    Effect of cannabis smoking on lung function and respiratory symptoms: a structured literature review

    As cannabis use increases, physicians need to be familiar with the effects of both cannabis and tobacco on the lungs. However, there have been very few long-term studies of cannabis smoking, mostly due to legality issues and the confounding effects of tobacco. It was previously thought that cannabis and tobacco had similar long-term effects as both cause chronic bronchitis. However, recent large studies have shown that, instead of reducing forced expiratory volume in 1 s and forced vital capacity (FVC), marijuana smoking is associated with increased FVC. The cause of this is unclear, but acute bronchodilator and anti-inflammatory effects of cannabis may be relevant. Bullous lung disease, barotrauma and cannabis smoking have been recognised in case reports and small series. More work is needed to address the effects of cannabis on lung function, imaging and histological changes

    Production of placental alkaline phosphatase (PLAP) and PLAP-like material by epithelial germ cell and non-germ cell tumours in vitro

    Placental and placental-like alkaline phosphatase (PLAP) levels in the culture media of 87 cell lines of neoplastic and 'normal' origin were measured by a conventional immunosorbent enzymatic assay (IAEA) and by a new immunoradiometric assay (IRMA). The IRMA detected immunoreactive PLAP in 37 of 80 (46%) human epithelial and germ cell cultures, while the IAEA detected PLAP in only 25 (33%). Of the 52 non-germ cell tumour cultures, the IRMA detected expression in 24 (46%) and the IAEA in only 16 (31%). In 17 cases (21%) the IRMA recorded levels double that of the IAEA, while in five cultures (6%) the reverse was true. The IRMA was much more robust than the IAEA and had considerably lower inter- and intra-assay coefficients of variation (3.75-8.5% vs 5.2-46%). Detection of PLAP(-like) expression by IAEA is dependent on neoplastic expression of enzymatically functional molecules and quantification assumes constant enzyme kinetics. PLAP-like material has a higher catalytic rate constant than PLAP and thus will give higher values on a stoichiometric basis in an IAEA. The higher detection rate and levels of PLAP-like material in neoplastic cultures when measured by the IRMA clearly demonstrate ectopic expression of non-enzymatic PLAP and PLAP-like genes. The incidence of PLAP(-like) expression by non-germ cell and possible germ cell tumours has been underestimated and its utility as a tumour marker should be re-examined using assays which measure antigen mass rather than phosphatase activity

    This study aims to carry out business planning and system design for the creation of a business startup marketplace based on a Muslim fashion mobile application with the name MALIK. The research method used is by using observation, questionnaires, interviews, and literature study. For system design, the approach method used is object-oriented analysis design. The results of this research will be a business plan in the form of business plan documentation complete with analysis using the business model canvas, industry analysis using Porter's Five Forces Model, and SWOT analysis. The next stage is to design a system in the form of a diagram with a unified modeling language notation, and a user interface. The results of the research that can be taken are that the target sample of customers is interested in Muslim clothing, buying Muslim clothing using the marketplace media with feature development carried out by MALIK in the form of online Muslim clothing tailoring, and virtual fitting rooms

    Laparoscopic hysterectomy with or without pelvic lymphadenectomy or sampling in a high-risk series of patients with endometrial cancer

    BACKGROUND: The purpose of the study was to determine the outcome of all patients with endometrial adenocarcinoma cancer treated by laparoscopic hysterectomy at our institution, many of whom were high-risk for surgery. METHODS: Data was collected by a retrospective search of the case notes and Electronic Patient Records of the thirty eight patients who underwent laparoscopic hysterectomy for endometrial cancer at our institutions. RESULTS: The median body mass index was 30 (range 19–67). Comorbidities were present in 76% (29 patients); 40% (15 patients) had a single comorbid condition, whilst 18% (7 patients) had two, and a further 18% (7 patients) had more than two. Lymphadenectomy was performed in 45% (17 patients), and lymph node sampling in 21% (8 patients). Median operating time was 210 minutes (range 70–360 minutes). Median estimated blood loss was 200 ml (range 50–1000 ml). There were no intraoperative complications. Post-operative complications were seen in 21% (2 major, 6 minor). Blood transfusion was required in 5% (2 patients). The median stay was 4 post-operative nights (range 1–25 nights). In those patients undergoing lymphadenectomy, the mean number of nodes taken was fifteen (range 8–26 nodes). The pathological staging was FIGO stage I 76% (29 patients), stage II 8% (3 patients), stage III 16% (6 patients). The pathological grade was G1 31% (16 patients), G2 45% (17 patients), G3 24% (8 patients). CONCLUSION: Laparoscopic hysterectomy can be safely carried out in patients at high risk for surgery, with no compromise in terms of outcomes, whilst providing all the benefits inherent in minimal access surgery

    Pentingnya penelitian ini adalah untuk mengetahui pemikiran dan perjuangan Abdurrahman Wahid tentang HAM dan Minoritas, dan juga untuk mengetahui apakah pemikiran dan perjuangan Abdurrahman Wahid masih relevan dan bisa menjadi solusi penegakkan HAM dan menghilangkan diskriminasi Minoritas saat ini dan masa yang akan datang. Permasalahan pada penelitian melihat pada angka pelanggaran HAM dan diskriminasi terhadap minoritas masih terus terjadi, bahkan angka pelangggarannya meningkat setiap tahunnya, tentu hal yang miris jika melihat negara yang memakai sistem demokrasi tidak menegakkan dua tema tersebut, hal itulah yang menjadi pemicu munculnya tokoh-tokoh pegiat HAM salah satunya adalah KH.Abdurrahman Wahid yang sudah terbukti sangat konsisten dalam penegakkan HAM dan menghilangkan diskriminasi terhadap Minoritas. Penelitian ini adalah penelitian tokoh dan penyajian sumber datanya diambil berdasarkan kajian literatur (library-research), pendekatan penelitian ini menggunakan metode penelitian historis-faktual mengenai tokoh yaitu mengikuti cara dan arah pikiran Abdurrahman wahid melalui karya-karya beliau dan karya milik orang lain yang meneliti tentang Gus Dur yang berkaitan dengan HAM dan Minoritas lalu di deskripsikan dengan sedemikian rupa, lalu mencari dan mengetahui kejadian semasa hidup Gus Dur yang relevan dengan permasalahan HAM dan Minoritas dimasa sekarang ini untuk menemukan pemahaman atau ide baru. Hasil dari penelitian ini yaitu mengungkapkan bahwa Pemikiran dan perjuangan Abdurrahman Wahid tentang HAM dan Minoritas ini masih relevan dan bisa menjadi solusi jika diimplikasikan pada zaman modern sekarang ini, karena apa yang dialami oleh Indonesia sekarang ini dengan masa lampau kurang lebih sama yaitu rentan terpecah belah oleh berbagai faktor salah satunya yaitu isu SARA dan saling fitnah akibat perbedaan pandangan, Abdurrahman Wahid sudah merasakan dan mengahadapi momen permasalahan itu semasa hidupnya dan apa yang Gus Dur lakukan dalam mengahadapi permasalahan itu patut kita teruskan dan bisa menjadi panutan oleh pemerintah yang berkuasa sekarang, karena apa yang Gus Dur lakukan pada masa lalu terbukti cukup efektif untuk menekan angka pelanggaran HAM bahkan beliau mendapat hati tersendiri dimata kaum minoritas yang pernah beliau bela, bahkan Komnas HAM dan LIPI pun pernah mengemukakan bahwa wilayah rawan konflik seperti papua pemerintah harus menurunkan keluarga Gus Dur untuk berdialog dengan tokoh masyarakat papua, karena keluarga Gus Dur disana masih sangat dihormati. Dari hal itu dapat disimpulkan bahwa pemikiran dan perjuangan Abdurrahman Wahid mengenai HAM dan Minoritas masih sangat relevan untuk direlevansikan dimasa sekarang dan masa yang akan datang
