    Implementasi pemungutan PBB-P2 dilaksanakan dengan mengacu pada Perbup Pamekasan Nomor 45 Tahun 2013 Tentang tata cara pemungutan PBB-P2, penelitian ini menggunakan metode penelitian kualitatif dengan tujuan menganalisis dan mendiskripsikan implementasi berdasarkan teori Edward III. Temuan penelitian menunjukkan bahwa sosialisasi telah dilaksankan ke seluruh kelompok kerja namun, tidak ada pelibatan masyarakat, komunikasi yang dibangun oleh pihak terkait khususnya pelaksana kebijakan belum optimal, Ketersediaan SDM yang masih sangat minim baik secara kualitas maupun kuantitas, dan sarana yang dimilikipun sangat terbatas. Pemerintah Kecamatan Galis hendaknya melibatkan masyarakat dalam sosialisasi, memberikan pembinaan secara berkala dan melakukan pembinaan di bidang IT dan Penambahan serta perbaikan sarana, serta melakukan evaluasi dan pengawasan terhadap program yang dibuat.     Palliative Surgery for Biliary Drainage in an Unresectable Pancreatic Cancer

    Pancreatic cancer, known for its rapid progression and poor prognosis, usually presents with obstructive jaundice. Biliary drainage can be achieved by various techniques and approaches, with endoscopic drainage as the preferred method. However, open drainage of the biliary tree is indicated when unresectable tumor is found during resection surgery. This is a case of biliary drainage with a double bypass biliodigestive construction, which could be performed in patients with unresectable cancer in the head of the pancreas presenting with obstructive jaundice and gastric outlet obstruction

    Transcranial magnetic stimulation in mild to severe hemiparesis early after stroke: a proof of principle and novel approach to improve motor function

    Abstract Low-frequency repetitive transcranial magnetic stimulation (rTMS) of the unaffected hemisphere can enhance function of the paretic hand in patients with mild motor impairment. Effects of low-frequency rTMS to the contralesional motor cortex at an early stage of mild to severe hemiparesis after stroke are unknown. In this pilot, randomized, double-blind clinical trial we compared the effects of low-frequency rTMS or sham rTMS as add-on therapies to outpatient customary rehabilitation, in 30 patients within 5-45 days after ischemic stroke, and mild to severe hand paresis. The primary feasibility outcome was compliance with the interventions. The primary safety outcome was the proportion of intervention-related adverse events. Performance of the paretic hand in the JebsenTaylor test and pinch strength were secondary outcomes. Outcomes were assessed at baseline, after ten sessions of treatment administered over 2 weeks and at 1 month after end of treatment. Baseline clinical features were comparable across groups. For the primary feasibility outcome, compliance with treatment was 100% in the active group and 94% in the sham group. There were no serious intervention-related adverse events. There were significant improvements in performance in the Jebsen-Taylor test (mean, 12.3% 1 month after treatment) and pinch force (mean, 0.5 Newtons) in the active group, but not in the sham group. Low-frequency rTMS to the contralesional motor cortex early after stroke is feasible, safe and potentially effective to improve function of the paretic hand, in patients with mild to severe hemiparesis. These promising results will be valuable to design larger randomized clinical trials

    Hypertension control during the COVID-19 pandemic: results of the MMM2021 in Russia

    Repetitive quarantines and social restrictions during the coronavirus disease 2019 (COVID-19) pandemic have negatively affected the population health in general, and the control of hypertension (HTN) in particular.Aim. To evaluate the control of HTN in the Russian population during the COVID-19 period based on the results of screening for HTN May Measurement Month 2021 (MMM2021).Material and methods. During May-August 2021, 2491 participants from 11 Russian regions took part in the screening. Participation was voluntary without restrictions on sex. All participants were over 18 years of age. During the screening, blood pressure (BP) was measured three times using automatic and mechanical BP monitors. In addition, a questionnaire was filled out on behavioral risk factors, comorbidities and therapy. HTN was diagnosed with systolic BP ≥140 mmHg and/ or diastolic blood pressure ≥90 mmHg and/or taking antihypertensive therapy. The questionnaire included questions about prior COVID-19, vaccinations and their impact on the intake of antihypertensive drugs.Results. The analysis included data from 2461 respondents aged 18 to 92, of which 963 were men (39,1%). The proportion of hypertensive patients was 41,0%, while among them 59,0% took antihypertensives and 30,9% were effectively treated. In comparison with pre-pandemic period according to MMM2018-2019, the higher proportion of HTN patients in the Russian sample was revealed during MMM2021 (41,0% vs 31,3%, p<0,001) with a comparable proportion of patients receiving antihypertensive therapy (60,7% vs 59,0%, p=0,05) and treatment efficacy (28,7% vs 30,9%, p=0,36). Monotherapy was received in 44,7% of cases, while dual and triple combination therapy — in 30,9% and 14,1%, respectively. The majority of respondents (~90%) did not adjust their antihypertensive therapy during the COVID-19 pandemic.Conclusion. According to HTN screening in Russia, there is persistent ineffective control of HTN, which may be due to both the worsening pattern of behavioral risk factors, limited access to healthcare during COVID-19, and the inertia of physicians and low adherence of patients due to the asymptomatic HTN course in the majority

The relevance of research: Relevance of the researching problem is caused by the revival of all people national cultures, which is happening in Russia and in this connection there are various problems, one of which concerns the international relations in the student's environment. The purposes of article consists in studying of the international relations among students of various nationalities. The leading method of research of this problem is the questioning allowing to consider and study the attitude of students towards culture of the and other people, the relation to religion, studying of interpersonal interaction, to give an assessment to the international relations in the student's environment. Results of questioning on the following blocks are presented in article: 1st block - the attitudes of respondents students of the different nations towards the culture and towards cultures of other people; The 2nd block - the relations of respondents students to various religious trends, associations, the organizations; The 3rd block - interpersonal interactions among respondents students, the 4th block - studying of qualities of Russians and Chechens in representations of respondents - the students having belonging to the Ukrainian and Tatar nations. The existing international relations revealed by means of questioning in the student's environment, can be useful to deputy directors of educational work, methodologists, curators, teachers, experts psychologists, sociologists, ethnologists during the work with students of multinational university
