evaluación de los conocimientos y prácticas sobre la norma para la atención, control y prevención del dengue por parte del personal médico, enfermería, inspectores de saneamiento ambiental y promotores de salud del sistema básico de salud integral San Miguel año 2008

    RESUMEN: En ésta investigación se trató de evaluar los conocimientos y prácticas acerca de la norma para la atención, control y prevención del dengue por parte del personal médico, enfermería, inspectores de saneamiento ambiental y promotores de salud pertenecientes al SIBASI San Miguel. El estudio se llevó a cabo durante los meses de septiembre y octubre del año 2008. Se realizó una investigación de tipo prospectiva, transversal, descriptiva, documental y de campo; utilizando para la recolección de información, técnica documental y la técnica de trabajo de campo como es la encuesta a través de un cuestionario administrado a una población conformada por 56 médicos, 127 enfermeras, 202 promotores de salud y 41 inspectores de saneamiento ambiental. Los resultados encontrados son los siguientes: En relación a la vigilancia epidemiológica el personal médico y de enfermería manejan este apartado en un 24.69%. En cuanto a los promotores e inspectores de saneamiento ambiental lo conocen en un 40.92%. Para la vigilancia serológica los médicos y enfermeras tienen conocimientos en un 50%; en el caso de los promotores e inspectores de saneamiento ambiental en un 57.42%. ABSTRACT: The purpose of this study was to evaluate the knowledge and practices of medical personnel, nurses, environmental sanitation inspectors and health promoters belonging to SIBASI San Miguel regarding the standard of care, control and prevention of dengue fever. The study was carried out during September and October 2008. A prospective, cross-sectional, descriptive, documentary and field research was carried out, using a documentary technique and a fieldwork technique such as a survey through a questionnaire administered to a population made up of 56 physicians, 127 nurses, 202 health promoters and 41 environmental sanitation inspectors. The results found are as follows: In relation to epidemiological surveillance the medical and nursing personnel handle this section in 24.69%. As for the promoters and environmental sanitation inspectors, 40.92% are familiar with it. For serological surveillance, doctors and nurses have 50% knowledge; in the case of environmental sanitation promoters and inspectors, 57.42%

    A community based participatory approach to improving health in a Hispanic population

    ABSTRACT: BACKGROUND: The Charlotte-Mecklenburg region has one of the fastest growing Hispanic communities in the country. This population has experienced disparities in health outcomes and diminished ability to access healthcare services. This city is home to an established practice-based research network (PBRN) that includes community representatives, health services researchers, and primary care providers. The aims of this project are: to use key principles of community-based participatory research (CBPR) within a practice-based research network (PBRN) to identify a single disease or condition that negatively affects the Charlotte Hispanic community; to develop a community-based intervention that positively impacts the chosen condition and improves overall community health; and to disseminate findings to all stakeholders. METHODS/DESIGN: This project is designed as CBPR. The CBPR process creates new social networks and connections between participants that can potentially alter patterns of healthcare utilization and other health-related behaviors. The first step is the development of equitable partnerships between community representatives, providers, and researchers. This process is central to the CBPR process and will occur at three levels -- community members trained as researchers and outreach workers, a community advisory board (CAB), and a community forum. Qualitative data on health issues facing the community -- and possible solutions -- will be collected at all three levels through focus groups, key informant interviews and surveys. The CAB will meet monthly to guide the project and oversee data collection, data analysis, participant recruitment, implementation of the community forum, and intervention deployment. The selection of the health condition and framework for the intervention will occur at the level of a community-wide forum. Outcomes of the study will be measured using indicators developed by the participants as well as geospatial modeling.On completion, this study will: determine the feasibility of the CBPR process to design interventions; demonstrate the feasibility of geographic models to monitor CBPR-derived interventions; and further establish mechanisms for implementation of the CBPR framework within a PBRN

    It is very important to know the appropriate techniques for the restoration of hand injuries, protecting as much as possible their configuration and functionality, since it performs multiple tasks in different areas of life. Hence, we present the case of a patient with traumatic injury due to crushing in the right hand, which treatment was considered the thoracoabdominal flap that was carried out in three times, the patient evolved favorably without complications of infection, dehiscence or necrosis of the flap. Despite the introduction of microsurgery for the reconstruction of these lesions, pedicle flaps continue to be feasible with optiomal results in situations in which microsurgery cannot be considered. Although there are multiple techniques for the correction of defects in hand, the skill and creativity of the surgeon following the basic principles of reconstruction will be conclusive for an optimal result.Es de gran importancia conocer las técnicas apropiadas para la restauración de las unidades funcionales de la mano a consecuencia de lesiones traumáticas, resguardando en lo posible su configuración y funcionalidad, puesto que desempeña múltiples tareas en distintos ámbitos de la vida. A continuación, se expone el caso de un paciente con lesión traumática por aplastamiento en mano derecha, cuyo tratamiento de consideración fue el colgajo toracoabdominal que fue llevado a cabo en tres tiempos, el paciente evolucionó de forma favorable sin complicaciones de infección, dehiscencia o necrosis del colgajo. A pesar de la introducción microquirúrgia para la reconstrucción de estas lesiones, los colgajos pediculados continúan siendo factibles con resultados óptimos en situaciones en las que la microcirugía no puede ser considerada. Si bien existen múltiples técnicas para la corrección de los defectos en mano, la habilidad y creatividad del cirujano siguiendo los principios básicos de la reconstrucción serán concluyentes para un resultado óptimo

    EntreCompEdu is a professional development framework for educators, designed as a complement to the European Commission’s Entrepreneurship Competence Framework (EntreComp). The purpose of this chapter is to reflect upon EntreCompEdu’s potential to stimulate teachers’ entrepreneurial thinking and activity. The chapter is structured in three main sections. The first describe the policy context and the gap in teacher education that EntreCompEdu seeks to address. The second offers an overview of the framework’s main components. The third reflects on the pilot phase of implementation, focusing on two case studies to illustrate the factors that influence EntreCompEdu’s impact and future direction. The reflections are those of the lead author of EntreCompEdu, three of those who facilitated the professional development sessions and the project manager. We follow Borton’s ‘(1970) simple ‘What? So What? and Now What?’ model to shape our reflections. Our subjective narratives are based on specific professional experiences as we critically reflect on the framework’s implementation within the context of Covid-19 and other challenges associated with transnational projects. We conclude that EntreCompEdu can stimulate teachers’ entrepreneurial thinking, although its sustainability in school is contingent on a range of individual and organisational factors discussed

    Labia minora hypertrophy is the prolongation of the labia minora beyond the anatomical limits of the labia majora. The belief in the simplicity of labia minora reduction and the failure to observe important aspects of surgical technique leads to total lip resection. In such cases, reconstructive surgery is the only possible way to rectify the situation. We present the case of a 36-year-old woman who presented with amputation of the labia minora secondary to labiaplasty surgery for hypertrophy performed by a medical esthetician. Reconstruction of labia minora was performed in two surgical stages. V-Y advancement flaps of the clitoral hood, with remnants of tissue from the posterior fork, can achieve satisfactory results and allow adaptation to the genital anatomy and unique aesthetic desires of each woman.La hipertrofia de labios menores es la prolongación de estos más allá de los límites anatómicos de los labios mayores. La creencia de la simplicidad en la reducción de los labios menores y la falla en observar importantes aspectos de la técnica quirúrgica que llevan a la resección total del labio. En tales casos, la cirugía reconstructiva es la única forma posible de rectificar la situación. Presentamos el caso de una mujer de 36 años que acude por amputación de los labios menores secundario a cirugía de labioplastia por hipertrofia realizada por médico esteticista. Se realiza reconstrucción de labios menores en dos tiempos quirúrgicos. Los colgajos de avance en V-Y del capuchón del clítoris, con remanentes de tejido de la horquilla posterior, pueden lograr resultados satisfactorios y permitir la adaptación a la anatomía genital y los deseos estéticos únicos de cada mujer

    Panniculus morbidus is a disease associated with morbid obesity that generates hygienic problems and functional disability. Massive panniculectomy involves several complications, however, it allows restoring the patient’s quality of life and functionality, thus becoming a challenge for the surgeon in his planning, execution and perioperative management and even more if the materials are not available. That is why this case series aims to present our surgical experience in the management of morbidly obese patients undergoing functional panniculectomy with satisfactory results. From our experience in the approach of these patients, we highlight that the absence of suspension materials does not make it impossible to carry out the surgery with acceptable results as long as the concepts of resection and synthesis of the abdominal wall were taken into account.Panniculus morbidus es una enfermedad asociada a la obesidad mórbida que genera problemas higiénicos y discapacidad funcional. La paniculectomía masiva conlleva varias complicaciones, sin embargo, permite restaurar la calidad de vida y funcionalidad del paciente convirtiéndose así en un reto para el cirujano en su planificación, ejecución y manejo peri operatorio y más aún si no se tienen los medios. Es por ello que esta serie de casos tiene como objetivo presentar nuestra experiencia quirúrgica en el manejo de pacientes con obesidad mórbida sometidos a paniculectomía funcional con resultados satisfactorios. Desde nuestra experiencia en el abordaje de estos pacientes destacamos que la ausencia de materiales de suspensión no imposibilita llevar a cabo la cirugía con resultados aceptables siempre y cuando se tomen en cuenta los conceptos de resección y síntesis de la pared abdominal
