Silicone adhesive multilayer foam dressings as adjuvant prophylactic therapy to prevent hospital-acquired pressure ulcers : a pragmatic noncommercial multicentre randomized open-label parallel-group medical device trial
Background: Silicone adhesive multilayer foam dressings are used as adjuvant therapy to prevent hospitalâacquired pressure ulcers (PUs).
Objectives: Determine if silicone foam dressings in addition to standard prevention reduce PU incidence category 2 or worse compared to standard prevention alone.
Methods: Multicentre, randomised controlled, medical device trial conducted in eight Belgian hospitals. At risk adult patients were centrally randomised (n=1633) to study groups based on a 1:1:1 allocation: experimental group 1 (n=542) and 2 (n=545) â pooled as the treatment group â and the control group (n=546). Experimental groups received PU prevention according to hospital protocol, and a silicone foam dressing on these body sites. The control group received standard of care. The primary endpoint was the incidence of a new PU category 2 or worse at these body sites.
Results: In the intentionâtoâtreat population (n=1605); 4.0% of patients developed PUs category 2 or worse in the treatment group and 6.3% in the control group (RR=0.64, 95% CI 0.41 to 0.99, P=0.04). Sacral PUs were observed in 2.8% and 4.8% of the patients in the treatment group and the control group, respectively (RR=0.59, 95% CI 0.35 to 0.98, P=0.04). Heel PUs occurred in 1.4% and 1.9% of patients in the treatment and control group respectively (RR=0.76, 95% CI 0.34 to 1.68, P=0.49).
Conclusions: Silicone foam dressings reduce the incidence of PUs category 2 or worse in hospitalised atârisk patients when used in addition to standard of care. Results show a decrease for sacrum, but no statistical difference for heel/trochanter areas

Vasodilator factors in the systemic and local adaptations to pregnancy
We postulate that an orchestrated network composed of various vasodilatory systems participates in the systemic and local hemodynamic adaptations in pregnancy. The temporal patterns of increase in the circulating and urinary levels of five vasodilator factors/systems, prostacyclin, nitric oxide, kallikrein, angiotensin-(1â7) and VEGF, in normal pregnant women and animals, as well as the changes observed in preeclamptic pregnancies support their functional role in maintaining normotension by opposing the vasoconstrictor systems. In addition, the expression of these vasodilators in the different trophoblastic subtypes in various species supports their role in the transformation of the uterine arteries. Moreover, their expression in the fetal endothelium and in the syncytiotrophoblast in humans, rats and guinea-pigs, favour their participation in maintaining the uteroplacental circulation. The findings that sustain the functional associations of the various vasodilators, and their participation by endocrine, paracrine and autocrine regulation of the systemic and local vasoactive changes of pregnancy are abundant and compelling. However, further elucidation of the role of the various players is hampered by methodological problems. Among these difficulties is the complexity of the interactions between the different factors, the likelihood that experimental alterations induced in one system may be compensated by the other players of the network, and the possibility that data obtained by manipulating single factors in vitro or in animal studies may be difficult to translate to the human. In addition, the impossibility of sampling the uteroplacental interface along normal pregnancy precludes obtaining longitudinal profiles of the various players. Nevertheless, the possibility of improving maternal blood pressure regulation, trophoblast invasion and uteroplacental flow by enhancing vasodilation (e.g. L-arginine, NO donors, VEGF transfection) deserves unravelling the intricate association of vasoactive factors and the systemic and local adaptations to pregnancy

Advertising, earnings prediction and market value: An analysis of persistent UK advertisers
YesThis paper examines whether major media advertising expenditures help in predicting future earnings. We consider the role of media advertising in firmsâ marketing efforts and posit that persistent advertisers are more likely to benefit from advertising activities in creating longâlived intangible assets. Employing a sample of persistent UK advertisers over the period 1997â2013, we find that advertising expenditures are significantly positively associated with firmsâ future earnings and market value. We also report size and sectorâbased differences in the association between advertising and firmsâ future earnings. Our additional analysis provides support for the arguments that despite the recent rise in digital advertising budgets, traditional advertising media are still effective in positively influencing firmsâ performance. Overall, the results of this study are consistent with the view that advertising expenditures produce intangible assets, at least for firms in certain sectors. These findings have implications for marketers in providing evidence of the value generated by firmsâ advertising budgets, for investors in validating the relevance of advertising information in influencing future earnings, and for accounting regulators in relation to the provision of useful insights for any future deliberations on financial reporting policies for advertising expenditures
Resale Price Maintenance: Output Increasing or Restricting? The Case of Distilled Spirits in the United States.
Theories of resale price maintenance (RPM) differ strongly on the pro- and a nti-competitive effects on RPM. There are few tests of the welfare ga ins or losses from RPM in industry case studies. This paper tests RPM 's welfare effects in the retail sale of distilled spirits in the U.S Using regression analysis to estimate demand, comparing liquor stor e license values over time, and analyzing political contributions, th e study finds that efficiency arguments for RPM are unsupported, with RPM leading to a reduction in distilled spirits consumption. Evidenc e shows substantial wealth transfers from consumers to retail liquor store owners due to RPM. Copyright 1987 by Blackwell Publishing Ltd.
