Die endovaskulĂ€re Ausbildung fĂŒr GefĂ€Ăchirurgen an der UniversitĂ€tsklinik Bern
Zusammenfassung: Die technische Entwicklung im Bereich von Katheterinterventionen hat auch zunehmenden Einfluss auf das TĂ€tigkeitsfeld der GefĂ€Ăchirurgen. Im Operationssaal werden hĂ€ufiger additive Katheterinterventionen oder Kombinationsverfahren, auch Hybrideingriffe genannt, durchgefĂŒhrt. Dies setzt zwingend kathetertechnische Fertigkeiten voraus. Deshalb werden an der Klinik und Poliklinik fĂŒr Herz- und GefĂ€Ăchirurgie in Bern in Zusammenarbeit mit interventionell tĂ€tigen Angiologen und Radiologen die GefĂ€Ăchirurgen in kathetertechnischen Interventionen trainiert. Verwendungsraten von Prothesen der unteren ExtremitÀt in Deutschland und der Schweiz: Ein Vergleich der Jahre 2005-2008
Zusammenfassung: Hintergrund: In den USA hat die Verwendung der totalen HĂŒftprothese innerhalb der letzten Dekade stark zugenommen. Es ist jedoch nicht bekannt, ob sich dieser Trend auf andere LĂ€nder ĂŒbertragen lĂ€sst. Ziel dieser Untersuchung war daher der detaillierte Vergleich der Verwendungsraten von HĂŒft-, Knie- und OSG-Prothesen in der BRD und der Schweiz im Zeitraum 2005-2008 und ein SekundĂ€rvergleich mit den USA. Patienten und Methoden: Aus Daten der beiden statistischen BundesĂ€mter wurden die Einwohnerzahl, Geschlechts- und Altersverteilung sowie die Anzahl von PrimĂ€r- und Revisionseingriffen ermittelt. Aus diesen Kennzahlen konnte die Anzahl von PrimĂ€r-, Revisions- und Gesamteingriffen, der Revisionslast, von PrimĂ€r- und Revisionsraten pro 100.000 Einwohner und Jahr sowie geschlechts- und altersspezifische PrimĂ€r- und Revisionsraten berechnet werden. Ein im Vergleich dazu reduzierter Datensatz aus den USA stammt direkt vom Autor der entsprechenden Auswertungen. Ergebnisse: In Deutschland, der Schweiz und den USA stieg die Anzahl der Voll- und TeilhĂŒftprothesenimplantationen pro 100.000 Personen von 235,8, 238,2 und 116,8 im Jahr 2005 auf 254,7, 262,7 und 127,3 an. FĂŒr Voll- und Teilknieprothesen lagen die Raten bei 156,3, 140,1 und 178,2 Implantationen in 2005 und 188,3, 176,8 und 213,6 in 2008. Die Revisionslast fĂŒr HĂŒftprothesen lag in der BRD in 2005 mit 13,6% um 3,6% höher als in der Schweiz und betrug 11,2% in den USA. In 2008 lag sie mit 15,1% um 4,6% höher als in der Schweiz und betrug wiederum 11,2% in den USA. FĂŒr Knieprothesen lag die Last in der BRD in 2005 bei 11,1% und damit 3,5% höher als in der Schweiz; in den USA betrug sie 7,4%. In 2008 lag sie bei 12,8% und somit 4,2% höher als in der Schweiz und betrug 8,9% in den USA. In allen 3 LĂ€ndern lag die Revisionslast fĂŒr Knieprothesen stets unter derjenigen fĂŒr HĂŒftprothesen. Schlussfolgerung: In allen 3 LĂ€ndern stiegen die PrimĂ€rraten fĂŒr HĂŒft- und Knieprothesen an, diejenigen fĂŒr Knieprothesen stĂ€rker. Die Revisionslasten lagen in 2008 fĂŒr beide Prothesentypen in der BRD am höchsten. In der Schweiz gab es eine vorĂŒbergehende Senkung der Revisionslasten und erst ab 2007 wieder einen Anstieg. Die Verwendungsrate von HĂŒftprothesen pro 100.000 Einwohner lag in den USA stark unter den Raten der BRD und der Schweiz, fĂŒr Knieprothesen lag sie leicht darĂŒbe
Bedeutung von Dysfunktionen der NebenschilddrĂŒse in der spĂ€teren Postmenopause
Zusammenfassung: Die hĂ€ufigste NebenschilddrĂŒsendysfunktion in der spĂ€ten Postmenopause ist der primĂ€re Hyperparathyreoidismus. Meistens ist es eine Zufallsdiagnose. Typische Symptome sind die hyperkalzĂ€mische Krise, Myopathie, Nierensteine, Osteoporose. Bei allen symptomatischen und allen asymptomatischen Patienten mit einem Serumkalzium >â0,25mmol/l der oberen Normgrenze, einer Niereninsuffizienz (GFR <â60ml/min) oder einem T-Score <âââ2,5 an Femur oder LWS sollte die Indikation zur Operation gestellt werden. Bei asymptomatischen Patienten, die keiner Operation bedĂŒrfen, wird einmal pro Jahr die Kontrolle von Serumkalzium, Kreatinin und einmal alle 2Jahre eine Osteodensitometrie empfohlen, da ein Drittel der FĂ€lle progredient ist. Der Hypoparathyreoidismus tritt insgesamt selten und meistens nach Operationen der Schild-, NebenschilddrĂŒse oder Karzinomchirurgie im Halsbereich auf. Die enge Zusammenarbeit zwischen GefĂ€Ăchirurgen und Angiologen hat an der UniversitĂ€t Bern eine lange Tradition. Der vorliegende Artikel legt die grundlegende Philosophie unserer gefĂ€Ăchirurgisch-angiologischen Freundschaft und Zusammenarbeit dar und schildert deren Umsetzung im klinischen Allta
May direct-to-consumer genetic testing have an impact on general practitioners' daily practice? a cross-sectional study of patients' intentions towards this approach.
Direct-to-consumer genetic testing (DTCGT) offers individuals access to information on their probable risks of suffering from a wide range of chronic diseases. General practitioners (GPs) will probably play a major role in supporting its use, but patients' perception of DTCGT remain unclear. This study aimed to describe those attitudes and expectations and how they might affect GPs' daily practices.
In 2018-2019, a study related to the use of DTCGT for preventive care in general medicine was conducted among patients in Switzerland's French-speaking areas. Data were collected in the waiting room using a self-administrated questionnaire about patients' interest in DTCGT and what their attitudes might be if testing revealed an elevated risk of diabetes, colorectal cancer, or Alzheimer's disease.
About 40% of the 929 participating (participation rate about 80%) patients had heard about DTCGT and, once the test had been explained, 43% reported that they would be interested in being tested. If that testing suggested an elevated risk of disease, the majority of patients reported that they would change their lifestyle (65%-81%, depending on the disease), request more examinations (63%-77%), and expect changes in their GP's follow-up (48%-59%). Personal characteristics such as sex, age, urbanity, marital status, and perceived health were factors predictive of patients' attitudes.
Findings indicated that the generalization of DTCGT might affect GPs' daily practices in terms of workload and knowledge about this approach. However, this result must be qualified by the fact that it is based on hypothetical situations

Fe2As has been studied in situ by synchrotron powder X-ray diffraction (PXRD) over the range of temperatures 25â850 °C and under a neutral atmosphere to understand its thermal behavior, which is potentially important for gold extraction. For the first time, incongruent high-temperature reactions of Fe2As are observed as it breaks down and the existence of a previously undiscovered high-temperature FeAs phase with an NiAs-type structure has been determined experimentally. No evidence has been found for the existence of the high-temperature Fe3As2 phase. Hence, the previously published phase diagram for the FeâAs system has to be modified accordingly

Electrical current distribution across a metal-insulator-metal structure during bistable switching
Combining scanning electron microscopy (SEM) and electron-beam-induced
current (EBIC) imaging with transport measurements, it is shown that the
current flowing across a two-terminal oxide-based capacitor-like structure is
preferentially confined in areas localized at defects. As the thin-film device
switches between two different resistance states, the distribution and
intensity of the current paths, appearing as bright spots, change. This implies
that switching and memory effects are mainly determined by the conducting
properties along such paths. A model based on the storage and release of charge
carriers within the insulator seems adequate to explain the observed memory

Long-term safety of secukinumab in patients with moderate-to-severe plaque psoriasis, psoriatic arthritis, and ankylosing spondylitis: integrated pooled clinical trial and post-marketing surveillance data.
BACKGROUND: Secukinumab, a fully human immunoglobulin G1-kappa monoclonal antibody that directly inhibits interleukin (IL)-17A, has been shown to have robust efficacy in the treatment of moderate-to-severe psoriasis (PsO), psoriatic arthritis (PsA), and ankylosing spondylitis (AS) demonstrating a rapid onset of action and sustained long-term clinical responses with a consistently favorable safety profile in multiple Phase 2 and 3 trials. Here, we report longer-term pooled safety and tolerability data for secukinumab across three indications (up to 5âyears of treatment in PsO and PsA; up to 4âyears in AS).
METHODS: The integrated clinical trial safety dataset included data pooled from 21 randomized controlled clinical trials of secukinumab 300 or 150 or 75âmg in PsO (14 Phase 3 trials and 1 Phase 4 trial), PsA (3 Phase 3 trials), and AS (3 Phase 3 trials), along with post-marketing safety surveillance data with a cut-off date of June 25, 2017. Adverse events (AEs) were reported as exposure-adjusted incident rates (EAIRs) per 100 patient-years. Analyses included all patients who received â„â1 dose of secukinumab.
RESULTS: A total of 5181, 1380, and 794 patients from PsO, PsA, and AS clinical trials representing secukinumab exposures of 10,416.9, 3866.9, and 1943.1 patient-years, respectively, and post-marketing data from patients with a cumulative exposure to secukinumab of ~â96,054 patient-years were included in the analysis. The most frequent AE was upper respiratory tract infection. EAIRs across PsO, PsA, and AS indications were generally low for serious infections (1.4, 1.9, and 1.2, respectively), Candida infections (2.2, 1.5, and 0.7, respectively), inflammatory bowel disease (0.01, 0.05, and 0.1, respectively), and major adverse cardiac events (0.3, 0.4, and 0.6, respectively). No cases of tuberculosis reactivation were reported. The incidence of treatment-emergent anti-drug antibodies was low with secukinumab across all studies, with no discernible loss of efficacy, unexpected alterations in pharmacokinetics, or association with immunogenicity-related AEs.
CONCLUSIONS: Secukinumab demonstrated a favorable safety profile over long-term treatment in patients with PsO, PsA, and AS. This comprehensive assessment demonstrated that the safety profile of secukinumab was consistent with previous reports in patients with PsO, PsA, and AS, supporting its long-term use in these chronic conditions
