    Rapid intravenous rehydration of children with acute gastroenteritis and dehydration: a systematic review and meta-analysis

    Background: The World Health Organization (WHO) recommends rapid intravenous rehydration, using fluid volumes of 70-100mls/kg over 3–6 h, with some of the initial volume given rapidly as initial fluid boluses to treat hypovolaemic shock for children with acute gastroenteritis (AGE) and severe dehydration. The evidence supporting the safety and efficacy of rapid versus slower rehydration remains uncertain.Methods: We conducted a systematic review of randomised controlled trials (RCTs) on 11th of May 2017 comparing different rates of intravenous fluid therapy in children with AGE and moderate or severe dehydration, using standard search terms. Two authors independently assessed trial quality and extracted data. Non-RCTs and non-English articles were excluded. The primary endpoint was mortality and secondary endpoints included adverse events (safety) and treatment efficacy.Main results: Of the 1390 studies initially identified, 18 were assessed for eligibility. Of these, 3 studies (n = 464) fulfilled a priori criteria for inclusion; most studied children with moderate dehydration and none were conducted in resource-poor settings. Volumes and rates of fluid replacement varied from 20 to 60 ml/kg given over 1-2 h (fast) versus 2-4 h (slow). There was substantial heterogeneity in methodology between the studies with only one adjudicated to be of high quality. There were no deaths in any study. Safety endpoints only identified oedema (n = 6) and dysnatraemia (n = 2). Pooled analysis showed no significant difference between the rapid and slow intravenous rehydration groups for the proportion of treatment failures (N = 468): pooled RR 1.30 (95% CI: 0.87, 1.93) and the readmission rates (N = 439): pooled RR 1.39 (95% CI: 0.68, 2.85).Conclusions: Despite wide implementation of WHO Plan C guideline for severe AGE, we found no clinical evaluation in resource-limited settings, and only limited evaluation of the rate and volume of rehydration in other parts of the world. Recent concerns over aggressive fluid expansion warrants further research to inform guidelines on rates of intravenous rehydration therapy for severe AG

    Predictors of olfactory improvement after endoscopic sinus surgery in chronic rhinosinusitis with nasal polyps

    Objective. This study aimed to determine the predictors of olfactory improvement after endoscopic sinus surgery among patients with chronic rhinosinusitis with nasal polyps. Method. This prospective cohort study included patients admitted to a university hospital between 2006 and 2012. Assessment using odour identification testing, a sinonasal symptom questionnaire, the Rhinosinusitis Disability Index and mucus biomarker levels was performed at various time points. Correlation of variables with identification score differences at six postoperative time points and at baseline was performed, followed by multiple linear regression to determine significant predictors at each of the six post-operative time points. Results. Baseline absence of acute sinusitis, elevated serpin F2 and anterior rhinorrhoea predict early olfactory improvement, whereas baseline allergic rhinitis predicts late olfactory improvement. Baseline odour identification score was the strongest predictor across all time points. Conclusion. Patients with chronic rhinosinusitis and nasal polyps with worse disease or baseline olfactory function may benefit more from endoscopic sinus surgery in terms of olfactory improvement

    Classical multivariate Hermite coordinate interpolation on n-dimensional grids

    In this work, we study the Hermite interpolation on n-dimensional non-equally spaced, rectilinear grids over a field k of characteristic zero, given the values of the function at each point of the grid and the partial derivatives up to a maximum degree. First, we prove the uniqueness of the interpolating polynomial, and we further obtain a compact closed form that uses a single summation, irrespective of the dimensionality, which is algebraically simpler than the only alternative closed form for the n-dimensional classical Hermite interpolation [1]. We provide the remainder of the interpolation in integral form; moreover, we derive the ideal of the interpolation and express the interpolation remainder using only polynomial divisions, in the case of interpolating a polynomial function. Finally, we perform illustrative numerical examples to showcase the applicability and high accuracy of the proposed interpolant, in the simple case of few points, as well as hundreds of points on 3D-grids using a spline-like interpolation, which compares favorably to state-of-the-art spline interpolation methods

    We compare theoretical results for the characteristic frequency of the Rayleigh peak calculated in one-loop order within the field theoretical method of the renormalization group theory with experiments and other theoretical results. Our expressions describe the non-asymptotic crossover in temperature, density and wave vector. In addition we discuss the frequency dependent shear viscosity evaluated within the same model and compare our theoretical results with recent experiments in microgravity.Comment: 17 pages, 12 figure

    EPIC Fields: Marrying 3D Geometry and Video Understanding

    Neural rendering is fuelling a unification of learning, 3D geometry and video understanding that has been waiting for more than two decades. Progress, however, is still hampered by a lack of suitable datasets and benchmarks. To address this gap, we introduce EPIC Fields, an augmentation of EPIC-KITCHENS with 3D camera information. Like other datasets for neural rendering, EPIC Fields removes the complex and expensive step of reconstructing cameras using photogrammetry, and allows researchers to focus on modelling problems. We illustrate the challenge of photogrammetry in egocentric videos of dynamic actions and propose innovations to address them. Compared to other neural rendering datasets, EPIC Fields is better tailored to video understanding because it is paired with labelled action segments and the recent VISOR segment annotations. To further motivate the community, we also evaluate two benchmark tasks in neural rendering and segmenting dynamic objects, with strong baselines that showcase what is not possible today. We also highlight the advantage of geometry in semi-supervised video object segmentations on the VISOR annotations. EPIC Fields reconstructs 96% of videos in EPIC-KITCHENS, registering 19M frames in 99 hours recorded in 45 kitchens, and is available from: http://epic-kitchens.github.io/epic-field

    Understanding the atmospheric properties and chemical composition of the ultra-hot Jupiter HAT-P-7b II. Mapping the effects of gas kinetics

The atmospheres of ultra-hot Jupiters (UHJs) are commonly considered to be at thermochemical equilibrium. We aim to provide disequilibrium chemistry maps for a global understanding of the chemistry in the atmosphere of HAT-P-7b and assess the importance of disequilibrium chemistry on UHJs. Methods. We applied a hierarchical modeling approach using 97 1D atmospheric profiles from a 3D general circulation model of HAT-P-7b. For each atmospheric 1D profile, we evaluated our kinetic cloud formation model consistently with the local gas-phase composition in chemical equilibrium. This served as input to study the quenching of dominating CHNO-binding molecules. We evaluated quenching results from a zeroth-order approximation in comparison to a kinetic gas-phase approach. Results. We find that the zeroth-order approach of estimating quenching points agrees well with the full gas-kinetic modeling results. However, it underestimates the quenching levels by about one order of magnitude at high temperatures. Chemical disequilibrium has the greatest effect on the nightside and morning abundance of species such as H, H2O, CH4, CO2, HCN, and all CnHm molecules; heavier CnHm molecules are more affected by disequilibrium processes. The CO abundance, however, is affected only marginally. While dayside abundances also notably change, those around the evening terminator of HAT-P-7b are the least affected by disequilibrium processes. The latter finding may partially explain the consistency of observed transmission spectra of UHJs with atmospheres in thermochemical equilibrium. Photochemistry only negligibly affects molecular abundances and quenching levels. Conclusions. In general, the quenching points of the atmosphere of HAT-P-7b are at much lower pressures than in the cooler hot-jupiters. We propose several avenues to determining the effect of disequilibrium processes on UHJs that are in general based on abundance and opacity measurements at different local times. It remains a challenge to completely disentangle this from the chemical effects of clouds and that of a primordial nonsolar abundance.Publisher PDFPeer reviewe

    Effect of structural defects on anomalous ultrasound propagation in solids during second-order phase transitions

    The effect of structural defects on the critical ultrasound attenuation and ultrasound velocity dispersion in Ising-like three-dimensional systems is studied. A field-theoretical description of the dynamic effects of acoustic-wave propagation in solids during phase transitions is performed with allowance for both fluctuation and relaxation attenuation mechanisms. The temperature and frequency dependences of the scaling functions of the attenuation coefficient and the ultrasound velocity dispersion are calculated in a two-loop approximation for pure and structurally disordered systems, and their asymptotic behavior in hydrodynamic and critical regions is separated. As compared to a pure system, the presence of structural defects in it is shown to cause a stronger increase in the sound attenuation coefficient and the sound velocity dispersion even in the hydrodynamic region as the critical temperature is reached. As compared to pure analogs, structurally disordered systems should exhibit stronger temperature and frequency dependences of the acoustic characteristics in the critical region.Comment: 7 RevTeX pages, 4 figure

    A complex fracture of the hyoid bone and the larynx after a bicycle accident – Case report and review of literature

    Introduction and importance Reports about laryngeal trauma and fractures of the hyoid bone are rare in the literature. Most cases are forensic cases and the results of postmortem analysis. Traumatic larynx and hyoid bone fractures represent a rare but important differential diagnosis of the common symptom hoarseness. Case presentation A 60-year-old female patient presented with unclear dysphonia and globus sensation following intubation for a surgical treatment for a lower leg fracture after a bicycle accident two months ago. Endoscopy and the computed tomography (CT) of the neck revealed a fixed and immobile fractured piece of the larynx, a hyoid bone fracture and a pseudarthrosis between the greater horn of the hyoid bone and the upper edge of the thyroid cartilage. The hyoid bone fracture led to a distortion of the supraglottis. After surgical removal of the fractured part of the hyoid bone and the pseudarthrosis separation, the supraglottis appeared symmetrical again. Four weeks after surgery the patient was symptom-free. Clinical discussion Though combined hyoid bone and larynx fractures after traumatic injuries are rare, they represent an important differential diagnosis in trauma patients with dysphagia or dysphonia. The clinical symptoms can vary and occur immediately or within a latent period taking weeks or months until the proper diagnosis. Depending on the symptoms, surgical management can be effective. Conclusion An isolated partial resection of the hyoid bone with separation of the pseudarthrosis is a reasonable therapeutic option and can lead to completely resolving symptom.
