Limits of Competition in the Health Care SystemMany health economists demand more competition in the health-care system. They focus on the competition between the individual health-insurance funds for those who want such insurance as well as on the competition between health-care providers for contracts with the health-insurance funds. However, they neglect the competition between health-care providers for patients; such competition is crucial for medical quality. This latter area of competition is in conflict with the two former ones. The empirical evidence on the effects of competition on cost and quality are also ambiguous. Verteilungsfragen der GRV – Gleichzeitig ein Kommentar zu aktuellen Vorschlägen

Für eine fundierte Beurteilung des Status quo sowie der intendierten und nicht intendierten Effekte sozial- und verteilungspolitischer Maßnahmen sind distributive Analysen als Grundlage für zieladäquate und rationale Entscheidungen unerlässlich. Dabei sind die intertemporalen und interpersonellen Dimensionen zu beachten und zudem die Komplexität eines sozialen Sicherungssystems zu berücksichtigen. Insgesamt gesehen steht eine umfassende Verteilungsanalyse der über die letzten Jahrzehnte erfolgten sozial-, verteilungs-, wirtschafts- sowie steuerpolitischen Maßnahmen mit ihren Wirkungen auf die gesetzliche Rentenversicherung (GRV) und die privaten Haushalte aus. Welche Aspekte bei einer Analyse der distributiven Wirkungen von Maßnahmen zu berücksichtigen sind, dazu enthält der vorliegende Artikel einige Hinweise. Die Beispiele beziehen sich u. a. auf aktuelle Vorschläge zu weiteren Änderungen des Altersvorsorgesystems.   Distributional Questions of Public Pension Schemes – Simultaneous Comment on Current Suggestions Distributional analyses are essential for a well-grounded evaluation of the status quo and the intended and unintended effects of social- and distributional policy measures. They are the indispensable basis for rational and purpose and goal-oriented decisions. For that intertemporal and interpersonal dimensions have to be considered and the complexity of social security systems has to be taken into account. Overall a comprehensive distributional analysis of the social, distributional, economic and fiscal measures – over the last decades – and their effects on the statutory old age pension system (GRV) of Germany and the private households is still missing. The article points out some aspects in the context of a distributional analysis of the effects of the measures which should be considered. The examples refer to current suggestions for additional modifications of Germany’s old age pension system. JEL-Klassifizierung: D63, H55, J2
