Poisoning with the recreational drug paramethoxyamphetamine ("death")

An external link to the publisherâs copy is included.Objective: To describe the clinical features of paramethoxyamphetamine (PMA; "death") poisoning and to compare these with those of people with self-reported "ecstasy" poisoning. Design: Retrospective casenote review. Participants and setting: 22 patients who presented to the Emergency Department of the Royal Adelaide Hospital (RAH), a major metropolitan teaching hospital, between 1 January 1996 and 31 December 1998 with PMA poisoning identified through urine drug screens; and 61 patients with self-reported ecstasy poisoning between 1 September 1997 and 31 December 1998 found through the hospital databases. Results: Patients with PMA poisoning presented with tachycardia (64%), hyperthermia (temperature > 37.5ÂșC; 36%), coma (41%), seizures (32%), arrhythmias (23%), and QRS intervals 100 ms (50%) with greater frequency and often greater severity than those with self-reported ecstasy poisoning. Two patients with PMA poisoning presented with severe hypoglycaemia (blood glucose level, 7.5 mmol/L). Conclusions: At our hospital, PMA poisonings accounted for most of the severe reactions among people who believed they had taken ecstasy. Hypoglycaemia and hyperkalaemia may be specific to PMA poisoning. PMA toxicity should be suspected with severe or atypical reactions to "ecstasy", and confirmed by chromatographic urine drug screens.Liang Han Ling, Colin Marchant, Nicholas A. The influence of the cosmological expansion on local systems
Following renewed interest, the problem of whether the cosmological expansion
affects the dynamics of local systems is reconsidered. The cosmological
correction to the equations of motion in the locally inertial Fermi normal
frame (the relevant frame for astronomical observations) is computed. The
evolution equations for the cosmological perturbation of the two--body problem
are solved in this frame. The effect on the orbit is insignificant as are the

Influence of grape juice extraction methods on basic analytical parameters
Currently, for monitoring the ripening of grape berries, different devices are used to produce the juices to be analysed. Crushing the berries is a key step that determines the quantity of juice extracted and may impact it composition. The effect of different devices on analytical parameters of the musts produced were compared in this study. Samples from four grape varieties ('Cabernet-Sauvignon', 'EkigaĂŻna', 'Marselan' and 'Vermentino'), showing a variability of berry size and precocity, were crushed using six different devices (ASieves, Bag mixerÂź, Crusher, Manual, TPress and Blender). Whatever the pressing equipment, sugar concentrations of the must were not modified by the extraction method, unlike other parameters. pH and titratable acidity were slightly impacted by the crushing method without changing the ranking of the varieties. However, potassium concentrations were more impacted by the pressing method. Differences in mechanical forces applied to skins and seeds according to the pressing equipment used may release more or less potassium. This study clearly discarded a complete grinding of the samples for grape ripening monitoring: this method strongly modified the potassium content and, consequently, the pH and the titratable acidity of the musts
Crossing the `Yellow Void' -- Spatially Resolved Spectroscopy of the Post- Red Supergiant IRC+10420 and Its Circumstellar Ejecta
IRC +10420 is one of the extreme hypergiant stars that define the empirical
upper luminosity boundary in the HR diagram. During their post--RSG evolution,
these massive stars enter a temperature range (6000-9000 K) of increased
dynamical instability, high mass loss, and increasing opacity, a
semi--forbidden region, that de Jager and his collaborators have called the
`yellow void'. We report HST/STIS spatially resolved spectroscopy of IRC +10420
and its reflection nebula with some surprising results. Long slit spectroscopy
of the reflected spectrum allows us to effectively view the star from different
directions. Measurements of the double--peaked Halpha emission profile show a
uniform outflow of gas in a nearly spherical distribution, contrary to previous
models with an equatorial disk or bipolar outflow. Based on the temperature and
mass loss rate estimates that are usually quoted for this object, the wind is
optically thick to the continuum at some and possibly all wavelengths.
Consequently the observed variations in apparent spectral type and inferred
temperature are changes in the wind and do not necessarily mean that the
underlying stellar radius and interior structure are evolving on such a short
timescale. To explain the evidence for simultaneous outflow and infall of
material near the star, we propose a `rain' model in which blobs of gas
condense in regions of lowered opacity outside the dense wind. With the
apparent warming of its wind, the recent appearance of strong emission, and a
decline in the mass loss rate, IRC +10420 may be about to shed its opaque wind,
cross the `yellow void', and emerge as a hotter star.Comment: To appear in the Astronomical Journal, August 200
