    LEADING SECURITY AND INTELLIGENCE: CURRENT TRENDS AND ESSENTIAL ABILITIES

    On November 15, 2023, Ms. Jennifer Irish, Associate and Program Director of the Executive Security and Intelligence Leadership Certificate program at, Telfer Executive Programs of the University of Ottawa, presented Leading Security and Intelligence: Current Trends and Essential Abilities at the West Coast Security Conference. The presentation was followed by a question-and-answer period with questions from the audience and CASIS Vancouver executives. The presentation was informed by structural interviews she conducted with senior leaders across Canada’s Security and Intelligence community to inform a refresh of the leadership program offered by uOttawa’s Telfer Executive Programs for federal government executives having security and intelligence responsibilities.  The key points focussed on the key competencies and skills required of contemporary S&I leaders as they navigate through changes in the community, the evolving global threatscape, and as result of emerging technologies and increased expectations for public accountability.   Received: 01-07-2024 Revised: 01-29-202

    The effects of natural structure on estimated tropical cyclone surge extremes

    Abstract The past 12 years have seen significant steps forward in the science and practice of coastal flood analysis. This paper aims to recount and critically assess these advances, while helping identify next steps for the field. This paper then focuses on a key problem, connecting the probabilistic characterization of flood hazards to their physical mechanisms. Our investigation into the effects of natural structure on the probabilities of storm surges shows that several different types of spatial-, temporal-, and process-related organizations affect key assumptions made in many of the methods used to estimate these probabilities. Following a brief introduction to general historical methods, we analyze the two joint probability methods used in most tropical cyclone hazard and risk studies today: the surface response function and Bayesian quadrature. A major difference between these two methods is that the response function creates continuous surfaces, which can be interpolated or extrapolated on a fine scale if necessary, and the Bayesian quadrature optimizes a set of probability masses, which cannot be directly interpolated or extrapolated. Several examples are given here showing significant impacts related to natural structure that should not be neglected in hazard and risk assessment for tropical cyclones including: (1) differences between omnidirectional sampling and directional-dependent sampling of storms in near coastal areas; (2) the impact of surge probability discontinuities on the treatment of epistemic uncertainty; (3) the ability to reduce aleatory uncertainty when sampling over larger spatial domains; and (4) the need to quantify trade-offs between aleatory and epistemic uncertainties in long-term stochastic sampling

    Risk aversion in the use of complex kidneys in paired exchange programs: Opportunities for even more transplants?

    This retrospective review of the largest United States kidney exchange reports characteristics, utilization, and recipient outcomes of kidneys with simple compared to complex anatomy and extrapolates reluctance to accept these kidneys. Of 3105 transplants performed, only 12.8% were right kidneys and 23.1% had multiple renal arteries. 59.3% of centers used fewer right kidneys than expected and 12.1% transplanted zero right kidneys or kidneys with more than 1 artery. Five centers transplanted a third of these kidneys (35.8% of right kidneys and 36.7% of kidneys with multiple renal arteries). 22.5% and 25.5% of centers currently will not entertain a match offer for a left or right kidney with more than one artery, respectively. There were no significant differences in all-cause graft failure or death-censored graft loss for kidneys with multiple arteries, and a very small increased risk of graft failure for right kidneys versus left of limited clinical relevance for most recipients. Kidneys with complex anatomy can be used with excellent outcomes at many centers. Variation in use (lack of demand) for these kidneys reduces the number of transplants, so systems to facilitate use could increase demand.     Two contemporaneous mitogenomes from terminal Pleistocene burials in eastern Beringia

    Pleistocene residential sites with multiple contemporaneous human burials are extremely rare in the Americas. We report mitochondrial genomic variation in the first multiple mitochondrial genomes from a single prehistoric population: two infant burials (USR1 and USR2) from a common interment at the Upward Sun River Site in central Alaska dating to ~11,500 calendar years before present (cal B.P.). Using a targeted capture method and next-generation sequencing we determined that the USR1 infant possessed variants that define mitochondrial lineage C1b, while the USR2 genome falls at the root of lineage B2, allowing us to refine younger coalescence age estimates for these two clades. C1b and B2 are rare to absent in modern populations of Northern North America. Documentation of these lineages at this location in the Late Pleistocene provides evidence for the extent of mitochondrial diversity in early Beringian populations, which supports the expectations of the Beringian Standstill Model

    The Impact of Climate Change on Virginia\u27s Coastal Areas

    As part of HJ47/SJ47 (2020), the Virginia General Assembly directed the Joint Commission on Technology and Science (JCOTS) to study the “safety, quality of life, and economic consequences of weather and climate-related events on coastal areas in Virginia.” In pursuit of this goal, the commission was to “accept any scientific and technical assistance provided by the nonpartisan, volunteer Virginia Academy of Science, Engineering, and Medicine (VASEM). VASEM convened an expert study board with representation from the Office of the Governor, planning district commissions in coastal Virginia, The Port of Virginia, the Virginia Economic Development Partnership, state universities, private industry, and law firms. In producing the report, the board followed methods similar to those used by the National Academies of Science, Engineering, and Medicine by convening an expert committee tasked with studying and reporting on the topic. As a result, the report represents the views and perspectives of the study board members but was not submitted for public review or comment. This report is the product of those efforts. It finds that climate change will have an increasingly disruptive effect on people living in Virginia’s coastal areas during the 21st century — and that these disruptions will have repercussions across the Commonwealth. It includes an explanation of the physical forces driving climate change, an analysis of the current and projected effects of climate change on the Commonwealth, perspectives that legislators might consider as they face these challenges, and recommendations that could help Virginia implement more productive and effective strategies to address them

    Diagnostic accuracy of Abbott Architect Assay as a screening tool for human T-cell leukaemia virus type-1 and type-2 infection in a London teaching hospital with a large solid organ transplant centre.

    AIM: In the United Kingdom, organ donors/recipients are screened for evidence of human T-cell leukaemia virus type-1 and type-2 (HTLV-1/2) infections. Since the United Kingdom is a low prevalence country for HTLV infections, a screening assay with high sensitivity and specificity is required. Samples with repeat reactivity on antibody testing are sent to a reference lab for confirmatory serological and molecular testing. In the case of donor screen, this leads to delays in the release of organs and can result in wastage. We aim to assess whether a signal/cut-off (S/CO) ratio higher than the manufacturer's recommendation of 1.0 in the Abbott Architect antibody assay is a reliable measure of HTLV-1/2 infection. METHODS: We conducted a 5 year retrospective analysis of 7245 patients from which 11 766 samples were tested on the Abbott Architect rHTLV I/II assay. Reactive samples (S/CO >1) were referred for confirmatory serological and molecular detection (Western Blot and proviral DNA) at UK Health Security Agency, (formerly PHE, Colindale), the national reference laboratory. Electronic, protected laboratory and hospital patient databases were employed to collate data. RESULTS: A total of 45 patients had initially reactive samples. 42.2% (n = 19/45) had an S/CO ratio > 20, with HTLV infection confirmed in n = 18/19 and indeterminate confirmatory results in n = 1/19. No samples with an S/CO ratio <4 (48.9%, n = 22/45) or 4-20 (8.9%, n = 4/45) had positive confirmatory results on subsequent confirmatory testing. CONCLUSION: Samples with an S/CO >20 likely represent a true HTLV-1/2 infection. Reactive samples with an S/CO <4 were unlikely to confirm for HTLV infections. Interpretation of these ratios can assist clinicians in the assessment of low reactive samples and reiterates the need for faster access to confirmatory testing

    Perioperative pain management and opioid-reduction in head and neck endocrine surgery: An American Head and Neck Society Endocrine Surgery Section consensus statement

    BACKGROUND: This American Head and Neck Society (AHNS) consensus statement focuses on evidence-based comprehensive pain management practices for thyroid and parathyroid surgery. Overutilization of opioids for postoperative pain management is a major contributing factor to the opioid addiction epidemic however evidence-based guidelines for pain management after routine head and neck endocrine procedures are lacking. METHODS: An expert panel was convened from the membership of the AHNS, its Endocrine Surgical Section, and ThyCa. An extensive literature review was performed, and recommendations addressing several pain management subtopics were constructed based on best available evidence. A modified Delphi survey was then utilized to evaluate group consensus of these statements. CONCLUSIONS: This expert consensus provides evidence-based recommendations for effective postoperative pain management following head and neck endocrine procedures with a focus on limiting unnecessary use of opioid analgesics
