    Estratègies d'intervenció i avaluació de l'hàbit tabàquic

    Objectiu: Donar informació sobre els riscos del tabac, avaluar l’hàbit tabàquic i el compliment de la normativa vigent per part de la comunitat universitària a través d’una campanya de sensibilització. Metodologia: Prèviament a l’inici de les activitats es van dissenyar i passar unes enquestes per avaluar l’hàbit tabàquic als estudiants (a l’aula) i treballadors de l’Escola de Ciències de la Salut. Durant la setmana d’activitats, que coincidia amb el Dia Internacional de LLuita Antitabàquica, es va crear una taula informativa a càrrec dels estudiants de l’assignatura Atencions d’Infermeria a Addiccions i Toxicomanies, per tal d’informar de forma àmplia sobre el tabaquisme, conseqüències i tractaments. Al mateix temps i de forma passiva en uns monitors de TV es donava més informació, amb una material creat per aquesta campanya. Resultats: Els mitjans de comunicació van demostrar, des de l’inici de la campanya, un gran interès. Respecte a la participació per part de la comunitat universitària podem dir que va ser acceptable, al voltant de 1000 persones van passar per la taula informativa. El perfil dels estudiants que van contestar l’enquesta va ser dona (66.4%), alumne de fisioteràpia (49%), fumadors (regulars i ocasionals 52%). Dintre del grup de fumadors regulars per gèneres les dones fumen més 37%, front als homes 27.7%. Consum setmanal de cigarretes (regulars i ocasionals), presenta una mitjana de 59 (D.E=49). La majoria ha fet algun intent de deixar de fumar, 61%. Respecte al temps d’ abstinència un 69.9% no ha passat de 3 mesos i un 96% pensa que fumar en un espai tancat és perjudicial pels altres. Per últim, de tots el entrevistats la gran majoria (97.8%), declaren que fumen davant seu i a un 41.4 % els molesta. Conclusions: La Comissió fa una valoració positiva general, de com es van desenvolupar les diferents activitats. No obstant s’observa poca sensibilització per una part de la comunitat universitària respecte a la llibertat de viure

    Chemokine Receptor Ccr6 Deficiency Alters Hepatic Inflammatory Cell Recruitment and Promotes Liver Inflammation and Fibrosis.

    Chronic liver diseases are characterized by a sustained inflammatory response in which chemokines and chemokine-receptors orchestrate inflammatory cell recruitment. In this study we investigated the role of the chemokine receptor CCR6 in acute and chronic liver injury. In the absence of liver injury Ccr6-/- mice presented a higher number of hepatic macrophages and increased expression of pro-inflammatory cytokines and M1 markers Tnf-α, Il6 and Mcp1. Inflammation and cell recruitment were increased after carbon tetrachloride-induced acute liver injury in Ccr6-/- mice. Moreover, chronic liver injury by carbon tetrachloride in Ccr6-/- mice was associated with enhanced inflammation and fibrosis, altered macrophage recruitment, enhanced CD4+ cells and a reduction in Th17 (CD4+IL17+) and mature dendritic (MHCII+CD11c+) cells recruitment. Clodronate depletion of macrophages in Ccr6-/- mice resulted in a reduction of hepatic pro-inflammatory and pro-fibrogenic markers in the absence and after liver injury. Finally, increased CCR6 hepatic expression in patients with alcoholic hepatitis was found to correlate with liver expression of CCL20 and severity of liver disease. In conclusion, CCR6 deficiency affects hepatic inflammatory cell recruitment resulting in the promotion of hepatic inflammation and fibrosis

    We would like to introduce our group of research, [CONTRA TAEDIUM], created by professionals from different fields, that have contributed in this article. Our purpose is to expose our reflections based on our own experiences, not only in research, but also in teaching. We propose new forms of writing history in order to understand the dairy life of the women and men of the past, from birth to death. We would like to point out that interacting all types of sources is essential to understand our history. But, what really makes sense is to bring our students in the historical methodology and involve them in their education.     Spanish Cell Therapy Network (TerCel) : 15 years of successful collaborative translational research

    In the current article we summarize the 15-year experience of the Spanish Cell Therapy Network (TerCel), a successful collaborative public initiative funded by the Spanish government for the support of nationwide translational research in this important area. Thirty-two research groups organized in three programs devoted to cardiovascular, neurodegenerative and immune-inflammatory diseases, respectively, currently form the network. Each program has three working packages focused on basic science, pre-clinical studies and clinical application. TerCel has contributed during this period to boost the translational research in cell therapy in Spain, setting up a network of Good Manufacturing Practice-certified cell manufacturing facilities- and increasing the number of translational research projects, publications, patents and clinical trials of the participating groups, especially those in collaboration. TerCel pays particular attention to the public-private collaboration, which, for instance, has led to the development of the first allogeneic cell therapy product approved by the European Medicines Agency, Darvadstrocel. The current collaborative work is focused on the development of multicenter phase 2 and 3 trials that could translate these therapies to clinical practice for the benefit of patients

    Gestión de los residuos sanitarios en el ámbito hospitalario. Nivel de conocimiento y valoración global

    Los residuos sanitarios representan un peligro potencial para los trabajadores sanitarios. Dado el alto riesgo de infección en los accidentes laborales, la gestión correcta de los residuos sanitarios minimiza el riesgo y mejora las condiciones labora­les y ambientales. OBJETIVOS. Identificar el nivel de conocimiento que tienen los profesionales sanitarios, en relación con la clasificación y ges­tión avanzada de los residuos sanitarios (RS) y valorar de for­ma global la gestión de los mismos. METODOLOGÍA. Estudio descriptivo transversal con 178 partici­pantes (auxiliares, enfermeras, médicos y técnicos de labora­torio), de 3 hospitales de la provincia de Barcelona. Se utilizó un cuestionario de elaboración propia, que anali­zaba las variables laborales, el conocimiento y la valoración global de la gestión de los RS. RESULTADOS. El nivel medio de conocimiento sobre los RS de los auxiliares, las enfermeras, los médicos y los técnicos de laboratorio, en una escala de 1 (valor mínimo) a 15 (valor máximo), es de 10.59, 10.61, 8.92 y 8.39, respectivamente. La valoración global que hacen los auxiliares, las enfermeras, los médicos y los técnicos de laboratorio de la gestión de los RS en una escala de 1 (valor mínimo) y 5 (valor máximo), es de 2.89, 2.93, 2.88, 2.94, respectivamente. C0NCLUSI0NES. Los resultados sugieren que las enfermeras y los auxiliares tienen un nivel de conocimiento aceptable con res­pecto a la gestión correcta de los RS. En cambio, los médicos y técnicos de laboratorio tienen un nivel de conocimiento infe­rior sobre la segregación y eliminación adecuada de los RS. En cuanto a la valoración global, se obtienen valores aceptables, y muy similares, entre las diferentes categorías de profesiona­les estudiadas, en relación con la gestión correcta de los RS en los centros asistenciales donde realizan su actividad laboral

    CCL20 mediates lipopolysaccharide induced liver injury and is a potential driver of inflammation and fibrosis in alcoholic hepatitis

    OBJECTIVE: Chemokines are known to play an important role in the pathophysiology of alcoholic hepatitis (AH), a form of acute-on-chronic liver injury frequently mediated by gut derived lipopolysaccharide (LPS). In our study, we hypothesise that chemokine CCL20, one of the most upregulated chemokines in patients with AH, is implicated in the pathogenesis of AH by mediating LPS induced liver injury. DESIGN: CCL20 gene expression and serum levels and their correlation with disease severity were assessed in patients with AH. Cellular sources of CCL20 and its biological effects were evaluated in vitro and in vivo in chronic, acute and acute-on-chronic experimental models of carbon tetrachloride and LPS induced liver injury. RNA interference technology was used to knockdown CCL20 in vivo. RESULTS: CCL20 hepatic and serum levels were increased in patients with AH and correlated with the degree of fibrosis, portal hypertension, endotoxaemia, disease severity scores and short term mortality. Moreover, CCL20 expression was increased in animal models of liver injury and particularly under acute-on-chronic conditions. Macrophages and hepatic stellate cells (HSCs) were identified as the main CCL20 producing cell types. Silencing CCL20 in vivo reduced LPS induced aspartate aminotransferase and lactate dehydrogenase serum levels and hepatic proinflammatory and profibrogenic genes. CCL20 induced proinflammatory and profibrogenic effects in cultured primary HSCs. CONCLUSIONS: Our results suggest that CCL20 upregulation is strongly associated with LPS and may not only represent a new potential biomarker to predict outcome in patients with AH but also an important mediator linking hepatic inflammation, injury and fibrosis in AH

    Gestión de los residuos sanitarios en el ámbito hospitalario (2). Percepción de riesgo frente a la gestión

    OBJETIVO. Identificar la percepción de riesgo que tienen los profesionales sanitarios, en relación con la clasificación y ges­tión avanzada de los residuos sanitarios (RS). METODOLOGIA. Estudio descriptivo y/o explicativo. El tamaño de la muestra fue de 177 participantes (técnicos auxiliares en cuidados de enfermería, enfermeras, médicos y técnicos de laboratorio) de tres hospitales de la provincia de Barcelona. Se utilizó un cuestionario de elaboración propia, para ana­lizar las variables laborales y de percepción de riesgo para la salud laboral y la seguridad personal, tanto para la población en general como para la salud pública, con preguntas en una escala tipo Likert y preguntas de asociación libre y espontá­nea y de frases incompletas. RESULTADOS. El nivel medio de percepción de riesgo de los téc­nicos auxiliares de cuidados de enfermería, las enfermeras, los médicos y los técnicos de laboratorio, en una escala de 1 (baja percepción) a 5 (alta percepción), es de 3.71, 3.75, 3.83 y 4.03, respectivamente. En referencia a los ítems con respuesta de asociación libre y espontánea, el 44.8 % de profesionales consideran los RS un riesgo biológico, el 29.6 % lo consideran material de desecho, el 22.1 % manifiestan que deben gestionarse correctamente y un 3.5 % los describen como residuos desconocidos. CONCLUSIONES. Los resultados sugieren que todos los profesio­nales de la salud tienen en general una percepción de riesgo alta, siendo los técnicos de laboratorio los que tienen una percepción más alta respecto al riesgo real de la gestión in­adecuada de los RS. El 63.2 % manifiesta que se ha de realizar una gestión adecuada para preservar su salud laboral; el 59 % responde que son un riesgo biológico para la población en general y el 47.8 % que son nocivos para la salud pública. Aunque cabe destacar que solo el 44.8 considera que los RS son tóxicos y peligrosos

    Integrative miRNA and Gene Expression Profiling Analysis of Human Quiescent Hepatic Stellate Cells.

    Unveiling the regulatory pathways maintaining hepatic stellate cells (HSC) in a quiescent (q) phenotype is essential to develop new therapeutic strategies to treat fibrogenic diseases. To uncover the miRNA-mRNA regulatory interactions in qHSCs, HSCs were FACS-sorted from healthy livers and activated HSCs (aHSCs) were generated in vitro. MiRNA Taqman array analysis showed HSCs expressed a low number of miRNAs (n = 259), from which 47 were down-regulated and 212 up-regulated upon activation. Computational integration of miRNA and gene expression profiles revealed that 66% of qHSC-associated miRNAs correlated with more than 6 altered target mRNAs (17,28 ± 10,7 targets/miRNA) whereas aHSC-associated miRNAs had an average of 1,49 targeted genes. Interestingly, interaction networks generated by miRNA-targeted genes in qHSCs were associated with key HSC activation processes. Next, selected miRNAs were validated in healthy and cirrhotic human livers and miR-192 was chosen for functional analysis. Down-regulation of miR-192 in HSCs was found to be an early event during fibrosis progression in mouse models of liver injury. Moreover, mimic assays for miR-192 in HSCs revealed its role in HSC activation, proliferation and migration. Together, these results uncover the importance of miRNAs in the maintenance of the qHSC phenotype and form the basis for understanding the regulatory networks in HSCs

    Kinase analysis in alcoholic hepatitis identifies p90RSK as a potential mediator of liver fibrogenesis

    Objective Alcoholic hepatitis (AH) is often associated with advanced fibrosis, which negatively impacts survival. We aimed at identifying kinases deregulated in livers from patients with AH and advanced fibrosis in order to discover novel molecular targets. Design Extensive phosphoprotein analysis by reverse phase protein microarrays was performed in AH (n=12) and normal human livers (n=7). Ribosomal S6 kinase (p90RSK) hepatic expression was assessed by qPCR, Western blot and immunohistochemistry. Kaempferol was used as a selective pharmacological inhibitor of the p90RSK pathway to assess the regulation of experimentally-induced liver fibrosis and injury, using in vivo and in vitro approaches. Results Proteomic analysis identified p90RSK as one of the most deregulated kinases in AH. Hepatic p90RSK gene and protein expression was also upregulated in livers with chronic liver disease. Immunohistochemistry studies showed increased p90RSK staining in areas of active fibrogenesis in cirrhotic livers. Therapeutic administration of kaempferol to carbon tetrachloride-treated mice resulted in decreased hepatic collagen deposition, and expression of profibrogenic and proinflammatory genes, compared to vehicle administration. In addition, kaempferol reduced the extent of hepatocellular injury and degree of apoptosis. In primary hepatic stellate cells, kaempferol and small interfering RNA decreased activation of p90RSK, which in turn regulated key profibrogenic actions. In primary hepatocytes, kaempferol attenuated proapoptotic signalling. Conclusions p90RSK is upregulated in patients with chronic liver disease and mediates liver fibrogenesis in vivo and in vitro. These results suggest that the p90RSK pathway could be a new therapeutic approach for liver diseases characterised by advanced fibrosis

    LPS-TLR4 Pathway mediates ductular cell expansion in alcoholic hepatitis.

    Alcoholic hepatitis (AH) is the most severe form of alcoholic liver disease for which there are no effective therapies. Patients with AH show impaired hepatocyte proliferation, expansion of inefficient ductular cells and high lipopolysaccharide (LPS) levels. It is unknown whether LPS mediates ductular cell expansion. We performed transcriptome studies and identified keratin 23 (KRT23) as a new ductular cell marker. KRT23 expression correlated with mortality and LPS serum levels. LPS-TLR4 pathway role in ductular cell expansion was assessed in human and mouse progenitor cells, liver slices and liver injured TLR4 KO mice. In AH patients, ductular cell expansion correlated with portal hypertension and collagen expression. Functional studies in ductular cells showed that KRT23 regulates collagen expression. These results support a role for LPS-TLR4 pathway in promoting ductular reaction in AH. Maneuvers aimed at decreasing LPS serum levels in AH patients could have beneficial effects by preventing ductular reaction development
