    The upper and lower airway microbiome in severe asthma

    Introduction: Severe asthma is complex and immunologically heterogenous; airways infection and innate immune dysregulation may confer inhaled corticosteroid resistance and remains clinically challenging. We hypothesise that marked airway microbiome compositional shifts in severe asthma represent a ā€˜treatable traitā€™, with dominance of pathogenic species, warranting targeted therapy. Additionally, these changes are thought to be distinct in the paucibacillary lower airway vs. heavily colonised nasopharynx. Methods: We performed Oxford Nanopore metagenomic sequencing, and RT-qPCR of induced sputum (n=67), bronchoalveolar lavage (BAL;n=71) and nasal lavage (NL;n=28 paired with sputum) samples from the Wessex Severe Asthma Cohort encompassing mild/moderate/severe asthma and health. Findings were integrated with clinical and cytokine data. Key Results: A dominant pathogenic organism (H.influenzae, S.pneumoniae, M.catarrhalis) was identified in sputua of 21% of patients with severe asthma and accompanied by sputum neutrophilia and elevated type-1 cytokines (including IL-1Ī², IL-6, IL-8, TNF;p<0.01, unpaired t-test, Benjamini-Hochberg correction). From BAL, airways infection was identified infrequently (Severe asthma, n=1/21 H.influenzae; Mild-Moderate asthma,n=2/25 T.whipplei). Metagenomic analysis of NL demonstrated a distinct microbiome; presence of pathogenic organisms in the upper airways did not predict concurrent presence in sputum of severe asthmatics. Conclusion: Airways infection is reliably identified in sputum using metagenomic sequencing and is associated with neutrophilic inflammation. The microbiome in the upper and lower airway are distinct and ongoing work is exploring its impact on mucosal immune responses at these sites

    Too Big to Fail ā€” U.S. Banksā€™ Regulatory Alchemy: Converting an Obscure Agency Footnote into an ā€œAt Willā€ Nullification of Dodd-Frankā€™s Regulation of the Multi-Trillion Dollar Financial Swaps Market

    The multi-trillion-dollar market for, what was at that time wholly unregulated, over-the-counter derivatives (ā€œswapsā€) is widely viewed as a principal cause of the 2008 worldwide financial meltdown. The Dodd-Frank Act, signed into law on July 21, 2010, was expressly considered by Congress to be a remedy for this troublesome deregulatory problem. The legislation required the swaps market to comply with a host of business conduct and anti-competitive protections, including that the swaps market be fully transparent to U.S. financial regulators, collateralized, and capitalized. The statute also expressly provides that it would cover foreign subsidiaries of big U.S. financial institutions if their swaps trading could adversely impact the U.S. economy or represent the use of extraterritorial trades as an attempt to ā€œevadeā€ Dodd-Frank. In July 2013, the CFTC promulgated an 80-page, triple-columned, and single-spaced ā€œguidanceā€ implementing Dodd-Frankā€™s extraterritorial reach, i.e., that manner in which Dodd-Frank would apply to swaps transactions executed outside the United States. The key point of that guidance was that swaps trading within the ā€œguaranteedā€ foreign subsidiaries of U.S. bank holding company swaps dealers were subject to all of Dodd-Frankā€™s swaps regulations wherever in the world those subsidiariesā€™ swaps were executed. At that time, the standardized industry swaps agreement contemplated that, inter alia, U.S. bank holding company swaps dealersā€™ foreign subsidiaries would be ā€œguaranteedā€ by their corporate parent, as was true since 1992. In August 2013, without notifying the CFTC, the principal U.S. bank holding company swaps dealer trade association privately circulated to its members standard contractual language that would, for the first time, ā€œdeguaranteeā€ their foreign subsidiaries. By relying only on the obscure footnote 563 of the CFTC guidanceā€™s 662 footnotes, the trade association assured its swaps dealer members that the newly deguaranteed foreign subsidiaries could (if they so chose) no longer be subject to Dodd-Frank. As a result, it has been reported (and it also has been understood by many experts within the swaps industry) that a substantial portion of the U.S. swaps market has shifted from the large U.S. bank holding companies swaps dealers and their U.S. affiliates to their newly deguaranteed ā€œforeignā€ subsidiaries, with the attendant claim by these huge big U.S. bank swaps dealers that Dodd-Frank swaps regulation would not apply to these transactions. The CFTC also soon discovered that these huge U.S. bank holding company swaps dealers were ā€œarranging, negotiating, and executingā€ (ā€œANEā€) these swaps in the United States with U.S. bank personnel and, only after execution in the U.S., were these swaps formally ā€œassignedā€ to the U.S. banksā€™ newly ā€œdeguaranteedā€ foreign subsidiaries with the accompanying claim that these swaps, even though executed in the U.S., were not covered by Dodd-Frank. In October 2016, the CFTC proposed a rule that would have closed the ā€œdeguaranteeā€ and ā€œANEā€ loopholes completely. However, because it usually takes at least a year to finalize a ā€œproposedā€ rule, this proposed rule closing the loopholes in question was not finalized prior to the inauguration of President Trump. All indications are that it will never be finalized during a Trump Administration. Thus, in the shadow of the recent tenth anniversary of the Lehman failure, there is an understanding among many market regulators and swaps trading experts that large portions of the swaps market have moved from U.S. bank holding company swaps dealers and their U.S. affiliates to their newly deguaranteed foreign affiliates where Dodd- Frank swaps regulation is not being followed. However, what has not moved abroad is the very real obligation of the lender of last resort to rescue these U.S. swaps dealer bank holding companies if they fail because of poorly regulated swaps in their deguaranteed foreign subsidiaries, i.e., the U.S. taxpayer. While relief is unlikely to be forthcoming from the Trump Administration or the Republican-controlled Senate, some other means will have to be found to avert another multi-trillion-dollar bank bailout and/or a financial calamity caused by poorly regulated swaps on the books of big U.S. banks. This paper notes that the relevant statutory framework affords state attorneys general and state financial regulators the right to bring so-called ā€œparens patriaeā€ actions in federal district court to enforce, inter alia, Dodd- Frank on behalf of a stateā€™s citizens. That kind of litigation to enforce the statuteā€™s extraterritorial provisions is now badly needed

    Effects of oral adenosine 5'-triphosphate and adenosine in enteric-coated capsules on indomethacin-induced permeability changes in the human small intestine: a randomized cross-over study

    <p>Abstract</p> <p>Background</p> <p>It is well-known that nonsteroidal anti-inflammatory drugs (NSAIDs) can cause damage to the small bowel associated with disruption of mucosal barrier function. In healthy human volunteers, we showed previously that topical administration of adenosine 5'-triphosphate (ATP) by naso-intestinal tube attenuated a rise in small intestinal permeability induced by short-term challenge with the NSAID indomethacin. This finding suggested that ATP may be involved in the preservation of intestinal barrier function. Our current objective was to corroborate the favourable effect of ATP on indomethacin-induced permeability changes in healthy human volunteers when ATP is administered via enteric-coated capsules, which is a more practically feasible mode of administration. Since ATP effects may have been partly mediated through its breakdown to adenosine, effects of encapsulated adenosine were tested also.</p> <p>Methods</p> <p>By ingesting a test drink containing 5 g lactulose and 0.5 g L-rhamnose followed by five-hour collection of total urine, small intestinal permeability was assessed in 33 healthy human volunteers by measuring the urinary lactulose/rhamnose excretion ratio. Urinary excretion of lactulose and L-rhamnose was determined by fluorescent detection high-pressure liquid chromatography (HPLC). Basal permeability of the small intestine was assessed as a control condition (no indomethacin, no ATP/adenosine). As a model of increased small intestinal permeability, two dosages of indomethacin were ingested at 10 h (75 mg) and 1 h (50 mg) before ingesting the lactulose/rhamnose test drink. At 1.5 h before indomethacin ingestion, two dosages of placebo, ATP (2 g per dosage) or adenosine (1 g per dosage) were administered via enteric-coated hydroxypropyl methylcellulose (HPMC) capsules with Eudragit<sup>Ā© </sup>L30D-55.</p> <p>Results</p> <p>Median urinary lactulose/rhamnose excretion ratio (g/g) in the control condition was 0.032 (interquartile range: 0.022ā€“0.044). Compared to the control condition, lactulose/rhamnose ratio after ingestion of indomethacin plus placebo was significantly increased to 0.039 (0.035ā€“0.068); P < 0.01). The indomethacin-induced increase was neither affected by administration of encapsulated ATP (0.047 (0.033ā€“0.065)) nor adenosine (0.050 (0.030ā€“0.067)). Differences in L/R ratios between the conditions with indomethacin plus placebo, ATP or adenosine were not significant.</p> <p>Conclusion</p> <p>In this study, either ATP or adenosine administered via enteric-coated capsules had no effect on indomethacin-induced small intestinal permeability changes in healthy human volunteers. The observed lack of effect of encapsulated ATP/adenosine may have been caused by opening of the enteric-coated supplement at a site distal from the indomethacin-inflicted site. Further studies on site-specific effectiveness of ATP/adenosine on intestinal permeability changes are warranted.</p

    Inference of development activities from interaction with uninstrumented applications

    Studying developersā€™ behavior in software development tasks is crucial for designing effective techniques and tools to support developersā€™ daily work. In modern software development, developers frequently use different applications including IDEs, Web Browsers, documentation software (such as Office Word, Excel, and PDF applications), and other tools to complete their tasks. This creates significant challenges in collecting and analyzing developersā€™ behavior data. Researchers usually instrument the software tools to log developersā€™ behavior for further studies. This is feasible for studies on development activities using specific software tools. However, instrumenting all software tools commonly used in real work settings is difficult and requires significant human effort. Furthermore, the collected behavior data consist of low-level and fine-grained event sequences, which must be abstracted into high-level development activities for further analysis. This abstraction is often performed manually or based on simple heuristics. In this paper, we propose an approach to address the above two challenges in collecting and analyzing developersā€™ behavior data. First, we use our ActivitySpace framework to improve the generalizability of the data collection. ActivitySpace uses operating-system level instrumentation to track developer interactions with a wide range of applications in real work settings. Secondly, we use a machine learning approach to reduce the human effort to abstract low-level behavior data. Specifically, considering the sequential nature of the interaction data, we propose a Condition Random Field (CRF) based approach to segment and label the developersā€™ low-level actions into a set of basic, yet meaningful development activities. To validate the generalizability of the proposed data collection approach, we deploy the ActivitySpace framework in an industry partnerā€™s company and collect the real working data from ten professional developersā€™ one-week work in three actual software projects. The experiment with the collected data confirms that with initial human-labeled training data, the CRF model can be trained to infer development activities from low-level actions with reasonable accuracy within and across developers and software projects. This suggests that the machine learning approach is promising in reducing the human efforts required for behavior data analysis.This work was partially supported by NSFC Program (No. 61602403 and 61572426)

    Where Are All the Mycobacterium avium Subspecies paratuberculosis in Patients with Crohn's Disease?

    Mycobacterium avium subspecies paratuberculosis (MAP) causes a chronic granulomatous inflammation of the intestines, Johne's disease, in dairy cows and every other species of mammal in which it has been identified. MAP has been identified in the mucosal layer and deeper bowel wall in patients with Crohn's disease by methods other than light microscopy, and by direct visualization in small numbers by light microscopy. MAP has not been accepted as the cause of Crohn's disease in part because it has not been seen under the microscope in large numbers in the intestines of patients with Crohn's disease. An analysis of the literature on the pathology of Crohn's disease and on possible MAP infection in Crohn's patients suggests that MAP might directly infect endothelial cells and adipocytes and cause them to proliferate, causing focal obstruction within already existing vessels (including granuloma formation), the development of new vessels (neoangiogenesis and lymphangiogenesis), and the ā€œcreeping fatā€ of the mesentery that is unique in human pathology to Crohn's disease but also occurs in bovine Johne's disease. Large numbers of MAP might therefore be found in the mesentery attached to segments of intestine affected by Crohn's disease rather than in the bowel wall, the blood and lymphatic vessels running through the mesentery, or the mesenteric fat itself. The walls of fistulas might result from the neoangiogenesis or lymphangiogenesis that occurs in the bowel wall in Crohn's disease and therefore are also possible sites of large numbers of MAP. The direct visualization of large numbers of MAP organisms in the tissues of patients with Crohn's disease will help establish that MAP causes Crohn's disease

    Paternal effects on early embryogenesis

    Historically, less attention has been paid to paternal effects on early embryogenesis than maternal effects. However, it is now apparent that certain male factor infertility phenotypes are associated with increased DNA fragmentation and/or chromosome aneuploidies that may compromise early embryonic development. In addition, there is a growing body of evidence that the fertilizing sperm has more function than just carrying an intact, haploid genome. The paternally inherited centrosome is essential for normal fertilization, and the success of higher order chromatin packaging may impact embryogenesis. Epigenetic modifications of sperm chromatin may contribute to the reprogramming of the genome, and sperm delivered mRNA has also been hythesized to be necessary for embryogenesis. There is less information about the epigenetic factors affecting embryogenesis than genetic factors, but the epigenetics of gamete and early embryogenesis is a rapidly advancing field
