    Who is absent and why? Factors affecting doctor absenteeism in Bangladesh

    Absenteeism by doctors in public healthcare facilities in rural Bangladesh is a form of chronic rule-breaking and is recognised as a critical problem by the government. We explored the factors underlying this phenomenon from doctors’ perspectives. We conducted a facility-based cross-sectional survey in four government hospitals in Dhaka, Bangladesh. Junior doctors with experience in rural postings were interviewed to collect data on socio-demographic characteristics, work and living experience at the rural facilities, and associations with professional and social networks. Multiple logistic regression was used to determine the factors associated with rural retention. Of 308 respondents, 74% reported having served each term of their rural postings without interruptions. The main reasons for absenteeism reported by those who interrupted rural postings were formal training opportunities (65%), family commitments (41%), and a miscellaneous group of others (17%). Almost half of the respondents reported unmanageable workloads. Most (96%) faced challenges in their last rural posting, such as physically unsafe environments (70%), verbally abusive behaviour by patients/caregivers (67%) and absenteeism by colleagues that impacted them (48%). Respondents who did not serve their entire rural posting were less likely to report an unmanageable workload than respondents who did (AOR 0.39, 95% CI 0.22–0.70). Respondents with connections to influential people in the local community had a 2.4 times higher chance of serving in rural facilities without interruption than others (AOR 2.40, 95% CI 1.26–4.57). Our findings demonstrate that absenteeism is not universal and depends upon doctors’ socio-political networks. Policy interventions rarely target unsupportive or threatening behaviour by caregivers and community members, a pivotal disincentive to doctors’ willingness to work in underserved rural areas. Policy responses must promote opportunities for doctors with weak networks who are willing to attend work with appropriate support

    Incentivising doctor attendance in rural Bangladesh: a latent class analysis of a discrete choice experiment

    OBJECTIVE: Doctor absenteeism is widespread in Bangladesh, and the perspectives of the actors involved are insufficiently understood. This paper sought to elicit preferences of doctors over aspects of jobs in rural areas in Bangladesh that can help to inform the development of packages of policy interventions that may persuade them to stay at their posts. METHODS: We conducted a discrete choice experiment with 308 doctors across four hospitals in Dhaka, Bangladesh. Four attributes of rural postings were included based on a literature review, qualitative research and a consensus-building workshop with policymakers and key health-system stakeholders: relationship with the community, security measures, attendance-based policies and incentive payments. Respondents' choices were analysed with mixed multinomial logistic and latent class models and were used to simulate the likely uptake of jobs under different policy packages. RESULTS: All attributes significantly impacted doctor choices (p<0.01). Doctors strongly preferred jobs at rural facilities where there was a supportive relationship with the community (β=0.93), considered good attendance in education and training (0.77) or promotion decisions (0.67), with functional security (0.67) and higher incentive payments (0.5 per 10% increase of base salary). Jobs with disciplinary action for poor attendance were disliked by respondents (-0.63). Latent class analysis identified three groups of doctors who differed in their uptake of jobs. Scenario modelling identified intervention packages that differentially impacted doctor behaviour and combinations that could feasibly improve doctors' attendance. CONCLUSION: Bangladeshi doctors have strong but varied preferences over interventions to overcome absenteeism. We generated evidence suggesting that interventions considering the perspective of the doctors themselves could result in substantial reductions in absenteeism. Designing policy packages that take account of the different situations facing doctors could begin to improve their ability and motivation to be present at their job and generate sustainable solutions to absenteeism in rural Bangladesh

    Household food insecurity after the early monsoon flash flood of 2017 among wetland (Haor) communities of northeastern Bangladesh: A cross-sectional study

    The livelihoods of wetland (Haor) communities living in northeastern region of Bangladesh are largely dependent on agriculture. Unseasonably heavy rainfall in Haor basin and upstream Indian catchments triggered a devastating early monsoon flash flood in Haor basin at the beginning of April in 2017, which destroyed the nearly ready-to-harvest annual rice crop and resulted in significant income loss of Haor dwellers. The present study aims at assessing post-flood household food insecurity and the factors associated with it in the aftermath of this flash flood. A cross-sectional study design was adopted, and a survey covering 1845 flood-affected households was conducted to measure the post-flood household food insecurity. A set of questionnaires was adopted from Household Food Insecurity Access Scale (HFIAS) to assess the level of household food insecurity, and a modified poisson regression model was used to identify the risk factors of household food insecurity. A staggering 62% of the surveyed households were found food insecure following the flash flood. Being ultra-poor, loss of livestock in flood, household head's occupation being natural resource-based, and household reliance on market purchase of food were identified as significant risk factors of post-flood household food insecurity. Household being headed by relatively younger, educated people, and household owning agricultural land were found to be protective against post-flood food insecurity. Therefore, post-flood emergency programs, especially the food assistance programs should be designed prioritizing these most food insecure groups instead of considering household poverty as the only selection criteria. As a long-term plan, a multi-sectoral approach focusing on disaster risk reduction by taking into account the risk of disaster-induced food insecurity is essential to reduce the vulnerability of wetland communities.PRIFPRI3; ISI; DCA; 2 Promoting Healthy Diets and Nutrition for allPHN

    Genomic analysis of SARS-CoV-2 variants of concern identified from the ChAdOx1 nCoV-19 immunized patients from southwest part of Bangladesh

    Background Bangladesh introduced ChAdOx1 nCoV-19 since February, 2021 and in six months, only a small population (12.8%) received either one or two dose of vaccination like other low-income countries. The COVID-19 infections were continued to roll all over the places although the information on genomic variations of SARS-CoV-2 between both immunized and unimmunized group was unavailable. The objective of this study was to compare the proportion of immune escaping variants between those groups. Methods A total of 4718 nasopharygeal samples were collected from March 1 until April 15, 2021, of which, 834 (18%) were SARS-CoV-2 positive. The minimum sample size was calculated as 108 who were randomly selected for telephone interview and provided consent. The prevalence of SARS-CoV-2 variants and disease severity among both immunized and unimmunized groups was measured. A total of 63 spike protein sequences and 14 whole-genome sequences were performed from both groups and phylogenetic reconstruction and mutation analysis were compared. Results A total of 40 respondents (37%, N = 108) received single-dose and 2 (2%) received both doses of ChAdOx1 nCoV-19 vaccine, which significantly reduce dry cough, loss of appetite and difficulties in breathing compared to none. There was no significant difference in hospitalization, duration of hospitalization or reduction of other symptoms like running nose, muscle pain, shortness of breathing or generalized weakness between immunized and unimmunized groups. Spike protein sequence assumed 21 (87.5%) B.1.351, one B.1.526 and two 20B variants in immunized group compared to 27 (69%) B.1.351, 5 (13%) B.1.1.7, 4 (10%) 20B, 2 B.1.526 and one B.1.427 variant in unimmunized group. Whole genome sequence analysis of 14 cases identified seven B.1.351 Beta V2, three B.1.1.7 Alpha V1, one B.1.526 Eta and the rest three 20B variants. Conclusion Our study observed that ChAdOx1 could not prevent the new infection or severe COVID-19 disease outcome with single dose while the infections were mostly caused by B.1.351 variants in Bangladesh.PRIFPRI3; ISI; DCA; 5 Strengthening Institutions and GovernancePHN

    Exploring the knowledge, attitudes, practices and lived experiences of frontline health workers in the times of COVID-19: A qualitative study from Bangladesh

    Objective: This study explored Frontline Health Workers’ (FLWs) knowledge, attitude and practice (KAP) on COVID-19 and their lived experiences, in both their personal and work lives, at the early stage of the pandemic in Bangladesh. Design, setting and participants: This was a qualitative study conducted through telephone interviews in May 2020. A total of 41 FLWs including physicians, nurses, paramedics, community healthcare workers and hospital support staff from 34 public and private facilities of both urban and rural parts of Bangladesh participated in the interview. A purposive sampling technique supplemented by a snowball sampling method was followed to select the participants. The in-depth interviews followed a semi-structured interview guide, and we applied the thematic analysis method for the qualitative data analysis. Findings: Except physicians, the FLWs did not receive any institutional training on COVID-19, including its prevention and management, in most instances. Also, they had no training in the use of personal protective equipment (PPE). Their common source of knowledge was the different websites or social media platforms. The FLWs were at risk while delivering services because patients were found to hide histories and not maintaining safety rules, including physical distancing. Moreover, inadequate supply of PPE, fear of getting infected, risk to family members and ostracisation by the neighbours were mentioned to be quite common by them. This situation eventually led to the development of mental stress and anxiety; however, they tried to cope up with this dire situation and attend to the call of humanity. Conclusion: The uncertain work environment during the COVID-19 pandemic simultaneously affected FLWs’ physical and emotional health in Bangladesh. However, they showed professional devotion in overcoming such obstacles and continued to deliver essential services.     Dominant clade-featured SARS-CoV-2 co-occurring mutations reveals plausible epistasis: An in silico based hypothetical model

    Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has evolved into eight fundamental clades with four of these clades (G, GH, GR, and GV) globally prevalent in 2020. To explain plausible epistatic effects of the signature co-occurring mutations of these circulating clades on viral replication and transmission fitness, we proposed a hypothetical model using in silico approach. Molecular docking and dynamics analyses showed the higher infectiousness of a spike mutant through more favorable binding of G614 with the elastase-2. RdRp mutation p.P323L significantly increased genome-wide mutations (p < 0.0001), allowing for more flexible RdRp (mutated)-NSP8 interaction that may accelerate replication. Superior RNA stability and structural variation at NSP3:C241T might impact protein, RNA interactions, or both. Another silent 5′-UTR:C241T mutation might affect translational efficiency and viral packaging. These four G-clade-featured co-occurring mutations might increase viral replication. Sentinel GH-clade ORF3a:p.Q57H variants constricted the ion-channel through intertransmembrane–domain interaction of cysteine(C81)-histidine(H57). The GR-clade N:p.RG203-204KR would stabilize RNA interaction by a more flexible and hypo-phosphorylated SR-rich region. GV-clade viruses seemingly gained the evolutionary advantage of the confounding factors; nevertheless, N:p.A220V might modulate RNA binding with no phenotypic effect. Our hypothetical model needs further retrospective and prospective studies to understand detailed molecular events and their relationship to the fitness of SARS-CoV-2.PRIFPRI3; ISI; DCAPHN

    Butterfly diversity in a tropical urban habitat (Lepidoptera: Papilionoidea)

    <p>Butterflies are important pollinators and indicators of environmental health. Habitat destruction and fragmentation caused by developmental activities and poor natural resource management are the main reasons for the drastic decline of butterfly populations throughout the world, and prompt the need for effective conservation management. We assessed the diversity, richness, seasonal distribution and conservation priorities of butterflies in an inner-city parkland habitat (University of Dhaka, Bangladesh) and investigated the biotic, and abiotic factors affecting butterfly diversity. A total of 93 species from 5 families were recorded from January to December 2014. Nymphalidae and Papilionidae were the most and least dominant families in terms of species number. Butterflies across families generally selected nectar plants randomly, although some showed preference towards herbs. Spatial interpolation of butterfly richness indicated that the most suitable area was around Curzon Hall, an area containing different types of vegetation. Precipitation was the only abiotic factor with a significant relationship with butterfly species richness. The establishment of green areas with complex structural vegetation will improve microclimatic conditions for butterflies, promoting healthier urban ecosystems, and the conservation of butterflies and other associated species in inner-city areas.</p
