    Prevalence of admission plasma glucose in 'diabetes'Ā or 'at risk' ranges in hospital emergencies with no prior diagnosis of diabetes by gender, age and ethnicity

    Aims To establish the prevalence of admission plasma glucose in 'diabetes'Ā and 'at risk' rangesĀ in emergency hospital admissions with no prior diagnosis of diabetes; characteristics of people with hyperglycaemia; and factors influencing glucose measurement. Methods Electronic patient records for 113Ā 097 hospital admissions over 1Ā year from 2014 to 2015 included 43Ā 201 emergencies with glucose available for 31Ā 927 (74%) admissions, comprising 22Ā 045 people. Data are presented for 18Ā 965 people with no prior diagnosis of diabetes and glucose available on first attendance. Results Three quarters (14Ā 214) were White Europeans aged 62 (43-78)Ā years, median (IQ range); 12% (2241) South Asians 46 (32-64)Ā years; 9% (1726) Unknown/Other ethnicities 43 (29-61)Ā years; and 4% (784) Afro-Caribbeans 49 (33-63)Ā years, Ā 24Ā hours. Conclusions Hyperglycaemia was evident in 21% of adults admitted as an emergency; various aspects related to follow-up and initial testing, age and ethnicity need to be considered by professional bodies addressing undiagnosed diabetes in hospital admissions

    Determination of the pharmacokinetics and oral bioavailability of salicylamine, a potent Ī³-ketoaldehyde scavenger, by LC/MS/MS

    Levels of reactive Ī³-ketoaldehydes derived from arachidonate increase in diseases associated with inflammation and oxidative injury. To assess the biological importance of these Ī³-ketoaldehydes, we previously identified salicylamine as an effective Ī³-ketoaldehyde scavenger in vitro and in cells. To determine if salicylamine could be administered in vivo, we developed an LC/MS/MS assay to measure salicylamine in plasma and tissues. In mice, half-life (t1/2) was 62 minutes. Drinking water supplementation (1-10 g/L) generated tissue concentrations (10-500 Ī¼M) within the range previously shown to inhibit Ī³-ketoaldehydes in cells. Therefore, oral administration of salicylamine can be used to assess the contribution of Ī³-ketoaldehydes in animal models of disease.     Can HbA1c detect undiagnosed diabetes in acute medical hospital admissions?

    Objective: to study hyperglycaemia in acute medical admissions to Irish regional hospital.Research design and methods: from 2005 to 2007, 2061 white Caucasians, aged &gt;18 years, were admitted by 1/7 physicians. Those with diabetes symptoms/complications but no previous record of hyperglycaemia (n = 390), underwent OGTT with concurrent HbA1c in representative subgroup (n = 148). Comparable data were obtained for 108 primary care patients at risk of diabetes.Results: diabetes was diagnosed immediately by routine practice in 1% (22/2061) [aged 36 (26ā€“61) years (median IQ range)/55% (12/22) male] with pre-existing diabetes/dysglycaemia present in 19% (390/2061) [69 (58ā€“80) years/60% (235/390) male].Possible diabetes symptoms/complications were identified in 19% [70 (59ā€“79) years/57% (223/390) male] with their HbA1c similar to primary care patients [54 (46ā€“61) years], 5.7 (5.3ā€“6.0)%/39 (34ā€“42) mmol/mol (n = 148) vs 5.7 (5.4ā€“6.1)%/39 (36ā€“43) mmol/mol, p = 0.35, but lower than those diagnosed on admission, 10.2 (7.4ā€“13.3)%/88 (57ā€“122) mmol/mol, p &lt; 0.001. Their fasting plasma glucose (FPG) was similar to primary care patients, 5.2 (4.8ā€“5.7) vs 5.2 (4.8ā€“5.9) mmol/L, p = 0.65, but 2hPG higher, 9.0 (7.3ā€“11.4) vs 5.5 (4.4ā€“7.5), p &lt; 0.001.HbA1c identified diabetes in 10% (15/148) with 14 confirmed on OGTT but overall 32% (48/148) were in diabetic range on OGTT. The specificity of HbA1c in 2061 admissions was similar to primary care, 99% vs 96%, p = 0.20, but sensitivity lower, 38% vs 93%, p &lt; 0.001 (63% on FPG/23% on 2hPG, p = 0.037, in those with possible symptoms/complications).Conclusion: HbA1c can play a diagnostic role in acute medicine as it diagnosed another 2% of admissions with diabetes but the discrepancy in sensitivity shows that it does not reflect transient/acute hyperglycaemia resulting from the acute medical event.</p

    Quenched complexity of the p-spin spherical spin-glass with external magnetic field

    We consider the p-spin spherical spin-glass model in the presence of an external magnetic field as a general example of a mean-field system where a one step replica symmetry breaking (1-RSB) occurs. In this context we compute the complexity of the Thouless-Anderson-Palmer states, performing a quenched computation. We find what is the general connection between this method and the standard static 1-RSB one, formulating a clear mapping between the parameters used in the two different calculations. We also perform a dynamical analysis of the model, by which we confirm the validity of our results.Comment: RevTeX, 11 pages, including 2 EPS figure

    We revisit two classic Thouless-Anderson-Palmer (TAP) studies of the Sherrington-Kirkpatrick model [Bray A J and Moore M A 1980 J. Phys. C 13, L469; De Dominicis C and Young A P, 1983 J. Phys. A 16, 2063]. By using the Becchi-Rouet-Stora-Tyutin (BRST) supersymmetry, we prove the general equivalence of TAP and replica partition functions, and show that the annealed calculation of the TAP complexity is formally identical to the quenched thermodynamic calculation of the free energy at one step level of replica symmetry breaking. The complexity we obtain by means of the BRST symmetry turns out to be considerably smaller than the previous non-symmetric value.Comment: 17 pages, 3 figure

    Idiopathic congenital nystagmus is characterized by involuntary, periodic, predominantly horizontal oscillations of both eyes. We identified 22 mutations in FRMD7 in 26 families with X-linked idiopathic congenital nystagmus. Screening of 42 singleton cases of idiopathic congenital nystagmus (28 male, 14 females) yielded three mutations (7%). We found restricted expression of FRMD7 in human embryonic brain and developing neural retina, suggesting a specific role in the control of eye movement and gaze stability

    Ribosomal proteins (RP) regulate specific gene expression by selectively translating subsets of mRNAs. Indeed, in Diamond-Blackfan anemia and 5q- syndrome, mutations in RP genes lead to a specific defect in erythroid gene translation and cause anemia. Little is known about the molecular mechanisms of selective mRNA translation and involvement of ribosomal-associated factors in this process. Ribonuclease inhibitor 1 (RNH1) is a ubiquitously expressed protein that binds to and inhibits pancreatic-type ribonucleases. Here, we report that RNH1 binds to ribosomes and regulates erythropoiesis by controlling translation of the erythroid transcription factor GATA1. Rnh1-deficient mice die between embryonic days E8.5 and E10 due to impaired production of mature erythroid cells from progenitor cells. In Rnh1-deficient embryos, mRNA levels of Gata1 are normal, but GATA1 protein levels are decreased. At the molecular level, we found that RNH1 binds to the 40S subunit of ribosomes and facilitates polysome formation on Gata1 mRNA to confer transcript-specific translation. Further, RNH1 knockdown in human CD34+ progenitor cells decreased erythroid differentiation without affecting myelopoiesis. Our results reveal an unsuspected role for RNH1 in the control of GATA1 mRNA translation and erythropoiesis

    A transcriptomic snapshot of early molecular communication between Pasteuria penetrans and Meloidogyne incognita

    Ā© The Author(s). 2018Background: Southern root-knot nematode Meloidogyne incognita (Kofoid and White, 1919), Chitwood, 1949 is a key pest of agricultural crops. Pasteuria penetrans is a hyperparasitic bacterium capable of suppressing the nematode reproduction, and represents a typical coevolved pathogen-hyperparasite system. Attachment of Pasteuria endospores to the cuticle of second-stage nematode juveniles is the first and pivotal step in the bacterial infection. RNA-Seq was used to understand the early transcriptional response of the root-knot nematode at 8 h post Pasteuria endospore attachment. Results: A total of 52,485 transcripts were assembled from the high quality (HQ) reads, out of which 582 transcripts were found differentially expressed in the Pasteuria endospore encumbered J2 s, of which 229 were up-regulated and 353 were down-regulated. Pasteuria infection caused a suppression of the protein synthesis machinery of the nematode. Several of the differentially expressed transcripts were putatively involved in nematode innate immunity, signaling, stress responses, endospore attachment process and post-attachment behavioral modification of the juveniles. The expression profiles of fifteen selected transcripts were validated to be true by the qRT PCR. RNAi based silencing of transcripts coding for fructose bisphosphate aldolase and glucosyl transferase caused a reduction in endospore attachment as compared to the controls, whereas, silencing of aspartic protease and ubiquitin coding transcripts resulted in higher incidence of endospore attachment on the nematode cuticle. Conclusions: Here we provide evidence of an early transcriptional response by the nematode upon infection by Pasteuria prior to root invasion. We found that adhesion of Pasteuria endospores to the cuticle induced a down-regulated protein response in the nematode. In addition, we show that fructose bisphosphate aldolase, glucosyl transferase, aspartic protease and ubiquitin coding transcripts are involved in modulating the endospore attachment on the nematode cuticle. Our results add new and significant information to the existing knowledge on early molecular interaction between M. incognita and P. penetrans.Peer reviewedFinal Published versio
